The Basic Design on Dispatch Life Support Pre-arrival Instruction Protocol (응급통신소생술 도착전지시 프로토콜 기본설계 방안)

  • As technology advances rapidly, emergency medical dispatching systems must also advance. Optimal emergency medical dispatching requires the courage to practice medicine at dispatch. Modern emergency medical dispatch provides appropriate resource responses with the use of an (Advanced Medical priority Dispatch System(AMPDS) in U.S.A. The AMPDS is a systematic protocol for all aspects of the dispatch process. Based on the AMPDS chief complaint list, we initiated a basic design on pre-arrival instruction protocol and completed the scripted pre-arrival instruction protocol, using Cardiopulmonary resuscitation(CPR) protocol. Implementation of this study creates computerized emergency medical dispatching system for future enhancements, such as the AMPDS.

A Strategic Study on National Disaster Medical System (국가재난의료체계에 대한 정책적 고찰)

  • Due to major disasters Korea has been damaged, and they caused lots of casualties: for last ten years natural disasters caused 1288 deaths including missing people; human disasters including industrial disasters brought as many as 4,512.148 casual ties (126,372 deaths with 4,385,400 injuries); and they cost 44.1 trillion property damage. However, even though major disasters have brought about tremendous human loss and property damage, Koreas National Disaster Medical System to rescue casualties is insufficient, and it has not been activated. Fortunately, through major disaster management process, the National Disaster Management System has been developed, increasing its own efficiency, and resulting in to organize an Office of Firefighting and Prevention of Disasters under the central government. Considering the value of human lives, the disaster medical part, in the U.S.A. as well as in Korea, must have an independent organization in the government, not as one sector of the government department. It will have its own organizational structure, such as disaster planning, operation, and logistics, and interact with central and local government or between local government agencies. So each agency will cooperate and supply resources interchangeably. Also, with the system of disaster management and restoration, the disaster medical system must be advanced in keeping step. Its role must be extended due to the possibility of biological terror or SARS around the world, resulting in severe casualties. Korea has the Emergency Medical Service System based on the regulation of emergency medical care, yet it is a part of the National Disaster Management System. It must be managed independently apart from it. As we see the emergency medical technicians playing as the backbone in disaster medical care in the US, we should have legal foundations for Koreas emergency medical technicians, emergency medical providers, to participate in rescue operation actively. At the same time, we need to have a national register system to classify disaster medical resources, and a total plan to place resources according to the impact of disaster, and how to organize teams. We also need to draw up a scheme to activate civil disaster medical resources, as integrating public and private or voluntary organizations.

How to cope with medical emergencies in the Dental Clinic (치과진료실에서 흔한 긴급상황의 처치)

  • Conceptually, the emergency is classified as an urgency and an emergency. The Urgency is not immediately life threatening, but could become so if not resolved promptly. So, it requires prompt patient care same as in the emergency situation. However, the emergency is immediately life threatening and requires immediate action, such as calling 119 and basic life support. Most medical emergencies in the dental clinic cases are urgencies. The incidence of true emergencies is approximately 1/1,000,000. Adequate managing medical urgencies are important because the chance of encountering medical emergencies in the dental clinic is high and higher these days especially because of rapidly aging Korean society. Many dentists indicate that many dentists feel difficulties to recognize and treat medical problems. This paper reviews the concept of medical emergencies and how to cope with commonly occurring urgencies in the dental clinic such as loss of consciousness, hypoglycemia, hyperventilation syndrome. The best treatment for medical urgencies and emergencies in the dental clinic is prevention. Also, it is required to make preparation for emergency situations such as CPR education for dentists and being well-acquainted with equipment and drugs for the emergency care as well as to systemic medical evaluation, patient monitoring, and sedations for controlling patient's anxiety and pain. In this paper, simple algorithms based on guidelines for common urgencies in the dental clinic are suggested. In conclusion, every dentist has competencies to do the urgency care adequately and basic life support. Also, advanced cardiac life support is strongly recommended when sedation is performed in the clinic.

