  • Objectives Two conflicting readings of "Donguisusebowon" so far have been presented that Sasang Constitutional Medicine inherited the traditional Five Elements theory and that it was a new perspective on its own right. The main purpose of this paper is to review the theoretical ground of Sasang Constitutional Medicine to see whether it has inherited the Traditional Korean Medicine, and show that it had been developed as an original view independent of the Five Elements theory. Methods To do this, I inquired into the theoretical background of Sasang Constitutional Medicine as well as its philosophical premises and its leading ideas. Results and Conclusions As a result, I have found that Sasang Constitutional Medicine is an original medical theory developed in empirical terms on its own right, divorced from the transcendental view of the Five Elements theory, while borrowing some concepts from I-Ching and Neo-Confucianism only for technical tools. In short, Sasang Constitutional Medicine may well be put as calling for a reappraisal of the Han-medical and Confucian accounts of human being, and a transition from speculative method based on the Five Elements theory to a verifiable empirical method. In this vein, my reading of Sasang Constitutional Medicine in empirical perspective may well be appraised as presenting a new way to further inquiries into the theoretical nature of it, and to getting itself across with Western medicine as well.

A Study on the Basic Concepts of Lee Jema's Emotion Control Method - Focusing on nature-temperament(性情), Junghwa(中和), Jiin(知人) - (이제마의 감정조절법의 기본 개념들에 대한 고찰- 성정(性情), 중화(中和), 지인(知人)을 중심으로 -)

  • Objectives : In order to understand and apply Lee Jema's emotion control method, the basic concepts of his nature-temperament theory was examined. Lee's thoughts on emotion control, and the basic principles of his method were deduced from this process. Methods : The meaning and set-up of basic concepts in his nature-temperament theory as written in text such as the Donguisusebowon, Gyeokchigo, and Donguisusebowon Sasang Chobongwon were examined, and compared with similar concepts of Confucianism. Results & Conclusions : Out of concepts set by Lee, the Ae(哀)-No(怒)-Hui(喜)-Rak(樂) nature-temperament is the condition for emotion control. Junghwa(中和) is the aim of emotion control according to different stages of emotional expression, and Jiin(知人) is the precondition for emotion control. Lee's basic principles of emotion control could be summarized as following. First, it must be done with the aim for 'Goodness[善]' to be manifested. Second, it must be based on clear understanding of 'Insa(人事)'. Lastly, Hoyeonjigi(浩然之氣) and Hoyeonjiri(浩然之理) must be consistently cultivated for stable emotion control.

The Origin, Change, and Composition of Soeumin's Bojungikgi-tang (소음인(少陰人) 보중익기탕(補中益氣湯)의 기원, 변천과정 및 구성 원리)

