Improvement Devices on the Law and Institution and Current Situation of Health and Medical Treatment for the Aged (노인보건의료의 현황과 법 제도적 개선방안)

  • As the population is getting older, medical expenses amount of the whole is keep increasing. So, the pressure of the finances, Health Insurance, Medical Care Assistance Act and etc, is getting higher. The share of healthcare-expense is increasing due to elderly illness. And it became a social problem; we analysed present state of senior healthcare in South Korea-looked into current laws and policies, and found problems. We tried to suggest improvements that drew from the current state of foreign country senior healthcare of those problems. For the result, we found the problem in relevant-law system of senior healthcare guarantee. In this study, we proposed the ways to qualitatively upgrade of medical standard that considered on elderly' features: the strengthened guarantee for healthcare, financial secure for long-term convalescence benefit, linking and functional reinforcement for elderly welfare and long-term convalescence insurance, the solution for overlapped laws about convalescence in long-term convalescence insurance and elderly welfare, a betterment of grading, and a home service consolidation. We need to secure right amount of emergency medical service budget, and effective management system for the improved level of senior severely emergency medical service. Furthermore, we suggested that South Korea needs to legislate [The Law for Senior Medical Secure] to respond to rapidly increasing senior healthcare fee.

The Effects of Rhythmic Sensorimotor Training in Unstable Surface on Balance Ability of Elderly Women (불안정한 지지면에서의 율동적 감각-운동훈련이 여성노인의 균형능력에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate the effect of sensorimotor training in unstable surface on balance ability of elderly women. METHODS: Total of 40 subject were recruited from two separated institution for elderly women in Changwon and conveniently allocated into experimental and control groups. Twenty women were included into experimental group and another twenty women were into control group. Experimental group performed sensorimotor training in unstable surface and control group were only activities of daily living during eight weeks. To assess static balance ability used One leg stance test (OLST) and to assess dynamic balance ability used timed up and go test (TUGT) and Functional reach test (FRT). RESULTS: Control group did not show any difference in balance ability. However static balance ability in experimental group improved on hard or soft surface only eye open condition. Also dynamic balance ability in experimental group improved TUGT or FRT. CONCLUSION: Sensorimotor training in unstable surface improved balance ability of elderly women.

A Study on Relational Aggression Experience by Old Users of the Institution for Seniors - A Convergent Approach Based on Colaizzi's Methodology (기관이용노인의 관계적 공격성 경험에 관한 연구 -Colaizzi의 방법론을 중심으로 한 융합방법론 접근-)

  • The purpose of this study was to explore the experience of relational aggression that old users of institutions for seniors have, understanding what that experience means. In detail, the study tried to determine the nature of the experience through a structural description of what and how the elderly users experience relational aggression. For the purpose, this study made profound interviews with 7 elderly persons who sustained such aggression while using the seniors' institution in Busan. In the interview, a non-structured questionnaire survey was conducted. Data were analyzed using Colaizzi's phenomenological approach. As a result, the study identified 32 meanings of relational the experience which were in turn divided into 16 subjects, and 5 categories like 'I who am powerless', 'I who am in an unmanned island', 'whirlwinds in my mind', 'I who send myself into the world' and 'I who am dwindling'. Based on this result, this research made practical suggestions for preventing relational aggression as mentioned above.

The Effect of Sleep Quality on Activity Participation in the Elderly Living in Community (지역사회 거주 노인의 수면의 질이 활동참여에 미치는 영향)

  • Objective : The purpose of this study was to verify the effect of sleep quality on activity participation in the elderly living in the community. Methods : This study was conducted on 200 elderly people using welfare institutions located in Jeollabuk-do from August 21 to September 29, 2019. Data collection was performed using the Korean version of the Pittsburgh Sleep Quality Index (PSQI-K) and the Korean-Activity Card Sort (K-ACS). The data were analyzed by descriptive statistics, independent t-test, Pearson correlation, and simple linear regression analysis. Results : The group with high quality of sleep showed significantly higher participation in instrumental activities, social activities and leisure activities than the group with low quality of sleep (p<.001). The quality of sleep was significantly correlated with instrumental activities, social activities and leisure activities (p<.01). The quality of sleep significantly affected the instrumental activities (R2=.224), social activities (R2=.142) and leisure activities (R2=.227) (p<.001). Conclusion : The quality of sleep needs to be approaches to promote the participation of elderly people living in the community, and development of an intervention program will be needed to improve quality of sleep.

  • The purpose of this study is to find ways to improve the quality of life in old age. 'Act on Long-Term Care Insurance for the Aged' has been enforced since 2008 in Korea. But This is not encompass all of the elderly policies. Aging policies service separately each part by residential, welfare, and so on. There are 'Local Community Support Service' and 'Community General Support Center' for seniors as a comprehensive institution in Japan. Local Community Support Service is preventive and regional. And Community General Support Centers are sufficiently provide for each region. Japan can be a leading model of Korea. Therefor it analyzes Japanese cases and compares to the domestic situations in this study. Through this process, this study measures the necessity of the General Support Center for the elderly.

