Objective: Independent walking is the most essential prerequisite to maintain quality of life in older persons. The purpose of this review was to investigate the effect of fall prevention strategies on fall risk for type 2 diabetes mellitus (T2DM) within community-dwelling older adults aged 65 and over. Design: A systematic review and meta-analysis. Methods: PubMed and three other databases were searched up to October 31st, 2018 and randomized controlled trials (RCTs) evaluating fall prevention strategies for fall risk in persons who were 65 years of age or above with T2DM were included. The review extracted the following information from each study selected: first author's surname, published year, country, study population, type of intervention, intensity of intervention, comparison, measurement variables, additional therapy, summary of results, and mean and standard deviation from selected studies. Results: This review selected fourteen RCTs with 460 older adults with diabetes mellitus. Of the 14 studies, the types of intervention used to improve the risk of falls were strengthening (5), aerobic exercises (2), multimodal exercises (4), one virtual reality exercise (1), whole body vibration with balance exercise (1), and Tai Chi exercise (1). Seven RCTs were eligible for the meta-analysis. Therapeutic interventions were more effective than the control group for the Timed Up-and-Go test (-1.11; 95% CI, -1.82 to -0.41) and the 6-minute Walk Test (-1.89; 95% CI, -8.33 to 4.54). Conclusions: The results of the review suggest that interventions to prevent fall risk in older adults with T2DM should focus on strengthening, balance, aerobic, and multimodal exercises.
The effect on strength and endurance training (SET) (2 patients) were compared with those of strength training(ST) (2 patients) in patients with-chronic obstructive pulmonary disease. The result of training was assessed by 4 tests: maximal inspiratory pressure(PImax), sustainable inspiratory pressure (SIP), maximal voluntary ventiiation(MVV) and bronchitis-emphysema symptom checklist(BESC). Measurements were repeated before and after training per week for 6 weeks. The SET group performed inspiratory muscle training, using a inspiratory muscle trainer 30 minutes per day, 6 days per week and performing endurance training-12-minute walking-2 days per week for 6 weeks, whereas the ST-only group trained for 30 minutes daily, 6 days per week using inspiratory muscle trainer. SET was no significant increase in exercise performance, whereas ST produced an increase in SIP and a decrease in BESC. There was significant change in BESC betweet the two groups. A simple at home training program using inspiartory muscle trainer was more effective than that of SET program in improving exercise performance of some patients with COPD.h COPD.
The purpose of this study was performed to evaluate the prevalence of overweight and to compare the dietary behaviors, nutrient intake and physical activities of specialized game high school students. Total of 163 subjects participated and their weight, height, waist circumference, hip circumference and bone status by a quantitative ultrasound method were measured. The subjects were surveyed by a self-administered questionnaire about general characteristics, dietary behaviors and physical activities. Nutrient intakes of the subjects were assessed by semi-quantitative food frequency questionnaire. The subjects were divided into four groups on their obesity level by BMI. The prevalence of underweight, normal, overweight and obese group was 6%, 58%, 16%, and 20% respectively. BMI was negatively correlated with bone mineral density (p < 0.01) and positively correlated with WHR (p < 0.01). The dietary guideline compliance score for "Enjoy Korean rice food style" was 2.63, followed by "Prepare food sanitarily"2.48, "Do not skip breakfast"2.29, "Eat a variety of vegetables, fruits, dairy products daily"2.25, "Drink water instead of beverage"2.10, "Choose less fried foods"2.09 and "Maintain healthy weight"1.91. The exercise frequency of walking was not significantly different between groups; however, heavy exercise frequency was significantly lower in underweight group than the other groups (p < 0.05). The energy intake was 2153 kcal, which was 81.2% of the Estimate Energy Requirement, and the intake of calcium and vitamin $B_2$ was 66.7% and 77.8% of KDRIs. Particularly, the intake of iron, vitamin A and vitamin C was about 59.4%, 52.2% and 55.4% of KDRIs and INQ was 0.71, 0.63 and 0.65 respectively, and intake of folic acid fell behind 39.1% of KDRIs and INQ was 0.46. Our study suggests that the systematic and continuous nutrition education will have to be provided at schools to improve dietary and health behaviors and prevent chronic metabolic disease for students of specialized game high school.