  • 1. Objective : The purpose of this study is to understand the origin, changes and composition of Soeumin's Bojungikgi-tang. 2. Methods : 1) Analyzed the context of the progressive transformation of soeumin's Bojungikgi-tang in related passages from "Donguibogam (東醫寶鑑)", "Donguisusebowon Chobongwon (東醫壽世保元 草本卷)", "Donguisusebowon Gabobon (東醫壽世保元 甲午 本)", "Donguisusebowon Sinchukbon (東醫壽世保元 辛丑本)", "Dongmuyugo (東武遺稿)" 2) Examined the differences between the utilization of Soeumin's Bojungikgi-tang in pre-existing manuscripts according to pathological pattern and more specifically at a pathological stage. 3) Analyzed the total primary effect of the herbs as a prescriptive category, as well as the herbal properties of each component. 3. Results and Conclusions : 1) Originally Dongwon's Bojungikgi-tang was prescribed as a stomach & spleen organ tonifying medicine so it was easily applied and established as a prescription appropriate for Soeumin constitutional types with a weaker spleen organ. 2) From Gabobon(甲午本), Soeumin's Bojungikgi-tang was modified, excluding Bupleurum falcatum(柴胡), Cimicifuga heracleifolia (升麻) and substituting Teucrium chamaedrys(藿香), Ophiopogon japonicus(蘇葉) which are herbs appropriate for the Soeumin constitutional type. 3) In the Gabobon(甲午本), the Gegi-tang which has yang raising properties is included in the Soeumin's Bojungikgi-tang. In Sinchukbon(辛丑本), the Gegi-tang of the Soeumin's Bojungikgi-tang in the Gabobon was excluded and the Gegi-tang mutated into Hwangigyeji-tang. 4) In the Gabobon(甲午本), as the Soeumin's Bojungikgi-tang was prescribed for Soeumin disease regardless of favorable-unfavorable patterns and mild-severe patterns, the stage of the disease when to prescribe Soeumin's Bojungikgi-tang was not clarified. 5) As of the Sinchukbon(辛丑本) edition, as Soeumin's prescriptions have been obvious, the soeumin's Bojungikgi-tang was applied to Mang-yang disease with Hwangigyeji-tang and Seungyangikki-tang 6) As of the Sinchukbon(辛丑本) edition, the Soeumin's Bojungikgi-tang has been applied to parients having both exterior-heat Mang-yang disease and Greater-yin disease, at the same time. 7) As of the Sinchukbon(辛丑本) edition, finally, the Soeumin's Bojungikgi-tang formula has been established as gathering Qi and middle tonification with Panax ginseng(人蔘), Astragalus membranaceus(黃芪), keeping the spleen healthy with Artractylodes japonica(白朮), keeping the spleen strong with Angelica acutiloba(當歸), rooting the spleen with Citrus reticulata(陳皮), raising Yang and middle jiao with Teucrium chamaedrys(藿香), Ophiopogon japonicus(蘇葉), middle tonification with Glycyrrhiza glabra(甘草灸), Zingiber officinale(生薑), Zizyphus jujuba(大棗).

A study on the schematic organization of the sub-classification system of the Taeeumin symptomatology (태음인(太陰人) 병증(病證) 분류(分類)에 관한 연구(硏究))

  • 1. Objectives: We aimed to propose a sub-classification system for the Taeeumin symptomatology by examining the Taeeumin pathology and symptomatology descriptions appearing in "Donguisusebowon". 2. Methods: The Gabo Edition and the Sinchuk Edition (the upgraded and revised edition) of "Donguisusebowon" were reviewed and examined for relevant information on the Taeeum pathology and symptomatology. 3. Results and Conclusions: 1) In the Taeeumin symptomatology, the Exterior disease develops from the basic pathology of Esophagus-Cold and the Interior disease from that of Liver-Heat, eventually progressing to damage of the expirational and dispersive energy of the Lung Sector, the Prime Core Organ or the excessively small organ of the Taeeum constitutional type. The resulting pathology can be broadly defined as the "Lung-Dryness symptomatology". 2) The case reports introduced in the Exterior disease section, including the Zhang Zhongjing Mahuang-tang treatment, Prolonged-affliction disease treatment, and Exterior disease Pestilential disease treatment, share several points in common. They all arise from the pathology of "weakness in the Lung sector and deficiency in the Exterior sector", and they can all be assigned to the same symptomatological division that presents with systemic heat and cold intolerance; this symptomatology can be defined as the "Esophagus-Cold symptomatology", the milder subdivision of the exterior symptomatology. 3) The body of text appearing in the last part of the Interior disease section commonly referred to as the "Taeeumin Conspectus" is in fact not a conspectus when its contents are actually examined. Instead, it can be understood from its pathological and symptomatological descriptions that the passage is explaining the more severe subdivision of the exterior symptomatology that has progressed from Esophagus-Cold to a pathology characterized by damaged expirational and dipersive energy of the Lung Sector. 4) The relocation of the "dry-related pathology" indicates a change in perspective regarding the "Dry-related symptomatology", which caused the rearrangement of the Interior disease into divisions of Liver-Heat symptomatology that is characterized by fulminant heat pathology and Dry-Heat symptomatology that is also accompanied by Lung-Dryness. 5) The Interior disease Yin-Blood Consumptive symptomatology should be included in the Dry-Heat symptomatology in the pathological scheme. 6) Based on the above, the subdivisions of the Taeeumin symptomatology should be arranged as "Esophagus-Cold symptomatology" and "Lung-Dry-Cold symptomatology" in the Exterior disease and "Liver-Heat symptomatology" and "Dry-Heat symptomatology" in the Interior disease.

