Lack of Utility of Cytokeratins in Differentiating Pseudocarcinomatous Hyperplasia of Granular Cell Tumors from Oral Squamous Cell Carcinoma

  • Granular cell tumor (GCT) of the oral cavity is a benign lesion. Half of oral GCTs demonstrate pseudocarcinomatous hyperplasia (PCH) of the mucosa which can mimic invasive islands of oral squamous cell carcinoma (SCC). Such similarity can be confusing when diagnosing or evaluating the two conditions, potentially leading to misdiagnosis or misclassification. Indeed, several misdiagnosed cases of oral GCT have been reported in the literature as OSCC or malignant oral GCT that resulted in unnecessary aggressive treatment for the affected patients. The aim of this study was to investigate if the cytokeratin pattern of the PCH can help in differentiating GCT from oral SCC. To distinguish between these two entities, we examined 12 patient specimens of oral GCT-PCH and oral SCC histologically and via immunohistochemistry (IHC) for CK13, CK17 and P75. The results suggest that the cytokeratin profile of PCH is similar to that of oral SCC. Therefore, consideration of IHC findings for epithelial markers alone may lead to erroneous diagnosis; thus, the presence of the granular tumor underneath the PCH and its immunopositivity for P75 or other neural definition markers can be essential to identify the underlying tumor and exclude oral SCC. Finally we recommend more studies on the molecular biology of PCH to understand how it can mimic oral SCC histologically without harboring its malignant phenotype clinically, which could have significant translational potential for understanding invasive oral SCC.

고내압$\cdot$대용량 GCT 사이리스터와 그 응용

  • 대용량 파워일렉트로닉스기기의 보다 대용량화$\cdot$소형경량화 및 저손실화 요구에 응하기 위하여 조지전압 4.5KV, 가제어전류 4KA의 고내압$\cdot$대용량 CGT(gate commutated turn-off : 게이트 전류형 턴오프) 사이리스터를 개발하여 상품화하였다. GCT사이리스터는 대용량 파워일렉트로닉스기기의 키파트로서 폭넓게 사용되고 있는 GTO(Gate turn-off)사이리스터와 Turn-on동작이 같고 gto사이리스터의 이점이 저On전압특성을 그대로 갖는 한편 턴오프동작은 ''턴오프 게인이 1''인 새로운 원리에 기초하고 있으며 턴오프특성에서는 GTO사이리스터에 비해 다음과 같은 특징을 갖고있다. (1)GTO사이리스터응용에서 턴오프시 dv/dt를 억제하기 위하여 필요한 스나버회로가 없어도 턴오프동작이 가능하다. (2)축적시간을 종래 GTO사이리스터의 약 1/10로 저감할 수 있다. (3)게이트축적전하를 종래의 GTO사이리스터의 약 1/2로 저감할 수 있다. 이러한 턴오프특성에 의하여 GCT사이리스터는 대용량 파워 일렉트로니스기기에 스나버회로 손실발생의 억제에 의한 손실의 저감, 고속동작화, 직병렬접속 응용에 의한 대용량화의 용이성, 게이트구동회로의 용량을 반감하는 등 많은 메리트를 가져다 준다.

  • Basic design of RC-GCTs (Reserve-Conducting Gate-Commutated Thyristors) by novel punch-through (PT) concept with 5,500v rated voltage is described here. A PT and NPT (non punch-through) concept for the same blocking voltage has been compared in detail. The simulation work indicates that GCT with such PT design exhibits that the forward breakdown voltage is 6,400V which is enough for supporting 5500V blocking. Additionally, the real IGCT turn-off in the mode of PNP transistor has been realized. However, the carrier extraction from N-base to gate terminal will be drastic slowly in terms of NPT structure except for the high on-state voltage drop.

  • Major factors that are considered to determine lease charges of container terminals are, among others, construction cost of berth, discount rate, financing cost, and size of annual equivalent recovery. This paper aims to calculate construction costs at PECT and GCT and their annual equivalent recovery on the basis of historical data, and to identify whether or not the relationship of the above result and current lease charges at the two terminals are justifiable.

  • Giant cell tumor(GCT) occurs very unusually in the rib(less than 1% of GCT). We present the cytologic features of GCT of the rib. It showed multiple cellular clusters composed of characteristic, benign looking osteoclast-like multinucleated giant cells and fibroblast-like mononuclear cells. The multinucleated giant cells contained numerous nuclei (average, 30 to 40 per cell, which were closely packed. The nuclei in giant cells were remarkably uniform and round to oval. The mononuclear, neoplastic stromal cells were elongated and spindle-shaped. There was no cytologically malignant portion in the tumor.

  • This paper is concerned with developing a standard costing model based on the case study of PECT and GCT and analysing their profitability in order to improve operation efficiency and design business strategy. In doing so, the model can be a useful tool to analyze current calculation system of lease charge at the two terminals and to judge whether the level of lease charge currently applied to them is justifiable for their profitability. This paper also deals with break-even analysis of container terminal operating companies on the basis of the model and forecast of their profitability. On the top of that, it tries to look into the arguments and to suggest proposals for improving their profitability.

Sacrococcygeal Teratoma : A Tumor at the Center of Embryogenesis

  • Sacrococcygeal teratoma (SCT) is an extragonadal germ cell tumor (GCT) that develops in the fetal and neonatal periods. SCT is a type I GCT in which only teratoma and yolk sac tumors arise from extragonadal sites. SCT is the most common type I GCT and is believed to originate through epigenetic reprogramming of early primordial germ cells migrating from the yolk sac to the gonadal ridges. Fetal SCT diagnosed in utero presents many obstetrical problems. For high-risk fetuses, fetal interventions (devascularization and debulking) are under development. Most patients with SCT are operated on after birth. Complete surgical resection is the key for tumor control, and the anatomical location of the tumor determines the surgical approaches. Incomplete resection and malignant histology are risk factors for recurrence. Approximately 10-15% of patients have a tumor recurrence, which is frequently of malignant histology. Long-term surveillance with monitoring of serum alpha fetoprotein and magnetic resonance imaging is required. Survivors of SCT may suffer anorectal, urological, and sexual sequelae later in their life, and comprehensive evaluation and care are required.

  • 악성 건초 거대세포종은 매우 드물며 양성 거대 세포 종양 이후 발생하거나 함께 발생할 수 있다. 저자들은 임파종을 가진 73세 남자 환자에서 왼쪽 무릎의 표재성 연부 종양으로 나타난 드문 악성 거대 세포 종양에 대해 보고한다.

  • 과립 세포 종양은 드문 연조직 종양으로써 주로 혀의 줄무늬 근육에서 발생한다. 과립 세포 종양이 대흉근에서 발생한 예는 매우 드물게 보고되었다. 이에 저자들은 유방 아래의 대흉근에 발생한 과립 세포 종양의 증례를 다양한 영상 소견과 함께 보고하고자 한다.
