A case of anemia caused by combined vitamin B12 and iron deficiency manifesting as short stature and delayed puberty

  • Anemia caused by vitamin B12 deficiency resulting from inadequate dietary intake is rare in children in the modern era because of improvements in nutritional status. However, such anemia can be caused by decreased ingestion or impaired absorption and/or utilization of vitamin B12. We report the case of an 18-year-old man with short stature, prepubertal sexual maturation, exertional dyspnea, and severe anemia with a hemoglobin level of 3.3 g/dL. He had a history of small bowel resection from 50 cm below the Treitz ligament to 5 cm above the ileocecal valve necessitated by midgut volvulus in the neonatal period. Laboratory tests showed deficiencies of both vitamin B12 and iron. A bone marrow examination revealed dyserythropoiesis and low levels of hemosiderin particles, and a cytogenetic study disclosed a normal karyotype. After treatment with parenteral vitamin B12 and elemental iron, both anemia and growth showed gradual improvement. This is a rare case that presented with short stature and delayed puberty caused by nutritional deficiency anemia in Korea.

열가소성 폴리머 필름의 마이크로 점착 거동에 대한 온도의 영향 (Effect of Temperature on the Micro-scale Adhesion Behavior of Thermoplastic Polymer Film)

  • Adhesion tests were carried out in order to investigate the effect of temperature on the adhesion behavior between a PMMA film and a fused silica lens in the micro scale. For the tests, a microtribometer system was specially designed and constructed. The pull-off forces on the PMMA film were measured under atmospheric condition as the temperature of the PMMA film was increased from 300 K to 443 K and decreased to 300 K. The contact area between the PMMA film and the lens was observed during the test. The adhesion behavior was changed with the change of the PMMA surface state as the temperature increased. In glassy state below 363 K, the pull-off force did not change with the increase of temperature. In rubbery state from 383 K to 413 K, the pull-off force increased greatly as the temperature increased. In addition, the area of contact was enlarged. In viscous state above 423 K, the fingering instability was observed in the area of contact when the PMMA film contacted with the lens. It was also found that the adhesion behavior can be varied with the thermal history of the PMMA film. The residual solvent in the PMMA film could emerge to the PMMA surface due to the heating and reduced the pull-off force.

Foraminal Synovial Cyst Associated with Ankylosing Spondylitis

  • Ankylosing spondylitis (AS) is frequently associated with inflammatory lesions of the spine and continuous fatigue stress fractures; however, an association with an intraspinal synovial cyst has not been previously reported. A 55-year-old man with a five year history of AS who presented with back pain and a right radiculopathy was admitted to the hospital. Five years previously, he underwent a percutaneous vertebroplasty for an osteoporotic L1 compression fracture, and was diagnosed with AS at that time. Plain radiographs showed aggravated kyphosis and a stress fracture through the ossified posterior element, below the prior vertebroplasty. Magnetic resonance images revealed a right foraminal cystic lesion at the L2-L3 level with effacement of the nerve root. A 1.6 cm cystic lesion that appeared to arise from the L2-L3 facet joint without direct communication was excised from the L2-L3 foramen. Pathological examination confirmed synovial cyst. The patient's symptoms resolved immediately after surgery except for a mild dysesthesia of the right leg. We report herein a rare case of foraminal synovial cyst associated with AS accompanying posterior element fracture with a review of literature.

노인 운동가에 있어 장기적인 운동에 따른 척추, 힙과 신체 총 골밀도 감소에 관한 재현성 (Reproducibility of Measures of Lumbar Spine, Hip and Whole Body BMD according to Longitudinal Physical Activity in Older Athletes)

  • With the aging of society a great deal of interest is being placed on the value of longitudinal data in evaluating physiological losses. We present data on test-one/test-two reliability and reproducibility for measures of training, bone density from a longitudinal study of master athletes. Fifty-two males (mean age at test $1=58.2{\pm}9.8\;years$) and thirty-two females ($54.4{\pm}8.8\;years$) were selected from the study population. Bone mineral density was determined using DEXA (Hologic 1500). The characteristics of the subjects are presented below as $means\;{\pm}\;S.D$. The data was imported into the Statistical Package for the Social Science (SPSS 9.0, Chicago, IL). Paired t-tests were performed between visit 1 and visit 2 in subjects. Pearson correlations were performed. The results of this study indicate the measures of training history, body mass and bone density are reasonably stable and reproducible. We conclude that body composition and bone density parameters are stable and reproducible over time in active older subjects. Physiologic measures in master athletes are fairly stable, and reproducible over time Longitudinal studies investigating age-related changes in master athletes need to be conducted on a time schedule of greater than two years.

