A Study on Abdomen Ultrasonography Classified by Particular Disease Practiced in Health Promotion Center of a University Hospital (한 대학병원 종합건강진단센터에서 시행한 복부 초음파검사의 유소견 연구)

  • This study is to get preliminary data for an effectiveness evaluation of abdominal examination and improvement of it. Abnormal cases of abdominal ultrasonography are classified by sex, frequency, diagnosis and age. 4,924 examinees were included at a university hospital of health promotion center from January to December in 1999. The results are as follows. 1. According to the distribution of sex, there are more male patients(55.0%) than females patients(48.0%). For men, 40's showed the highest percentage among examinees. For women, 50's were the highest. 2. The reason that they visited the health promotion center was that 'they wanted to check their health status'. This answers were reported the highest(59.3%). 3. Patients that had abnormal cases of abdominal ultrasonography were 48.3%. Liver, kidney, gallbladder showed the highest percentage of abnormal cases in order of organs. Additionally, abnormal cases were discovered in liver cases. 4. According to the frequency of abnormal cases among examinees, the slight fatty liver were the highest regardless of sex. Men had the slight fatty liver, kidney simple cyst, liver calcification and liver simple cyst in order of abnormal cases. Women showed the slight fatty liver kidney simple cyst, kidney calcification, liver simple cyst, and blood vessel tumor in order of abnormal cases. 5. For the abnormal cases of liver by sex and age, the 50's reported the highest number of abnormal cases in men(299 patients). In addition, 60's had the highest of disease rata 47.8%. For women, 50's reported the highest number of abnormal cases(361 patients). Over 70's patients had the highest of disease rata 52.6%. For kidney, men and women showed the highest number of abnormal cases -62 vs 44 respectively. Over 70's patients had the highest percentage of disease rata-23.2% vs 14.0% respectively. For gallbladder, the number of abnormal cases were the most in men's 60's (31 patients) and in women's in the same age group (32patients). Disease showed the highest percentage in men's 60's(7.6%) and in women's 70's (14.0%). 6. According to malignant tumor, 17patients were liver cancer, 2patients stomach ca and 1pt kidney cancer. 7. The relationship between the malignant tumor and the examination motive was that 'they wanted to check their health status(41.0%)' and 'regular checkup (24.0%)'.

Health Problem of the Middle-Aged Women (종합 건강검진 과정에서 나타난 일부 중년기 여성의 건강문제)

  • This study was carried out to offer the basic data for more qualitative lives of the middle-aged women through their health maintenance and control. So, the researcher reviewed and analyzed the health problems of the middle-aged women, the differences of main symptoms in each subject, positive ratcs in screening tests, the difference of screening tests with age and the correlation of obesity and blood pressure with screening tests. All subjects were 218 women(40-59years) who had a check-up at a comprehensive health check-up center in K University Hospital in Seoul from July 1, 1998 to August 31. 1998. Questionaires were developed to get subjects, general characteristics. main symptoms, medical examination and diagnosed disease by the researcher referring through the questionaires of comprehensive health check-up center and the medical recorders were used as a tool of study. The researcher analyzed the data with SPSS PC+ ; the distribution of the subjects, general characteristics and main symptoms with percentage : the difference between main symptoms and screening tests with X$^2$-test, ANOVA. The results are as follows. 1) Among the age distribution, most were 40-45 years old and unemployed. 2) The most common complaints were indigestion in digestive system, cough in respiratory system, heart beat in cardiovascular system, general sweat in endocrine system. edema in urinary system, easy bruise in hematologic system, backache in musculoskeletal system, headache in psychoneurologic system, lochia in gynecology, itch in dermatology system, eye ball pain in visual system and tinnitus in auditory system. 3) In main symptoms according to age, the 40-45 age group had a higher rate of complaints in digestive system and gynecology than other age group ; the 46-49 age group in endocrine system. 4) The main symptoms according to diagnosis had no difference statistically. 5) The screening tests which showed high rates of abnormality were mammography(the highest), abdominal ultrasonography, upper gastrointestinal series/gastrofiberscopy, PFT. In all age group the highest rate of abnormality was seen in mammography. 6) In screening tests according to age. the 46-49 age group showed higher positive rates of pap smear, blood sugar test, urine test than any other age group ; the 50-55 age group showerd higher positive rates of obesity, BP, chest X-ray, abdominal ultrasonography, ESR, SGOT, CRP and urine micro. 7) In correlation of obesity with screening test, the obesity group showed higher positive rates of blood pressure, chest X-ray, abdominal ultrasonography, HCT and CRP than the normal weight group. 8) In correlation of blood pressure with screening test, the hypertension group showed higher positive rate of obesity, chest X-ray, abdominal ultrasonography, pap smear and blood sugar than the normal blood pressure group. In conculusion, the middle-aged women have various physical symptoms and affected by age. The obesity and BP have an important effect on the health of the middle aged women. Therefore, this study is considered significant as data for qualitative lives of the middle-aged women playing an important part of family health care by catching of their health problem complaints for prevention of disease and promotion of health.

