Korean Gastric Cancer Association Nationwide Survey on Gastric Cancer in 2014

  • Purpose: The Korean Gastric Cancer Association (KGCA) has conducted nationwide surveys every 5 years, targeting patients who received surgical treatment for gastric cancer. We report the results of the 2014 nationwide survey and compare them to those of the 1995, 1999, 2004, and 2009 surveys. Materials and Methods: From March 2015 to January 2016, a standardized case report form was sent to every member of the KGCA via e-mail. The survey consisted of 29 questions, regarding patient demographics as well as tumor-, and surgery-related factors. The completed data forms were analyzed by the KGCA information committee. Results: Data on 15,613 patients were collected from 69 institutions. The mean age was $60.9{\pm}12.1$ years, and the proportion of patients more than 70 years of age increased from 9.1% in 1995 to 25.3% in 2014. Proximal cancer incidence steadily increased from 11.2% in 1995 to 16.0% in 2014. Early gastric cancer incidence consistently increased and accounted for 61.0% of all cases in 2014. The surgical approach was diversified in 2014, and 7,818 cases (50.1%) were treated with a minimally invasive approach. The most common anastomosis was Billroth I (50.2%) after distal gastrectomy, and the proportion of Roux-en-Y anastomoses performed increased to 8.6%. Conclusions: The results of this survey are expected to be important data for future studies and to be useful for generating a national cancer control program.

Study of the Current State and Future Directions of Medical Ethics Education on the Colleges of Korean Medicine in Korea (전국 한의과 대학의 의료윤리교육 현황 및 발전방향)

  • In this research, we examined the courses of the colleges of Korean medicine in Korea to find out how well the education on medical ethics is being conducted so that we could make good use of our findings as basic material to make arrangements to develop ethics education in Korean medicine. In the same research, we conducted a survey to find detailed information on the current state of ethics education and its actual application at 12 colleges of Korean medicine in Korea. As a result, 8 colleges out of 12 included medical ethics in their courses. They were among the subjects in the first year or second year of a preparatory program. Most of them were classified as requirements while some of them were classified as electives in humanities or liberal arts. As ethical issues in the medical community become more diversified and important, it is imperative that health care providers maintain firm ethics. As the need for medical ethics and ethics education is getting larger, ethics education at colleges of oriental medicine is becoming a more important focus. Therefore, it is necessary to expand ethics education at the colleges of Korean medicine in Korea and set a precedent for studying ethics in Korean medicine based on that.

  • Number of fixtures supporting prosthesis for rehabilitation of partial edentulism in distal area is an important factor in distal area to the bone tissue response around dental implant. Optimal number and optimal positioning of dental implant has leaded to the stable condition of bone tissue and successful long-term treatment outcome. This clinical and radiographic study was performed to document and evaluate the short-term result of occlusal rehabilitation by means of implant-supported fixed prostheses (ISPs) especially for partial edentulism in distal area in patients treated for advanced periodontal disease and to verify the number of fixture affecting the bone tissue response. A total of 30 consecutive patients referred because of advanced periodontal disease were included. Before the implant therapy was initiated, periodontal treatment was performed and the outcome evaluated during at least a 6-month period. An individual maintenance care program was designed for each patient. All 75 implants were placed using a 2-stage surgical approach. The patients were divided into 2 groups, in one of which two fixtures were placed and in the other of which three fixtures were placed with tripodal geometry. Following installation of the ISPs, all patients underwent a baseline examination including evaluation of i) oral hygiene, and ii) periodontal/ peri-implant conditions, and iii) radiographs. These examinations were repeated annually during the 1 or 2-year observation period. The results were as follows: 1.No single implant was lost during the observation follow-up period. 1.The percentage of plaque harboring surfaces and bleeding units upon probing were found to be low (<10%), and no soft tissue complications were recorded. 1.Two-fixture group showed bone destruction ranged from 0.0mm to 1.5mm and the mean was 0.31mm. Three-fixture group showed more bone destruction of 0.51mm. There was no statistically difference between two groups. These results suggested that the factor for success is not the number of fixture but the strict maintenance of peri-implant tissue health and initial stability of fixture.

