한의학 고문헌 텍스트 분석을 위한 비지도학습 기반 단어 추출 방법 비교 (Comparison of Word Extraction Methods Based on Unsupervised Learning for Analyzing East Asian Traditional Medicine Texts)

  • Objectives : We aim to assist in choosing an appropriate method for word extraction when analyzing East Asian Traditional Medical texts based on unsupervised learning. Methods : In order to assign ranks to substrings, we conducted a test using one method(BE:Branching Entropy) for exterior boundary value, three methods(CS:cohesion score, TS:t-score, SL:simple-ll) for interior boundary value, and six methods(BExSL, BExTS, BExCS, CSxTS, CSxSL, TSxSL) from combining them. Results : When Miss Rate(MR) was used as the criterion, the error was minimal when the TS and SL were used together, while the error was maximum when CS was used alone. When number of segmented texts was applied as weight value, the results were the best in the case of SL, and the worst in the case of BE alone. Conclusions : Unsupervised-Learning-Based Word Extraction is a method that can be used to analyze texts without a prepared set of vocabulary data. When using this method, SL or the combination of SL and TS could be considered primarily.

한의학 고문헌 텍스트에서의 인용문 추정과 탐색 (Detecting Local Text Reuse in the Texts of East Asian Traditional Medicine)

  • Objectives : The purpose of this paper was to examine quantitative methods for estimating and detecting local text reuse in the texts of East Asian Traditional Medicine. Methods : We introduce techniques that estimate the volume of local text reuse with n-gram and those that directly detect the reuse with the Smith-Waterman algorithm (SW algorithm). Based on this, the estimation and detection of local text reuse were carried out for 『Donguibogam』 and 『Huangdineijing·Suwen』. Results : Estimates with n-gram had more errors than methods with SW algorithms. SW algorithms detected suspected strings directly with local text reuse, resulting in more accurate results. Conclusions : Although n-gram does not accurately find local text reuse, its high speed makes it a preferable method for certain purposes, such as screening similar documents. On the other hand, SW algorithms have the advantage of being relatively good at finding similar phrases suspected as local text reuse even if the strings do not completely match. However, due to its excessive consumption of time and computing resources, its benefits are limited to cases where precise results are required.

Updated Trans-Ethnic Meta-Analysis of Associations between Inflammation-Related Genes and Intracranial Aneurysm

  • Objective : We performed an expanded multi-ethnic meta-analysis to identify associations between inflammation-related loci with intracranial aneurysm (IA) susceptibility. This meta-analysis possesses increased statistical power as it is based on the most data ever evaluated. Methods : We searched and reviewed relevant literature through electronic search engines up to August 2022. Overall estimates were calculated under the fixed- or random-effect models using pooled odds ratio (OR) and 95% confidence intervals (CIs). Subgroup analyses were performed according to ethnicity. Results : Our meta-analysis enrolled 15 studies and involved 3070 patients and 5528 controls including European, Asian, Hispanic, and mixed ethnic populations. Of 17 inflammation-related variants, the rs1800796 locus (interleukin [IL]-6) showed the most significant genome-wide association with IA in East-Asian populations, including 1276 IA patients and 1322 controls (OR, 0.65; 95% CI, 0.56-0.75; p=3.24#x00D7;10-9) under a fixed-effect model. However, this association was not observed in the European population (OR, 1.09; 95% CI, 0.80-1.47; p=0.5929). Three other variants, rs16944 (IL-1β), rs2195940 (IL-12B), and rs1800629 (tumor necrosis factor-α) showed a statistically nominal association with IA in both the overall, as well as East-Asian populations (0.01<p<0.05). Conclusion : Our updated meta-analysis with increased statistical power highlights that rs1800796 which maps on the IL-6 gene is associated with IA, and in particular confers a protective effect against occurrence of IA in the East-Asian population.