SATISFACTION OF HANDICAPPED PATIENT TREATED UNDER OUTPATIENT ANESTHESIA (외래 전신마취를 이용한 치과치료에 대한 만족도 조사 -장애인환자 보호자를 대상으로-)

  • 90 handicapped patients with dental problem treated under outpatient general anesthesia at one day operation room of kyungpook National University Hospital during 2000-2005 were analyzed. Following are the conclusions. 1. For those 90patients treated under general anesthesia, 62(68.9%)patients had autism or mental retardation. 2. The most difficult work for the preparation of general anesthesia was preoperative examination(53.3%). 3. The parent's satisfaction level after treatment under general anesthesia was high(75.6%). 4. 36(40%) of the patients responded to periodic recall check till now.

An Association between Factor of Metabolic Syndrome and Serum Levels of Gamma-glutamyltranspeptidse at Age 40 Years

  • We have evaluated an usefulness and association between serum levels of gamma-glutamyltranspeptiase (GGT) and metabolic syndrome (MS) in 40-year-old men and women. The prevalence of MS in men (11.8%) was higher than that of women (4.0%). Serum levels of GGT were higher in men and women with MS than in them without MS. In the prevalence of higher serum GGT, men group with MS (35.8%) was greater than women group with MS (22.7%). In the relative risk of MS to an increased level of GGT, women group was higher than men group (about 3 times) (female, OR=7.48 vs male, OR=2.46). The present study reveals that routine check of serum GGT in health examination can be useful for the prevention and management of MS including cardiovascular and/or liver diseases.

Prevalence of Metabolic Syndrome and Its Predicting Factors among Small-sized Company Workers

  • Purpose: This study was aimed to examine the prevalence of metabolic syndrome (MetS) and cardiovascular risk factors among workers at small-sized companies having fewer than 50employees in Korea. Methods: A descriptive cross-sectional study was conducted using a secondary data analysis on workers' health examination data. Data from 21,784 employed workers were analyzed, including 16,488 males (mean age 38.7years)and 5,296 females (mean age 40.5 years). Participants were newly enrolled in annual health check-ups at a professional health clinic from 2009 to 2011. Logistic regression analysis was performed to identify age-adjusted gender specific predicting factors of MetS incidence. Results: The prevalence of MetS was 13.4% in male and 7.7% in female workers. Logistic regression analysis showed that, when age was adjusted for, family history of cardiovascular disease (CVD) was a predicting factor in both male and female workers. For male workers, heavy alcohol drinking and non-manual occupation (office workers/drivers) predicted MetS. Conclusion: Health care providers should screen for MetS periodically in small work places especially for those with a family history of CVD. Educational counseling needs to be given to male workers with sedentary occupations with an aim to reduce heavy drinking and encourage lifestyle modification.

The Awareness Survey of Vision Correction Surgery Side Effect (시력교정술 부작용 인지도 조사)

  • Purpose. To investigate awareness of vision correction surgery side effects and investigated side effects affect vision correction surgery decisions. Methods. This study was performed on 85 college students. The awareness of vision correction surgery, the awareness of vision correction surgery side effects, and investigated side effects affect surgery decisions. Results. 37.6 % (32 people) replied that the drying effects is representative side effects of the eye correction surgery. 68.2% (58 people) answered the reason of eye surgery is uncomfortable wearing glasses. 74.1%(63 people) answered the side effects does not affect vision correction surgery decision. Prior to hear a description of side effects, 31.8% percent people hope the surgery. after listening to explain of side effects, only 10.6% percent people hope the surgery. Conclusions. People who want to get vision correction surgery should check their vision condition through prior examination and consultation. What side effects in the operation should be reviewed. That is the only way to reduce side effects.

A Study on the Compliance of the Occupational Safety and Health Act by Busan and Gyungsangnam-do Province Working Environment Measurement Institutions (부산·경남지역 작업환경측정기관의 산업안전보건법 준수 실태 및 준수율 제고를 통한 측정기관 종사자 건강보호 방안 고찰)

