Development and Validation of the Classification of Home-based Long-term Care Activities (노인장기요양보험 재가서비스 분류 틀 개발 및 타당도 검증)

  • The purpose of this study was to develop the classification of home-based long-term care activities and to test its validity. In this study, the taxonomy of long-term care activities was structured according to the service domain and process. Two expert groups participated in making a draft of the taxonomy that was composed of 7 service domains, 22 care needs, 22 service objectives, and 114 activities. Reliability and validity of the taxonomy was tested in a sample of 152 elderly subjects who used the home-based long-term care services. Based on the factor analysis of 114 activities, 21 factors were extracted from 114 activities. Internal consistency of the factors was high. Content validity was confirmed by the CVI. Long-term care insurance grade was used to assess the criterion validity. Among 21 care needs, 12 cares needs were significantly different from their grade. The classification of home-based long-term care activities demonstrated reliability and validity. In conclusion, the use of this classification is recommended while communicating with the elderly subjects, service providers, and the 3rd party payers.

Database for Hospice Nursing in Electronic Medical Record (호스피스 전자기록을 위한 데이터베이스 개발)

  • Purpose: The purpose of this study was to create an electronic nursing record form to build a hospice nursing process database to be used in the u-hospital EMR system. Specific aims of the study were: 1. To generate a complete, accurate, and simple electronic nursing record form. 2. To verify its appropriateness following documentation with the standardized hospice protocol. 3. To verify its validity and finalize the hospice nursing process database through discussion among hospice professionals. Methods: Nursing records from three independent hospice organizations were collected and analyzed by five expert hospice nurses with more than 10 years of experience, and a nursing record database was developed. This database was applied to 81 hospice patients at three hospice organizations to verify its completeness. Results: 1. An electronic nursing record form with completeness, accuracy, and simplicity was developed. 2. The completeness of the standardized home hospice service protocol was 95.86 percent. 3. The hospice nursing process database contains 18 items on health problems, 79 items on related causes and major symptoms, and 229 items on nursing interventions. Conclusion: The new nursing record form and database will reduce documentation time and articulate and streamline the working process among team members. They can also improve the quality of hospice services, and ultimately enable us to estimate hospice service costs.

The Status of Drug Use of Juveniles and their Deliquent Behaviors -Based on those of Boys and Girls on the Third Years (청소년의 약물 사용과 비행 실태 - 충청남도 일부 고등학교 3학년 학생을 중심으로 -)

  • This study had conducted to push our society for getting some proper measures of the anti-drug abuse and adopting this study as a fundamental data for the school health education. Sample subjects were the 3rd grade of high school in Chungnam Province and the number were 1,014. The data had been collected on Dec. 3th-8th, 1990 according to the tool of Institute on Drug Abuse of Korea. And then Frequency and $x^2-test$ were need to analyse of data by SPSS-X program. The results of this study could be summarized as follows. 1. They had used 14 kinds of drugs: $70.1\%$ of the subjects students used alcohol, $69.2\%$ analgesics, $38.6\%$ cigarette, $15.0\%$ stimulants, $14.7\%$ antitussives, $11.2\%$ antithistamines, $8.9\%$ hypnotics, $7.1\%$tranquilizers, $4.2\%$ inhalants, $2.3\%$ marijuana, $1.4\%$ hallucinogens, $0.9\%$ cocaine, $0.6\%$ amphetamines, and $0.6\%$narcotics. The boys' drug using rate is higher than girls' except in analgesics and stimulants. 2. With respect to misbehaving, $43.0\%$ of the students said they had pop houses, $42.4\%$ adult video, $17.4\%$ unexcused absence from school, $11.0\%$ dangerous weapon, $9.1\%$ sexual intercourse, $70.1\%$ disappearance from home, 8.0% blackmail, and $6.0\%$ assault. In every kinds of misbehaving, the boys' misbehaving rate is higher than the girls'. 3. Almost every kinds of drugs have the students first used at the age of 15 through. 18. And $13.8\%$ of the students smoked every da·y and $1.7\%$ drank every day. 4. The frequency of the drug using such as cigarette (p<.01), marijuana (p<.05), cocaine (p<.05) and narcotics (p<.05) showed the stastically significant differences according to the religions of the subjects 5. The subjects who has complaints about someone use more drugs than the ones who doesn't, and who doesn't have any complaints about five elements have never used drugs such as marijuana, cocaine, amphetamines, narcotics, hallucinogens, and inhalants. 6. The drug using subjects have the more experiences of adult video, pop houses, and unexcused absent from school than the non-drug using ones. It means that the subjects who use drug were apt to do misbehaving more often than the ones who have never used them, and delinquent boys and girls use more drugs.

