Effect of Social Exclusion on Depression and Suicidal Ideation among the Elderly in Korea: Focus on Gender Differences (노인단독가구 노인의 사회적 배제, 우울 및 자살생각 : 성 차이를 중심으로)

  • The ratio of suicide among the elderly in Korea has nearly tripled within the last 20 years. With this in mind, this study examined the effects of social exclusion on depression and suicidal ideation among the elderly living in elderly-only households with a focus on gender differences. Social exclusion is composed of economic, health, housing, education, social participation, and relational aspects. For this study, data were gathered from 527 participants aged 65 years and over. The statistical methods used for data analysis were descriptive statistics, chi-square test, t-test, and hierarchical regression analysis with the SPSS Win 20.0 program. The results of this study are summarized as follows: First, more than 80% of the elderly experienced social exclusion. Women were shown to be more vulnerable than men in terms of housing exclusion, educational exclusion, health exclusion, and multiple exclusion, whereas men were more vulnerable than women in terms of social participation exclusion. Second, social exclusion had significant effects on suicidal ideation among the Korean elderly. We also observed gender differences in the results. Regression analysis of suicidal ideation showed that relational exclusion had a significant effect on suicidal ideation in men, whereas health exclusion had a significant effect on suicidal ideation in women. Results of the study show that stronger social support networks and policies focusing on the elderly are needed in order to reduce elderly suicide in Korea.

  • In this paper, short-term power demand forecast using exclusion of week periodicity presented. Week periodicity excluded from weekday change ratio. Forecast term of five and multiple regression model of the three form was composed. Forecast result was good. Therefore, It Could be the power demand forecast of special day(weekend). This method may contribute improvement of forecast accuracy.

Development of Composite Deprivation Index for Korea: The Correlation with Standardized Mortality Ratio (표준화사망비와 지역결핍지수의 상관관계: 지역사회 통합결핍지수 개발)

  • Objectives : The aims of this paper were to develop the composite deprivation index (CDI) for the sub-district (Eup-Myen-Dong) levels based on the theory of social exclusion and to explore the relationship between the CDI and the standardized mortality ratio (SMR). Methods : The paper calculated the age adjusted SMR and we included five dimensions of social exclusion for CDI; unemployment, poverty, housing, labor and social network. The proxy variables of the five dimensions were the proportion of unemployed males, the percent of recipients receiving National Basic Livelihood Security Act benefits, the proportion of households under the minimum housing standard, the proportion of people with a low social class and the proportion of single-parent household. All the variables were standardized using geometric transformation and then we summed up them for a single index. The paper utilized the 2004-2006 National Death Registry data, the 2003-2006 national residents' registration data, the 2005 Population Census data and the 2005-2006 means-tested benefit recipients' data. Results : The figures were 115.6, 105.8 and 105.1 for the CDI of metropolitan areas (big cities), middle size cities and rural areas, respectively. The distributional variation of the CDI was the highest in metropolitan areas (8.9 - 353.7) and the lowest was in the rural areas (26.8 - 209.7). The extent and relative differences of deprivation increased with urbanization. Compared to the Townsend and Carstairs index, the CDI better represented the characteristics of rural deprivation. The correlation with the SMR was statistically significant and the direction of the CDI effects on the SMR was in accordance with that of the previous studies. Conclusions : The study findings indicated mortality inequalities due to the difference in the CDI. Despite the attempt to improve deprivation measures, further research is warranted for the consensus development of a deprivation index.

With fertilizer ingredients in pig sludge chemical qualities, the content of the nitrogen and the phosphoric acid comes 4.4% to be 6.29%, pH 7.02 and content of the copper and the zinc which is a heavy metal which contains in pig sludge with 805 mg/kg and 1,704 mg/kg, it is a restrictive standard of the fertilizer, 300 mg/kg and 900 mg/kg it sees to be high, it manufactures citric acid 1 hydrate with the organic acid solution, heavy metals of pig sludge where it is a mixture ratio of the organic acid solution, it divides to 25%, 50%, 75% and 100% 4 kind levels, the result which measures the heavy metal exclusion ratio of the copper and the zinc, the mixture ratio of the organic acid solution to be many exclusion ratio of the copper and the zinc is showing a just interrelation, from organic acid solution 100% level content of pig sludge remains copper and zinc 330.03 mg/kg and 41.28 mg/kg, it shows the exclusion ratio of copper 59% zinc 97%. 'Cheonganok' growth characteristics with citric acid 1 hydrate, Treatment 2 and control growth characteristics etc, it exclusion the copper and the zinc it doesn't appear on significant difference statistically but, treatment 3 after only pig sludge in resources disposal where it seeding, growth characteristics of leaf area etc. is badness, it compares in control and treatment 2 the growth characteristics badness, it is appearing, it is caused by with disease and insects occurrence of $Ostrinia$ $furnacalis$ and brown spot, the damage was many. From forage value, Treatment 2 where it exclusion the heavy metal with the citric acid 1 hydrate with control it compares and there are not significant difference from crude protein and ADF and NDF contents etc., seeding only Pig Sludge in resources disposal treatment 3, it is caused by with $Ostrinia$ $furnacalis$ etc., trunk and aging of the leaf to be high ADF content is low. but from crude protein, the nitrogen ingredient which pig sludge has and interrelation it seemed and high numerical value were confirmed.

