Attitudes toward Caring for the Elderly and Geriatric Nursing Practice of Nurses in a General Hospital (일 병원 간호사의 노인돌봄태도와 노인간호실천)

  • The purpose of this study was to identify the relationship among attitude toward caring for the elderly and geriatric nursing practice and the influencing factors on geriatric nursing practice of nurses in a general hospital. Data were collected from October 1 to October 30, 2017 by questionnaires from 228 nurses who work at a general hospital in B city. The data were analyzed by IBM SPSS statistics 20.0 program using descriptive statistics, t-test, ANOVA, Scheffe's test, Pearson's correlation coefficient and Stepwise Multiple Regression. The average score of attitude toward caring the elderly was 3.39 and geriatric nursing practice was 3.21. Significant correlation were found among attitude toward caring the elderly and geriatric nursing practice. According to the research, the significant factors influencing geriatric nursing practice were attitude toward caring the elderly, total clinical careers, elderly education experience influences the attitude toward the elderly, the present working department, have a friendly elderly person, and these factors explained 30.4%. Based on these findings, it is necessary to develop and apply a program to improve positive attitude of care for the elderly to hospital nurses and a program to promote the geriatric nursing practice.

Nurses' Experiences of Caring with Demented Oder Persons (간호사의 치매노인에 대한 돌봄 경험)

  • Purpose: The aim of this study was to understand the nurses' experiences of caring for demented older persons in geriatric in care facilities. Methods: The subjects were ten nurses employed in geriatric care facilities. Open in-depth interviews were used to collect data from January, 2010 to October, 2010. The Colaizzi phenomenological method was used for data analysis. Results: The experiences of nurses' caring for patients with dementia were divided into the following six categories: 'new encounter', 'understanding of intention and satisfying', 'dilemma of caring', 'contriving of comfort', 'comprehensive coordinator', and 'being present'. There were 15 clusters of themes. The subjects stated that the best caring for the demented older persons were 'encounter, knowing, and being with them'. Conclusion: Nurses' caring could improve the quality of life for demented older patients in geriatric care facility. These results suggest that institutional, administrational strategies and schemes need to cluster around the relationship with patients who would include encounter knowing and being with them.

Users' Evaluation of Interior Design Features of Patients Rooms in Geriatric Hospital - From the perspectives of Nurses and Care-Givers - (노인요양병원 입원실의 실내디자인 특성에 대한 사용자 평가 - 간호사와 간병인 및 보호자를 대상으로 -)

  • Demands for geriatric hospital have increased in an era of rapidly aging population. Most of aged patients tend to stay in institutions for long terms. This means that the patient rooms of geriatric hospital should be given different considerations from those of normal hospital in designing interior features. They should be a homelike places for the aged patients and designed to take care of specific needs of the aged. However, most of geriatric hospitals are designed with little attention to such point. They appear almost same to normal ones. This study attempts to examine how users evaluate patients' rooms. The users are nurses, care-givers and family members of aged patients in six geriatric hospitals in Busan. They rated 12 features of patient rooms from 0 point to 100 points and described reasons why they rated in that way. Also, the walk-through was done for these six hospitals. 12 features are sizes of patient rooms, sizes and fixtures of bathrooms, sizes and locations of windows, bed layout, numbers and types of chairs, sizes and types of closet, lighting, color scheme, finishes of floor and wall, and interior design tone. Followings are findings : The users evaluated patients' rooms relatively positive. However, extra chairs for visitors, closet in patients rooms and storage in bathroom, and sizes of patients rooms and bathrooms were evaluated relatively negative.