Satisfaction Gaps among Physicians, Nurses, and Patient Family in the Emergency Department (응급실 서비스 만족도에 대한 환자 가족의 평가와 의료진의 인식 차이)

  • Background: The objective of this study was to explore patient family's evaluation of emergency department (ED) service satisfaction and to compare these with ED staff perception of patient family's evaluation. Methods: Based on two surveys of the National Emergency Medical Center: the 2008 National Survey for Recognition and Satisfaction towards Emergency Medical Services and the 2008 Opinion Survey of Emergency Medical Service Providers, satisfaction gaps among physicians, nurses, and patient family were evaluated by Kruskal-Wallis tests and Wilcoxon-Mann-Whitney tests. Furthermore, the factors associated with satisfaction of emergency medical service were identified by ordinal logistic regression models. Results: There were statistically significant gaps among physicians, nurses, and patient family in overall satisfaction with ED visit, length of stay in ED, enough explanation, physicians/nurses kindness, and ED facilities. Age and income in the patient family model, the number of beds in hospital, job satisfaction and year of service in the physicians model, and the number of beds in hospital, job satisfaction and the number of patients per duty hour in the nurses model were statistically significant factors associated with evaluation/ perception of ED service satisfaction. Conclusion: Patient satisfaction is an important indicator of the quality of care and service delivery in the ED. To improve and understand satisfaction in ED service, a dyadic view of the evaluation of service quality and satisfaction-that is, from the perspectives of both the patient and the emergency medical service providers-should be concerned.

Factors affecting use of the emergency department at superior hospitals on public holidays (상급종합병원 응급실 공휴일 이용의 영향 요인)

  • The purpose of this study was to identify the factors affecting the use of superior general hospitals on public holidays by using the medical use model (Dutton's medical use model) with the medical panel data. The study found that 34.2% of superior general hospital emergency rooms were used on public holidays and the factors which made statistically significant influences on the use of superior general hospital emergency rooms on public holidays were whether patients were operated or emergency care and inspection etc. Also, there was a difference depending on whether the type of establishment of the medical insititution is national or private. In other words, patients who received emergency care and examinations were found to make more frequent visit to hospital emergency room on pubic holiday, compared to patients who underwent surgery and those who visited emergency rooms in the private superior general hospital did so, compared to those who visited emergency room in the national general hospital(OR, 4.4, 3.386, respectively). Therefore, it is necessary to consider the introduction of integrated care of health and social care medical service that focuses on primary care in Denmark, which focuses on patients, and pre-The Canadian Triage and Acuity Scale(pre-CTAS) in the UK.

Analysis to Performance of Prehospital emergency care for cardiac arrest patient showing shockable rhythm on EKG (제세동 가능 심정지 환자의 병원전 응급처치 수행에 대한 분석)

  • Purpose : Prehospital emergency care for shockable rhythm is one of major concerns of emergency medical services. But, in Korea, prehospital medical service systems are not yet well established. We tried to offer one of the fundamental data to develop of these system. Method : After application of exclusion criteria, 200 patients who had shockable rhythm from January to December, 2008 were included in this study. Restrospective review of Prehospital care Reports of these patients was done. Result : Total 200 cases of shockable rhythm and prehospital arrest were analyzed. The rates of assessment of vital signs were 89.0%, the rate of level of consiousness was 99.5%. Just 6.0% were communicated with medical director providing the prehospital care. The frequency of defibrillation was performed 58.5%. Conclusion : Survival rate was higher in defibrillation group than that of nondefibrillation group(20.5% vs 2.4%, p=0.000).

