Acuros XB advanced dose calculation algorithm (AXB, Varian Medical Systems, Palo Alto, CA) has been released recently and provided the advantages of speed and accuracy for dose calculation. For clinical use, it is important to investigate the dosimetric performance of AXB compared to the calculation algorithm of the previous version, Anisotropic Analytical Algorithm (AAA, Varian Medical Systems, Palo Alto, CA). Ten volumetric modulated arc therapy (VMAT) plans for each of the following cases were included: head and neck (H&N), prostate, spine, and lung. The spine and lung cases were treated with stereotactic body radiation therapy (SBRT) technique. For all cases, the dose distributions were calculated using AAA and two dose reporting modes in AXB (dose-to-water, $AXB_w$, and dose-to-medium, $AXB_m$) with same plan parameters. For dosimetric evaluation, the dose-volumetric parameters were calculated for each planning target volume (PTV) and interested normal organs. The differences between AAA and AXB were statistically calculated with paired t-test. As a general trend, $AXB_w$ and $AXB_m$ showed dose underestimation as compared with AAA, which did not exceed within -3.5% and -4.5%, respectively. The maximum dose of PTV calculated by $AXB_w$ and $AXB_m$ was tended to be overestimated with the relative dose difference ranged from 1.6% to 4.6% for all cases. The absolute mean values of the relative dose differences were $1.1{\pm}1.2%$ and $2.0{\pm}1.2%$ when comparing between AAA and $AXB_w$, and AAA and $AXB_m$, respectively. For almost dose-volumetric parameters of PTV, the relative dose differences are statistically significant while there are no statistical significance for normal tissues. Both $AXB_w$ and $AXB_m$ was tended to underestimate dose for PTV and normal tissues compared to AAA. For analyzing two dose reporting modes in AXB, the dose distribution calculated by $AXB_w$ was similar to those of AAA when comparing the dose distributions between AAA and $AXB_m$.

Background: This study aims to predict the midline dose based on the entrance and exit doses from optically stimulated luminescence detector (OSLD) measurements for total body irradiation (TBI). Materials and Methods: For TBI treatment, beam data sets were measured for 6 MV and 15 MV beams. To evaluate the tissue lateral effect of various thicknesses, the midline dose and peak dose were measured using a solid water phantom (SWP) and ion chamber. The entrance and exit doses were measured using OSLDs. OSLDs were attached onto the central beam axis at the entrance and exit surfaces of the phantom. The predicted midline dose was evaluated as the sum of the entrance and exit doses by OSLD measurement. The ratio of the entrance dose to the exit dose was evaluated at various thicknesses. Results and Discussion: The ratio of the peak dose to the midline dose was 1.12 for a 30 cm thick SWP at both energies. When the patient thickness is greater than 30 cm, the 15 MV should be used to ensure dose homogeneity. The ratio of the entrance dose to the exit dose was less than 1.0 for thicknesses of less than 30 cm and 40 cm at 6 MV and 15 MV, respectively. Therefore, the predicted midline dose can be underestimated for thinner body. At 15 MV, the ratios were approximately 1.06 for a thickness of 50 cm. In cases where adult patients are treated with the 15 MV photon beam, it is possible for the predicted midline dose to be overestimated for parts of the body with a thickness of 50 cm or greater. Conclusion: The predicted midline dose and OSLD-measured midline dose depend on the phantom thickness. For in-vivo dosimetry of TBI, the measurement dose should be corrected in order to accurately predict the midline dose.
This paper is to establish a basis for a dose reduction strategy by confirming correlations with the factors that may affect the radiation dose based on the dose records in low-dose chest CT and abdominal non-contrast CT. In order to find out the causes of unnecessary exposure, the correlation between seven factors (age, gender, height, weight, BMI, patient status [inpatient and outpatient], and use of dose modulation) and CT dose were identified. Logistic regression was used as the statistical analysis for correlation verification. In the low dose chest CT, as the higher values of height and BMI and dose modulation off were associated with lowering the risk exceeding Diagnostic Reference Levels(DRL) (odds ration<1, p<0.05). However, as woman compared to man and the higher values of weight were associated with highering the risk exceeding DRL (odds ration>1, p<0.05). In the abdomen CT, as dose modulation off were associated with lowering the risk exceeding DRL (odds ration<1, p<0.05). Therefore It is necessary to conduct research on the relationship between various factors affecting radiation exposure and patient radiation dose for reducing the dose.