Pudendal nerve entrapment syndrome caused by ganglion cysts along the pudendal nerve

  • Pudendal nerve entrapment (PNE) syndrome refers to the condition in which the pudendal nerve is entrapped or compressed. Reported cases of PNE associated with ganglion cysts are rare. Deep gluteal syndrome (DGS) is defined as compression of the sciatic or pudendal nerve due to a non-discogenic pelvic lesion. We report a case of PNE caused by compression from ganglion cysts and treated with steroid injection; we discuss this case in the context of DGS. A 77-year-old woman presented with a 3-month history of tingling and burning sensations in the left buttock and perineal area. Ultrasonography showed ganglion cystic lesions at the subgluteal space. Magnetic resonance imaging revealed cystic lesions along the pudendal nerve from below the piriformis to the Alcock's canal and a full-thickness tear of the proximal hamstring tendon. Aspiration of the cysts did not yield any material. We then injected steroid into the cysts, which resolved her symptoms. Steroid injection into a ganglion cyst should be considered as a treatment option for PNE caused by ganglion cysts.

온실을 중심으로 토양 성분에 따른 세이보리의 생육현황에 대하여 (A Study on the Growth of Savory According to the Composition of the Soil Inside a Green House)

  • Herb has been categorized as a special plants from the beginning of human history and used in different medical systems in different cultures. This research has classified soil into 6 kinds that have diverse elements to see to which various kinds of savory(satureja hortensis) adapt well, experimenting from sowing to flowering for around 13 weeks, and also divided indoor conditions to get the result below. In conclusion, growth status of savory depending on the kinds of soil suggest that in indoor conditions the savory if planted in ⑤ bed soil compounded with saprolite and poultry manure grew better than any other condition. On the other hand, the growth status was bad in ① masato, ② clay, ④ bed soil mixed with saprolite, and ⑥ bed soil mixed with clay conditions. Though you can see the immediate effect of soil on the growth of savory, I'd like to reveal the details of how elements of savory operate in which kind of soil and outdoor conditions the goal of this research, in the next research.

  • We had been investigated the effect of ear acupuncture in alcoholics, but in Kang won do province never we have been investigated. so then in Sangji university oriental hospital from Mar. 1. 1995 to Feb. 28. 1996. we clinically analized 102 alcoholics treated The results were summarized as follow: 1. In Age distribution, 40s were the top as 28.4% and next 30s, 50s, 60s 70s, below 3Os. In Sex distribution, these were 92 person in male, 10 person in female. 2. Marrige Status revealed in descending order ; married, single, divorced Sibling order distribution in descending order; eldest son, middle son, youngest son, only son 3. Distribution of education in descending order ; high school, middle school, primary school, collage and graduate school, illiterate 4. Distribution of occupation in descending order; farmer, labor, merchent, salaried and inoccupation, service 5. Distribution of religion in descending order ; none, buddism, protestantism, catholicism 6. Distribution of family history of alcoholism; yes(24.5%), no(69.6%) 7. Distribution of onset of drinking in descending order ; 20s, below 20s, 30s and 40s, 50s 8. Distribution of duration of drinking in descending order ; 20-29 years, 10-19 and below years, 40-49 years, 30-39 years 9. Distribution of the reason of drinking in descending order ; habbit, business and reduce of stress, no reason, syndrome of stop drinking 10. Distribution of frequency of drinking in descending order ; daily, four or five times a week, irregularly, once a week, two or three times a week 11. Distribution of amount of drinking in descending order ; two bottles, one bottle, three bottles, half, bottle above four bottles 12. Distribution of Chief Complaint of Alcohol in descending order ; no appetite and anorexia, diarrhrea insomnia, fatigue, vomitting, tremor, drinking water, hallucination, abdominal pain, constipation 13. Distribution of total MAST score in descending order ; 26-48 score, 13-18 score, 19-25 score, 8-12 score, 0-7 score 14. In the treatment effects according to MAST score, these were complete stop drinking 18.0%, improved 53.0%, unchanged 29.0%. 15. Distribution of liver function test and treatment effects in descending order ; ALAT, ASAT, GGTP, Trigliceride, Alk-phosphatase and Total bilirubin The treatment effects in ear acupuncture were 70.5% effective ones ; stop drinking 17.6%, improved 52.9%. 17. These were headache, nausea, vomitting sense, weakness in revealing symtoms after treatment, but no severe side effect. 18. Distribution of the times of onset in the change of taste in descending order; 3-5 times, below 2 times and above 6 times.