  • There are various transversal structures (small dams or drop structures) in median and small streams in Korea. Most of them are concrete structures and it is so hard to exclude low-level water. Unless drainage valves and/or gates would not be installed near bottom of bed, sediment from upstream should be deposited and also contaminants attached to the sediments would devastatingly threaten the water quality and ecosystem. One of countermeasures for such problem is the bypass pipe installed underneath the transversal structure. However, there is still issued whether it would be workable if the gravels and/or stones would roll into and be not excluded. Therefore, in this study, the conditions to exclude the rip stone which enter into the bypass pipe was reviewed. Based on sediment transport phenomenon, the behavior of stones was investigated with the concepts from the critical shear stress of sediment and d'Alembert principle. As final results, the basis condition (${\tau}_c{^*}$) was derived using the Lagrangian description since the stones are in the moving state, not in the stationary state. From hydraulic experiments the relative velocity could be obtained. In order to minimize the scale effect, the extra wide channel of 5.0 m wide and 1.0 m high was constructed and the experimental stones were fully spherical ones. Experimental results showed that the ratio of flow velocity to spherical particle velocity was measured between 0.5 and 0.7, and this result was substituted into the suggested equation to identify the critical condition wether the stones were excluded. Regimes about the exclusion of stone in bypass pipe were divided into three types according to particle Reynolds number ($Re_p$) and dimensionless critical shear force (${\tau}_c{^*}$) - exclusion section, probabilistic exclusion section, no exclusion section. Results from this study would be useful and essential information for bypass pipe design in transveral structures.

Risk Factors for Gallstone Formation after Surgery for Gastric Cancer

  • Purpose: The incidence of gallstones after gastrectomy for gastric cancer is higher than in the general population. However, the causes and mechanisms of post-gastrectomy gallstones are unclear. The aim of this study was to evaluate the incidence of gallstone formation and the risk factors for their development after gastrectomy for gastric cancer. Materials and Methods: Of 1,744 gastric cancer patients who underwent gastrectomy at Seoul National University Bundang Hospital between January 2010 and December 2012, 1,284 were included in this study and retrospectively reviewed. Patients' age, sex, body mass index (BMI), tumor location, stage, type of gastrectomy, type of reconstruction, and extent of node dissection were evaluated. Results: The incidence of gallstones after gastrectomy for gastric cancer was significantly higher in men than in women (P=0.019). Exclusion of the duodenum during reconstruction was associated with a significantly higher incidence of gallstones (P=0.003). Overweight and obese patients with BMI ${\geq}23kg/m^2$ had significantly higher incidence of gallstones than those with a lower BMI (P=0.006). Multivariate analysis showed that obesity (hazard ratio, HR=1.614; 95% confidence interval, CI: 1.135~2.296; P=0.008), male sex (HR=1.515, 95% CI: 1.029~2.231, P=0.033), and exclusion of the duodenum (HR=1.648, 95% CI: 1.192~2.280, P=0.003) were significant, independent risk factors for gallstones after gastrectomy. Conclusions: The cumulative incidence of gallstones for 5 years after gastrectomy was 15.3%. Male sex, obesity, and exclusion of the duodenum were risk factors for gallstone formation after gastrectomy. Careful surveillance will be required for these patient groups after gastrectomy.

Effects of Ixeris dentata Extract on Radical Oxygen Species and Bcl-2 Family in Human Breast Cancer Cells (씀바귀 추출물이 인체유방암세포의 활성 산소 및 Bcl-2 Family에 미치는 영향)

  • The aim of the study was to determine the effects of Ixeris dentata extract (IDE) on anticancer activity in human breast cancer MDA-MB-231 cells at both cellular and molecular levels. The cells were cultured in the presence of 0, 20, 30 and $40{\mu}g/mL$ Ixeris dentata extract for 24 hours, respectively. At the end of culture, cytochemical analyses for MTT activity, trypan blue dye exclusion, Annexin V-FITC Apoptosis, and radical oxygen species (ROS) were conducted. RT-PCR was also performed to determine whether or not alterations in cell viability affect the Bax/Bcl-2 ratio. MTT assay showed that relative cell viability decreased in a dose-dependent manner (p<0.05). Reduction of cell viability matched well with increased cell membrane permeability as determined by trypan blue dye exclusion test (p<0.05). The rates of intracellular ROS also increased in a similar manner to those of TB-stained cells. There was an associated shift of apoptotic cells from early to late apoptosis between the 30 and $40{\mu}g/mL$. Bax/Bcl-2 ratio significantly increased along with significant decreases in Bcl-2 expression between 30 and $40{\mu}g/mL$ groups (p<0.05). In conclusion, anticancer activity of Ixeris dentata extract is modulated by a reduction in cell viability along with increased membrane permeability, leading to ROS accumulation within cells, and subsequently cell death through an apoptotic pathway that involves Bax and Bcl-2 in human breast cancer MDA-MB-231 cells.