A Study on How the Discourse on the Constitutional Symptoms and Diseases in "Donguisusebowon" Influenced the Formation of the Theory of Eight Constitution ("동의수세보원(東醫壽世保元)"의 병증론이 8체질론(體質論)의 형성에 미친 영향에 대한 고찰(考察))

  • 1. Purpose Lee Je-Ma had continued to revise "Donguisusebowon" by adding the contents of the Symptomatic-pharmacology(病證藥理論) on Soeumin, Soyangin and Taeeumin to it from the first establishment of the old version in 1894 to 1900.(new version). This paper tries to find out the contents and meaning of the Discourse on the Constitutional Symptoms and Diseases(病證論) between the old version and new version and what the Discourse on the Constitutional Symptoms and Diseases affected the Theory of Eight Constitution created in subsequent years. 2. Methods I classified the Discourse on the Constitutional Symptoms and Diseases in "DongYiSuseBowon" into old version and new version and tried to find out the Theory of Eight morbidities(8病證論) which is the predecessor of the Theory of Eight Constitution, before reviewing their relation. 3. Conclusions 1) The Discourse on the Constitutional Symptoms and Diseases in the old version specify that the externally sensed diseases are caused by the deep sediment of nature(性) and that the internally touched diseases are caused by the violent activities of emotion(情). In addition, it said that all diseases are caused by the specific characteristics of patient's smaller internal organs(偏小之臟). But in new version, the diseases were understood through the concept Exterior, Interior, Heat and cold(表裏寒熱). The new version said that the diseases may be caused not only the specific characteristics of patient's smaller organs but also by bigger organs(偏大之臟). 2) The Theory of Eight Constitution was derived from the Theory of Eight morbidities, in which all kinds of human diseases are caused by 8 disease-orgin organ and that the diseases therefore can be classified into 8 morbidities. The Theory of Eight morbidities reorganized the Discourse on the Constitutional Symptoms and Diseases in "DongYiSuseBowon" by classifying them into first morbidity and second morbidity. 3) The Discourse on the Constitutional Symptoms and Diseases in "DongYiSuseBowon" has decisively affected the creation of the Theory of Eight morbidities which is the predecessor of the Theory of Eight Constitution. The Theory of Eight morbidities may be deemed another interpretation of the Discourse on the Constitutional Symptoms and Diseases in "DongYiSuseBowon". As the Theory of Eight morbidities put emphasis on the genetic characteristics, the Theory of Eight Constitution stating that Eight morbidities are caused by different specific constitution was derived.

The Origins, Changes, and Compositive Principles of Hyungbangjihwang-tang (형방지황탕(荊防地黃湯)의 기원(基源), 변천과정(變遷過程) 및 구성원리(構成原理))

