Foreign body aspirations in dental clinics: a narrative review

  • Foreign body aspiration can produce a medical emergency. Obstruction of the airways can be life-threatening, and complications may develop in less-severe cases if it is left untreated. Although it is more prevalent in children by approximately three times, adults can still experience it, and it is more frequently related to healthcare in adults. Objects used in dental treatment are usually placed in the oral cavity and can be ingested or inhaled by accident. Dental treatment has been identified as an important cause of the misplacement of foreign bodies in the airway. However, few reports have been published on dentistry-related foreign body aspiration. This paper discusses the disease course, management, and clinical outcomes of foreign body aspiration, especially those associated with dentistry. The patient must be examined for respiratory distress. If the patient is unstable, urgent airway management and the maneuvers for removal should be performed. Radiographs and computed tomography can help identify and locate the object. The treatment of choice is often bronchoscopy, and both flexible and rigid endoscopes can be used depending on the situation. Preventive measures need to be implemented to avoid inhalation accidents given the potential consequences. Though the incidence is rare, healthcare levels need to be enhanced to avert morbidity and mortality. Radiological evaluation and bronchoscopy are vital for management.

  • At here, we investigated the historical background, current stage, and concrete therapies of alternatives in cancer therapy. We obtained the conclusion below. 1. Alternative medicine(AM) is a unothodoxial medical physiology, medical practice or intervension that overcome the side effects, faults, limits of coventional medicine. 2. Recentely, as emerging New-age movement, collapse of Decarte's mecanical cosmology, and emphasis on patient rights, the interest of AM is increased, and foreign & domestic use rates of AM are risen. 3. After coventional medicine in cancer therapies that depends on operations, radiations, and chemotherapy is bounded, Alternatives in cancer therapy are rapidly developed, and the majority of users are often considered satisfactory. 4. Alternatives in cancer therapy are classified six boundaries; Diet and Nutrition, Mind-body Technique, Integrated System, Pharmacologic and Biologic treatments, Immuno Enhanced Therapy, Natural and herbal medicine. 5. Looking at contents of Alternatives in cancer therapy, we get to know most of them are similar to the treatment of oriental medicine in holistic method, therapy, philosophy. If we synthesize the alternative medicine's treatment mentioned above and investigate it with oriental medical viewpoint, that may help us approach the conquest of cancer and improve the rate of cure. 6. It is certain that foreign AM will be imported reversely, and we must prepare for rebuilding of Korean traditional AM systemically, investing for activity positively, making national policy for medical system.

A Clinical Study of the Foreign Body in the Air Passage (기도이물의 임상적 고찰)

  • A clinical study was done on 24 cases with foreign body in the air passage, who were treated at the department of otolaryngology of Kyung Hee university hospital during the period from Apr. 1973 to Feb. 1983. The obtained results were as follows : 1) The incidence of sex was much higher in male than female and children under 4 years old were predominant. 2) Almost of all patients came in the hospital within 3 days after onset. However one patient came in hospital 5 months later. 3) The variety of foreign body was numerous, among which vegetables and plastic materials were most frequently found. 4) The most common sites of lodgement were trachea and right main bronchus. 5) The common clinical manifestations were dyspnea, decreasing breathing sound and cough. Negative X-ray findings did not exclude a foreign body in the air passage. 6) The important pulmonary complications due to foreign body were atelectasis, emphysema and pneumonia. 7) Foreign body was successfully removed by use of peroral or inferior endoscopy in all cases except one case, on whom thorachotomy was done. 8) The important complications due to the surgical procedure were decannulation difficulty and pneumothorax.

Clinical Analysis of Foreign Bodies in Gastrointestinal Tract in Children (소아에서 위장관내 이물질의 임상적 고찰)

  • Foreign body ingestion is a common problem among paediatric populations. Most of the ingested foreign bodies spontaneously pass through the gastrointestinal tract, but approximately less then 10% of them remain without being discharged, and trigger complications. Therefore, proper evaluation and treatment according to the situation is required. In this study, clinical progress and complications were analyzed according to the clinical features and treatment in children who ingested foreign bodies. Among pediatric patients under 18 who were admitted to Chonnam National University Hospital after ingesting foreign bodies between January 2008 to June 2012, only the patients who had their foreign body in the gastrointestinal tract were included in this study. Based on medical records, age, type of foreign body, time spent till admission, and whether the endoscopy was done or not, complication were researched retrospectively. According to symptoms and plain abdomen X-ray findings, treatment was chosen and conducted among endoscopy, observation and emergency operation. Among 273 patients, 9 (3.3%) of them had surgical removal. Seven (2.6%) of them had an emergency operation on the day of admission, and the rest 2 (0.7%) had operation during observation. Removal through initial endoscopic approach was tried in 157 (57.5%) patients. Eleven (70.8%) of them had their foreign body removed at the initial trial, and 5 (4.9%) of them at the second trial. Among 109, who were on observation status, 9 (8.3%) of them needed endoscopic removal, and 2 (1.8%) of them suffered from surgical removal. It is thought to be better to approach slowly considering the type, size and symptoms in foreign body ingestion of pediatric patients, rather than immediate and invasive removal.