The Effects of Aroma Inhalation on Sleep and Fatigue in Night Shift Nurses (아로마 흡입이 밤번근무 간호사의 수면과 피로에 미치는 효과)

  • Purpose: This study was conducted to describe the effects of aroma inhalation on sleep and fatigue in night shift nurses. Methods: The research design was one group pre-posttest design. The subjects were 17 night shift nurses who work at neurosurgery intensive care unit of K hospital of C university in Seoul. During three consequent night shifts, the participants were worn the Actigraph to measure the sleep pattern on their wrist since their first night shift work. The quality of sleep and fatigue was measured by survey when they awoke at 6 pm after second night shift work. And these measures were repeated during the night shifts in second and third week. Aroma inhalation was applied before going to bed after finishing each night shift work during second and third week. SPSS 12.0 program was used for data analysis. Results: There were no statistically significant differences in sleep latency and total sleeping time. However, significant differences were found in the sleep efficiency, awakening number, sleep quality, subjective symptom and intensity of fatigue. Conclusion: These results showed that aroma inhalation had positive effects on the pattern and quality of sleep and reducing fatigue in night shift nurses.

Prevalence of Types of Cancers in the Elderly Covered by Insurance of the Islamic Republic of Iran Broadcasting Company in 2015 - Comparison with Younger Groups

  • Presently, the world population of the elderly is growing. By improving health hygiene and welfare indicators, mortality and birth rates decrease and life expectancy increases, making the present century the century of elderly. Aging is one of the main risk factors for development of cancer, which itself is the second cause of death in old people. This study was conducted to assess the prevalence of cancer in the elderly covered by the Islamic Republic of Iran Broadcasting (IRIB) insurance program and to obtain suitable programs for cancer screening and early detection, increase patient survival, improve elderly care and to reclaim the cost of treatment in comparison to the national and international statistics. This is a cross-sectional study conducted on all elderly patients diagnosed with malignancy based on their pathology reports. In this study, of the total 75,500 patients covered by IRIB insurance, 17.2% belonged to the elderly group, males accounting for 53.3%. The most common cancers in old men were prostatic cancer (61.3%), colon cancer (10.3%) cancer of the hematologic system, bladder cancer (9.6%), lung cancer (9.1%), thyroid cancer (3.9%) and brain tumors (1.3%). In the elderly women, the most common cancers were breast cancer (80.1%), colon cancer (5.1%), thyroid cancers (4.4%), bladder and hematologic system malignancies (3.6), lung cancer (2.9%) and brain tumors (0.7%). In addition, the prevalence of cancer was almost the same as national and international statistics. With the exception of non-melanoma skin cancer no difference was shown in prevalence of cancer between IRIB elderly patients and the other groups of cancer patients in Iran.

An Analysis of Related Factors for Major Nursing Diagnoses Identified for Instituionalized Elders (일 시설노인에게 나타난 주요 간호진단의 관련요인 분석)

  • Purpose: The purpose of this study was to analyze related factors for major nursing diagnoses used in caring for institutionalized elders. Method: This study was a descriptive study. The participants were 92 residents in a long-term care facility in Wonju, selected by convenience sampling. The instrument was a checklist designed on the basis of the related factors suggested by NANDA (1997) and a literature review. Data was collected by chart review, observation, and interviews with the participants. Data was analyzed using descriptive statistics with the SPSS WIN program. Result: The most frequent nursing diagnosis was 'risk for injury', 'self-esteem disturbance', 'activity intolerance', 'impaired phyical mobility', and 'powerlessness'. The most frequent component for related factors for the five component for each nursing diagnosis was the physical component, followed by the social component. Common related factors for the nursing diagnoses were 'pain', 'change of emotional state/disorder', 'cognitive disorder', 'change of physical structure and function', and 'physical impairment', and 'immobility'. Conclusion: The results of this study can contribute to the development of appropriate nursing intervention programs for elders (eg. 'Injury Prevention Programs', 'Self-esteem Improvement Programs' etc.). Clinical guidelines that gerontological nurses can use to accurately assess health problems and select appropriate nursing interventions may be developed.

