The effect of chewing difficulty on depression in Korean adults over aged 40 years : The 7th Korean National Health and Nutrition Examination survey (2016~2018) (제7기(2016~2018) 국민건강영양조사를 이용한 40세 이상 한국 성인의 저작불편감이 우울에 미치는 영향)

  • Objectives: The purpose of this study was to analyze the effect of chewing difficulty on depression in Korean adults to provide basic data on improving the quality of life related to oral health of Korean adults. Methods: The raw data of the analysis were obtained from the dataset of the seven Korea National Health and Nutrition Examination Survey conducted between 2016 and 2018. The subjects included 9,535 people who underwent oral examination and answered questions. The collected data were analyzed using the SPSS (ver 21.0) program using composite samples, and the mean crossover analysis and logistic return analysis were performed. Results: An analysis of the effects of chewing difficulty on depression in Korean adults showed that those with chewing difficulty had a 2.24 fold higher risk of depression (95% CI: 1.73-2.90) and a statistically significant difference (p<0.05). Conclusions: The association between the chewing difficulty on depression could be confirmed, and measures improve of the oral health and preventing depression should be prepared.

Prevalence Estimation of Several Chronic Diseases through Community Based Health Examination Survey (지역사회 주요 만성질환의 유병률 추정 - 구리시 '97 건강진단조사 결과를 기초로 -)

  • Objectives. The health examination survey was conducted in order to estimate the prevalence of major chronic diseases in a community Methods. A multi-stage stratified probability sample of 1,656 persons, aged from 20 to 69 years, was drawn out from Kuri City. All sampled persons were personally informed of the purpose, content and procedures of the survey and mobile examination center was run between 6 AM and 9 PM for the convenience of the participants. Results. 854 persons (339 males and 515 females) completed this survey with a response rate of 53.4%. Prevalent diseases in male were hypertension(19.0%), HBs Ag carrier(9.8%), impaired glucose tolerance(8.7%), diabetes mellitus(6.3%), hypercholesterolemia(3.4%), anemia(2.7%) and degenerative arthritis(2.6%) in order of prevalence rate. Prevalent diseases in female were osteoporosis(22.5%), impaired glucose tolerance(17.3%), anemia(12.8%), hypertension(12.4%), degenerative arthritis (6.6%), HBs Ag carrier(5.8%), diabetes mellitus(5.4%) and hypercholesterolemia(3.5%) in order of prevalence rate. Conclusions. This results suggest that hypertension was most prevalent in adult male whereas diabetes mellitus including IGT, osteoporosis were most prevalent in adult female, in Kuri City.

Analysis of the Difference in Nutrients Intake, Dietary Behaviors and Food Intake Frequency of Single- and Non Single-Person Households: The Korea National Health and Nutrition Examination Survey (KNHANES), 2014-2016 (1인가구와 다인가구의 영양소섭취, 식행동 및 식품섭취빈도에 대한 차이분석 : 제 6, 7기 국민건강영양조사(2014~2016)자료 활용)

  • Objectives: This study was performed to compare the dietary life of single- and non single-person households in the Korea National Health and Nutrition Examination Survey (KNHANES). Methods: A nationally representative sample of 20,421 19-64-year-olds who had 24-hour recall data was taken from the 2014-2016 Korea National Health and Nutrition Examination Survey (KNHANES). Single- and non single-persons were compared for nutrient intake, dietary behaviors, food consumption patterns, nutrition education and confirm nutrition label. Results: The dietary intakes of dietary fiber and iron were lower in single-person households than in non single-person households. The lower the level of education and income, the lower the nutrient intake of single-person households. In the case of those aged 19 to 29, the breakfast skipping rate was higher in single-person households than in non single-person households. The higher the education level, the higher the breakfast skipping rate and the eating out frequency in the single-person households. In the food intake survey, the frequency of healthy food intake in single-person households was much lower than that of non single-person households. The confirmation rate of nutrition labeling was lower in single-person households than in non single-person households. Conclusions: This study shows that single-person households have poorer health-nutritional behaviors than multi-person households. Therefore, a nutrition education program based on the data of this study needs to be developed for health promotion of single-person households.

