Compliance of mothers' breastfeeding and complementary feeding practices with WHO recommendations in Turkey

  • BACKGROUND/OBJECTIVES: This study aimed to evaluate how breastfeeding and complementary nutrition practices of mothers of 0-24-month-old children comply with the World Health Organization (WHO) recommendations for infant and young child feeding and to compare the results with selected demographic parameters related to the mother and child. SUBJECTS/METHODS: The research sample comprised mothers (n = 250) with children less than 2 years old. Data were obtained via questionnaire and were analyzed using SPSS 20.0 package program. The Pearson χ2 or Fisher's exact tests were used for assessing relationships between categorical variables. The one-sample t-test was used for comparisons with reference values. RESULTS: Most mothers (97.2%) breastfed their babies immediately after birth. The mean time to breastfeeding after delivery was 47.8 ± 14.8 minutes, and 40.8% of the mothers complied with the WHO recommendation. Furthermore, 59.8% of the mothers exclusively breastfed their children for 6 months (mean 5.2 ± 1.5 months). The mean duration to the start of providing complementary food was 5.8 ± 0.6 months, and 76.1% of mothers who complied with the WHO recommendation. Only 12.3% of mothers breastfed their children for at least 12 months (mean 7.7 ± 3.3 months). On average, mothers gave cow milk to their children for the first time at 10.1 ± 1.7 months and honey at 11.8 ± 2.3 months. The mothers' rates of compliance with the WHO recommendations on cow milk and honey feeding were 32.0% and 71.6%, respectively. The rate of mothers who complied with the WHO minimum meal frequency recommendation was 88.3%. CONCLUSIONS: We suggest that the WHO recommendations on this subject will be realized more fully by emphasizing the importance of the positive effects of breastfeeding until the age of 2 years and of a timely start of complementary food provision. Such changes will affect child health over the long term.

An Analysis of Cancer Survival Narratives Using Computerized Text Analysis Program (컴퓨터 텍스트 분석프로그램을 적용한 암환자의 투병수기 분석)

  • Purpose: This study was done to explore experiences of persons living through the periods of cancer diagnosis, treatment, and self-care. Methods: With permission, texts of 29 cancer survival narratives (8 men and 21 women, winners in contests sponsored by two institutes), were analyzed using Kang's Korean-Computerized-Text-Analysis-Program where the commonly used Korean-Morphological-Analyzer and the 21st-century-Sejong-Modern-Korean-Corpora representing laymen's Korean-language-use are connected. Experiences were explored based on words included in 100 highly-used-morphemes. For interpretation, we used 'categorizing words by meaning', 'comparing use-rate by periods and to the 21st-century-Sejong-Modern-Korean-Corpora', and highly-used-morphemes that appeared only in a specific period. Results: The most highly-used-word-morpheme was first-person-pronouns followed by, diagnosis treatment-related- words, mind-expression-words, cancer, persons-in-meaningful-interaction, living and eating, information-related-verbs, emotion-expression- words, with 240 to 0.8 times for layman use-rate. 'Diagnosis-process', 'cancer-thought', 'things-to-come-after-diagnosis', 'physician husband', 'result-related-information', 'meaningful-things before diagnosis-period', and 'locus-of-cause' dominated the life of the diagnosis-period. 'Treatment', 'unreliable-body', 'husband people mother physician', 'treatment-related-uncertainty', 'hard-time', and 'waiting-time represented experiences in the treatment-period. Themes of living in the self-care-period were complex and included 'living-as-a-human', 'self-managing-of-diseased-body', 'positive-emotion', and 'connecting past present future'. Conclusion: The results show that the experience of living for persons with cancer is influenced by each period's own situational-characteristics. Experiences of the diagnosis and treatment-period are negative disease-oriented while that of the self-care period is positive present-oriented.

A Study on the Occurrences of Accidents among Children in Nursery School and Kindergartens (도시지역 어린이집 및 유치원 어린이의 안전사고 발생 실태)