  • Clinical observation is made on the total 205 cases of foreign bodies in the air and food passages. The entire foreign body cases that had visited ENT Dept. of Han Gang Sacred Heart Hospital from January, 1972 to March, 1976 are included in this study. The results summarized are as follows: 1) The majority of the cases of foreign bodies are found in the food passage, 200 cases (97.6%) while in the air passage only 5 cases was present. 2) The most common foreign bodies in the esophagus was coin (90%) and in the air passage, the sorts of foreign bodies are bean, peanut, plastic bullet, corn and towel clip. 3) In the locality of foreign bodies, the first narrowing is the most frequent site in the esophagus. 4) The majority of the cases with esophageal foreign body is male (M:F 1.4 : 1), in the air passage (M:F 3:2). 5) The patients with esophageal foreign body visited within 3 days in 87%, and with trachiobronchial foreign body visited within a day in all cases.

Health Literacy, Diabetic Knowledge, and Diabetic Self-care among Foreign Diabetic Patients at a Hospital in South Korea (일개 상급종합병원을 이용하는 미국과 유럽계 외국인 당뇨 환자의 건강정보이해능력, 당뇨지식, 당뇨자가간호)

  • Purpose: The purpose of this study was to investigate health literacy, diabetic knowledge, and diabetic self-care among foreign diabetic patients at a hospital in South Korea. Methods: Participants was 134 foreign patients diagnosed with diabetes and who understand English. Data were analyzed by SPSS version 20.0. Results: Health literacy significantly differed with HbA1c levels, whereas diabetic knowledge significantly differed with education levels, and time since diagnosed. Diabetic self-care significantly differed with time since the diagnosis. Health literacy significantly differed with diabetic knowledge and diabetic self-care. There was a correlation between health literacy and diabetic knowledge and between diabetic knowledge and diabetic self-care but not between health literacy and diabetic self-care. Conclusion: There are significant results on health literacy, diabetic knowledge and diabetic self-care. Subject with adequate health literacy had high scores on diabetic knowledge and self-care.

A Study on the Relationship of Cultural Competence, Self Efficacy and Job Stress in Nurses Caring for Hospitalized Foreign Patients (외국인 환자 입원병동 간호사의 문화적 역량, 자기효능감, 직무 스트레스 간의 상관관계)

  • Purpose: The present study was done to identify the relationship of cultural competence, self efficacy and job stress of nurses working on wards for foreign patients. Methods: The participants were 133 clinical nurses who agreed to participate in the study and were working on wards for foreign patients in three general hospitals in Seoul. A structured questionnaire that covered the three tested constructs was conducted between Dec. 15, 2014 and Jan. 15, 2015. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, and Pearson correlation coefficients with SPSS/WIN 21.0 program. Results: Cultural competence was found to have a significant positive correlation with self efficacy (r=.464, p<.001) and self efficacy had a significant negative correlation with job stress (r=-.397, p<.001). Conclusion: The findings of the study suggest that programs designed to nourish cultural sensitivity and cultural skills need to be developed in order to promote cultural competence. The results also suggest that in these programs to improve self efficacy and reduce job stress should be considered.

  • Objectives : This study was performed to investigate the knowledge on contacting experiences to foreign patients, the equipments and requirements to make the vitalization method for the oriental medical tourism for foreign patients. Methods : We adopted the focusing group interviews on the oriental medical doctors, nurses and personnels who had the experiences to deal with the oriental medical tourism. Results : For 3 parts, translation into each country's language in program, hospital information, treatment information and homepage should be done and consideration into each country's culture is very important part to know medical tourism patients and prepare the adequate products, and lastly the translater or medical coordinator plays a very important role to connect the hospital personnels with medical tourism patients. Conclusions : Translation in to each country's language concerning medical tourism material and understanding on one's country's culture and a medical coordinator are very important constituents for a successful medical tourism.

A case of a bronchial foreign body (기관이물 1례)

  • This is a case of a left lower bronchial foreign body in a 11 years old boy. This boy was admitted to E.N.T. clinic, Chonnam University Hospital with the complaints of paroxysmal attack of cough and dyspnea for 45 days. He had been treated at a local clinic under the diagnosis of active pulmonary tuberculosis, however, no significant response was achieved by the treatment. The chest X-ray film showed atelectatic shadow, however, no density of suspicious foreign body was observed. On the clinical points of view and the patient's history, bronchoscopy was carried out under the general anesthesia and a plastic gun bullet was removed from the left main bronchus. The boy was discharged from the hospital with good aeration of the left entire lung field and good recovery.

Convolutional Neural Network-Based Automatic Segmentation of Substantia Nigra on Nigrosome and Neuromelanin Sensitive MR Images

  • Recently, neuromelanin and nigrosome imaging techniques have been developed to evaluate the substantia nigra in Parkinson's disease. Previous studies have shown potential benefits of quantitative analysis of neuromelanin and nigrosome images in the substantia nigra, although visual assessments have been performed to evaluate structures in most studies. In this study, we investigate the potential of using deep learning based automatic region segmentation techniques for quantitative analysis of the substantia nigra. The deep convolutional neural network was trained to automatically segment substantia nigra regions on 3D nigrosome and neuromelanin sensitive MR images obtained from 30 subjects. With a 5-fold cross-validation, the mean calculated dice similarity coefficient between manual and deep learning was 0.70 ± 0.11. Although calculated dice similarity coefficients were relatively low due to empirically drawn margins, selected slices were overlapped for more than two slices of all subjects. Our results demonstrate that deep convolutional neural network-based method could provide reliable localization of substantia nigra regions on neuromelanin and nigrosome sensitive MR images.