Purpose : The measurement of skin dose is very important that treatment of breast cancer. On account of the cold or hot dose as compared with prescription dose, it is necessary to analyse the skin dose occurring during the various plan of the breast cancer treatment. At our hospital, we want to apply various analyses using a diversity of dosimeters to the breast cancer treatment. Subjectss and Methods : In the study, the anthropomorphic phantom is used to find out the dose difference of the skin(draining site), scar and others occurring from the tangential treatment plan of breast cancer. We took computed tomography scan of the anthropomorphic phantom and made plans for the treatment planing using open and wedge, Field-in-Field, Dose fluence. Using these, we made a comparative analysis of the dose date points by using the Eclipse. For the dose comparison, we place the anthropomorphic phantom in the treatment room and compared the measurement results by using the TLD and MOSFET on the dose data points. Results : On the central point of treatment planing basis, the upward and downward skin dose measured by the MOSFET was the highest when the fluence was used. The skin dose of inner and outer was distinguished from the figure(5.7% ~ 10.3%) when the measurements were fulfilled by using TLD and MOSFET. The other side of breast dose was the lowest in the open beam, on the other hand, is highest in the Dose fluence plan. In the different kinds of treatment, the dose deviation of inner and outer was the highest, and so this was the same with the TLD and MOSFET measurement case. The outer deviation was highest in the TLD, and the Inner'was highest in the MOSFET. Conclusion : Skin dose in relation to the treatment plan was the highest in the planing using the fluence technique in general and it was supposed that the high dose had been caused by the movement of the MLC. There's some differences among the all the treatment planning, but the sites such as IM node occurring the lack of dose, scar, drain site are needed pay close attention. Using the treatment planning of dose fluence is good to compensate the lack of dose, but It increases the dose of the selective range rather than the overall dose. Therefore, choosing the radiotherapy technique is desirable in the lights of the age and performance of the patient.
Dose distributions around Co- 60 moving source in high dose rate remote afterloading unit, Buchler 3K unit, were experimented with X-omat V films and calculations. In our experiments, film dosimetries have achieved to evaluated the axial dose distributions for source oscillations were 0, 3.5, 5.0 and 6.0 cm in periodically, In results, the dose distributions in axial of source movement showed apparently higher than in transverse direction caused by source locations, dwelling time and air gap in the applicator. In the calculations, the dose rate was derived by using the inverse square law, filteration corrections and Meisberger constant for scatter corrections as source movings. In our experiments and calculations, the average dose uncertainties were showed -2.1$\pm$1.9% in fixed sourdce, -2.9$\pm$1.8%, -7.4$\pm$6.1% and -6.7$\pm$4.6% at 3.5 cm, 5.0 cm and 6.0 cm source oscillations, but the calculations have showed very close to experimental dose rate within 4 cm distance from source.
This study uses digital imaging and communications in medicine (DICOM) files acquired after CT scan to obtain the absorbed dose distribution inside the body by using the patient's actual anatomical data; uses geometry and tracking (Geant)4 as a way to obtain the accurate absorbed dose distribution inside the body. This method is easier to establish the radioprotection plan through estimating the absorbed dose distribution inside the body compared to the evaluation of absorbed dose using thermo-luminescence dosimeter (TLD) with inferior reliability and accuracy because many variables act on result values with respect to the evaluation of the patient's absorbed dose distribution in diagnostic imaging and the evaluation of absorbed dose using phantom; can contribute to improving reliability accuracy and reproducibility; it makes significance in that it can implement the actual patient's absorbed dose distribution, not just mere estimation using mathematical phantom or humanoid phantom. When comparing the absorbed dose in polymethly methacrylate (PMMA) phantom measured in metal oxide semiconductor field effect transistor (MOSFET) dosimeter for verification of Geant4 and the result of Geant4 simulation, there was $0.46{\pm}4.69%$ ($15{\times}15cm^2$), and $-0.75{\pm}5.19%$ ($20{\times}20cm^2$) difference according to the depth. This study, through the simulation by means of Geant4, suggests a new way to calculate the actual dose of radiation exposure of patients through DICOM interface.
