Changing Pattern and Comparison of Nutritional States before and after Nasogastric Tube Feeding for the Severe Brain Injury Patients in Critical Period (초급성기 중증 뇌 손상 환자의 영양 상태 변화양상과 인공영양 전·후의 영양 상태에 대한 비교)

  • Purpose: The purpose of the study were to examine the nutritional status of severe brain injury adult patients in critical period, and to compare the nutritional states before and after tube feeding. Methods: Data from 19 patients admitted to the SICU in a university hospital due to severe brain injury were analyzed. Nutritional states were measured by anthropometric and blood biochemical indicators. Results: MAC and MAMC were significantly decreased only at 7 days after admission compared with those on the day of admission. TSF was significantly decreased from 7 days to 14 days after admission. Fat rate was significantly decreased from 3 days to 14 days after admission. Hb was significantly decreased only at 3 days after admission. Albumin was significantly decreased from 3 days to 14 days after admission. However, lymphocyte was significantly increased at 14 days after admission. TSF and Albumin became significantly worse even after initiating tube feeding. Conclusions: Nutritional status of severe brain injury patients in SICU became worse after admission whichever indicators were adopted to evaluate nutritional status, anthropometric or blood biochemical indicators, and became worse even after initiating tube feeding.

Field Application and Performance of Continuously Reinforced Concrete Pavement via Mechanical Tube-feeding Method (기계식 연속철근콘크리트포장의 현장 적용성 및 거동 분석 연구)

  • PURPOSES : The field application and performance of continuously reinforced concrete pavement (CRCP), constructed by using the mechanical tube-feeding method, are evaluated in this study. METHODS: The location of the rebar was evaluated by using the MIRA system. The early-age CRCP performance was evaluated via visual survey, in which the crack spacing and crack width were examined. RESULTS: The location of longitudinal reinforcing bars was evaluated via MIRA testing and the results showed that the longitudinal rebars all lie within a given tolerance limit (${\pm}2.5cm$) of the target elevation. In addition, owing to the low temperature when the concrete was pured, the crack spacing in the Dae-Gu direction is slightly wider than that of the Gwang-Ju direction. Almost all of the crack spacings lay within the range of 1.0 m~3.0 m. A crack width of <0.3 mm was measured at the pavement surface. However, as revealed by the field survey, the crack spacing was not correlated with the crack width. CONCLUSIONS : In CRCP constructed by using the mechanical tube-feeding method, almost all of the longitudinal reinforcing bars lay within the tolerance limit (2.5 cm) of the target elevation. The concrete-placing temperature affects the crack spacing, owing to variations in the zero-stress temperature. Crack survey results show that there is no correlation between the crack spacing and crack width in CRCP.

Effects of Non-nutritive Sucking on the Physiological and Behavioral States of Pre-term Infants during Tube Feeding (달래기 젖꼭지의 제공이 위관수유 중인 미숙아의 생리적 상태 및 행동적 상태에 미치는 영향)

  • Purpose: This study was to investigate the effects of non-nutritive sucking on physiological and behavioral state of pre-term infants during tube feeding. Method: This nonequivalent, non-synchronized experimental study included 50 pre-term infants. An attempt was made to match gestational age and birth weight of infants in each group. Infants in the experimental group were given a pacifier 2 minutes before, during, and for 2 minutes after tube feeding. Infants in the control group did not get a pacifier. Both groups were tested at three stages for changes in the physiologic state and behavioral state - 2 minutes before, during, and 2 minutes after feeding. Date was analyzed with SPSS WIN 10.0 using an $X^2$-test, t-test, and repeated measures ANOVA. Result: Heart rates and oxygen saturation levels of the two groups were significantly different(P=.001, P=.000). The behavioral states of the two groups were significantly different during and post feeding(P=.000, P=.000). Conclusion: This result suggests non-nutritive sucking by using a pacifier is an effective intervention for pre-term infants during tube feeding.

Early Postoperative Retrograde Jejunojejunal Intussusception after Total Gastrectomy with Roux-en-Y Esophagojejunostomy: A Case Report

  • Intussusception is a rare cause of postoperative intestinal obstruction in adults. Many retrograde intussusceptions occur during the period following gastrectomy. A 77-year-old woman visited our hospital because of detected gastric adenocarcinoma. She received radical total gastrectomy with Roux-en-Y esophagojejunostomy. On the fifth postoperative day, she complained of abdominal pain, and we found leakage at the esophagojejunostomy site and dilatation of the Roux limb and the afferent limb of the jejunojejunostomy. Emergency surgery was performed. Retrograde jejunojejunal intussusception accompanied with a nasojejunal feeding tube was found at the efferent loop of the jejunojejunostomy. No ischemic change was found; therefore, manual reduction and primary repair of esophagojejunostomy was performed. She was discharged without complications on the 23rd re-postoperativeday. We suggest that the nasojejunal feeding tube acted as a trigger of intussusception because there was no definitive small bowel mass or postoperative adhesion. We present our findings here along with a brief review of the literature.