Health Intended Food Use by Korean Adults: Data from the Korean National Health and Nutrition Examination Survey 2001 (2001년 국민건강영양조사 자료를 이용한 한국 성인의 건강증진용 식품 사용실태)

  • Data from the 2001 Korea Health and Nutrition Examination Survey, a nationally representative, cross-sectional survey, were analyzed to assess prevalence of health intended food use overall and in relation to socio-demographic and lifestyle characteristics. Health intended food were classified into four broadly defined categories: vitamin and mineral supplements, health aid food, animal protein based restorative food and medicinal plants. Thirty three percent of adults aged 20 years or older reported taking health intended food in the past year : twenty four percent took vitamin or mineral supplements and medicinal plants were taken by 17 % of adults, health aid food by 15% and animal protein based restorative food by 6%. In multivariate logistic regression, female gender, older age, more education, regular exercise and non smoking were associated with greater use of vitamin or mineral supplements. Dietary quality was significantly associated with use of vitamin or mineral supplements or health aid food but not with use of medicinal plants or animal protein based restorative foods. Obese persons were less likely to take vitamin or mineral supplements. These findings suggest that epidemiologic studies of diet, demographic or lifestyle and health take health intended food use into account because of high prevalence of use of the food by the population and differential use of the food by socio-demographic and lifestyle characteristics.

Comparison of customer satisfaction with health examination programs provided by the Korea National Health Insurance and private healthcare organizations in Korea (건강보험공단 건강검진과 자비 건강검진 수검자간의 만족도 비교)

  • Backgrouds : This study was performed to compare the satisfaction levels for health examination programs provided by between the Korea National Health Insurance Corporation (KNHIC) and private healthcare organizations in Korea. We investigated factors associated with the satisfaction level for the KNHIC health examination. Also, factors explaining for the additional receipt of private healthcare organization's examination were identified. Methods : A self-administered questionnaire survey was conducted in June 2004 with 250 beneficiaries of the KNHIC industrial workers' program and 100 beneficiaries of the KNHIC selfemployed program. A total of 297 completed the questionnaire, including 213 (85.2%) and 84 (84.0%) for each insurance program. Ten questions measuring satisfaction levels for each examination program were rated on a scale ranging from 1 (strongly unsatisfied) to 5 (strongly satisfied). Results : Among the subjects receiving both examinations, the mean satisfaction level for the KNHIC examination (3.07) was significantly lower than that for the private healthcare organization's examination (3.50) (p<0.05). The KNHIC examination showed lower satisfaction for all of the 10 items than the counterpart. In particular, the KNHIC examination had the scores of lower than 3.0 for the items reflecting post-examination management services. According to the multiple regression analysis results, the satisfaction for the KNHIC examination was positively associated with the positive attitude toward health examination(${\bullet}$,=0.38, p<0.00). The logistic regression results showed that the likelihood of receiving the private healthcare organization's examination in addition to the KNHIC examination increases as the respondents were less healthier(OR=0.29, 95% CI = 0.10¢¶0.84), the satisfaction level for the KNHIC examination decreased(OR=0.46, 0.28¢¶0.75) or the attitude toward health examination was more positive (OR=2.56, 1.31¢¶5.12). Conclusion : The relatively low satisfaction level for both examination programs suggests that there's ample room for improvement of health examination services in Korea. The negative association between additional receipt of private healthcare organization's examination and satisfaction level for the KNHIC examination implies that the improvement of the satisfaction for the KNHIC examination would help to reduce the national expenditure spent on the additional examination.

Inappropriate Survey Design Analysis of the Korean National Health and Nutrition Examination Survey May Produce Biased Results

  • Objectives: The inherent nature of the Korean National Health and Nutrition Examination Survey (KNHANES) design requires special analysis by incorporating sample weights, stratification, and clustering not used in ordinary statistical procedures. Methods: This study investigated the proportion of research papers that have used an appropriate statistical methodology out of the research papers analyzing the KNHANES cited in the PubMed online system from 2007 to 2012. We also compared differences in mean and regression estimates between the ordinary statistical data analyses without sampling weight and design-based data analyses using the KNHANES 2008 to 2010. Results: Of the 247 research articles cited in PubMed, only 19.8% of all articles used survey design analysis, compared with 80.2% of articles that used ordinary statistical analysis, treating KNHANES data as if it were collected using a simple random sampling method. Means and standard errors differed between the ordinary statistical data analyses and design-based analyses, and the standard errors in the design-based analyses tended to be larger than those in the ordinary statistical data analyses. Conclusions: Ignoring complex survey design can result in biased estimates and overstated significance levels. Sample weights, stratification, and clustering of the design must be incorporated into analyses to ensure the development of appropriate estimates and standard errors of these estimates.