  • Objectives: This study aims to investigate whether working environment measurement institutions(WEMIs) are conducting safety and health education, risk assessment, and oversight of special management materials, and whether working environment measurement and special medical examinations are being conducted as prescribed in the Occupational Safety and Health Act(OSHA). Methods: In of February 2021, a questionnaire was prepared and distributed to 33 WMEIs registered with the Ministry of Employment and Labor(MoEL) in Busan and Gyeongsangnam-do Province. The responses were collected and then analyzed. Results: The findings show that 5 WEMIs(15%) complied fully with OSHA. Risk assessment was conducted by 13 WMEIs(39%) and safety education by 11 WMEIs(33%). Eighteen WMEIs(55%) conducted working environment measurement, and 29 WMEIs(88%) conducted special medical examinations. The implementation rate of the risk assessment in the health industry(85%) was higher than the one in the special technology industry(11%)(p<0.05). The implementation rate of the special medical examination in the examiners(54%) was not as high as the one in the analysts(91%)(p<0.05). Conclusions: The MoEL needs to check whether basic OSHA requirements are being observed during regular inspections by WEMIs. These findings indicate that it is necessary to prepare a plan to improve the rate of compliance with OSHA regulations.

Correlation of Coffee Consumption, Lifestyle, and Nutrient Density (커피 소비와 생활습관 및 영양밀도의 관련성)

  • This study evaluated the nutrient intake with increasing coffee consumption for 403 adults aged over 30 years in Korea. The 403 subjects were national health screening examinees, who visited Sahmyook Seoul Hospital's Comprehensive Check-up Center between 2017.11.01 and 2018.12.18. The subjects were asked to answer questionnaires covering a 24-hour recall fluid and dietary intake before the health examination. The research ethics council of Sahmyook University (2-7001793-AB-N-012019036HR) approved this study. Coffee consumption exceeding two servings daily was more likely in males, in those aged between 30 and 40 years, and in the smoking, drinking, non-exercise, non-breakfast groups compared to each counterpart. The correlation between the coffee consumption frequency and current nutrient density showed negative correlations in most micronutrients. The mean nutrient density decreased gradually with increasing coffee consumption (<1 serving daily, ${\leq}1{\sim}2$ servings daily, >2 servings daily) in the ANOVA analysis. Therefore, the progressive adverse health effects of excessive coffee consumption needs to be researched further, and a daily total caffeine limit should be suggested in education of the nation levels.

A Study of Musculoskeletal Disorders at a Subway Train Repair Plant in Korea (국내 지하철 정비 사업장의 근골격계질환 실태조사에 관한 연구)

  • A research project was conducted to study work-related musculoskeletal disorders (MSDs) at subway train repair plant in Korea. The project was consisted of 4 main parts; education on the topics of MSDs for all workers, symptom survey, medical check-up, investigation of MSD risk factors. The result of symptom survey showed that 95.2% of the respondents complained pains on at least one part of the body. After cross-sectional analysis of various information and risk factors, 86.8% of respondents were considered as active health surveillance level 1 that require continuous monitoring on their symptoms and working conditions. And 57.1% of the respondents were considered as active health surveillance level 2 that needed medical check-ups for proper medical treatment. The analysis of occupational risk factors revealed that handling of heavy object (46.15%) and repeated awkward postures (46.15%) were two most contributing risk factors for the on-set of MSD at this work site followed by static strain (7.7%), and vibration and impact (3.8%). Medical examination was performed by an industrial medicine MD on 156 workers those considered as active health surveillance level 2. The result showed that 35 workers (20.7%) were considered as MSD patient group at severe level, and 68.6 workers (68.6%) were considered as moderate group and 18 workers (10.6%) were considered minor or normal group those have no symptom.

Audiometric Calibration of Aural Acoustic Immittance Instrument: A Review of Acoustic Immittance Instrument's Calibration

  • Audiometric calibration is a prerequisite for securing the reliability of audiometric test results by checking the internal consistency of the relevant instrument. The purpose of this review is to help instrument operators understand the calibration procedure of aural acoustic immittance instrument which is frequently used for objective assessment. By referring to the latest international standards and the national standards relevant to the aural acoustic immittance instrument, the following parameters will be reviewed: 1) introduction of performance characteristics, 2) detailed procedure of the calibration method. According to the newest international and national standards [IEC 60645-5 (2004), ANSI S3.39-1987 (R2012)], the aural acoustic immittance instrument basically includes six components: 1) calibration cavity, 2) acoustic immittance analysis system, 3) probe assembly/unit and signal, 4) pneumatic air-pressure pump system, 5) acoustic reflex activator system and 6) tympanogram and acoustic reflex plotting system, each of these components should meet set standards. The result of behavioral hearing tests is influenced by various complex factors including the examinee's cooperation, background noise of the examination room, measurement method, skill level of the audiologist and calibration status, but the objective hearing tests is more influenced by the calibration status of the instrument than any other factors. The audiologist should take full responsibility for the reliability of the hearing test result, so he/she should carry out the calibration check and adjustments of aural acoustic immittance instrument periodically and maintain the instrument continuously by referring to the newest standards and the manufacturer's instruction manual.