A Study on the Experience of Back Pain and Developmental Factors of Male High School Students in an Urban area (일 도시지역 남자고교생들의 요통경험과 발생요인에 관한 연구)

  • The purpose of this study was to obtain basic data for the prevention and control of adolescent back pain through analyzing connections between study environments and physical posture. The subjects were 960 male general high school students in the Chonju area and the data were collected by a self-reported questionnaire from Mar. 17 to Mar. 22, 1999. The collected data were analyzed by a frequency, chi-square and t-test using an SPSS program. The results are as follows; 1) The experience rate of back pain perceived by subjects was 67.5% and by each grade: 1st-27.5%, 2nd-35.4%, 3rd-37.1%. The relationship between grades and the experience of back pain didn't show any significant difference. 2) The causes of back pain perceived by subjects such as 'postures are not good' was 56.7%, 'sitting too much time in a chair' was 39.1%, and 'too severe exercise' was 32.4%. 27.8% had back pain first during the 3rd grade of middle school, and 23.9% had it first during the 1st grade of high school. 3) Intensity of subjects' back pain spread from 'moderate' at 49.6%, to 'severe' at 16.4%. Concerning the frequency of back pain, 58.6% said it was 'irregalar'. 4) Among interventions to deal with back pain: 'move by exercising my back or ask friends to beat my back' was 41.0%, 'just bear it' was 23.1%, and at home 'don't have 'any treatment' was ranked first, at 54.9%. 5) The relationship between subjects' general characteristics and back pain experiences: height (t=-1.99, P=.046), sitting/height (t=-2.61, P=.009), self-perceived condition of health (${\chi}^2=23.530$, P=.000), family history (${\chi}^2=43.903$, P=.000) showed significant difference, but the kinds of transportation, sleeping postures, sleeping method and smoking didn't show significant differences. 6) The relationship between subjects' learning environment and back pain experiences, the height of students' desk and chair showed significant difference (${\chi}^2=23.054$, P=.000), but the sitting time didn't show significant difference. 7) The relationship between the characteristics of subjects' physical postures and back pain experiences: standing postures (${\chi}^2=15.105$, P=.001), and sitting postures (${\chi}^2=20.264$X2, P=.001) showed significant difference, but lifting postures didn't show significant difference.

Related Factors for Not Washing Hands at School among Adolescents (청소년의 학교 내 손씻기 미실천율과 관련요인)

  • Objectives: Handwashing is one of the most effective methods to prevent the spread of infectious diseases. This study assessed the related factors and reasons for not practicing handwashing at school among adolescents. Methods: We analyzed data collected from 57,303 adolescents who participated in the 15th Korea Youth Risk Behavior Survey 2019. Results: The proportions of not washing hands "before meals at school" and "after using the toilet at school" were 15.9% and 4.4%, respectively. The adjusted odds ratio for not washing hands before meals at school was significantly higher in girls (Odds Ratio [OR]=1.52, 95% Confidence Intervals [CI]=1.42-1.63), metropolitan city (OR=1.32, 95% CI=1.11-1.56), city (OR=1.29, 95% CI=1.08-1.54), higher grade, higher academic performance, lower economic status, not handwashing at home (OR=14.36, 95% CI=13.37-15.42), and without annual personal hygiene education (OR=1.41, 95% CI=1.33-1.49). Reasons for not washing hands at school among adolescents who do not wash their hands before meals at school included 'it is bothersome (52.3%)', 'there is no soap or hand sanitizer (13.8%)', and 'I do not feel the need (9.5%)'. Conclusions: Improving handwashing before meals at school among adolescents requires raising awareness of the importance of handwashing before meals and establishing a suitable environment and handwashing-encouraging culture.