새로운 항암제 처방 감사 시스템 도입을 통한 의료의 질 향상 (Efficacy of new inspection system of Anticancer Drug Prescription)

  • Background : The number of outpatient injected anticancer drug is increasing. and the pathway of prescribing, compounding, and injecting anticancer drug is processed very rapidly in out-patient department. Moreover, Dose of anticancer drug is often changed depending on side effect of patients. So we need more effective inspection of anticancer drug prescriptions. The purpose of this study was to analyze the prescription errors for anticancer drugs in Out-Patient Department and to suggest system to prevent them. Method : The study took place at Asan Medical Center from July to September 2007. The pharmacists performed inspection of anticancer drug prescriptions before compounding and injecting. We used protocol-based anticancer drug order program and Electronic Medical Record (EMR). Result : During the study period, we analyzed 4683 prescriptions for out-patient. And we detected 55 medication errors (1.2%). Most common errors included dosage above or below the correct ones (56.3%), followed by incorrect treatment duration. Because most of dosing errors were in the range of usual dosage, it was hard to detect them. So when inspecting the prescription, we considered the medical records of individual patients. As a result, we could raise the efficiency of intervention. Therefore inspection using EMR could possibly reduce the number of anticancer drug errors. Conclusion : we are preventing the medication errors on stability and dosage above or below the maximum therapeutic dose according to the previous inspection system. However most of dosing errors were in the range of usual dosage according to the result of this study. Because of there was interpatient variability of dosage depending adverse effect. For improvement of quality assurance, we suggest inspection system based on patient's medical history.

결함 이중 Perovskite La1/3TaO3 단결정의 유전 및 전도특성 (Dielectric and conductivity properties of defect double Perovskite La1/3TaO3 single crystal)

  • A-자리 결함 이중 Perovskite La1/3TaO3 단결정 시편을 제작하여 10~800 K 온도범위에서 유전특성을 조사하였다. 500 K 이하에서는 상유전성 거동이 나타났으며, 550 K 이상에서 유전이상과 유전상수의 열이력(thermal hysterisis)가 나타났다. 교류전도도 측정에 의한 활성화 에너지는 560 K 이하 영역에서 1.83 eV로 가장 크며, 560~690 K 영역에서 0.35 eV, 690 K 이상인 고온 영역에서 0.28 eV로 나타났다 이러한 결과로부터 500 K 이하 영역에서는 La3+-이온과 공공-자리가 무질서하게 배열하여 상유전상을 유지하며, 가장 높은 활성화 에너지를 나타낸 560 K 부근 영역에서는 La3+-이온이 공공-자리로의 전도 혹은 점프에 의한 재배열에 기인하여 유전이상이 나타난 것으로 판단된다.

석탑 기단부 적심구성방법에 대한 특성 고찰 - $7{\sim}8$세기 석탑 중 해체 수리한 석탑을 중심으로 - (A study on characteristics of composition method of inner foundation in stone stupa)

  • Through analysing on construction cases of stupa built in A.D. 7,8th, I have researched about these : constructive methods of inner soil of stupa, spatial compositions, characteristics of structures, arrangements of inner soil and etc. And cases analysed are six ; Mireuksajiseoktap(stone pagoda of Mireuksa Temple site), Gameunsajisamcheumgseoktap(three storied stone pagoda of Gameunsa Temple site), Goseonsajisamcheungseoktap(three storied stone pagoda of Goseonsa Temple site), Wolseong nawolliocheungseoktap(five storied stone pagoda in Nawonri, Wolseong), Guksagokseoktap(three storied stone pagoda in Guksa valley), Giamgokseoktap(three storied stone pagoda in Giam valley). Additionally we researched about inner soil of Sacheonwangsaji tapji(basement of stone stupa site in Sacheonwang Temple site) to speculate on composition of Synthetically, the foundation could be divided as core space and outer space. ; the former as structural function and the latter as ornamental function. And the core area could be divided again as center column space and buffer space. The relationship between core spaces and its formation are as belows; First, according to the area of foundation and scale of stone pagoda, formations of core are differed. As the scale of stone pagoda goes bigger, and the area of foundation goes larger, the structure of stone pagoda comprised by center column type and layered-core which endure upper load independently. On the contrary, as the scale of stone pagoda goes smaller, and the area of foundation goes lesser, the structure of stone pagoda tend to use only center column to endure upper part. Second, spatial composition of core area is comprised as two spaces, one which endure upper load and buffer space which absorb side pressure and upper pressure. The buffer space tend to be used in case of those structures which could not endure side pressure or have lots of joint. In some cases, it was located below the cover stone of foundation and gained upper load. And in case that have not gained pressure from side stone, the buffer space are comprised by smalle stone or roof tile to get structural supplement.
