경막외 Morphine 투여에 의한 극심한 호흡억제 및 두개강내 공기음영 (Severe Respiratory Depression and Intracranial Air after Epidural Morphine -Subdural or Epidural Injection?-)

  • Massive extradural spread, distinguished from subarachnoid injection that sometimes follows the introduction of small amounts of local anesthetics or narcotics during attempted epidural anesthesia or analgesia, has been attributed to subdural injection. A 64-year-old woman was admitted for partial radical hysterectomy under general anesthesia after insertion of lumbar epidural cathter by loss of resistance technique with 5 ml of air. In this case, we experienced severe respiratory depression and loss of consciousness after administration of 4 mg of morphine for postoperative pain control. We confirmed air shadows at right silvian and suprasella cisterna region by CT scanning. Patients was recovered without sequele after 2 days, As this case resembles a "massive epidural", it is suggested that subdural injection rather than epidural injection may explain the phenomenon.

Using Online Information Support to Decrease Stress, Anxiety, and Depression

  • Today, online education is becoming more important. The effectiveness of online education has been measured by student satisfaction and the possibility of substituting offline education. This study proposes a plan to increase the effectiveness of education in a new form by using online information. Education is the process of socializing and growing learners. Representative negative emotions experienced by learners are stress, anxiety, and depression (SAD). A reduction in SAD will promote student growth and improve educational outcomes. This paper considers online information by dividing it into online educational information support (OEDIS) and online emotional information support (OEMIS). We demonstrate that OEDIS reduces SAD, and OEMIS reduces stress and anxiety. By providing online information, negative emotions can be reduced, and educational outcomes can be improved. This study suggests a new role for online information support, such as emotional change in individuals and solving psychological problems. Online information support goes beyond knowledge transfer and can be used in various fields, such as online education that promotes human growth and positive change, and even healthcare.

청소년의 우울 관련 요인 (Relationships of Individual and Family Variables with Adolescents' Depression)

  • 본 연구는 고등학교에 재학 중인 남녀 청소년을 대상으로 성별에 따라 청소년의 우울에 차이가 있는지를 살펴보고 개인요인과 가족 요인이 청소년의 우울과 어떠한 관련성을 가지고 있는지 파악하고자 하였다. 이러한 연구목적을 위해 서울시에 재학 중인 고등학교 2학년 800명을 대상으로 자료를 수집한 후 735부(남학생 342명, 여학생 393명)의 설문 자료를 최종 분석하였다. 본 연구에서 얻어진 결과를 분석한 결과, 청소년의 우울에서 성별에 따른 유의한 차이가 나타나 남자 청소년보다 여자 청소년이 우울을 더 많이 경험하는 것으로 나타났다. 둘째, 청소년의 우울에 영향을 미치는 요인은 자아존중감, 또래친구 관계, 부부 갈등으로 나타났고 자아존중감이 청소년의 우울을 가장 잘 예측하는 요인으로 나타났다. 그 다음으로는 또래친구 관계가 부정적일수록 그리고 부부 갈등 정도가 심할수록 청소년은 우울을 더 많이 경험하였다. 또한 남자 청소년은 자아존중감이 낮을수록, 부부 갈등이 심할수록, 또래친구 관계가 부정적일수록 우울을 더 많이 경험하였고, 여자 청소년도 자아존중감이 낮을수록, 또래친구 관계가 부정적일수록, 부부 갈등이 심할수록 우울을 더 많이 경험하는 것으로 나타났다.

농촌 고령자의 일상활동능력 및 우울감이 생활만족도에 미치는 영향 (The Effects of the Instrumental Activities of Daily Living and Depression on the Life Satisfaction of the Rural Elderly)

  • The purpose of this study was to investigate the effects of the instrumental activities of daily living (IADL) and depression on the life satisfaction of the rural elderly. The research subjects of this study were 1,000 people aged 65 years and over living in the rural areas of Korea. The data were collected using a questionnaire with direct interviews and analyzed using SPSS ver. 18.0 for Windows. The results of this study were as follows: First, the rural elderly's depression was affected by the IADL, cost of living, education level, and farming variables. The higher the IADL, the more sufficient the cost of living, the higher the education level and the more the farming time that the rural elderly had, the lower was the level of depression that they had. Second, the life satisfaction of the rural elderly was affected by the cost of living, depression, IADL, need for life services, and gender. The more sufficient the cost of living, the lower the level of depression, the higher the IADL, the less the need for life services, the higher was the life satisfaction that they experienced. Third, the results of the path analysis revealed that life satisfaction was influenced both directly and indirectly. The factors that affected life satisfaction directly were the cost of living, IADL, need for life services, and gender. The factors that affect life satisfaction indirectly were the IADL, cost of living, education level, and farming. Among them, farming and education level influenced life satisfaction indirectly through the parameter variables of depression.