  • The purpose of this study is to analyze the propriety of portable digital X-ray Equipment for Emergency Medical Services in Daejon Emergency Medical Center in Korea. The major instruments of this study were Korean Self-Analysis Opinionnaire, Questionnaire contains 35 items which measure emergency medical personal opnions. To take the analysis of data, the total of 92 persons were investigated in Medical Information Center in Daejon Metropolitan City from 2005. 20. April to 2004. 25. May. The data were analyzed by the path analysis SPSS program. First, portable X-RAY equipment is needed to apply it to emergency. Second, it should have small and light structure compared with old equipments and have high voltage generator unit for X-RAY using inverter. Third, it should be able to send the shot data that is digital detector type without film to doctors in emergency center.

  • During New year's day and Thanksgiving holiday, the 1339 Information Center in Gwangju and Jeonnam received an emergency medical situation and carried out the analysis as follows ; 1. The reason for the higher phone guidance(72%) of the hospital was based on the simple query for the pain control rather than emergency care. This was explained by the unfamiliar environment of the holiday movers. Pharmaceutical guidance(12.6%) in rural area was also given during the holidays. 2. The disease counselling(2.4%) and emergency treatment instruction(First Aid)(1.6%) decreased because of the small number of request for the diseases. This explained the decrease of the disease counselling. 3. The phone calls increased 8-13 times more than usual. Five-day holidays could have more emergency patients than three-day holidays. During New year's day and Thanksgiving day, the number of received phone calls was 6,444(25.7%) and this accounted for one fourth of the total number in 2007. 4. The number of the patients increased on New year's day and Thanksgiving day because most of the medical institutions and pharmacy were off duty. 5. The patients were centered in Internal medicine and pediatrics. This showed the food-borne diseases and air-borne infection such as common cold. 6. During Thanksgiving day, ophthalmologic patients having viral keratoconjunctivitis(Apollo eye disease) increased 8 folds than in New year's day. It was estimated from the continuous epidemic of Apollo eye disease. 7. There broke out the traffic accidents, food poisoning, infectious diseases, dermatological diseases due to seasonal and environmental changes during the holiday move.

The 2018 Fire department emergency medical technician survey (2018년 소방공무원 응급구조사 총조사)

  • Purpose: The 2018 General survey of emergency assistance was conducted to examine the working conditions and welfare, including educational direction, interests, and awareness of work, of the fire department emergency medical technicians (EMT). This would be used as basic data for future policy directions. Methods: Among the fire-fighting officers in 16 cities nationwide, emergency rescue workers engaged in first-aid activities were targeted. With prior consent, a survey was conducted through electronic documents. Of the total 1,227 people, responses from 1,151 were finally analyzed, excluding 76 who did not respond appropriately. Results: The working conditions and welfare of 119 firefighters were moderate, but in the fields of education and interest, the learning according to the regulations was high. In particular, satisfaction with the scope of work was found to be below average. However, it was positive that it will play a role as a social safety net in the future and will converge with cutting-edge science. Conclusion: Although this study was a total investigation of the EMT survey, conducting an EMT survey on all fire fighters in Korea is difficult. Further research is needed, particularly on first-class emergency medical personnel who play a major role in 119 paramedics.

Images of Hospital Emergency Medical Technicians Perceived by Emergency Medical Service Providers (Focusing on tertiary medical institutions in Daejeon and Chungcheongnam do) (응급의료종사자가 인식하는 병원 내 응급구조사의 이미지 (대전, 충남 3차의료기관 중심으로))

  • This study surveyed the image of hospital emergency medical technicians with 122 emergency medical service providers (doctors and nurses) working at tertiary medical institutions in Daejeon and Chungcheongnam.do in order to provide basic materials for enhancing the status and professional image of hospital emergency medical technicians. According to the results of this study, the overall image was $3.27{\pm}0.34$, and by its sub.area, professional image was highest and role image was lowest. By item, 'Maintain a friendly relations with doctors' was highest, and 'Arrogant and negligent in job performance' was lowest. In order to enhance their image, hospital emergency medical technicians need to assume a sincere attitude toward patients and colleagues consistently, to strengthen their professionalism, and to carry out their duties and roles faithfully. What is more, the association should take proper measures and to establish the sound image of emergency medical technician as a professional through active campaigns and monitoring using mass media.
