농촌지역 고혈압노인을 위한 동기강화 자가관리프로그램의 개발 및 적용 (Development and Application of Motivation-enhancing Self-management Program for Rural Aged with Hypertension)

  • Objective: The purpose of this study was to develop a motivation-enhancing self-management program and apply to rural aged with hypertension to evaluate the applicability of the Program. Methods: The motivation-enhancing self-management program was on the evidence of Orem's self-care theory and Ford's Motivational system theory which consists of group education and tele-coaching conducted 12-week. The program evaluation was done by structured questionnaires of motivation (self-efficacy, perceived benefit, perceived barrier and emotional salience), self-care and blood pressure. There were totally 31 aged with hypertension in 2 rural areas enrolled in the program. Results: There was only perceived benefit statistically significant between the intervention and control group (t=2.05, p=.04) and self-care level was no difference between the two groups (t=0.21, p=.84) after program. But there are statistically significant differences of systolic blood pressure between the intervention and control group(t=-2.62, p=.01). Conclusion: The motivation-enhancing self-management program is necessary for the rural aged with hypertension, but it's needed modify and revise in the future study.

장기현장실습 프로그램의 만족도 영향요인 탐색: K대학 IPP 프로그램을 중심으로 (An exploration of factors affecting the Satisfaction of long-term on-site training: Focusing on IPP of K University)

  • The purpose of this study was to investigate the factors affecting the satisfaction of the program for the quality improvement of the long-term on-site training operated by the university. The survey questionnaire data of 393 students who participated in the IPP program of K university were used to analyze the data and the results were as follows: First, factors affecting the satisfaction of long-term on-site training were corporation factors and university factors. Specifically, job contents, coaching, job scope, center support were found to have a significant effect in that order. Second, the factors affecting the satisfaction of long-term on-site training were different according to group characteristics(IPP participation types, major types, corporation types). It is necessary to intensively manage the common and important corporation factors, and it is necessary to closely examine each factor derived from group characteristics, and to support and strive at university level according to group characteristics in student counseling and matching.

유방암 생존자의 운동실천을 위한 앱 기반 자가관리 프로그램 개발 및 효과: 비무작위 대조군 실험연구 (Development and Evaluation of an App-Based Self-Management Program for Exercise Practice of Breast Cancer Survivors: A Non-Randomized Controlled Trial)

  • Purpose: This study aimed to develop an app-based self-management program based on the transtheoretical model (TTM) for breast cancer survivors' exercise practice, as well as to investigate the program's effects on the stage of change for exercise, exercise self-efficacy, exercise decisional balance, exercise amount, and body composition. Methods: This non-randomized controlled study included 52 participants (26 in each of the experimental and control groups, respectively). An app-based self-management program based on the TTM was conducted with the experimental group for a 12-week period. The program comprised three components: individual coaching for each stage of change for exercise based on TTM, amount of exercise and body composition monitoring, and online self-help meetings. Results: Compared with the control group, the experimental group had significantly higher stages of change for exercise (p < .001), exercise self-efficacy (p < .001), exercise decisional balance (p = .002), exercise amount (p < .001), and body composition (body weight [p = .006], body mass index [p = .005], and body fat percentage [p = .010]) immediately and four weeks after the intervention. Conclusion: An app-based self-management program based on the TTM improves exercise behaviors in breast cancer survivors and provides physical benefits.

자기효능 정보자원 프로그램이 관상동맥 질환 환자의 지각된 자기효능감과 자가간호행위에 미치는 효과 (An Effect of a Self-Efficacy Information Resources Program on Perceived Self-Efficacy and Self-Care in Patients with Coronary Artery Disease)

  • Purpose: This study was conducted to investigate whether a self-efficacy information resources program based on the self-efficacy theory of Bandura (1986) would increase self-efficacy and self-care in patients with CAD. Method: The study was a one group, pretest-posttest pre-experimental design research. The Self-Efficacy Information Resources Program was composed of a video tape program and a telephone coaching program and based on vicarious experiences and verbal persuation. The Program was used as an experimental treatment from February 27. to May 27, 1999. Twenty patients participated from a university hospital located in Seoul. Three days later after a pretest the telephone coaching program was given. It was repeated weekly during a 3 week period. At the end of this period post-test data were collected by mail. Data were analyzed with a random permutation test using SAS program. Result: 1) Scores on self-efficacy after the Self-Efficacy Information Resources Program were significantly higher than before the program (p=0.06). 2) Scores on self-care after the Program were significantly higher (subjective self-care, p=0.0005; objective self-care, p= 0.0002) than before the program. Conclusion: The Program was found to be an effective nursing intervention for increasing self-efficacy and self-care. When nurses who care for CAD patients plan to increase their patients' self care ability, they should always consider the concept of self-efficacy.