  • The purpose of this study was to determine how often toddlers and preschoolers in kindergartens have accidents and what factors affect the accident rate. The study population consisted of 150 children who were attending at nursery schools and 150 children who were attending at kindergartens. The data was collected from ten nursery schools and five kindergartens from five districts in Daegu. The questionnaires were distributed to teachers of participating nursery schools and kindergartens to be completed using nursing care records in the institutions and by interviewing children's parents about all of the accidents happened in a previous year. Data were collected during the period of August 1 through 31, 2002. The results of the study are as follow: 1. Among 300 children, 282 had 506 accidental injuries during the study period. 2. The month, the day and the time with the highest accident rate were April. Monday, and between 2 and 4 o'clock in the afternoon, respectively. 3. Locations where the injury took place most included nursery schools or kindergartens, around the homes of the children, and inside the home. 4. Most accidents were occurred due to lack of carefulness of the children, and the most prevalent forms of injury was abrasions. 5. Most frequently injured part of the body was legs of the children. 6. Most injuries were healed within three days and required first aid measures to disinfect the wound. Mostly, these were performed by family members at home. Some children go to the hospital to suture the open wound. Most frequent type of complication was scar formation and the cost of the treatment ranged from 9,000 to 30,000 won. 7. Children's age. sex, birth order. personality. type of family composition. type of residency. father's occupation, father's age, and mother's age were significantly related to the frequency of injury among children. Children who were in nursery schools and kindergartens need their assessment for accident involving condition according to seasons, time. place. This study provided a very useful and important data to prepare accident prevention education program and accident prevention strategies, and to develop Injury Surveillance System.

Game Use and School Life of Korean Adolescents: Gender Difference and Moderating Effects of Social Relationship (청소년의 게임이용과 학교생활 - 성별 차이와 사회적 관계의 조절효과)

  • This study examines links between Korean adolescents' uses of games and their school lives. A gender-based analysis was conducted and moderating effects of social relationship variables were tested. The major findings are as follows. First, some gender differences were found in impacts of game use. For example, although a game addiction variable appeared to influence both male and female adolescents negatively on the school-related conscientiousness, only male students displayed a negative impact of game time. It represents a gender difference in the preferred game genres and ways of using them. Second, the analysis showed that variables of social relationship could moderate the negative effects of game use. While the relationship with father appeared to moderate the negative effects of game time in male adolescents, the relationship with mother turned out to relieve the negative impacts of game addiction in female ones. In the case of male adolescents, however, the relationship with the parents did not show any moderating effect on game addiction. This study supports the claim that it is necessary to take a gendered approach in studying gaming.

Young Married Women's Labor Market Exit: Focused on the Effects of the Child Birth and Available Family-Friendly Policies (첫 자녀 출산 여부와 가족친화제도에 따른 유배우 기혼 여성의 취업 중단에 관한 연구)

  • The purpose of this study is to understand why female labor force participation rates decline in early times after their marriage. Data were derived from the 4th(2001) to 9th(2006) Korea Labor & Income Panel Study. 194 Korean married women in twenties and thirties who had a job before marriage were analyzed. Survival analysis was used to explore the first labor force exit of married women longitudinally. The major findings are as follows. First, nearly half of them went away from labor market in the first 3 years after marriage. Second, child birth was the most significant factor in predicting women's labor force exit. Married women's employment discontinuity tend to be lowered after child birth, with working hour decreasing, and with the number of available family-friendly policies increasing. Married women's income encouraged them to hold on their career, though husband's income and household income were not significant. Third, married women tended to leave their job before giving birth. Women who remained in the labor market at child birth or until a year after birth were inclined to continue their job thereafter. Fourth, maternity leave and childcare leave diminished the probability of employment discontinuity. Many working wives could not use a maternity leave or childcare leave. This study shows married women usually underwent labor market exit in their newly married time. They cannot help facing conflict between the role of mother's and a worker's. Family-friendly policies could encourage working wives to rear child and continue work at the same time. The findings of this study could serve as fundamental material for further studies and would be a key to find effective solution for problematic issues on reconciling work and family.

The Effects of Education on Knowledge and Confidence in Postpartal Self-care and New baby care (산모교육의 효과 측정 - 산욕기 자가간호와 신생아양육에 대한 지시와 자신감을 중심으로-)