Sex-related and racial variations in orbital floor anatomy

  • Background: Repair of the orbital floor following trauma or tumor removal remains a challenge because of its complex three-dimensional shape. The purpose of the present study is to understand normal orbital floor anatomy by investigating its differences across four groups (Caucasian American and East Asian, males and females) via facial bone computed tomography (CT). Methods: A total of 48 orbits in 24 patients between 20 and 60 years of age were evaluated. Although most patients underwent CT scanning following trauma, the orbital walls were intact in all patients. Linear and angular measurements of the orbital floor were obtained from CT images. Results: Orbital floor width, length, angle between the orbital floor and medial wall, and distance from the inferior orbital rim to the lowest point of the orbital floor did not show a statistically significant difference between groups. Angles made by the infraorbital rim, the lowest point of the floor, and the anterior border of the infraorbital fissure were statistically significantly wider in East Asian females than in male groups. The floor depth in East Asian females was significantly smaller compared to all the other groups. Conclusion: East Asian female population had smaller curvature and depth of an orbital floor than the other groups, which means racial and sex-related differences should be considered in the orbital floor reconstruction.

  • The history of medicine has a strong correlation to the ancient development of epidemics. Although the study of the history of East Asian Traditional Medicine does not put much emphasis in understanding the flow of medical history in relation to epidemics, it largely impacted the development of this epidemic and the compilation and evolution of treatment methods. The same was true for Korea as they linked development of epidemics, unusual natural conditions, and social phenomenons from ancient documents. This study methodically classifies the epidemics mentioned in ${\ulcorner}$三國史記${\lrcorner}$ and concludes on how ancient Koreans understood epidemics.

Correlation of CT Perfusion Images with VEGF Expression in Solitary Brain Metastases

  • Objectives: To obtain permeability surface (PS) values using multi-slice helical CT perfusion imaging and to evaluate the spatial distribution correlation between PS values and vascular endothelial growth factor (VEGF) expression in solitary brain metastases. Methods: Imaging was performed on 21 patients, PS values being calculated from the central, border and peripheral parts of tumours. VEGF expression was determined by immunohistochemical staining. Results: Rim enhancement was found in 16 cases, the border of the tumour featuring PS elevation with high VEGF expression in 13 cases. In the 5 cases with nodular enhancement, the border and the central part had high permeability and VEGF expression was high in all cases, the correlation being significant (P<0.01). Conclusion: VEGF expression in brain metastases positively correlates with PS values from CT perfusion imaging, so that the latter can be used in the surveillance of angiogenic activity in brain metastases.

생체 전기 임피던스 분석의 한의학적 적용을 위한 연구동향 (Review on Bioelectrical Impedance Analysis in Traditional East Asian Medicine)

  • Bioelectrical Impedance Analysis (BIA) is a non-invasive and low-cost technique that estimates body composition based on the distribution of water and electrolytes in the body by analyzing body's electrical responses to source voltages. In this work, we carried out a systematic literature review on BIA researches in traditional East Asian medicine (TEAM). For comparison, firstly we introduced the concept and principle of BIA, and offered a general overview of research trends in western medical perspectives. We searched through the databases of Oriental Medicine Advanced Searching Integrated System and DataBase Periodical Information Academic for the articles published between 1994 and 2013, with keywords such as 'BIA', 'bioelectrical impedance' and 'impedance'. Among the rough-searched 274 articles, we finally selected 21 articles appropriate to the intended research field. The selected articles were categorized into diagnosis in Sasang medicine, impedance analysis in meridian system, and change of body composition after taking herbal medicine. We found that most of BIA researches in TEAM were preliminary and remained in the peripheral levels which is far behind the western medical research activities. Therefore, more efforts are needed to study BIA in association with major subjects such as pattern identification or physiological/pathological phenomena. In addition, methodological breakthrough of BIA is possible by applying the diagnostic concepts of the TEAM in relation to the balance of Qi and Blood.