We developed the shoes which is specially designed to decrease the leg's fatness during walking and to improve the body figure. The analysis of the gait cycle and the shoes gave changes at the swing phase. We examined the results to 30 girl students of Yeong Dong Junior College for 5 weeks from 27, October to 29, Nobember in 1996, and the results were also obtained in decrease of their thigh's size in circumference. The results of the analysis are as followings; 1) After experiment circumference of the right thigh was decreased 0.04cm in noncontrol group and significantly decreased 1.04 cm in control group(p<0.01). 2) After experiment circumference of the left thigh was decreased 0.27 cm in noncontrol group and significantly decreased 1.17cm in control group(p<0.01). 3) After experiment circumference of the right calf of the leg decreased 0.09 cm in noncontrol group and significantly decreased 0.54 cm in control group(p<0.01). 4) After experiment circumference of the left calf of the leg decreased 0.21 cm in noncontrol group and significantly decreased 0.47 cm in control group. 5) After experiment body weight were not changed significantly in both group. In these results, if the people want to exercise the analysis of fat in the thigh, put on the shoes which improves the body figure and do daily work, and it will be naturally decreased the fat in thigh and they will maintain the beauty lines of the legs. There weren't any changes in the body weight while the fat of thigh in circumsference was analysed and decreased. It means that because the weight of the fat is very light, there weren't any changes in body weight.
Purpose: This study examined the effects of a person-centered fall prevention program for older adults with dementia in long-term care hospitals. Methods: A nonequivalent control group pretest-posttest design was used. The study sample included 42 older adults with dementia (experimental group: 21, control group: 21) and 42 caregivers (experimental group: 21, control group: 21). The program comprised 48 sessions held over 12 weeks and included exercise intervention with resistance and balance, dance walking (45~60 min, three times/week), cognitive and emotional intervention (35~50 min, once per week), and person-centered fall prevention education (10 min, once per week). The program for caregivers consisted of six educational sessions (i.e., fall prevention competency enhancement and person-centered care strategy education, 80 min, once per week) for six weeks. Data were collected before participation and 12 weeks after program completion from February 18 to May 12, 2019. Data analysis was conducted using the chi-square test, t-test, and Mann-Whitney U test with SPSS/WIN 21.0. Results: The experimental group of older adults with dementia showed significant improvement in physical and cognitive functions, and a decrease in depression, and behavioral and psychological symptoms, when compared with the control group. caregivers in the experimental group exhibited significant improvement in fall-related knowledge and person-centered care of older adults with dementia compared to the control group. Conclusion: The study findings indicate that this program was effective as a nursing intervention for fall prevention among older adults with dementia in long-term care hospitals.

The purpose of this study was to investigate the effect of somatosensory input on the gait ability and equilibrium sensory of elderly women. The subjects who participated in this study were 31 elderly women with a mean age of 75 years or older who no using a walking stick and had no abnormality in otolaryngology. The control group consisted of 30 elderly women who did not exercise regularly more than twice a week. Dependent variables consisted of 2.44m timed up and go test, 10m usual gait, 10m fast gait and 6minute gait. The equilibrium sensory test was performed using EquiTest (NeuroCom, USA). The results of the study on gait ability were not statistically significant for 2.44m timed up and go test, and 10m usual gait. However, 10m fast gait (P<.001) and 6min gait (P<.05) showed significant differences. According to the results of the study on equilibrium sensory ability, there was no significant difference between Condition 1 and Condition 5 however Condition 2 (P<.01), Condition 3 (P<.01), Condition 4(P<.01) and Condition 6 (P<.05) showed statistically significant differences. In conclusion, walking stick have beneficial effects on walking and equilibrium sensation, and elderly women need to actively use walking stick when going out and walking.
The purpose of this study is to provide information searching for health promotion, nutrition improvement, and health care of the impaired elderly by ADL(Activity of Daily Living) and IADL(Instrumental Activity of Daily Living). The subjects were divided into the Assistant Needed Group and No Assistant Needed Group for living in line with the responses of ADL(10 items like dressing, washing, move etc.) and IADL(10 items like housekeeping, using transportation, shopping, Phone call etc.). Survey was made for health behavior, health risk habit, dietary management status and diet intake by 24 hr-recall and questionnaire method. 242 subjects were collected in 12 cities or Gun districts in Gyeonggi Province, S. Korea. Survey was carried out by regional home extension workers using interview method. Statistical analyses were made using SAS (Version 8.1). Chi-Square Tests and General Linear Models. The subjects of impaired ADL elderly was 26.5% and it composed 30% of the total male and 22.2% of the total female. The demographic status of the impaired ADL elderly showed no difference from that of the normal elders, elementary school educated (73.4%), with spouse (43.8%) or with adult children(37.5%), using monthly living cost of 500-1,000 thousand won(35.9%). Mean age was 74.05 years compared to 72.25 years of normal elders. However, there was no significant difference from the normal and impaired ADL group, regular exercise(60.0%), with walking (90.0%), no-smoke(54.7%) and no-drink(48.4%). Kind of disease was not different from the one in impaired and normal group, with cardiovascular disease(32.3%), with diabetes mellitus(8.1%), joint lumbago neuralgia(32.3%) and osteoporosis(9.7%). Gastrointestinal complaints of the impaired ADL group were nausea(57.8%), chronic indigestion (23.4%), constipation (14,0%) and vomiting(3.7%). Sleeping time required for the impaired was longer than that for the normal group by 10hours(4.7%) or 8-10hours(20.3%), which consisted 1.7% and 16.6% respectively. Nutrient intake of the impaired ADL group was low compared to normal range elders: Energy(1260kca1), Protein(52.75g). There was gender difference in nutrient intake; the male impaired group showed no significant difference from the normal group but it was significantly lower in female impaired group. These results suggest that low quality of life and low economic status of the impaired ADL elderly require congregate meal in village hall to cover the lack of side dishe variety. And nutrition education program including community assistance would be required for the impaired ADL elderly together with the sufficient food and exercise practice. By operating nutrition education program, the impaired ADL elderly would maintain more enhanced quality of life and ameliorate the ADL capability.