The Influence of Job Satisfation on Turnover Intention of Nurses in Geriatric Hospital (요양병원 간호사의 직무만족이 이직의도에 미치는 영향)

  • This study aims to analyze the effects of job satisfaction on turnover intention among the nurses in geriatric hospitals. In terms of study methods, this study developed structured questionnaires, and distributed them to nurses working in 11 geriatric hospitals in metropolitan area. A total of 418 questionnaires (response rate: 87.0%) were used for final analysis. We conducted multiple regression analyses to analyze the influence of job satisfaction on turnover intention, adjusting for the characteristics of the research subjects. The results of this study are as follows. First, both job satisfaction and turnover intention were statistically significantly related to individual and hospital characteristics. Second, turnover intention was lower in older age group, and higher in smaller hospitals. As job satisfaction increases, turnover intention decreases. Third, the results of the regression analysis adjusting for subject's characteristics and job satisfaction's sub-categories show that the turnover intention is lower in nurses who are older, day worker, and work in bigger hospitals. Fourth, the result of subgroup analysis by hospital volume show that both of two groups are influenced by hospital ownership and satisfaction about compensation. In addition, professionalism had a significant effect on decreasing turnover intention in smaller hospitals. However, there was no statistically significant relationship between job satisfaction's sub-categories and turnover intention in larger hospitals. The results of this study confirms that job satisfaction lowers turnover intention among nurses in geriatric hospitals. It is suggested that the administrators of those hospitals continue to monitor critical factors associated with job satisfaction, and thus, reduce turnover intention, which helps improve quality of services.

Effects of Empathy for the Elderly, the Nursing Work Environment, and Person-centered Care on Geriatric Nursing Practice Among University Hospital Nurses (대학병원 간호사의 노인에 대한 공감능력, 간호근무환경 및 인간중심돌봄이 노인간호실천에 미치는 영향)

  • Purpose: We investigated the association between empathy for the elderly, the nursing work environment, person-centered care, and geriatric nursing practice among university hospital nurses and factors that affect geriatric nursing practice. Method: We administered questionnaires to 178 nurses between May 23 and June 21, 2022 at a university hospital in city D. We used descriptive statistics, the t-test, one-way analysis of variance, the scheffé test, Pearson correlation coefficients, and stepwise multiple regression analysis for data analysis. Results: We observed a statistically significant positive correlation between nurses' geriatric nursing practice and empathy for the elderly, the nursing work environment, and person-centered care. Empathy for the elderly was positively correlated with the nursing work environment and person-centered care, and the nursing work environment showed a statistically significant positive correlation with person-centered care. Empathy for the elderly and the nursing work environment significantly affected geriatric nursing practice. The overall explanatory power of the regression model was 33.4%. Conclusion: Greater awareness and implementation of various educational programs that promote empathy for the elderly are necessary to improve nursing care for the elderly population. Furthermore, adequate manpower and material support are important to improve the nursing work environment among nurses employed at university hospitals.

Development and Evaluation of Geriatric Visiting Nurses' Educational Program (노인전담 방문간호사 교육 프로그램 개발 및 평가)

  • Purpose: The purpose of this study was to evaluate the validity of an educational program developed for geriatric visiting nurses in Seoul by assessing their satisfaction level and level of learning goal achievement. Methods: This descriptive research study was conducted to develop, implement, and evaluate the educational program in accordance with the ADDIE Instructional System Model. Participants were 170 nurses hired for the 2016 Seoul Metropolitan Government visiting service for older people. Based on Kirkpatrick's Training Evaluation Model, reaction and learning evaluations were conducted during and after the educational program. Data were analyzed using descriptive statistics. Results: The developed educational program consisted of basic and professional courses. The evaluations showed that participants were highly satisfied with the lectures and field placement. Over 90% of the participants achieved the learning achievement goals. Conclusion: The program developed for geriatric visiting nurses in Seoul is considered a valid educational program because of the participants' high levels of satisfaction and academic achievement.