심장판막 치환술 후 한의학적 치료에 대한 체계적 문헌고찰 및 메타분석 (East Asian Traditional Medicine Treatment for Patients after Heart Valve Replacements: A Systematic Review with Meta-Analysis)

  • Objectives: To compare the effectiveness and safety of East Asian traditional medicine treatments (EATMT) versus conventional management in patients following heart valve replacement surgery. Methods: We searched several databases, including the Korean Studies Information Service System, PubMed, China National Knowledge Infrastructure, and Citation Information by NII. The search range included randomized controlled trials from each first issue until June 27, 2021. Two review authors independently extracted the data. We assessed the risk of systematic errors by evaluating risk domains using the "Risk of bias" tool. Results: We included 5 trials in the review. In the EATMT, the investigators reported significant improvements in reshaping of the heart structure: left ventricular end diastolic diameter (MD -4.43, 95% CI -6.06 to -2.79; 130 participants; 2 studies; high evidence). Comparisons with usual care revealed a significant decrease in gastrointestinal complications rate (OR 0.30, 95% CI 0.20 to 0.47; 503 participants; 2 studies; high evidence). We assessed 4 studies as having a low risk of bias and 1 study as having a high risk of bias. Conclusion: This systematic review suggests that East Asian traditional medicine interventions may be effective in preventing and alleviating complications, but we found evidence of important trade-offs between known benefits and known adverse effects in cardiac dysfunction and inflammation following heart valve replacement. Consequently, additional high-quality studies should be conducted.

대만 건강보험청구데이터(NHIRD)를 이용한 전통 동아시아 의학(TEAM) 임상연구의 현황 (Current Status of Clinical Study on Traditional East Asian Medicine Using Taiwan Health Insurance Claim Data)

  • Objectives The study of the clinical effects of traditional east asian medicine (TEAM) using Taiwan national health insurance claim dataset (NHIRD) is useful in Korean Medicine research. We reviewed the clinical studies of TEAM using NHIRD as a whole through this study. Methods We comprehensively searched PUBMED and NHIRD DB for clinical effects of TEAM study using NHIRD from inception to 17, January 2017. As a result, 40 studies investigating the contribution of TEAM intervention to health benefit have been confirmed. We analyzed publication time, target disease, sample size, outcome measurement and main result of 40 searched studies. Results The number of TEAM studies using NHIRD grdually increasing. The topics of the team study using NHIRD covered a wide range of subjects including cardiovascular disease, tumor, gynecological disease, diabetes and kidney disease. The studies have shown large samples and reported significant effects on severe diseases. Conclusions The results of this study suggest that the study of Korean Medicine using Big data will be useful for decision making related to health care in Korea. However, considering the limited domestic Korean health insurance data, it will be necessary to activate the big data research of Korean Medicine through the establishment of a separate cohort in Korea.

계층적 군집분석(hierarchical clustering)을 통한 침구자생경(鍼灸資生經) 경혈 선택 요인 분석 (Deduction of Acupoints Selecting Elements on Zhenjiuzishengjing using hierarchical clustering)

  • Objectives : There are plenty of medical record of acupuncture & moxibustion in Traditional East Asian medicine(TEAM). We performed this study to find out the hidden criteria lies on this record to choose proper acupoints. Methods : "Zhenjiuzishengjing", ancient TEAM book was analysed using document clustering techniques. Corpus was made from this book. It contained 196 texts driven from each symptoms. Each texts converted to vector representing frequency of 349 acupoints. Distance of vectors calculated by weighted Euclidean distance method. According to this distances, hierarchical clustering of symptoms was builded. Results : The cluster consisted of five large groups. they had high corelation with body part; head and face, chest, abdomen, upper extremity, lower extremity, back. Conclusions : It assumes that body part of symptom is the most importance criteria of acupoints selecting. some high similar symptom vectors consolidated this result. the other criteria is cause and pathway of illness. some symptoms bound together which had common cause and pathway.