  • A maternity ward in a hospital in Seoul has an educational program for postpartal mothers since its opening, but evaluation on the program has not been done. This study was to find whether the educational program contributed to increase of knowledge and confidence of the post-partal mothers in the area of postpartal self-care and the newborn baby care or not. This study aimed at improvement of the educational program for posrpartal mothers and the newborn babies. Subjects were 40 primiparae who were admitted to the obstetric ward in a general hospitals in Seoul from August 20th to September 10th, 1995. Subjects were those who had no labor pain at the admission time, had no complications during labor and delivery, gave a birth to a healthy baby, and agreed to participate in this study. All subjects were well educated and were in well-to-do group. Most of them received antenatal care adequately. They were tested on knowledge and self-confidence in the area of postpartal self-care and infant care two times, one at the admission time and the other prior to discharge. After the first test, nurses in a maternity ward and nursery taught them on postpartal self-care and infant care. Education consisted of a 1 hour planned program and incident teachings done at bed side. Four tools were developed by authors based literature review. The test tool for knowledge of postpartal self-care consisted of 15 items which included the definition of postpartum, dangerous symptoms in postpartum, lochia, time to begin coitus, postpartum exercise, sitz-bath, and perineal care. The test tool for self-confidence in postpartal self-care included such items as emergency care on dangerous symptoms in postpartum, sexual life and contraception after postpartum, self-confidence in postpartum exercise and perineal care. The test tool for knowledge of newborn baby care consisted of 18 items which included bathing, umbilical cord care, vaccination, breast feeding, abnormal symptoms of neonate. The test tool for self confidence in the newborn baby care included umbilical cord care, vaccination, breast feeding, emergency care for baby. Analysis of demographic data were analyzed with calculation of percentage. Score differences between the first test and the second test were analyzed with paired t-test. SAS / PC (Release 6.04 for DOS) program are as follows. 1. Mothers' knowledge of postpartal self-care increased significantly after education (t=10.04, df=38, p=0.0001). 2. Mothers' self-confidence in postpartal self-care increased significantly after education (t=6.53, df=38, p=0.0001). 3. Mother's knowledge of the newborn abay care increased significantly after education (t=9.74, df=38, p=0.0001). 4. Mothers' self-confidence in the newborn baby care increased significantly after education(t=8.22, df=38, p=0.0001) Suggestions for further studies and nursing practice were as follows. 1. We suggest studies by randomized control-group pretest-posttest design or nonequivalent control group pretest-posttest design will be done. 2. We suggest follow-up studies to find if mothers's confidence will last or not after discharge. 3. We suggest general hospitals to establish a phone-counseling system.

Paternal Childcare Time for Preschool Children and Its Determinants on Working and Nonworking Days (미취학자녀를 둔 아버지의 근무일과 비근무일의 자녀돌봄시간과 영향 요인 - 맞벌이 여부 및 돌봄유형별 차이를 중심으로 -)

  • This study analyzed 2019 time survey data from the National Statistical Office to examine the childcare behaviors of fathers with preschool children and their determinants, considering types of care and working/nonworking days. The main results of this study are as follows. First, paternal childcare time of nonworking days was three times more than that of working days, and the participation rate was also higher on nonworking days. Second, there was no significant difference in the amount of time spent on primary care and developmental care by fathers, whether from dual- or single-income families. Third, it seems that fathers adjust their participation in childcare between working days and nonworking days in consideration of the mother's time availability. Fourth, the variables related to childcare needs had a significant influence on paternal childcare time on both working and nonworking days. Fathers' developmental care time was not explained by the independent variables entered into the regression analysis. As a result of the study, it is necessary to reduce fathers' working hours and increase family-friendly systems to increase fathers' participation in childrearing. Fathers' perception of parental responsibility must also be changed.

Plasma Concentrations of Fe, Cu, Mn, and Cr of Maternal and Umbilical Cord Blood during Pregnancy

  • Anemia is prevalent among pregnant women in Korea, and Fe deficiency anemia is a major nutritional problem throughout the world. Because studies of Cu, Mn, and Cr levels excluding Fe are rare, we were interested in changes in the nutritional status of these trace minerals and their relationship to hematogenesis. Accordingly, we determined the changes in plasma Fe, Cu, Mn, and Cr concentrations of maternal and umbilical cord blood during pregnancy, and evaluated the relationships between them at different time points during pregnancy. A total of 81 women participated in the study: 26 subjects in the first trimester, 23 in the second, and 32 in the third trimester. Plasma Fe levels were lower significantly (p<0.05) in the third trimester. Plasma Cu level ($\mu\textrm{g}$/dL) in each trimester were 86.6$\pm$13.8, 111.6$\pm$27.9, and 114.0$\pm$29.7, respectively; with significant increases (p<0.()5) in the second and third trimester. Plasma Mn concentrations (pg/dL) in each trimester were 212.6$\pm$89.0, 234.0$\pm$140.0, and 240.3$\pm$166.0, respectively and tended to increase, though not significantly, as the pregnancies progressed. The plasma concentrations of Cr (pg/dL) in each trimester were 3.7$\pm$2.0, 3.1$\pm$1.0, and 2.4$\pm$1.2, respectively; and was significantly lower (p<0.05) in the third trimester. In umbilical cord blood, the plasma level of Fe was 194.8$\pm$74.6 $\mu\textrm{g}$/dL, Cu was 57.5$\pm$10.9 $\mu\textrm{g}$/dL, Mn was 482.4$\pm$111.1 pg/dL, and Cr was 9.3$\pm$2.8 pg/dL. Plasma concentrations of Fe, Cu, Mn, and Cr of cord blood were 300 %, 50 %, 200 %, and 370% as compared to those of maternal blood in the third trimester. These results suggest that an active transport mechanism for the transport of Fe, Mn, and Cr from mother to fetus may exist, whereas, for Cu, the placenta appears to have a blocking effect on the transport from mother to baby.

