The effect of Problem-Based Learning and Simulation Practice Convergence Education for Nursing Students (간호대학생의 문제중심학습과 시뮬레이션 실습 융합교육의 효과)

  • The purpose of this study is to investigate the effect of simulation practice combining problem-based learning on nursing knowledge, self-confidence, critical thinking tendency and problem solving ability. The subjects of this study were 45 students who took two courses of nursing situation practice in the fourth grade of S university in S area. Data were collected before and after the simulation exercise using the self-report questionnaire. The problem-based learning and the simulation practice convergence training were conducted for three weeks with two hours per week, and the post- integration nursing knowledge, self-confidence, critical thinking disposition, and problem solving ability scores were improved. Nursing knowledge, self-confidence, and critical thinking tendency were positively correlated with problem solving ability and proved the effectiveness of problem-based learning and simulation training. Therefore, education programs combining problem-based learning and simulation training on various topics should be developed and utilized.

Effect of Neck Exercise with PNF for Dysphagia and Pain of Neck in Patients with Stroke (PNF를 이용한 경부 운동이 뇌졸중 환자의 연하기능과 경부 통증에 미치는 영향)

  • Purpose: This study aimed to investigate the effect of neck exercises using proprioceptive neuromuscular facilitation (PNF) on dysphagia and neck pain in stroke patients. Methods: This study included 20 patients with stroke who performed the following neck exercises using PNF: flexion, lateral flexion, and rotation. The PNF technique was repeated under the guidance of verbal commands from a physical therapist. The exercise program consisted of 5 times, 30-minute sessions each week for four weeks. The Mann Assessment of Swallowing Ability (MASA) and the visual analogue scale (VAS) were used to evaluate the patients before and after the training. Swallowing ability was evaluated using MASA, and the patients' pain was evaluated using VAS. All data were analyzed using SPSS 21.0. Results: Significant differences were observed for the MASA and VAS in the subjects: MASA was significantly improved (p<0.00) and VAS was significantlyincreased (p<0.00). Conclusion: The neck is a very important part of the body in stroke patients. Neck exercises using PNF are effective for improving dysphagia and decreasing neck pain in patients with stroke. Thus, neck exercises using PNF are very effective for stroke patients, and this approach would be effective in clinical practice.

  • Active and therapeutic sport recovery is becoming a popular and important component in approving performance for pro and recreational athletes alike. It is also helping in the realm of injury prevention. In the search of finding modalities that are widely effective, natural, and safe, acupuncture is a viable and cost-effective treatment for helping athletes achieve this goal. More direct related research is needed, but testimonials from pro athletes and the body of research that currently exists provides powerful evidence on acupunctures ability to help with enhancing recovery. Specializing in acupuncture and exercise science, Chris integrates acupuncture into musculoskeletal rehabilitation therapy or fitness training for pain modulation, speedy recovery, and enhanced performance. Clients can choose to focus on one-on-one corrective exercise therapy, manual and massage therapy, or acupuncture. However, for best results, Chris recommends all three. Other modalities that he uses in therapy are acu-taping, herbal therapy, nutrition supplementation, cupping, guasha, and stretching techniques. Effect of Knee Stabilization Exercise on Balance and Walking Ability in Patients with Total Knee Replacement (무릎관절 전치환술 환자에게 적용한 무릎 안정화 운동이 균형과 보행능력에 미치는 영향)

  • Background: Degenerative arthritis accounts for a large portion of the elderly, causing a lot of inconvenience in daily life. Total knee replacement (TKR) are performed to relieve pain in the knee joint. The purpose of this study was to determine whether knee joint stabilization exercises are effective in improving balance and walking ability in degenerative arthritis patients who have undergone TKR. Methods: A total of 30 TKR patients participated in this study. They were assigned to two groups. The experimental group, the group that underwent knee stabilization exercises, joint mobilization and general physical therapy. On the other hand, and the controlled group, the group that underwent joint mobilization and general physical therapy. All exercises were conducted thrice a week, for four weeks. The main balance outcomes were evaluated using the posture balance training system, while walking ability was assessed using a wireless 3-axis accelerometer. Results: The experimental group had significantly higher scores in postural stability testing (PST), limits stability testing, and waling ability. The post-intervention evaluation, there were significant differences in all variables between the two groups. However, no significant difference was noted in the foam eyes closed test in PST. Conclusion: This study suggests that knee joint stabilization exercises effectively improve the balance and walking ability of TKR patients. Thus, it could be presented as useful in clinical practice.