근로자의 직업불안정성이 직업 관련 우울감 및 불안감에 미치는 영향: 대규모와 소규모 사업장 근로자를 중심으로 (Effect of Job Insecurity on Job related Depression and Anxiety: Large- and Small-sized Company Employees)

  • Purpose: The purpose of this study was to investigate the influences of job insecurity on job related depression and anxiety in large- and small-sized company employees. Methods: Data of the third Korean Working Condition Survey in 2011 were used. Subjects were 2,050 large-sized company employees and 18,924 small-sized company workers. $x^2$ test and multiple logistic regression using SAS 9.2 were conducted. Results: Large- and small-sized company employees showed significant differences in terms of demographic, health-related, and job-related characteristics. From the bivariate analysis, gender, income, self-rated health, occupation, working hours per week, job-related stress, workplace violence, and job insecurity were significantly related to job-related depression/anxiety in large-sized company employees. From the multivariate analysis, higher income (AOR: 0.22, 95%CI: 0.07~0.71), better health perception (AOR: 0.05, 95%CL: 0.01~0.18), 40 or more working hours per week (AOR: 0.20, 95%CI: 0.05~0.79) showed lower risk for job-related depression/anxiety. From the bivariate and multivariate analysis, better health perception (AOR: 0.32, 95%CI: 0.20~0.53), higher job-related stress (AOR: 2.57, 95%CI: 1.68~3.93, workplace violence experience (AOR: 4.26; 95%CI: 2.88~6.30), and job insecurity experience (AOR: 1.90, 95%CI: 1.18~3.05) were significantly related to job-related depression/anxiety in small-sized company employees. Conclusion: Results of this study suggest that job insecurity experience was significantly related to job-related depression/anxiety in small-sized company employees but not in large-sized company employees. Therefore, small-sized company workers who have experienced feeling of job insecurity are vulnerable population in terms of job-related depression/anxiety.

집단심상치료가 혈액투석환자의 안위와 우울에 미치는 효과 (The Effects of Group Imago Psychotherapy on Comfort and Depression of Patients with Hemodialysis)

  • The hemodialysis patients with chronic renal disease have experienced negative emotions, especially depression among with physical, social, and psychological changes. Based upon a stress-coping theory, group imago psychotherapy which can induce effective coping through self awareness and positive emotional responses is implemented to the hemodialysis patients. The effects of the imago psychotherapy in regards to comfort and depression are studied here. Group imago psychotherapy was performed on forty-three subjects(twenty subjects in the experimental group and twenty-three subjects in the control group). The results of the study were as follows. After being given group imago psychotherapy, the comfort scores of the experimental group were significantly higher than those of the control group (F=15.33, p= .003). Moreover, after being given treatment, the depression scores of the experimental group were significantly lower than those of the control group (F=9.14. p=.0044). Specifically, the scores on comfort in the experimental group under emotion-focused coping style were significantly higher than those of the control group (F=18.59, p= .0002). The mean difference on comfort scores in the experimental group under problem - focused coping style was higher than that of the control group. But their scores were not significant (F=0.19, p= .6729). The scores on depression in the experimental group under emotion-focused coping style were significantly lower than those of the control group (F=14.62, p= .0006). The mean difference on depression scores in the experimental group under problem - focused coping style was much lower than that of the control group. But their scores were not significant (F=0.31, p=.5947). There was a significant positive correlation between comfort and depression variables. After group imago psychotherapy the hemodialysis patients recognized positive changes in emotional reponses, self awareness, self control, ease of mind, and felt overall more relaxed. Imago psychotherapy is a nursing intervention which as this study has shown can improve to comfort. The results of this study can be applied to general nursing practices. In the view of holistic nursing, the development of the nursing practice combined with imago psychotherapy will contribute to the enlargement of the nursing field with conventional nursing practices.