Effects of Nutritional Status, Activities Daily Living, Instruments Activities Daily Living, and Social Network on the Life Satisfaction of the Elderly in Home (재가노인의 영양상태, 일상생활 수행능력, 도구적 일상생활 수행능력 및 사회적 연결망이 삶의 만족도에 미치는 영향)

  • This study aimed to verify the effects of nutritional status, K-ADL, K-IADL, and social network on the life satisfaction of the elderly in home. Total 213 research subjects participated in this study, and their average age was 71.38±5.59. As the methods of analysis, using the SPSS 21.0, this study examined the differences between variables in accordance with the general characteristics, and then verified the correlations between independent variables of nutritional status, K-ADL, K-IADL, social network(family networks, friends networks), and life satisfaction. In order to verify the factors having effects on the life satisfaction of the elderly in home, the stepwise multiple regression analysis was conducted. In the results of this study, in the general characteristics, the life satisfaction showed statistically significant differences in accordance with education(F=5.280, p=.002), economic condition(F=22.407, p<.001), monthly income(F=3.181, p=.015), and subjective health status(F=14.933, p<.001). In the results of verifying the correlation between independent variables, the life satisfaction showed positive correlations with family networks(r=268, p<.001) and friends networks(r=.286, p<.001) while the nutritional status(r=-.222, p=.001), K-IADL(r=-.235, p=.001), and interdependent social support(r=-.283, p<.001) showed negative correlations. The predictive factors on the life satisfaction of the elderly in home included the economic condition(β=.358, p<.001), subjective health status(β=.245, p<.001), interdependent social support(β=-.158, p=.009), and K-IADL(β=-.153, p=.012), and the explanatory power was 30.1%. The regression model was statistically significant(F=23.778, p<.001). Based on such results of this study, it would be necessary to develop programs that could maintain and improve the health of the elderly, and also provide financial support to the elderly suffering from economic hardship, in order to improve the life satisfaction of the elderly in home. Moreover, there should be the concrete measures for vitalizing the community-connected activities for interdependent social support.

Effects of the Field Management Training Program for Home Care Services : Understanding and Professional Competence (현장관리중심 교육훈련프로그램의 방문건강관리 이해도 및 업무수행능력 인식에 대한 효과)

  • Objectives: The purpose of the present study is to examine the effects of the Field Management Training Program for home care services personnel on their understanding and professional competences. Methods: The subjects were 373 team managers of public home care services who participated in the training program. Data was collected with a self-administered questionnaire in April and September, 2007. The subjects' level of understanding of home care was measured by 35 questions divided into 8 categories while their professional competence was measured by 15 questions divided into 5 categories. Result: After attending the training, the subjects' understanding improved from 20.90 points (possible range: 4~32) to 26.11 points. The most improvement was evident in the Planning and Public Health Education categories. Their professional competences improved from 10.81 points (possible range: 4~16) to 12.51 points. The improvement of their understanding and professional competences differed across to training places. It was also evident that an increase in understanding brought about an increase in professional competence. Conclusion: The Field Management Training Program needs to be continued with efforts to reduce the differences of training effects between training places. And additional recommendations should be made through further evaluation of subsequent training programs.

The Correlation between Depression and Physical Health in the Elderly (노인의 신체적 건강과 우울과의 관계)