Purpose : Automatic exposure system(AOP mode) in DR Mammography of the STD and DOSE apply to women of childbearing age, the average glandular dose(AGD) and image quality by comparing was to demonstrate the usefulness of DOSE. Materials and Methods : Of the under 40 age 108 patients who visit to our hospital and examined STD and DOSE mammography from January 2008 to July 2013. AGD was obtained by DICOM header information provided by GE Senographe DS. STD and DOSE images were evaluated with obtained patients, Image J program was compared by calculating the SNR. Results : The average AGD of DOSE mode was 0.99mGy, and which decreased by 19% comparing to that of STD mode which was 1.18mGy. The two the average AGDs indicated statistically significant difference(p< .01). The average SNR of STD was 40.26, DOSE displayed, and to 39.68 in, there was no statistical significance. Results : The average AGD using DOSE mode which is one of DR mammography AOP modes decreased by comparing to that of STD mode showing no difference in image quality. The use of DOSE is considered to be useful.
Objective: The aim of this study is to investigate the effective managerial factors influencing dose reduction of the nozzle dam installation and removal tasks ranking within top 3 in viewpoint of average collective dose of nuclear power plant maintenance job. Background: International Commission on Radiation Protection (ICRP) recommended to reduce unnecessary dose and to minimize the necessary dose on the participants of maintenance job in radiation fields. Method: Seven sessions of nozzle dam installation and removal task logs yielded a multiple regression model with collective dose as a dependent variable and work time, number of participants, space doses before and after shield as independent variables. From the sessions in which a significant reduction in collective dose occurred, the effective managerial factors were elicited. Results: Work time was the most important factor contributing to collective dose reduction of nozzle dam installation and removal task. Introduction of new technology in nozzle dam design or maintenance job is the most important factor for work time reduction. Conclusion: With extended task logs and big data processing technique, the more accurate prediction model illustrating the relationship between collective dose reduction and effective managerial factors would be developed. Application: The effective managerial factors will be useful to reduce collective dose of decommissioning tasks as well as regular preventive maintenance tasks for a nuclear power plant.
Human Carcinogenic Potency (HCP) can be estimated based on human daily exposure dose to carcinogen (Dh), body weight (Wh), 10% tumorigenic dose (TD10), and slope factor at TD10 (Q10) from 2-yr bioassay data. This approach is more relevant to humans generally exposed to low doses of carcinogens and can reduce more of extrapolation errors from high dose in animal experiments to low dose in humans than HERP (human exposure dose/rodent potency dose) proposed by Ames et al. (Science, 236, 271-280, 1987). TD50 and HERP have been routinely used to compare rodent carcinogenic potency and human carcinogenic potency, but those approaches have had limitations in extrapolation of high dose to low dose in humans. The advantages of HCP are to estimate human exposure dose (Dh) by human monitoring instead of environmental monitoring, to consider slope factor (Q10) which reflects the tendency of curve at low dose, and to use TD10 which represents much lower dose thant TD50 or HERP. HCP will be a useful parameter for the estimation of human carcinogenic potency in risk assessment and management of carcinogens.

Generally uniform dose distribution is assumed to be formed in a target region when a conventional dose formation method using a broad proton beam, a fixed modulation technique, a bolus and an aperture is employed. However, actual situations differ. We usually find non-uniformity in the target region. This is due to the insertion of a range-compensating bolus before the patient. Since the range-compensating bolus has an irregular shape, the scattering in the bolus depends on the lateral position. Dose distribution is overlapping results of dose distribution of pencil-proton beams traversing different lateral positions of the bolus. The lateral extent of dose distribution of each pencil beam traversing the different position differs each other at the same depth in the target object. This is a cause of the non-uniformity of the dose distribution. Therefore the same lateral extent of dose distribution should be attained for different pencil beams at the same depth to obtain a uniform dose distribution. For that purpose, we propose here a bi-material bolus. The bi-material bolus consists of a low-Z material determining mainly the range loss and a high-Z material defining mainly the scattering in the bolus. After passing through the bi-material bolus, protons traversing different lateral positions will have different residual range yet with the same lateral spread at a certain depth. Using the optimized bi-material bolus, we can obtain a more uniform dose distribution in the target region as expected.