Effects of Various Addition and Exclusion Time of Glucose on Development of Mouse Two-Cell Embryos

  • This study was conducted to investigate the effect of various addition and exclusion time of glucose (Control: no addition, A: 24~72 h, B: 24~48 h, C: 48~72 h, D: 0~72 h, E: 0~48 h, F: 0~24 h and 48~72 h, G: 0~24 h) on embryonic developmental capacity of 2-cell embryos in mice. Developed blastocysts were assessed for mean cell number by differential staining. The zona-intact blastocyst (ZiB) rates were higher (p<0.05) in group B than control. However, the zona-escape blastocyst (ZeB) rates were not significantly different in all groups. At 72 h, total blastocyst (ZiB + ZeB) formation rates were not significantly different in all groups. The mean cell number was not significantly different among all groups. The inner cell mass (ICM) cell number was higher (p<0.05) in group F than control, group A, B and G. The trophectoderm (TE) cell number was higher (p<0.05) in control than group A and D. The %ICM was higher (p<0.05) in group C, D and F than control. The ICM : TE ratio was not significantly different in all groups. Between control and glucose group, no significant difference was observed in the total blastocysts (ZiB + ZeB) formation rates. Also, no significant difference was observed in the mean cell number, ICM cell number and ICM : TE ratio. However the TE cell number was higher (p<0.05) in control than glucose group and %ICM was higher (p<0.05) in glucose group than control. In conclusion, glucose added in culture medium was not inhibitory on blastocyst formation but glucose added for 48 ~72 h in culture medium increases %ICM of blastocysts in mice.

Determining Major Items of Scheduled Maintenance for Apartment through Case Data: Focused on Improving the Establishment Standard for Long-term Repair Program (사례 데이터 분석을 통한 공동주택 계획수선 주요 항목 도출: 법령 상의 장기수선계획 수립기준 현실화를 중심으로)

  • The long-term repair program is significant as the execution standard for apartment in repair construction, and provides the basis for effective facility management through appropriation reserve and preventive maintenance. Considering its importance, the government legislates the items, cycle, and ratio for long-term repair. However, the frequent changes, controversies on repair, and difficulties in using appropriation reserve are caused as the items have not been revised on time reflecting up-to-date construction technologies. In order to resolve these problems, it is required to improve the standard by evaluating the appropriateness of repair items throughout the analysis of actual conditions. This study aims to suggest the revision plan for the criteria with the major items based on the status identified by the actual long-term repair programs and historical repair construction data. To do this, the adoption ratios of items to program are reviewed by collecting the real cases of the long-term repair programs. Developing improvement plan was approached in two ways such as "exclusion method" deleting minimum unnecessary items and "selection method" sorting out items with high adoption ratio and evidence of repair in a positive manner. Aa a result, the major items were identified as 118 items and 73 items by exclusion method and selection method respectively from the current 147 items. The outcomes of this study are expected to contribute enhancing the practicality of the standard in that the regulation for long-term repair program should periodically reflect the status of currently applied construction methods and materials.

Bile Duct Ligation and Insulin-like Growth Factor-I on the Ischemia-Reperfusion Injury of the Small Bowel (쥐에서 허혈-재관류 소장 손상에 대한 담관결찰 및 Insulin-like Growth Factor-I의 영향)

  • To determine whether bile juice exclusion can prevent the mucosal damage, and Insulin-like growth factor-I can promote mucosal regeneration in ischemia-reperfusion injury of the bowel, 39 weanling rats with 10 cm of Thiry-Vella loop were studied. Animal groups were; Control, BL(common bile duct ligation), IGF{insulin-like growth factor-I(IGF-I) infusion} and IGF-BL(combined treatment). IGF-I(1.5 mg/kg/day) was continuously delivered through a subcutaneously implanted miniosmotic pump. After 15 minutes of superior mesenteric artery clamping, a tissue specimen(P) was taken after 30 minutes of reperfusion. Intestinal continuity was restored to allow oral feeding. A specimen of main tract(M) and another of the Thiry-Vella loop(T) were collected for histomorphometry after 48 hours of reperfusion and free feeding. Villus size ratio(VSR), crypt depth(CD), crypt-depth/villus-height ratio(CVR) and injury score(IS) were measured in 15 consecutive villi. The postoperative mortalities of bile duct ligation groups(BL and IGF-BL) were higher than those of other groups. In control group, VSR of M was lower(P<0.05) than P or T, but not in the other groups. VSR of M in control was lower than those in other groups. CD of T in control, IGF and IGF-BL group were higher than those of M. CD of M and T showed gradual increments from control, IGF and IGF-BL group, respectively. CVR of M and T in IGF group were higher than those in control. CVR in IGF-BL group, T was higher than M, and M was higher than P. About IS, M of BL($20.1{\pm}2.5$) and IGF-BL($20.9{\pm}3.3$) groups were significantly lower than that of control($32.4{\pm}2.5$). These results suggest that the exclusion of bile juice reduces the severity of the reperfusion injury of the mucosa, by inability to activate pancreatic enzymes and IGF-I stimulates mucosal regeneration in injured bowel, and the effect is potentiated by bile juice exclusion.