Knowledge about the Examination System and Recognition for the Roles of Health Educator in Health College Students (보건계열 대학생의 보건교육사의 시험제도에 대한 지식과 역할에 대한 인식)

  • Purpose: This study was to survey the recognition for the roles of health educator and knowledge about the examination system in health college students at a university located in Chungnam. Methods: The participants were 140 health college students. The survey was conducted using a questionnaire consists of 10 items. The knowledge about the examination system was checked for 9 items with a five-point scale. Recognition for the roles of health educator was divided into four categories from know very well to do not know at all. The data were analyzed using SPSS 18.0 program. Results: In this study, the mean score of knowledge about the examination system was 1.95 points out of 5, and the mean score of recognition for the roles of health educator was 3.01 points out of 4. According to the general characteristics, there were statistically significant differences in their knowledge about the examination system for health educator regarding their gender (p=.034) and grade (p<.001). Recognition for the roles of health educator showed a statistically significant difference only in their grade (p<.001). Conclusion: These results provide the basic data to prepare the necessary measures for the revitalization and promotion of the health educators.

A Study of Health Behavior through Comparative Analysis of Self-perceived Health Status and Health Examination Results (주관적 건강인식과 건강검진 결과의 비교분석을 통한 건강행위 연구)

  • The purpose of this study is to analyze health behavior by comparing the difference between self-perceived health status and health examination results. The study subjects consist of 7,702 people aged over 20, surveyed by Health Interview survey, Health Examination survey, Dietary Life survey, Health Consciousness and Behavior survey. Data used in the study are drawn from raw data from a 1998 National Health and Nutrition survey. General characteristics variables are sex, age, education level, residential area, marital status, occupation, and living standard while dichotomous variables, ‘not healthy’ and ‘healthy’ are used to measure self-perceived health status. Variables for health examination results are high blood pressure, high cholesterol, diabetes, liver diseases, liver inflammation, kidney diseases, normal weight, regular diet, optimum sleeping time(7-8 hours), regular health examination and health behavior practice group. Major findings of the study are as follows: 1) Analysis of self-perceived health status and health behavior by disease: Variables significantly correlated with high self-perceived health status have strong associations with high health behavior practice, which supports the hypothesis that as one has high self-perceived health status, one is more likely to practice health promoting behavior. The results of analysis of health behavior differences by dividing subjects into two categories, ‘cases of illness’ and ‘cases of no illness’ indicate that drinking, sleeping time, health examination are significant variables (p〈0.001, 0.05) whereas smoking, weight control, regular exercise, regular diet are not significant. 2) Analysis of disparity patterns between self-perceived health status and health examination: The hypothesis that health behaviors would be different according to the disparity pattern between self-perceived health status and health examination is supported as a result of χ2 test. Among Type I : Self-perceived health status is high and actual health status is good (no disease) Type II: Self-perceived health status is high and actual health status is poor(have disease) Type III: Self-perceived health status is low and actual health status is good(no disease) Type IN: Self-perceived health status is low and actual health status is poor(have disease) Type I and Type IV show no disparity, Type I shows the highest health promoting behavior whereas Type IV shows the lowest health promoting behavior. Type II, and III, compared to Type I, practise lower health promoting behavior. Multi-logistics regression analysis was conducted to find out the degree of impact on health behavior. Independent variables are general characteristics, self-perceived health status and health examination result and presence of illness, while the dependent variable is health promoting behavior. The analysis of the impact of self-perceived health status on the health promoting behavior shows that smoking, drinking, weight control, regular exercise, health examination practice, and/or regular diet are significantly correlated to self-perceived health status. High self-perceived health status is inversely related to high health promoting behavior. This finding supports the hypothesis that the higher one perceives one's health, the more likely one is to practice health promoting behavior. On the contrary, the presence of illness has little impact on health promoting behavior. 3) Multiple logistics analysis on how disparity patterns between self-perceived health status and health examination affect health behavior: The results of multiple logistics analysis made on health behavior variables compared to the standard variable are as follows: When analyzed on the standard of Type I, smoking is a significant risk factor for the Type IV. In case of drinking, all the patterns show a high probability of relative risk ratio. With regard to weight control, it is a risk factor for Type II while all the patterns show high probability of not practising when analyzed on the standard of type IV. Type III and IV show high probability of not doing regular exercise while Type IV, shows a high probability of not taking appropriate sleeping time. When analyzed on the standard of type IV, all the patterns show a high probability of not taking health examinations. Type III and IV show a high probability of not having regular meals. As for overall health promoting behavior, Type III and IV show a high relative risk ratio. These two groups have low self-perceived health status. It implies that self-perceived health status has significant impact on health promoting behavior. This is also supported by the fact that Type I with high self-perceived health status and no illness shows a high practice rate of health promoting behavior. Types II and III the groups with high disparity between self-perceived health status and health examination results, show a low practice rate of health promoting behavior when compared to Type I. Type IV, that is the group with low self-perceived health status and actual illness, shows the lowest practice of health promoting behavior. It is highly probable that this type proves to be the poorest health group.