Background: This study was conducted to investigate the effects of unstable modified wall squat exercises accompanied by abdominal drawing-in on the gait variables of healthy adults. Methods: The total number of subjects was 30, and 15 were randomly placed in the training group (TG) and 15 in the control group (CG). To determine the gait variables of TG and CG, step length difference (SLD) stance phase difference (STPD), swing phase difference (SWPD), single support difference (SSD), and step time difference (STD) were measured using OptoGait, a gait analysis system. Results: When the pre-intervention and post-intervention results of TG and CG were compared, statistically significant differences in SLD, STPD, SWPD, SSD and STD of TG were seen. Conclusion: Unstable modified wall squat exercises accompanied by abdominal drawing-in might help reduce the deviation between left and right gait variables during walking.
Purpose: This study was conducted to assess the effects of robot-assisted gait training with visual feedback on gait, balance, and balance confidence in patients with chronic stroke. Methods: Thirty subjects with chronic stroke were randomly assigned to two groups: the experimental group (n=15) and the control group (n=15). The experimental group performed robot-assisted gait training for 30 minutes and the control group performed gait training with assisted devices training for 30 minutes after both groups performed conventional physical therapy for 30 minutes. Both groups performed the therapeutic interventions for 5 days per week, for a period of 4 weeks. For assessment of the 10 m walking test (10 MWT), Figure of 8 on the walk test (F8WT), Timed-Up and Go test (TUG), and Berg Balance Scale (BBS) were used to test the gait and balance, and the Korean version of the Activities-specific Balance Confidence Scale was used to test the balance confidence. Results: The experimental group showed significant improvement in the 10 MWT and the K-ABC (p<0.05), and the control group showed significant improvement in the BBS and the TUG (p<0.05). In four measurements, there were significant differences between the two groups (p<0.05), and the control group showed significant improvement in the F8WT at pre and post intervention (p<0.05). Conclusion: This study demonstrated that Robot-assisted gait training with visual feedback is an effective intervention for improving straight gait abilities and balance confidence, while the control group showed some improvement in curve gait and balance. Thus, we suggest both Robot-assisted gait training with visual feedback and gait training with assisted devices training exercise as a therapeutic intervention in chronic stroke rehabilitation.
Purpose: This study aimed to apply a health partnership program using commercially available mobile health apps to improve cardiovascular risk factors in male employees and verify its effectiveness. Methods: Using a randomized control group pretest-posttest design, male employees with cardiovascular risk factors from five small and medium-sized workplaces were randomly assigned to an experimental group (n = 32) and a control group (n = 31). The experimental group was encouraged to use three mobile health apps for 12 weeks to acquire the necessary cardiovascular disease-related information and practice strengthening training, walking, and diet management appropriate to their level. They also received feedback on their weekly activities and motivational text messages from health partners. Hypotheses were tested using the SPSS WIN 22.0. Results: The experimental group showed a significant difference compared to the control group in terms of their perception of mobile health app (p < .05), self-efficacy for exercise and diet, self-management partnership, and cardiovascular disease prevention health behavior (p < .001). In particular, there were significant decreases in the body mass index, ratio, serum fasting blood sugar, total cholesterol, and triglyceride in the experimental group (p < .001); however, there was no significant difference in high-density lipoprotein-cholesterol. Conclusion: Intervention using mobile apps based on partnership with health managers is effective in improving the objective cardiovascular risk index in male employees; therefore, such intervention should be continuously used as a useful lifestyle modification strategy in the workplace.