A Study on the Changes of Urinary Hormonal Excretion and Renal Function During Three-shift Nursing Practice (일일 3교대 간호활동시 호르몬분비 및 신장기능의 변화에 관한 연구 -간호학생을 대상으로-)

  • The sympathico-adrenergic system and the hypophyseal-adrenocortical system mediates the regulation of the internal milieu. And the kidneys regulate both water and electrolyte balance of the body fluid. The kidneys are the sites of production of renin which participate indirectly in maintaining renin. angiotensin-aldosterone system. These system de-serve special attention in the context of adjustment the effects on the body function. And so, maximal exercise and work load are associated with home-osthetic function. The nurses working in the hospital have been complained of fatigue and stress by frequent duty changes and overload. In order to define this, the possible changes of hormonal excretion during three-shift nursing practice were investigated. Urine samples were collected at pre-duty and post-duty, and were measured with chemical assay and radioimmunoassay in 30 nursing students, in nursing practice and 43 nursing students, in studying. The results obtained were as follows. 1. In nursing practice, urinary norepinephrine concentration showed a marked increase during day duty, urinary cortisol concentration showed a marked increase during evening duty, and urinary renin concentration was increased in night duty, 2. Corrected ratio of urinary sodium excreted by the urinary excretion of creatinin (UNa/UCr) and UCl/UCr showed a marked decrease during night duty. Nursing practice did not affect on the UK/UCr and urinary concentrating ability. From these results, it is suggested that further studies the define the effects on some physiological function of the three-shift nursing practice against circadian rhythm are needed for better working condition of nurses.

Gynostemma pentaphyllum extract and its active component gypenoside L improve the exercise performance of treadmill-trained mice

  • BACKGROUND/OBJECTIVES: The effectiveness of natural compounds in improving athletic ability has attracted attention in both sports and research. Gynostemma pentaphyllum (Thunb.) leaves are used to make traditional herbal medicines in Asia. The active components of G. pentaphyllum, dammarane saponins, or gypenosides, possess a range of biological activities. On the other hand, the anti-fatigue effects from G. pentaphyllum extract (GPE) and its effective compound, gypenoside L (GL), remain to be determined. MATERIALS/METHODS: This study examined the effects of GPE on fatigue and exercise performance in ICR mice. GPE was administered orally to mice for 6 weeks, with or without treadmill training. The biochemical analysis in serum, glycogen content, mRNA, and protein expressions of the liver and muscle were analyzed. RESULTS: The ExGPE (exercise with 300 mg/kg body weight/day of GPE) mice decreased the fat mass percentage significantly compared to the ExC mice, while the ExGPE showed the greatest lean mass percentage compared to the ExC group. The administration of GPE improved the exercise endurance and capacity in treadmill-trained mice, increased glucose and triglycerides, and decreased the serum creatine kinase and lactate levels after intensive exercise. The muscle glycogen levels were higher in the ExGPE group than the ExC group. GPE increased the level of mitochondrial biogenesis by enhancing the phosphorylation of peroxisome proliferator-activated receptor-γ coactivator-1α (PGC-1α) protein and the mRNA expression of nuclear respiratory factor 1, mitochondrial DNA, peroxisome proliferator-activated receptor-δ, superoxide dismutase 2, and by decreasing the lactate dehydrogenase B level in the soleus muscle (SOL). GPE also improved PGC-1α activation in the SOL significantly through AMPK/p38 phosphorylation. CONCLUSIONS: These results showed that GPE supplementation enhances exercise performance and has anti-fatigue activity. In addition, the underlying molecular mechanism was elucidated. Therefore, GPE is a promising candidate for developing functional foods and enhancing the exercise capacity and anti-fatigue activity.

Effects of Abdominal Exercise Methods on Breathing Ability (복부 운동 방법에 따른 호흡기능 변화 비교)

  • Purpose : The enhancement of abdominal muscles increases the activation and contraction of respiratory muscles, including the diaphragm. Generally, diaphragm exercises are applied to increase the breathing ability of patients with respiratory disease. Previous studies have shown that breathing capacity can be increased through abdominal muscle strengthening exercises. However, studies on breathing ability are rare and it is doubtful whether these affect respiratory ability more than diaphragm exercises. Therefore, this study seeks to compare whether abdominal exercises can improve breathing ability and whether any increase is comparable to diaphragm exercises. Methods : After selecting subjects, the place of intervention was separated for blindness. The plank group was allowed to relax for 30 seconds after 30 seconds of planking; this was set at three and increased by one set each week. Subjects in the draw-in group were allowed to relax for 30 seconds after maintaining the draw-in contraction state for 30 seconds and this was done for 15 minutes. Subjects in the control group underwent abdominal dilation for five seconds of inspiration time and expired air for five seconds by exposing the lips; breathing was performed repeatedly for 15 minutes. Subjects in each group measured their respiration function three times before intervention, three weeks after the commencement of intervention and after intervention. Spirovit SP-1 was used to measure respiratory function. In each group, repeated ANOVA was used to compare the respiratory function over time and one-way ANOVA was used to compare the respiratory function between groups. The post hoc was conducted using the LSD method. Results : There was a significant increase in respiratory ability between the forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and peak expiratory flow (PEF), forced expiratory volume in one second (FEV1)/forced vital capacity (FVC) according to the six-week period. However, there was no difference between each group. Conclusion : For patients with low respiratory muscle strength, plank exercises and abdominal draw-in are beneficial exercises for improving respiratory function. These are expected to be widely used in clinical practice for patients with weak respiratory muscles.

