Sound-Field Speech Evoked Auditory Brainstem Response in Cochlear-Implant Recipients

  • Background and Objectives: Currently limited information is available on speech stimuli processing at the subcortical level in the recipients of cochlear implant (CI). Speech processing in the brainstem level is measured using speech-auditory brainstem response (S-ABR). The purpose of the present study was to measure the S-ABR components in the sound-field presentation in CI recipients, and compare with normal hearing (NH) children. Subjects and Methods: In this descriptive-analytical study, participants were divided in two groups: patients with CIs; and NH group. The CI group consisted of 20 prelingual hearing impairment children (mean age=8.90±0.79 years), with ipsilateral CIs (right side). The control group consisted of 20 healthy NH children, with comparable age and sex distribution. The S-ABR was evoked by the 40-ms synthesized /da/ syllable stimulus that was indicated in the sound-field presentation. Results: Sound-field S-ABR measured in the CI recipients indicated statistically significant delayed latencies, than in the NH group. In addition, these results demonstrated that the frequency following response peak amplitude was significantly higher in CI recipients, than in the NH counterparts (p<0.05). Finally, the neural phase locking were significantly lower in CI recipients (p<0.05). Conclusions: The findings of sound-field S-ABR demonstrated that CI recipients have neural encoding deficits in temporal and spectral domains at the brainstem level; therefore, the sound-field S-ABR can be considered an efficient clinical procedure to assess the speech process in CI recipients.

  • It is very difficult to reduce traffic noise level in the downtown. The best method is to improve the sound amenity of the districts, considering concept of soundscape. ?his study aims to survey the effect of introducing sound and masking to reduce the background noise level. We carried out the evaluating test for 18 introducing sounds varied S/N ratio(-5, 0, +5) in the city riverside. In these test, subjects evaluated the sound environment with five bipolar adjective scales. It was evaluated that introducing sounds improved the sound environment and that S/N + 5dB was very appropriate to mask the background noise. But the hearing level was suggested to be S/N 0 dB or less.

  • Generally, noise from a indoor air conditioner has been measured by overall sound pressure level, dB. Customers evaluation, however, of the noise from an air conditioners should not be depend on the only overall level but also the sensory feeling. Moreover, in this sensory evaluation, irregular, time-varying and uneven sounds which are difficult to describe itself by overall level are more affectable. Unfortunately however the formal evaluation method for this kinds of noise is not determined yet. In this research, the evaluation method for tonal noise, which is one of the unmeasurable sounds by overall level, is proposed to evaluate its acceptability based on the consumer's sense of hearing. To consider the consumer's sense, several times of Jury evaluation are carried out. The proposed approach could be applied to the other sound, which acoustical characteristics are similar to the tonal noise .

  • Existing loudness models are specified only to diotic sounds in spite of the fact that normal human beings hear dichotic sounds. Approximately, the arithmetic mean of loudness values of both ear signals has been suggested for the resultant perceived loudness. In this study, the dependence of overall loudness perception on the interaural level differences was investigated by the subjective tests. It was found that the larger the interaural level difference, the louder the perception than the mean of calculated loudness values at both ears and the lower the critical band rate or the reference level, the louder the perception than the mean value. A modified loudness model was proposed to he applicable to dichotic sounds by using the equivalent diotic levels.

  • The noise inside a naval vessel is very important in considering the need for hearing protection, improving the working environment and maintaining good communications for crews living on board a naval vessel. The indoor noise of a ship usually is specified by the A-weighted sound pressure level, but other evaluating parameters are required to reflect human senses more effectively. This paper uses additional noise indices related to room acoustics, such as NR(noise rating), NC(noise criterion), RC(room criterion), PSIL(preferred speech interference level) and loudness level to evaluate the noise inside cabins on a naval vessel. Using these psychological noise indices, allowable limit of noise level in cabins is suggested through psycho-acoustic evaluation for the noise in cabins.

  • This research is aimed at cutting off hearing loss and other harmful factors due to noise and providing basic material for noise reduction plan. As the research method, this research assessed noise by measuring acoustic pressure level and frequency in various situation of non-treatment and treatment. As the measurement result, average noise degree of high speed handpiece of non-treatment, ultrasonic waves scaler, and low speed handpiece showed 58~66 dB(A). Average noise degree of scaling of treatment, tooth elimination, and denture adjust showed 73~81 dB(A). The result is inferior to recognized standards of noise induced hearing loss. But the result of assessing this with (noise rating) NR curve was NR-73~78, which exceeded general workplace noise standard. This level can cause hearing loss when exposed to a long time. Therefore, treatment office noise during dental treatment can cause psychological and physical damage in dental clinic employees, and it is urgently required to establish systematic and active noise reduction plan.