Importance and Satisfaction on Nursing Core Competency of Nurses & Nurse Managers in a Community Hospitals (일 지역 병원 간호사와 간호관리자가 인식하는 간호핵심역량 중요도 및 만족도)

  • This is a descriptive study to identify the importance and satisfaction on nursing core competencey(NCC) for the clinical practical centered nursing education curriculum revision. The subjects were 136 staff nurses and nurse managers at A and B hospitals in K city. The data were collected from December 1 to 15, 2012. It were analyzed using the SAS program through t-test, ANOVA, Duncan test. There was a significant differences in the score of the importance and satisfaction on NCC within nurses(t=12.64, p<0.0001) and within nurse managers(t=6.20, p<0.0001). Also there was higher in the score of importance on professional knowledge and personality area of the NCC, But there was the most lowest in the score of satisfaction on foreign language skills. Therefore education and health care systems should work closely to promote professional competency of nurses and to strengthen the liberal arts education, including personality and foreign language skills of students nurses.

Effect of Auricular Acupressure Therapy on Decreasing Smoking rates, Desire for Smoking, and Exhaled Carbon Monoxide in Male College Students (이압요법이 흡연 남자대학생의 흡연량, 흡연욕구, 호기 일산화탄소에 미치는 효과)

  • Purpose: This study was conducted to investigate the effect of auricular acupressure therapy on decreasing smoking rates, desire for smoking, and exhaled carbon monoxide in male college students. Methods: The quasi-experimental, nonequivalent control group pretest-posttest design was used in this study. The participants (n=56) were assigned to the experimental group (n=29) that received auricular acupressure therapy or the control group (n=27) that received placebo therapy. The therapy was applied for 5 days followed by 1 or 2 days of rest. The therapy was repeated 2 times. To identify the effect of the experimental treatments, assessments of smoking rates, desire for smoking and exhaled carbon monoxide were measured. Data were analyzed using $x^2$ test, Fisher's exact test, t-test and Repeated measure ANOVA with the SPSS version 21.0 program. Results: The experimental group showed significantly lower smoking rates (t=6.60, p<.001), desire for smoking (t=3.37, p=.002) and exhaled carbon monoxide levels (t=6.22, p<.001) after application of the therapy. Significant group${\times}$time interaction effect was found in exhaled carbon monoxide levels between the experimental group and the control group (F=8.73, p=.001). Conclusion: Auricular acupressure therapy was considered to likely be useful as a therapeutic intervention for smoking cessation in male young adults.

Actual conditions and recognition of dental hygienists for the smoking (치과위생사의 흡연실태 및 인식조사)

  • Objectives : There were reports about the decreasing of quit-smoking ratio because of taking professional advices. This study were to investigate smoking prevalence rate of the dental hygienist study in nationwide and quit-smoking counseling activity for patients in dental clinic. Methods : The registered Korea Dental Hygienists Association Meeting in 2005 were recruited as subjects. The personal surveyed, 486(87.7%) returned completed questionnaires. Results : The smoking rate of dental hygienist is 3.1%, past smoking rate 1.4% and smoking cessation 95.5%. In smoking cessation counseling activity, only 20.8% of dental hygienist would advise to quit smoking. However, 63.8% intended to advise to quit smoking but they have no idea about quit smoking program and 15.4% had no intention of advising to quit. It is a whole consent that Smoking dental hygienist is tend to against smoking and necessary training about smoking cessation same opinion. Conclusions : Dental hygienist is more effective for health care professional to help people stop smoking, therefore dental hygienist is important for them to have through knowledge of subject and confidence in their role in smoking cessation.

Study for Effective Factors of Emotional Labor in Radiation Therapist of Radiation Oncology Department (방사선종양학과에 종사하는 치료방사선사의 감정노동에 미치는 요인에 관한 연구)

  • The objectives of this study were to investigate the degrees of emotional labor, the effects to organizational effectiveness and the significant factors influencing organizational effectiveness in radiation therapy professional. The subjects of this study were the radiation therapists working at the hospital in Seoul and Gyungi. The self-reported questionnaires were administered to 205 radiation therapists and the data were analyzed with SPSS 19.0 package. The level of emotional labor of single or female subjects were revealed higher than married or male. The professionalism of job has significant correlation with emotional labor. Significant explanatory variables of the degrees of emotional labor included organization culture, distress and self-leadership. These three variables demonstrated the explanatory power of 13.9% of the emotional labor. Based on the findings, emotional labor of radiation therapist was correlated with occupational professionalism. These results suggest that the intervention program intervening emotional labor should be continued to develop in order to improve the effective self-leadership, organization culture and distress factor.