쪽방지역 주거기간과 우울증상 경험과의 관련성 (Association between period of living alone in a shabby one-room and depression experience)

  • Purpose: This study aims to substantiate a correlation between the period of living alone in a shabby one-room and a depression experience. Methods: We conducted a cross-sectional study using data from the Korea Homeless Survey carried out by the Korea Institute for Health and Social Affairs in 2016. In the study, the data included 485 survey participants who resided in a single room. The period of single-room occupancy being divided into three groups: less than 1 year, 1 to 4 years, and more than 5 years was considered a primary independent variable. The outcome variable which represented a depression experience was measured by CESD-11(The Center for Epidemiological Studies-Depression Scale). We carried out a Chi-square test and multiple logistic regression to evaluate the association. Results: The rate of depression experienced in this population was 77.5%. Almost 83.9% of the participants were reported as being unable to receive the mental health counselling in the last 12 months. After controlling for covariates, subjects who were living in a single room occupancy for 1-5 years(OR=3.89; 95% CI=1.43-10.56) and more than 5 years(OR=2.71; 95% CI= 1.05-6.95) were found to be more likely to experience depression. Conclusions: The results showed that living alone in a shabby one-room had a high correlation with experiencing depression. We proposed that there is a need to develop a new strategy for mental health promotion program for the poor, vulnerable and deprived population.

임부의 분만 자신감, 산전 우울, 분만 지식과 배우자 지지는 분만 두려움에 영향을 미치는가? (Do childbirth confidence, prenatal depression, childbirth knowledge, and spousal support influence childbirth fear in pregnant women?)

  • Purpose: This study investigated levels of childbirth fear and related prenatal factors (self-confidence for childbirth, prenatal depression, knowledge about childbirth, and spousal support) among pregnant women in South Korea. Methods: A correlational study design was used to explore levels of childbirth fear and related prenatal factors in 200 pregnant women over 28 weeks of gestation. A self-administered questionnaire was used to measure fear of childbirth and related factors, such as self-confidence for childbirth, prenatal depression, knowledge about childbirth, and spousal support. Results: One-third of the pregnant women were aged 35 years and older. Sixty-one percent of women were nulliparae, but only 26.0% had experienced prenatal education. The mean score for fear of childbirth was 66.99 out of 165. The prevalence of fear of childbirth was 72.0%, and childbirth fear was severe in 26.5% of the participants and moderate in 45.5%. Fear of childbirth was negatively related to self-confidence (r=-.45, p<.001), but positively related to prenatal depression (r=.21, p=.002). Two significant predictors were found to explain the fear of childbirth. Higher self-confidence for childbirth was associated with less severe fear of childbirth (β=-.44, p<.001), while higher prenatal depression was associated with more severe fear of childbirth (β=.13, p=.038). Conclusion: The level of fear of childbirth was higher among pregnant women with lower self-confidence and higher prenatal depression. Reasonable evidence should be provided for implementing prenatal and childbirth classes to reduce pregnant women's depression and to increase their confidence.

The Long-Term Effects of Familial Difficulties Experienced in Childhood: Predictors of Internalizing Behavior Problems during the Early Adolescent Period and Late Life Periods

  • This study utilized data (a sample of 18,553 people born in 1958 in England, Scotland, and Wales) from the National Child Development Study of 1968 and 1991 to explore the influence of familial difficulties on the internalizing behavioral patterns during the early adolescent period and late life outcomes periods. In this paper, internalizing behavioral problems include 'depression', 'anxiety', 'hostility to adults', 'hostility to children', and 'withdrawal'. Late outcomes were analyzed in two different variables and one marital management domain: 'unemployment', 'seen doctors about emotional problems', 'divorce or separation; never lived as a couple; arguments end in violent behavior' The results indicate that young adolescents who had experienced familial difficulties also have internalizing behavioral problems giving them emotional and behavioral instability. The findings also show that familial difficulties during childhood positively contribute to late life outcomes such as unemployment, emotional problems, and marital management. This study suggests that in order to effectively respond to the needs of children and adolescents who have experienced various familial difficulties, counselors and educators must guide parents.

친구의 자살이 청소년의 정신 건강에 미치는 영향 (Impact of Peer's Suicide on Mental Health of Adolescents)

  • Objectives : This study examined the impact of peer's suicide on mental health of middle-school students. The aim of this study was to describe the course of posttraumatic stress and grief reaction, suicide ideation, and depression score among adolescents after exposure to peer's suicide and to examine the relationship between posttraumatic stress and other mental health scores in these subjects. Methods : Thirty seven middle school students who were exposed to the suicidal death of a peer completed self-report measures to assess levels of depression, grief reaction, post-traumatic stress, and suicide ideation at 1 month and 8 months after the peer's suicide. Results : There was no significant change in mental health scores between 1 month and 8 months. Level of posttraumatic stress after 8 months was related to acute grief response. Subjects who experienced a clinically significant level of suicidal ideation and posttraumatic stress showed more mental health problems. Conclusion : The results suggested that peer's suicide strongly impacted mental health issues of adolescents at a critical time of adolescent development. Clinicians should perform a thorough evaluation of mental health problems for youth who experienced peer's suicide and help them in management of their grief reaction.