Factors Affecting Communication Satisfaction of Geriatric Caregiver : Focusing on the case of an urban-rural complex community (노인 돌봄서비스 제공자의 의사소통 만족도에 영향을 미치는 요인 : 도·농 복합 지역사회의 사례를 중심으로)

  • Objectives: The aim of this study was to identify factors influencing communication satisfaction between geriatric caregivers and older adults in urban-rural complex communities. The ultimate goal was to design local community educational programs and policies to enhance communication satisfaction among geriatric caregivers and improve the quality of care services for older adults. Methods: To identify factors influencing communication satisfaction between elderly caregivers and older adults, a survey titled "CCEP: Assessment of Communication Status between Elderly Care Service Providers and Recipients" was conducted from February to July 2020, focusing on rural-urban complex areas. The survey was administered based on providers of elderly healthcare services. The survey targeted 131 respondents involved in providing care services for older adults. The dependent variable of this study was the communication satisfaction reported by elderly caregivers in their interactions with the elderly. The independent variables included perceptions of older adults, factors associated with communication difficulties, and communication efforts. Additionally, gender, working environment, working experience, and the proportion of face-to-face interactions with older adults during caregiving were controlled for the hierarchical multiple regression analysis. Results: The analysis revealed that communication efforts with older adults significantly influenced communication satisfaction (β=.09, p<0.01). However, perceptions of the elderly and communication hindrance factors did not have a significant impact on communication satisfaction among geriatric caregivers. Conclusion: Effective communication between geriatric caregivers and older adults is crucial for identifying and meeting the needs and demands of caregiving services, and it plays a vital role in overall caregiving service satisfaction. To enhance communication skills and satisfaction among geriatric caregivers and ensure the appropriate fulfillment of elderly care needs in the local community, the development of community-centered, specialized health communication programs and other initiatives will be necessary in the future.

  • The elderly population will increase from 5.8% in 1996 to 12.5% in 2020. The related problems of health will also become a very important issue in the future. Therefore it is important to address the problems of geriatric nursing and geriatric health. 87.6% of the aged were ill with chronic degenerative diseases in 1994. In 1995, hospital admission rates (86.8) for those aged 60 or older were higher than that (56.3) of the total age group. Such high medical utilization will increase national health costs. For the development of geriatric nursing, active nursing intervention in various settings combined with education and research should be developed. Considering the health and welfare of the aged and the present status and views in Korea, I suggest the following: First, the health needs of the elderly in the institution, must be met by a plan that fosters geriatric nurses and programmed service development. Second, health service for the residential elderly must be provided in day care centers, short stays, nursing homes and geriatric hospitals. Geriatric nursing services should be provided in home residential areas, public health centers, public health subcenters as well as having, community health practitioners in primary health care posts and home health nurses. Third, geriatric nursing curriculum must be developed adjust to situations and culture of Korea and be included in the nursing curriculum. And gerontological nurse practitioner or geriatric specialist must be fosteraged to provide the professional care for the aged. Geriatric nursing research should be also achived for geriatric nursing improvements.

  • Korea in the era of population aging has a limitation on the welfare policy that treats the elderly as a weak person. Plans of supportive structure and maintenances of institution that can reanimate vitality of the healthy or motivated seniors in our society are very important to society that ageing is ongoing. When health and economic situation of the elderly has been weakened, our country needs to review and remake the social system or environment which helps to activate seniors` abilities and be able to become independent life through maintenance of seniors`participation structure. In this study, I will research the necessity of active support and suggest the improvement ways of legal system; not only focusing on living supports that treats unconditionally the elderly as weak person.

A Case Study on the Meaning of Performance of the Elderly Suicide Prevention Service (노인자살 예방서비스 성과의 의미에 관한 사례연구)

  • The purpose of this study was to identify the meaning of performance experienced in the process of practicing at the elderly suicide prevention center. Therefore, the actual results perceived from the subjective viewpoints of the chiefs, staff and service users from the public institutions, which offer the elderly suicide prevention services were examined. The results classified the service provider's view, service users view and multi-dimensional view. The research method is a case study of qualitative research method. The results of the study found that the analysis commonly perceived from the perspectives of both the chiefs and staff from the public institutions offering elderly suicide prevention service were 'the perception of structural obstacles during the process of practicing the services and focused on the achievement of experiencing changes in the service users and the community'. On the other hand, the analysis from the perspective of the service users turned out to be 'focusing on changes in the users'internal perception according to service experiences'. The meaning and essence of the analytical results perceived from each perspective are summarized as follows. First, the analytical results perceived of the chiefs of public institutions offering elderly suicide prevention service were 'the process in which the identification of the institution is rediscovered through the service'. Second, the analytical results perceived by the staff from the institution were 'being acknowledged in the community and organization while experiencing changes in the service users in the process of struggling to survive in such community and organization'. Third, the analytical results perceived by the service users were 'holding on to the lifeline at the edge of a precipice'. Based on the results, political and practical implications of elderly suicide prevention service were discussed.

A Study on the Usage Behavior of Elderly Welfare Facilities for the Elderly with Moderate Dementia: Focusing on Satisfaction with Memory School Users (경증치매노인 노인복지시설의 이용행태에 관한 융합연구: 기억학교 이용자만족도를 중심으로)

  • The purpose of this study is to provide basic data to confirm the necessity and effectiveness of the memory school by investigating the perception of users and caregivers about the services and programs provided by the memory school, a social welfare institution for the elderly with minor dementia. For this purpose, 363 students and students in Daegu City were surveyed about the satisfaction and attitude of using memory school. As a result of the analysis, it was found that the use of the memory school is very helpful to the users, and at the same time, it is very meaningful for the decrease of the caregiver burden of the caregiver. Thus, it can be seen that the memory school has the effect of appropriately matching the installation purpose of the facility. In particular, it was found that the degree of satisfaction with the program level, type of service, and future sustainability of the memory school exceeded 90%. Therefore, this study aims to provide policy implications for the social welfare response for the elderly with mild dementia and for the basic data on the memory school for this purpose.