  • Objectives This paper investigates the origins, changes, and the constructive principles of Hyungbangjihwang-tang(荊防地黃湯). Methods Hyungbangjihwang-tang and other related prescriptions were analyzed in terms of their pathological indications, based on previous literature including 『Gogeumuigam(古今醫鑑)』, 『Sanghannon(傷寒論)』, 『Uihagjeongjeon(醫學正傳)』, 『Donguisusebowon·Sasangchobongwon(東醫壽世保元·四象草本卷)』, 『Donguisusebowon·Gabobon(東醫壽世保元·甲午本)』, 『Donguisusebowon·Sinchukbon(東醫壽世保元·辛丑本)』. Results and Conclusions The origins of Hyungbangjihwang-tang can be seen in three sections depending on the classification of medicinal herbs. Schizonepeta tenuifolia Briq.(荊芥), Saposhnikovia divaricata Schischk.(防風), Notopterygium incisum Ting(羌活), Aralia cordata Thunb.(獨活), and Plantago asiatica L.(車前子) originated from Hyungbangpaedok-san(荊防敗毒散) included in 『Gogeumuigam』. Poria cocos Wolf.(茯苓) and Alisma canaliculatum(澤瀉) originated from Ohryoung-san(五苓散) included in 『Sanghannon』. Rehmannia glutinosa for hueichingensis(熟地黃) and Cornus officinalis S. et Z.(⼭茱萸) originated from Yukmijihwang-tang(六味地黃湯) included in 『Uihagjeongjeon』. Hyungbangpaedok-san was transformed into "Paedok-san" in 『Chobongwon(草本卷)』, Hyungbangpaedok-san in the 『Gabobon(甲午本)』 and 『Sinchukbon(辛丑本)』. Yukmijihwang-tang was mentioned as a Soyangin medicine in the 『Chobongwon』 and was later transformed into a Suhwagije-tang(水火旣濟湯) in 『Gabobon』. The meaning of Yukmijihwang-tang inherited in 『Sinchukbon』 as Dokhwaljihwang-tang and Hyungbangjihwang-tang. The medicinal herbs in Hyungbangjihwang-tang can be categorized in three sections by their functions, Schizonepeta tenuifolia Briq., Saposhnikovia divaricata Schischk., Notopterygium incisum Ting, Aralia cordata Thunb., and Plantago asiatica L. scatter the Pyohan(表寒) and make the Pyoeum(表陰) drop. Poria cocos Wolf.(茯苓) and Alisma canaliculatum(澤瀉) make the Pyoeum(表陰) drop through Lee-su(利水). Rehmannia glutinosa for hueichingensis(熟地黃) and Cornus officinalis S. et Z.(⼭茱萸) tonify Shin-won(腎元). The each principles of medicinal herbs corresponds to the stage of the "Cold-related Diarrhea in Soyangin Spleen Cold-based Exterior Cold disease(少陽人 脾受寒表寒病 亡陰病 身寒腹痛亡陰證)".

A Study on the Therapeutic Herbal Medications for Soyangin Mangeum Symptomatology (소양인(少陽人) 망음증(亡陰證)의 치방(治方)에 대한 고찰(考察))

  • 1. Background: This is the first documented study on the four medications for the Mangeum symptomatology (亡陰證) in the Soyangin disease category. The four medications, Hyeongbangsabaek-san (荊防瀉白散), Jeoryeongchajeonja-tang (猪苓車前子湯), Hyeongbangjihwang-tang (荊防地黃湯), and Hwalseokgosam-tang (滑石苦蔘湯), were comparatively analyzed on their compositive principles and respective symptomatologies. 2. Methods: Previous literature, including classical works on earlier Sasang Constitutional Medicine (different editions of Donguisusebowon and Dongmuyugo), were reviewed for relevant material on the Mangeum medications. 3. Results and Conclusions: 1) Hyeongbangsabaek-san (荊防瀉白散) is used in pathologies of early stages of invasion of the internal heat in the lower-most burn-center (locus related to the Kidney system). 2) Jeoryeongchajeonja-tang (猪苓車前子湯) is used when pathogenic dampness-heat invades the lower-most burn-center (locus related to the Kidney system). 3) Hyeongbangjihwang-tang (荊防地黃湯) is used to treat deficient heat involving injury of the lower-most burn-center (locus related to the Kidney system). 4) Hwalseokgosam-tang (滑石苦蔘湯) is used when the lower-most burn-center (locus related to the Kidney system) is in the early stages of exhaustion, expecially accompanied by concomitant dampness-heat. 5) The Mangeum symptomatology (亡陰證) was fully understood only after the completion of the 1901 Sinchuk edition (辛丑本) of Donguisusebowon (東醫壽世保元).