  • The purpose of this study was to identify the relationship between depression and physical health of the elderly and to provide fundamental data for programs which improve the health of this population. The subjects were 168 elderly people(55 years and older) who resided at home in Taegu. They were surveyed by interview using a closed- ended questionnaire. The survey was done from September 16 to October 16 in 2000. The instruments used in this study were general characteristics, Short form Geriatric Depression Scale(SGDS), Barthel Index, Muscular skeletal symptoms scale, Northern Illinois University's Health Self Rating Scale. The data were analyzed by using descriptive statistics, t-test, ANOVA, Pearson Correlation Coefficient, multiple regression with SPSS PC 10.0 version for Windows. The findings were as follows: 1. As compared 65-74 years elderly group, 75-84 years group was significantly higher score for depression(F=3.17, p=.026). As compared elderly group who has own spouse, the group who has no own spouse was significantly higher score for depression(t=- 2.44, p=.016). 2. The aged who have more limitation of Activities of Daily Living(ADL)(t=3.93, p=.000), pain of muscular skeletal symptoms(F=5.33, p=.002) and poor perceived health state(F=17.04, p=.000) showed the higher severity of depression than the aged who have not. 3. ADL correlated negatively with depression(r=- .293, p=.000), pain of muscular skeletal symptoms correlated positively(r=.251, p=.001), perceived health status correlated negatively(r=-.522, p=.000). 4. The combination of perceived health status and ADL explained 29.1% of the varience of depression. On the basis of the above findings the following recommendations are made; 1. Developing health programs is needed considering ADL, pain of muscular skeletal symptoms, perceived health status, demographic variables (age, spouse status) which have an significant effects on depression of the elderly. 2. In the following study, the use of the various scale is needed which reflects physical status of the elderly in home.

The Nature of Korean Elder의s Reminiscence : A Phenomenological Approach (한국노인의 회상(回想)의 본질에 관한 연구)

  • The phrases such as elders' powerless soliloquy or stammering and their positions unadjusted to the real world are, so far, one of negative views on elder's reminiscences. In other words, it means that elders can get psychological consolation from the getting themselves absorbed in their past. The starting point of this study is, however, that elders' reminiscence are their own mode of the existence. This dissertation is studied through Van Manen's phenomenological research method. The aim of this study is to examine the nature of Korean elders' reminiscences by both observing and describing the reminiscing content in everyday life of elders. The research participants consist of the ten elders who are community elders, hospitalized elders and nursing home residents. The period of the research is about a year from Jan., 1996 to Feb., 1997. The results of this study are as follows : The natures of elders' reminiscences are the regrecting on their life, mitigating the regrects, the confirmation of the powers and the confirmation of their life's mark. The regrecting on their life is the starting point of elders's reminiscences. Elders have tried to examine their life through reflection on their life that they did not live good life as son and daughter for parents and as parents for children and that they have foolishly lived, on dream that they did not come true, on their learning that they did not unsatisfied. But elders mitigate the regrects. They have felt their limitations of power in the conflict of human relation, the economic matter and the difficult predicament, and have finally accepted their life as it is by conquesting from their tenacity. It is dynamically found that both the regrecting on their life and mitigating of the regrection. And they pursuit the powers. It means the pursuit of the vital powers, the vigorous powers and the competency. Elders have pursued their vitality, vigor and competency through their reminiscences and have wanted to be recognised by others as a powerful being. These have dynamic and compounded aspects. The confirmation of their life's mark refer to through their own situation of health, condition, children, neighbourhood and doing their duty as a human. Elders have confirmed the present value of their being through the reflecting the present, past self, other men around themselves, children, neighborhood and the doing their human duty as a good men. Therefore the results of this study can offer new view on the elders' reminiscences that we have to understand them as a process as what they are, escaping from the simple logic that elders' reminiscence have a positive or a negative effects. Also, this study which have examined the natures of Korean elders' reminiscences can cast a new light on elders' nursing proper for Korean culture.

Development and evaluation of physical activity promotion program in the resident elderly (재가노인을 위한 신체활동증진 프로그램의 개발 및 효과)

  • The purpose of this study was to identify the effects of a visiting exercise program for physical activity promotion on physical (grip strength, static balance) and psychological (depression, perceived health status) function of frail elderly patients receiving home visiting service care. Subjects included a total of 49 patients; 24 in the control group and 25 in the experimental group. The visiting exercise program was carried out 16 times for 8 weeks. The collected data were analyzed by ${\chi}^2-test$ and t-test using SPSS/WIN 21.0 program. There was a significant improvement in the left grip strength (t=2.19, p<.000), right grip strength (t=5.07, p=.032), static balance (t=3.98, p=.050), and depression (t=-8.83, p=.009) in the experimental group compared with the control group. The visiting exercise program for physical activity promotion was effective on physical and psychological functions for frail elderly patients receiving home visiting service care. Further research is needed in a longitudinal study incorporating various centers to examine the maintenance of such effects.