  • Feeding gastrostomy is widely used for children with feeding impairment. The replacement of gastrostomy tube is known as an easy and safe procedure. However, various complications associated with replacement of gastrostomy tube were reported, including fistula disruption and colo-cutaneous fistula. For replacement of gastrostomy tube in small children with small stomach, special cautions are needed. Here, we report a rare case of duodenal perforation as an acute complication after the replacement of gastrostomy tube for a 33-month-old girl.

  • Early diagnosis followed by primary repair is the best treatment for spontaneous esophageal perforation. However, the appropriate management of esophageal leakage after surgical repair is still controversial. Recently, the successful adaptation of vacuum-assisted closure therapy, which is well established for the treatment of chronic surface wounds, has been demonstrated for esophageal perforation or leakage. Conservative treatment methods require long-term fasting with total parenteral nutrition or enteral feeding through invasive procedures, such as percutaneous endoscopic gastrostomy or a feeding jejunostomy. We report 2 cases of esophageal leakage after primary repair treated by endoscopic vacuum therapy with continuous enteral feeding using a Sengstaken-Blakemore tube.

A Systematic Review and Meta-Analysis on the Effects of Oral Stimulation Interventions on the Transition from Tube to Oral Feeding in Premature Infants (구강자극요법이 미숙아의 경구수유 이행에 미치는 효과에 대한 체계적 문헌고찰과 메타분석)

  • Purpose: The purpose of this study was to evaluate the effects of oral stimulation interventions on the transition from tube to oral feeding in preterm infants through systematic review. Methods: The randomized clinical trials published between 1980 and 2015 were searched using domestic and international databases, and five randomized studies were selected for this study. The quality of study was assessed by assessment tool from the SIGN and meta-analysis was performed using the Cochrane Review Manager software Version 5.3 (RevMan). Results: Oral stimulation intervention in preterm infants decreased the transition time from tube to oral feeding (Post Menstrual Age, Day of life). Especially, a significant medium effect size was found in the number of days needed on the transition (ES=-4.95, p=.02). The oral stimulation intervention also had a significant large effect on the length of stay at hospital (ES=-8.33, p<.001). Conclusion: Based on the findings, oral stimulation interventions could be useful to facilitate the transition from tube to oral feeding in preterm infants in terms of reducing the length of stay at hospital and the number of days needed on transition.

Experimental Study on the Oxygen Combustion Characteristics with $CO_2$ Feeding ($CO_2$ 첨가에 따른 순산소 연소기의 연소특성에 관한 실험적 연구)

  • The performance of oxygen combustion with $CO_2$ feeding was investigated in a pyrex tube furnace. The inverse type multi-hole burner was used for improving mixing and wide operating range. It introduced oxygen, fuel, and oxygen, respectively, from center tube to outer tubes. Oxygen combustion characteristics with excess oxygen ratio, oxygen feeding ratio, and $CO_2$ feeding flow rate were studied to optimize the operating condition and to apply the oxygen combustion with recirculation of flue gas to a real furnace. This paper presents results on the effect of $CO_2$ feeding flow rate on the structure of the flames and concentrations of NO and CO emissions. The visible flame length was shortest due to well mixing between fuel and oxygen when the oxygen feeding ratio was 0.25. The NO emission was reduced drastically regardless of excess oxygen ratio when the $CO_2$ feeding flow rate was larger than 15 lpm. The CO emission is varied by changing the $CO_2$ feeding flow rate but the CO emission characteristics is highly affected by excess oxygen ratio. When the excess oxygen ratio is below $\lambda=1.1$, the CO emission increased as the $CO_2$ feeding flow rate increased.

  • Some pediatric patients who can not eat orally depend on enteral tube feedings, and some patients require more nutrients and calories to achieve the catch-up growth. If a patient is counting on the parenteral nutrition, early initiation of enteral feeding, orally or enterally, is a very good for the intestinal mucosal maturity and motility. There are numerous kinds of formulas and supplements for the enteral feeding for neonates, infants, and children. Depending on the intestinal symptoms, allergic symptoms, requirement of special nutrients, we can choose regular infant formula (milk-based, soy-based), protein hydrolysate formula, amino acid hydrolysate formula, elemental formula. Proper use of these formulas would help for the pediatric patients to recover from their diseases, to facilitate the intestinal mucosal maturity and to achieve their goal of growth.
