Flap tear is a type of displaced meniscal fragments that is often clinically significant lesions requiring surgical intervention. If the displaced tear is located inferomedial to the tibial plateau and incarcerated into the articular capsule, it can be overlooked from preoperative magnetic resonance imaging evaluation and escape detection during arthroscopic examination. In addition, the clinical feature and treatment result of incarcerated flap tear has not been reported. We present 2 cases of medial meniscus flap tear incarcerated into the articular capsule that showed specific clinical features, in order to emphasize the importance of clinical suspicion of such a lesion for accurate preoperative diagnosis.

In the diagnosis of the meniscal tear of the knee, the high accuracy of the MRI diagnosis is well-known, but the accuracy of the MRI in the diagnosis of the very pattern of the tear of menisci is not well-established. The purpose of this study is to give some informations to consider in the diagnosis and therapeutic planning of torn menisci. The authors performed a retrospective study comparing the MRI and arthroscopic findings of 141 knees which had undergone arthroscopic surgery from Mar. 1997 to Mar. 1998. The results are as follows. In the diagnosis of tear of the menisci, MRI had sensitivity of 91%, specificity of 96%, accuracy of 95%. The mismatch of the tear patterns of the menisci between MRI and arthroscopic findings was identified in 59%, especially high in flap tear(100%), complex tear(84%), peripheral tear(55%). In conclusion, MRI is very accurate in the diagnosis of the tear of menisci, but the tear patterns of the menisci cannot be accurately determined by MRI. In the cases of flap tears, complex tears and peripheral tears, it is more difficult to determine the tear patterns of the menisci by MRI. False-negative rate of MRI was especially high(44%) in the peripheral tear type. So, other clinical correlations should be made in determination of the presence of the tear in the diagnosis of peripheral tear of the menisci.
In the patients of retracted massive rotator cuff tears, there are much of difficulty to functional recovery and pain relief. Nevertheless the development of treatment, there are still debates of the best treatments in the massive rotator cuff tears. Recenlty various of treatments are introduced; these are acromioplasty with debridement, biceps tenotomy, great tuberoplasty with biceps tenotomy, partial repair, mini-open rotator cuff repair, arthroscopic rotator cuff repair, soft tissue augmentation, tendon transfer, flap, hemiarthroplasty, and reverse total shoulder arthroplasty. That there is no difference of result for reverse total shoulder arthroplasty between patients who have massive rotator cuff tear without arthritis and patients who have cuff tear arthropathy. Reverse total shoulder arthroplasty is one of reliable and successful treatment options for massive rotator cuff tear. Especially it is more effective for patients who have a pseudoparalysis.
We reviewed the charts and photos taken during arthroscpy of 218 knees of 214 patients(240 menisci) retrospectively. The male was 156 cases(73%) and the female 58 cases(27%). The mean age of the patients was 35 years(range, 7-68). The patients who had definite trauma history were classified as trauma group(Group 1), and the patients who had no or could not recall trauma history were as atrauma group(Group 2). The trauma group was subclassified into the the patients with sports injury, traffic accident, fall down, slip down, direct injury, and miscellaneous according to the causes of the trauma. The patterns of meniscal tear were classified into longitudinal, bucket-handle, horizontal, transverse, flap, complex, and degenerative tear on the basis of O'Connor's classification. The aim of this study was to compare the meniscal tear patterns between trauma group(Group 1) and atrauma group(Group 2) and between the patients before and after the age of 40. The results were as follows ; 1) The difference in the incidence of tear between medial and lateral meniscus was not significant statistically. 2) In Group 1, 60% of the cases showed the longitudinal and bucket-handle tear and 52% of the cases of Group 2 were horizontal tear. 3) In the patients before the age of 40, the longitudinal and bucket-handle tear were 52% of the cases and in the patients over 40, tear patterns which were thought to be related to degenerative change, horizontal and degenerative tear were more than half of the cases (51%).