예비유아교사의 커리어 포트폴리오 제작 경험의 의미 (Meaning of the Experiences of Career Portfolio Production among Preservice Early Childhood Teachers)

  • 본 연구는 유아교사 양성과정에 커리어 포트폴리오 제작 프로그램을 적용해봄으로써 예비유아교사들이 겪게 되는 다양한 경험의 의미를 살펴보고 이를 통해 커리어포트폴리오 제작 프로그램의 교육적 가치를 살펴보고자 진행되었다. 프로그램은 K대학 3학년 1학기에 재학 중이면서 교직실무 교과목을 수강하는 예비유아교사 23명에게 적용되었으며 연구자는 자료수집을 위해 프로그램이 적용되는 9주 동안 예비유아교사의 반성적 저널, Coaching Professor 프로그램 지도 보고서, 소그룹 면담 자료 등을 수집하였다. 수집된 자료들은 주제별 약호화의 방법으로 분석되었으며 그 결과 8개 하위영역 즉, '시간의 흐름 지각하기', '흩어진 경험 모으기', '혼돈 속에서 나 찾아가기', '함께임을 인식하기', '심리적 안녕감 형성하기', '유아교사로서 나의 모습 그리기', '발전하는 나 설계하기', '세상으로 나아가기' 로 분류할 수 있었다. 이를 통해 커리어 포트폴리오 제작 프로그램의 참여가 예비유아교사들이유아교사로 성장하는데 필요한 가치 있는 경험을 하고 있음을 알 수 있었다.

농업인 참여식 실천학습모델 개발과 성과분석 -농촌진흥청 강소농 사업을 중심으로- (Case Study on Dynamics of RDA PLA Model with Agri-SMEs)

  • This case study aims to explore how RDA PLA model affects the agri-SMEs' empowerment. As an agri-business management renovation program from main workshop it was conducted on March to December 2011 with agri-SMEs and extension officials nationwide by RDA. Especially, as a packaged action learning process in the model used participatory action research. This study collected data with participants observation, interviews, situational analysis and systematic review of discourse in qualitative method. For the validity and identifying empirical results, this study used statistic analysis as a mixed method. Further including various pedagogic methods and business coaching skills, this model was conducted from workshop in RDA, in turn, on-farm business coaching as follow-up, CoPs' activities, and local ATCs extension services by each actors. The dynamic process and effects of each process led some change for farmers' innovative knowledge, skills, attitude, practice and aspiration on their farm business. RDA PLA model development based on the previous practices and research, which provided a configurated picture in the holistic action learning process. In statistic research, this study focused on 279 farmers as respondents who had participated in the program. It shows that their income and benefits increased from their renovative practices on farm business. Following the sampling group, it was surveyed by four indicators - products, customer, quality and cost. The level of contribution of education on economic impact 15% is quoted from previous paper. Even in some limitations of public sector, RDA PLA model actively suggests the paradigm shift of agricultural HRD and development of alternative extension-service system.

다 학제간 접근방식을 이용한 생활금연 프로그램개발과 그 효과에 관한 연구 (The Effect of Life Smoking Cessation Program- multidisciplinary approach)

  • This study was designed to develop the life moking cessation program with multidisciplinary approach using the Bandura's theory of self-efficacy and confirm the effect of it on the self-efficacy and smoking amount in college students. For this purpose non-equivalent control group, pretest-posttest design was used. The participating subjects in this study were 46 male college freshmen, 23 in experimental and 23 in control group. The experimental group received the 8 sessions of lecture and small group discussions for 4 weeks and a telephone coaching program for a period of 8 weeks. The control group received 5 sessions of lecture without further treatments. The data was collected from March 15 to June 11, 1999, and analysed by SAS/PC program with $\chi$$^2$test, simple t, paired t test. The results were as follows. 1. The score of self-efficacy was significantly increased over time in the experimental group than those of the control group. 2. The amount of urine cotinine was decreased over time in the experimental group, number of cigarettes smoked a day were significantly decreased, and they were significantly less than those of the control group. In conclusion it was found that the Life Smoking Cessation Program with Multidisciplinary Approach was an effective nursing intervention for increasing self efficacy and reducing the amount of smoking in male college students. Therefore, future smoking cessation programs should always consider the concept of self-efficacy and ways to positively reinforce it.