Effects of Squat Exercise Combined with Whole-Body Vibration on the Pulmonary Function and the Quadriceps Femoris Activity of Patients with Severe Chronic Obstructive Pulmonary Disease (전신진동을 결합한 스쿼트운동이 중증 만성폐쇄성폐질환 환자의 폐기능과 넙다리네갈래근 활성도에 미치는 영향)

  • PURPOSE: This study aimed to propose an exercise technique that helps improve the skeletal muscle function while suppressing the symptoms of respiratory distress, by mediating squat exercises in whole-body vibration for patients with severe COPD, and comparing the post intervention pulmonary function and activity of quadriceps. METHODS: Totally, 21 patients with severe COPD were randomly assigned to two groups through clinical sampling: experimental group I included 11 patients (Squat exercise combined with whole-body vibration exercise), and experimental group II included 10 patients (Only squat exercise). Before intervention, we measured pulmonary function using a pulmonary function tester, muscle activity of quadriceps using surface EMG, and gait ability using the 6MWT. RESULTS: Comparison of intra-group changes in both experimental groups showed a significant increase in the activity of rectus femoris, vastus medialis, and vastus lateralis, and also in the 6MWT. Intra-group comparisons also revealed significant difference in the activity of rectus femoris, vastus medialis, and vastus lateralis (p < .05). CONCLUSION: Squat exercise combined with whole-body vibration significantly increased the activity of the quadriceps muscle, suggesting that this intervention helps maintain the function of skeletal muscles and prevent muscle atrophy. Therefore, studies to develop protocols using whole body vibration in clinical practice as an exercise method can safely be performed in severe COPD patients, as considered necessary.

The Effects of Exercise Programs on Gait Ability of Elderly Dementia Patients (치매노인을 위한 운동프로그램이 보행능력에 미치는 영향)

  • The purpose of this study is to investigate the effects of the practice exercising and cycling on the gait abilities of dementia elderly patients. Forty forty elderly patients with dementia were participants. Participants were divided into three groups as A, B, and C, Group A was a standard group which had been treated with electric treatment, hot pack, and exercise. The second group B had been treated with cycling added to the treatments of group A. Finally, treatments of group C were arranged by subtraction of exercise from those of group A. The test was a 'timed up and go test' used for measuring the rates of the gait ability through four months. The gait ability of each group was measured after each month. The results of this study are as following: 1. After first month, the averages of changes in gait ability of group C, A and B were -1.69, -1.67, and -1.13 seconds respectively. That means that dementia control was achieved significantly in group A and B (p<0.05)whereas it was not in group C. 2. The averages after two months were -4.00 seconds for group C, -2.60 seconds for group A, and -1.56 seconds for B, respectively. All groups acquired significant effects in treating dementia (p<0.05). 3. After three months, -6.38, -3.00 and -2.31 seconds were average values of group C, A and B respectively. Same as results after two months, there were significant effects in all groups (p<0.05). 4. After four months, the averages of changes in gait ability of three groups were -8.00. -3.93 and -3.00 seconds for group C, A and B respectively. In the all patients treated analysis, treatments showed significant results (p<0.05). 5. Compared with each other through four months. three groups testing the timed up and go test showed more efficient in maintaining gait ability, in order of group B. A, and C

Factors Influencing the Work Ability of Building Construction Field Managers (건축공사 현장관리자 작업능력에 영향을 미치는 요인)

  • The success of construction projects is heavily dependent on the work ability of the field managers who manage the execution of projects. The work ability of field managers is influenced by various elements such as job stress and individual characteristics. The purpose of this study is to reveal the factors influencing the work ability of construction field managers in Korea. Work ability is evaluated bases on the Work Ability Index(WAI) developed by the Finnish Institute of Occupational Health, while job stress is measured using the Korean Occupational Stress Questionnaire Short Form developed by the Korean Occupational Safety & Health Agency. The results showed that field managers under higher job stress have lower work abilities. In addition, it is revealed that exercise or religious practice have a positive influence on work ability. This study will help to increase the work ability and enable the efficient management of organization at building construction sites.