Purpose: Cemental tear is an uncommon form of root fracture that can lead to rapid localized attachment loss. When it is exposed in the periodontal pocket, it should be removed to prevent accumulation of dental plaque and calculus. Material and Methods: 2 patients were diagnosed as a cemental tear and they were treated with conventional flap operation and subgingival curettage. Additional treatments such as bone graft or guided tissue regeneration were not performed. Result: Symptoms subsided after the treatment. Periodontal pocket has been reduced but no gain of clinical attachment was observed. Remnant of cemental fragment remained after curettage. However, periodontal pocket was stably maintained and there was no recurrence. Conclusion: Periodontal attachment loss associated with cemental tear can be successfully treated with conventional periodontal surgical and nonsurgical procedures.
Purpose: To investigate the arthroscopic findings, and to evaluate the clinical outcomes of the treatment of posterior internal impingement of the shoulder in baseball players. Materials and Methods: We followed up 5 cases who were diagnosed as posterior internal impingement for the mean 15 months. All of the cases complained of the pain in the posterior shoulder at late cocking, and were positive in the relocation test added by hyper-horizontal abduction at $120^{\circ}$ abduction of the arm. We sutured posterior labral tear and SLAP lesion arthroscopically, and conducted debridement for rotator cuff. Three cases were performed of anterior capsular plication and the other two were performed of thermal capsular shrinkage. Pain, range of motion, and level of return to sports activity were assessed for the results. Results: As to the arthroscopic findings, all the five cases showed the fraying in posterosuperior labrum, and two of them was accompanied with the flap tear in posterosuperior labrum and the other one was accompanied with type 2 SLAP lesion. All the cases showed the fraying in supraspinatus, and one case showed partial tear. Meanwhile, in all the cases, the rotator cuff was impinged to the labrum at $90^{\circ}{\sim}120^{\circ}$ of abduction and external rotation. As to the postoperative results, all the cases did not complain of pain or instability while pitching, and the competition was recovered to be the mean 88%($80{\sim}100%$) of that before the injury. Conclusion: Definite diagnosis for the posterior internal impingement would be possible through arthroscopic examination. Favorable outcomes could be obtained with capsular plication or shrinkage for anterior microinstability and stretching exercise for posterior capsule tightness inducing the internal impingement.

Purpose: First introduced by Buncke and Rose in 1979, the neurovascular partial $2^{nd}$ toe pulp free tissue transfer has been attempted to reconstruct posttraumatic finger tip injuries. Although some surgeons prefer other reconstructive methods such as skin graft and local flap, we chose the partial $2^{nd}$ toe pulp flap owing to its many advantages. We report three successful surgical cases in which the patients had undergone this particular method of reconstruction. Methods: We retrospectively examined three cases of fingertip injury patients due to mechanical injury. Bone exposure was seen in all three cases, All had undergone partial toe pulp free flap for soft tissue defect coverage. Results: All flaps survived without any complications such as partial necrosis, hematoma or dehiscence. Although tingling sensation has returned in both cases, two-point discrimination has not returned yet. Currently no patient is complaining of any pain which gradually improved during their course of recuperation. All stitches were removed on postoperative 2 weeks. Patients are satisfied with the final surgical result and there are no signs of any edema or hematoma. Conclusion: The homodigital reconstruction of finger tip injury using the partial $2^{nd}$ toe pulp flap has numerous advantages compared to other reconstructive modalities such as its resistance to wear and tear and in that it provides a non-slip palmar digital surface. However it requires microsurgery which may not be preferred by surgeons. Advanced age of the patient can be a relative contraindication to this approach since atheromatous plaque from the donor toe can compromise flap circulation after surgery. We report three successful cases which patient age was considered appropriate. Further investigation with a larger number of cases and long term follow-up is deemed necessary.

Purpose: The purpose of this study was to compare the preoperative magnetic resonance image (MRI) findings with postoperative arthroscopic findings on meniscus injury with anterior cruciate ligament (ACL) rupture. Materials and Methods: We reviewed MRI images and arthroscopic findings of 225 patients treated by ACL reconstruction due to rupture, from February 2001 to November 2010. There were 154 cases of meniscus tear in arthroscopic findings. We examined the sensitivity for detecting meniscal tears varied with the presence of a rupture of the ACL, with the location of the tear within the meniscus, and among configurations of meniscal tears. Results: In the presence of a rupture of the ACL, the sensitivity of MRI was 0.88 for medial meniscal tears and 0.69 for lateral meniscal tears. And sensitivity of MRI was lowest in posterior horn and peripheral portion tears in lateral meniscus injury accompanying ACL rupture, sensitivity was low in anterior horn and flap shape tears in medial meniscus injury accompanying ACL rupture. Conclusion: In Meniscus injury with ACL rupture, a special attention shoulder given to the posterior horn and peripheral portion injury in lateral meniscus during arthroscopic surgery due to difficulty in detecting on MRI.
A 56-year old female underwent total aortic arch replacement March 1995, because of an expanding chronic Debakey type I aortic dissection. This aortic dissection had an intimal tear at the origin of the right carotid artery. Retrograde and antegrade propagation of dissection resulted in aortic arch blood flow separation and expanding pseudolumen to the abdominal aorta. Sudden anuria(ARF) developed 3 hours later postoperatively and renal doppler ultrasonography and aortography showed diminished blood flow of renal arteries. We performed balloon aortic dilatation but failed. She could be restored good renal flow after intimal flap fenestration resection and thrombectomy of the abdominal aorta. This patient could be discharged in a state of mild CRF after 2 months of ICU care for respiratory and renal failure.