  • Donguisusebowon(東醫壽世保元) doesn't have pretty large quantity of contents, so indications of Four-Constitution Medicine formulas on that medical book are small. Therefore, indications of Four-Constitution Medicine formula need to be extended for broader clinical use of Four-Constitution Medicine. There would be some methods to do that, and we could review the literatures that Four-Constitution Medicine formulas are based on. We reviewed Bojungyikgi-tang on "Dongeuibogam(東醫寶鑑)", and we found that if the Four-Constitution Medicine formula is based on Sanghan formula(傷寒方) or non-classical formula(後世方), it is worth adding more indications of base formulas to those of Four-Constitution Medicine formula. For exmaple, Indications of Bojungyikgi-tang for So-Eum type could be extended by indications of non-classical formula(後世方) Bojungyikgi-tang and modified formula(加減方) Bojungyikgi-tang on Dongeuibogam, We supposed 23 indications of non-classical formula(後世方) Bojungyikgi-tang, and 20 indications of modified formula(加減方) Bojungyikgi-tang as possible indications of Bojungyikgi-tang for So-Eum type.

A literature study on the controlling methods of Seong-Jeong (性情) and covetous desire (心慾) for patients with cancer (암환자의 성정(性情)과 심욕(心慾) 조절 방법에 대한 문헌적 고찰)

  • Objective The management of psychological distress in patients with cancer is a major concern in clinical practice. In this paper, we discussed how to apply the controlling methods of Seong-Jeong (性情) and covetous desire (心慾) of Sasang constitutional medicine to patients with cancer. Methods We investigated the controlling methods of Seong-Jeong (性情) and covetous desire (心慾) through the literature of " Gyukchigo (格致藁)", "Donguisusebowon-Sasangchobongyun (東醫壽世保元 四象草本卷)", "Donguisusebowon (東醫壽世保元) and "Dongmuyougo (東武遺稿)", and presented the controlling methods of Seong-Jeong (性情) and covetous desire (心慾) for patients with cancer based on the clinical experience. Results We identified the equilibrium and harmony (中節) of sorrow-anger-joy-pleasure (喜怒哀樂) is the basis of treatment and suggested 'understanding of the others (知人)', 'understanding of the diseases (知病)' and 'knowing and acting (知行)' as methods to control of Seong-Jeong (性情) and covetous desire (心慾). The 'understanding of the others (知人)' and 'understanding of the diseases (知病)' could reduce the psychological distress. The 'knowing and acting (知行)' encourages the patients to overcome the covetous desire (心慾) and have a positive attitude to life and disease. Conclusion The controlling method of Seong-Jeong (性情) and covetous desire (心慾) of Sasang constitutional medicine could help cancer treatment controlling the psychological distress of patients with cancer, and could impact positively on the view of life and disease.

Two Cases of Drug-induced Secondary Parkinsonism Treated with Sasang Constitutional Therapy (사상처방을 활용한 약물 유발성 속발성 파킨슨증 치험 2례)

  • Objectives : This study is about two patients of drug-induced secondary parkinsonism caused by antipsychotic drugs. The purpose of this study is to report the clinical effects of Sasang constitutional medicine. Methods : These two patients were treated by Sasang constitutional herbal medications based on "Donguisusebowon". We evaluated the symptoms through the score of the Unified Parkinson's Disease Rating Scale(UPDRS). Results and Conclusions : The patients' chief complaints were improved. This study shows that Sasang constitutional herbal medications are an effective treatment for drug-induced secondary parkinsonism patients, but further studies are still necessary.