PARENTAL PERCEPTIONS OF CHILD ADJUSTMENT ASSOCIATED WITH PARENTING STRESS WITH AUTISTIC CHILDREN (자폐아동부모의 양육스트레스와 아동의 적응능력지각과의 관계)

  • This study were purposed to find out parental perceptions of child adjustment associated with parenting stress with autistic children and differences of parenting stress between autistic children's parents and normal children's. Subjects were 27 autistic children's parents, 28 normal children's, 4 teachers with autistic children. 'Scales were Parenting Stress Index(PSI), Eyberg Child Behavior Index(ECBI), Childhood Autism Rating Scale(CARS), Social Maturing Scale(SMS). The results of this study were as follows. 1) In total parenting stress, parents with autistic children had significantly higher scores than normal's. 2) In child domain of parenting stress, fathers with autistic children had significantly higher than mothers on the other components except Mood and Demandingness. 3) In parent domain of parenting stress, parents with autistic children had significantly higher than nomal's on Restrict of role, Attachment, Sense of Competence. All mothers of normal and autistic children had significantly higher than fathers on the other components except Repression. 4) In parenting perceptions associated with parenting stress level of autistic children's parents. Low level group perceived their children as higher adaptibility than teacher's evaluation and high level group perceived them as equal to teacher's.

Implementation of Wireless Charger with the Function of Auto-Shutdown for fully Implantable Middle Ear Hearing Devices (완전 이식형 인공중이를 위한 자동 충전종료형 무선 충전장치의 구현)

  • In the paper, a wireless charger with the function of auto-shutdown for fully implantale middle ear hearing devices (F-IMEHD) has been designed. The wireless charger can communicate with an implant module to be turned off automatically shutdown after an internal rechargeable battery has been fully-charged by electromagnetic coupling using two coils. For the communication with an implant module, the wireless charger uses the load shift keying (LSK) method. But, the variation of the mutual inductance due to the different distance between two coils can cause the communication error in receiving the fully-charged signal from an implant module. To solve the problem, the implemented wireless charger has a variable reference generator for LSK communication. The wireless charger generates proper level of the reference voltage for a comparator using an ADC (analog-to-digital converter) and a DAC (digital-to-analog converter). Through the result of experiment, it has been confirmed that the presented wireless charger can detect signals from implantable module. And wireless charger can stop generating electromagnetic flux after an implanted battery has been fully charged in spite of variable coil distance according to different skin thickness.

The Effects of Education on the Prevention of Noise-Induced Hearing Loss in Adolescence (청소년 소음성 난청 예방교육의 효과)

  • Purpose: The purpose of this study was to investigate the effects of health education based on the health belief model (HBM) to the prevention of noise-induced hearing loss (NIHL) in middle school students. Methods: A pretest-posttest nonequivalent control group was designed for the purpose of this study. A total of 212 students (134 in the experimental group and 78 in the control group) of two middle schools in Seoul were enrolled for the study. Health education of two 45-minutes sessions over two weeks were provided. The data were analyzed by descriptive statistics, Chi-square test, t-test, ANCOVA and repeated measures ANOVA with Bonferroni post-hoc test, using the SPSS version 21.0. Results: There were significant differences between experimental group and control group with reference to perceived susceptibility (F=7.862, p=.006), perceived severity (F=8.291, p=.004), perceived benefits (F=20.311, p<.001), and perceived barriers (F=5.628, p=.019) after health education were provided. We also observed sustained health education effects of perceived susceptibility, perceived severity, perceived benefits, and perceived barriers in the experimental group for up to 4 weeks after health education were provided. Conclusion: The health education to prevent NIHL based on the HBM improved the health beliefs of the middle school students. Health education on the HBM for the prevention of NIHL should be provided at the level of middle school.

Development of Korean Consonant Perception Test (자음지각검사 (KCPT)의 개발)

  • The purpose of this study was to develop Korean Consonant Perception Test (KCPT), that is a phonemic level including elementary data to evaluate speech and consonant perception ability of the normal and the hearing impaired qualitatively and quantitatively. KCPT was completed with meaningful monosyllabic words out of possible all Korean monosyllabic words, considering articulation characteristics, the degree of difficulty, and the frequency of the phonemic appearance, after assembling a tentative initial and final consonants testing items using four multiple-choice method which were applied to the seven final consonant regulation and controlled with the familiarity of the target words. Conclusively, the final three hundred items were developed including two- and one-hundred items for initial and final testing items, respectively, with the evaluation of the 20 normal hearing adults. Through this process, the final KCPT was composed upon the colloquial frequency following identification of no speakers' variances statistically and elimination of the highly difficult items. The 30 hearing impaired were tested with KCPT and found that the half lists, A and B, were not different statistically and the initial and final testing items were appropriate for evaluating initial and final consonants, respectively.