A Study on the Usefulness of an Examination Information Printout during Ordinary X-Ray Examinations (일반촬영 검사 시 검사안내 유인물의 유용성에 관한 고찰)

  • In a normal x-ray examination, the cooperation of a patient in positioning greatly influences the improvement of work performance, the quality of the images and the reduction in time spent on the examination. An attempt was made to discover the connection between an examination information printout and the effects of the examination by informing the patient before the examination on the examination positions in order to increase the level of cooperation by the patient and by surveying the degree of satisfaction of the patient during the examination proceedings. An examination information printout was prepared on five positions that patients consider to be difficult while receiving normal x-ray examinations and were shown to 257 male and female patients between the ages of 30 to 79 who had visited this hospital between January 1 to February 29, 2012 or to a person who had accompanied the patients before the examination to research the degree of satisfaction regarding the printout. In addition, the examination explanation printout was given to Radiologic Technologists with over 1 year experience in this hospital to use while taking the x-rays and the degree of satisfaction was surveyed and it was examined if the increase of the degree of satisfaction had an effect on the reduction in the examination time. The patients who utilized the examination information printout revealed results of approximately a 7.8% higher degree of satisfaction among the patients who had previous experience receiving x-ray examinations compared to those were having an x-ray for the first time. In the age groups between 30 to 70 years of age, the age group in the 30's revealed the lowest level of satisfaction at approximately 69% and the group in the 50's showed the highest degree of satisfaction of 87.94%. In the survey of the Radiologic Technologists, 85% responded that they were satisfied. In the question that stated "Will the examination information printouts reduce the time involved in unnecessary conversations with the patients?," the responses showed a 95% degree of satisfaction. The degree of satisfaction was high over the examination information printout by both the patients and the Radiologic Technologists. Although the effects of examination time reduction varied based on the ages of the patients and physical conditions that they were in and could not be given measurement values, it was able to be seen through the survey that reduction of the unnecessary communication between the patient and the Radiologic Technologists contributed to the increase in the degree of satisfaction. As a result, both the patients and the Radiologic Technologists had an increased degree of satisfaction over the examination information printout and it was possible for improvements to be made in the quality of medical service provided as well. Nevertheless, it is considered that a more developed manual in terms of quality and quantity must be produced using a more systematic approach and design.

The Influence of Preemployment Medical Examination, Pure Tone Audiometry, and Simple Lumbar Spine X-ray Test on the Worker's Employment - The Result of Survey at Incheon Metropolitan City and Gyeonggi Province in Korea, the Year 2003 - (채용시 건강진단과 순음청력검사 및 요추부 단순방사선 검사가 근로자 채용에 미치는 영향 - 인천, 경기 지역 2003년 실태 조사 -)

  • This study was conducted for investigating the status of management of preemployment health examination and to have an effect on the worker's employment. Health managers of 103 companies in Incheon metropolitan city and Gyeonggi were interviewed by telephone. Of 103 companies, 67(65.1%) said they don't hire the applicants who have an active pulmonary tuberculosis, 80(77.7%) companies said they health HBV carrier is acceptable but active HBV carrier is not 29(28.2%) companies said they don't hire the applicants who have a hypertension or diabetes mellitus, 42(40.8%) companies said they don't hire the applicants who have a hearing disturbance. If HIVD is suspicious in X-ray lumbar-sacral region, 37(78.7% of 47 companies) said they do not hire the applicants. 29(35% of 83 companies) said they cancel the employment of the applicants who are suspicious of noise induced hearing loss on preplacement health examination. From our survey, preemployment health examination was utilizing mainly as a tool for the selection of health employees who don't have a disease. Furthermore, in many companies, additional test items are being included and getting more strict the selection criteria for preemployment health examination. For the right use of preemployment health examination, author suggested that further studies were needed to select the adequate test items and establish the reasonable criteria for preemployment health examination.