Patterns of Ceremonial Foods for Middle-aged Residents in Ganghwa (강화 지역 중년 남.녀의 의례 음식 섭취 실태)

  • The data for this study were collected in a survey conducted in Ganghwa. The questionnaire was specifically designed to identify ceremonial and prohibitive foods in Ganghwa. Quantitative and qualitative data were reported as frequencies, and $X^2$ analysis was employed to assess the relationships among religious. Ceremonial foods were important on the 15th of January by the lunar calendar(87.5%), the Korean Thanks giving Day(84.4%), New Year's Day(79.8%), and the winter solstice(77.4%). A table in celebration of a baby's first birthday included baekseolgi, rice cake with Indian millet and red bean, songpyeon, injeolmi, fruits, and japchae. Women who had delivered a child ate boiled rice and seaweed soup. Birthday parties was hosted in 67.0% of the homes. The reasons for not having a birthday party were the inability to make enough time(38.2%) and difficulties with work(19.4%). Pyebaek foods were jerked beef, chicken, jujube and chestnuts. A 60th birthday anniversary was the reason for 31.4% of the respondents to eat ceremonial foods, and a Memorial Day service that carried in the eldest son was the reason in 53.4% of the families. The Memorial-Day service foods were learned by a Catholic mother(66.7%), by the husband's Buddhist mother(37.9%), or by Confucianism(54.5%)(p<0.05). Therefore, it is important to increase the understanding of celebrational foods and to enforce systematic public relations.

The factors to identify high risk family (고위험가족 선별을 위한 위험요인 분석)

  • The main purpose of the study is to identify critical risk factors for development of a family assessment tool to screen high risk family. This study used a conceptual framework of family diagnosis developed by Eui-sook Kim's (1993) and analyzed risk factors to identify the high risk family. As employing a explorative and methodological study design, this study has four stages. 1. In the first stage, 34 family risk factors were identified by doing intensive literature review on conceptual framework of family diagnoses. 2. In the second stage, above risk factors were tested for content validity by consultation with 29 persons in community health nursing, nursing education, family theory, and social work. 3. In the third stage, existing survey data was used for actual application of the identified risk factors. The survey data used for this purpose was previously collected for the community diagnosis in a region of Seoul. At the final stage, through the comparison between high risk and low risk families, initially identified 34 risk factors decreased to 25 risk factors. Among 34 risk factors, six factors did not agree with content of questionnaries sand two factors were not significant in differentiating the high risk family Also, two risk factors showed high correlation between themselves, so only one of those two factors was chosen. As a result, twenty-five risk factors chosen to identify the high risk family are following ; 1. A single parent family due to divorce or death of a partner, or unweded single mother 2. A family with an unrelated household members 3. A family with a working mother with a young child 4. A family with no regular income 5. A family with no rule in family or too strict rules 6. A family with little or no support from other lam-ily members 7. A family with little or no support from friends or relatives 8. A family with little or no time to share with each other 9. A family with family history of hypertension, diabetus, cancer 10. A family with a sick person 11. A family with a mentally ill person 12. A family with a disabled person 13. A family with an alcoholic person 14. A family with a excessive smoker who smokes more than 1 pack / day 15. A family with too much salt intake in their diet. 16. A family with inappropriate management skills for family health 17. A family with high utilization of drug store than hospital to solve the health problems of the family 18. A family with disharmony between husband and wife 19. A family with conflicts among the family members 20. A family with unequal division of labor among family members 21. An authoritative family structure 22. A socially isolated family 23. The location of house is not residential area 24. A family with high risk of accidents 25. The drinking water and sewage systems are not hygienic. The main implication of the results of this study is clinical use. The high risk factors can be used to identify the high risk family effectively and efficiently. The use of high risk factors woule contribute to develop a conceptual framework of family diagnosis in Korea and the list of risk factors need to be revised continuously. Further researches are needed to develop an index of weight of each risk factor and to validate the risk factors.