Nurses' Needs for the Spatial Composition of Geriatric Hospital (노인요양병원의 공간구성에 대한 간호사 요구)

  • Purpose : The purpose of this study was to examine the nurses' needs for the spatial composition of geriatric hospital. Methods : The survey method was used for collecting the data. The subjects were 110 nurses who worked at 6 geriatric hospitals in Busan. They answered the questionnaire by the self-administered method. The questionnaire consisted of items which asked the spatial composition of patient room, bathroom, dining space, resting space, care-givers' room, and garden. Results : 1) The patient room using a bed on ondol floor would be useful. Also, it would be desirable that most patient rooms consisted of 4 or 6 persons per patient room and the others were 1 or 2 persons per room. 2) The bathroom for only patients and the shower room for their families or care-givers are needed in the geriatric hospital. 3) The dining room for patients in each floor would be useful to old patients. In addition to that, the cafeteria for their families or care-givers are needed. 4) The resting space for patients and their families, for the staff, and for care-givers are needed. 5) The personal locker and refrigerator are necessary fixtures in care-givers' room. Also, table, sofa, sink, and shower booth are also needed. 6) On the base of activities of care-givers, the space for preserving, washing, and heating foods are needed. Also, the storage space for personal items and the fitting space are needed. Implications : The results of this study would be the fundamental data for space planning of the geriatric hospital.

A Comparative Study on Elderly Care Attitude and Dementia Communication Behavior beforeand after Geriatric Nursing Practice (노인간호실습 전후 간호대학생의 노인돌봄태도와 치매의사소통행위의 비교 연구)

  • This study is a one group pretest-posttest design study to confirm the effect of geriatric nursing practice on elderly care attitude and dementia communication behavior of student nurses. The study was conducted from September to December, 2018. Data were collected through self-report questionnaires of 59 student nurses. The data were analyzed by frequencies and paired t-test using SPSS 21.0 program. As a result, elderly care attitude and dementia communication behavior were significantly increased after the geriatric nursing practice compaired to before the geriatric nursing practice. The results of this study can be used as basic data for the composition of the practice curriculum for student nurses to improve their competence as nurses in the future.

  • Purpose: This descriptive research study is intended to develop practical strategies for enhancing the quality of elderly care by evaluating the levels of ageism and compassion competence among clinical nurses and their impact on geriatric nursing performance. Methods: This study involved 162 nurses who provided direct care to elderly patients in a department with many such patients. Of the 161 returned questionnaires, 150 were analyzed, excluding 11 that had insincere or missing responses. To analyze the data, a comprehensive approach was employed using the IBM SPSS 27.0 program that included calculations of frequency and percentage, mean, and standard deviation, as well as more complex statistical tests such as the independent t-test, one-way ANOVA, Pearson's correlation coefficients, and hierarchical linear regression analysis. Results: Compassion competence was significantly positively correlated with elderly care and negatively correlated with ageism. This study conclusively demonstrates that compassion competence among nurses in geriatric nursing situations plays a crucial role in provision of quality nursing grounded in understanding and respect for the elderly. Consequently, it is imperative to develop educational strategies tailored to each career stage of nursing practice to further enhance effective elderly care.   • The elderly population will increase from 5.8% in 1996 to 12.5% in 2020. The related problems of health will also become a very important issue in the future. Therefore it is important to address the problems of geriatric nursing and geriatric health. 87.6% of the aged were ill with chronic degenerative diseases in 1994. In 1995, hospital admission rates (86.8) for those aged 60 or older were higher than that (56.3) of the total age group. Such high medical utilization will increase national health costs. For the development of geriatric nursing, active nursing intervention in various settings combined with education and research should be developed. Considering the health and welfare of the aged and the present status and views in Korea, I suggest the following: First, the health needs of the elderly in the institution, must be met by a plan that fosters geriatric nurses and programmed service development. Second, health service for the residential elderly must be provided in day care centers, short stays, nursing homes and geriatric hospitals. Geriatric nursing services should be provided in home residential areas, public health centers, public health subcenters as well as having, community health practitioners in primary health care posts and home health nurses. Third, geriatric nursing curriculum must be developed adjust to situations and culture of Korea and be included in the nursing curriculum. And gerontological nurse practitioner or geriatric specialist must be fosteraged to provide the professional care for the aged. Geriatric nursing research should be also achived for geriatric nursing improvements.