단기 생애회고요법이 말기 암 환자의 영적 안녕, 우울 및 불안에 미치는 효과 (Effects of a Short-term Life Review on Spiritual Well-being, Depression, and Anxiety in Terminally Ill Cancer Patients)

  • Purpose: This study was done to evaluate the effects of a short-term life review on spiritual well-being, depression, and anxiety in patients with terminal cancer. Methods: The study used a pre posttest quasi experimental design with a nonequivalent control group. Measurement instruments included the Functional Assessment of Chronic Illness Therapy-Spiritual scale (FACIT-Sp12) and the Hospital Anxiety and Depression Scale (HADS). Participants were 32 patients with terminal cancer who were receiving chemotherapy or palliative care at hospitals or at home. Eighteen patients were assigned to the experimental group and 14 to the control group. A sixty minute short-term life review session was held twice a week as the intervention with the experimental group. Results: There was a statistically significant increase in spiritual well-being in the experimental group compared to the control group. There were also significant decreases in depression and anxiety in the experimental group compared to the control group. Conclusion: The results indicate that a short-term life review can be used as a nursing intervention for enhancing the spiritual well-being of patients with terminal cancer.

발의 유형과 발목 관절 피로 수준이 외발서기 시 압력중심점과 질량중심점 궤적에 미치는 영향 (Effects of Foot Type and Ankle Joint Fatigue Levels on the Trajectories of COP and COM during a Single-Leg Stance)

  • The purpose of this study was to investigate the effects of foot type and ankle joint fatigue levels on the trajectories of center of pressure and center of mass during a single-leg stance. The study subjects included 24 healthy women (normal foot group, n=10; pronated foot group, n=14). Ankle joint muscle fatigue was induced by using an isokinetic dynamometer, where the fatigue levels were measured on plantar flexion and dorsiflexion at angular velocities of $30^{\circ}/s$ at 50% and 30% of the peak torque of ankle plantar flexion. Following assessments in the anteroposterior direction according to the level of fatigue, the pronated foot group showed decreased single-leg stance ability at 50% and 30% of the fatigue level. Moreover, the normal foot group showed better single-leg stance ability than the pronated foot group at 30% of the fatigue level. Following assessments in the mediolateral direction, we noted that the single-leg stance ability did not differ significantly according to the levels of fatigue or foot type. In conclusion, ankle plantar flexion at 50% and 30% of the peak torque reduced the ability of the pronated foot group to achieve a single leg stance in the anteroposterior direction. Moreover, the normal foot group showed better single-leg stance ability than the pronated foot group.

진단 직후 유방암환자의 디스트레스 관리를 위한 심리교육프로그램의 효과 (Psychoeducational Approach to Distress Management of Newly Diagnosed Patients with Breast Cancer)

  • Purpose: The purpose of this study was to examine the effects of integrated psychoeducational program for distress management of newly diagnosed patients with breast cancer. Methods: A quasi-experimental trial was conducted. The participants consisted of 47 female patients with breast cancer assigned to an intervention group (n=25) and control group (n=22). The intervention group participated in integrated psychoeducational program, consisting of individual face-to-face education and telephone-delivered health-coaching sessions. Data were collected at three time points: pre-intervention (T1), post-intervention (T2), and 6-month follow-up (T3). Study instruments were Distress thermometer, Supportive Care Needs Survey Short Form 34 and Functional Assessment of Cancer Therapy-Breast. Results: Compared with the control group, breast cancer patients in the intervention group reported lower distress and supportive care needs than the control group. The intervention group reported higher quality of life (QOL) overall and higher emotional well-being than the control group. Conclusion: These findings indicate that the integrated psychoeducational program is an effective intervention for reducing distress and supportive care needs and increasing QOL of newly diagnosed patients with breast cancer. Oncology nurses need to provide psychoeducational intervention to support patients with breast cancer in managing their distress and helping them adjust to their life.
