Three-dimensional morphological evaluation of the hard palate in Korean adults with mild-to-moderate obstructive sleep apnea

  • Objective: The purpose of this study was to evaluate differences in three-dimensional (3D) morphology of the hard palate between Korean adults with and without mild-to-moderate obstructive sleep apnea (OSA) using cone-beam computed tomographic (CBCT) data. Methods: The protocol for the two-dimensional (2D) and 3D mathematical modeling was established by analyzing CBCT images of 30 adults with OSA and 30 matched controls without OSA, using MIMICS software. The linear and angular measurements were also determined using this software. The measurements were repeated for 30 palates, by the same operator, to assess reliability. Results: The palates of OSA patients were higher in the posterior part and narrower in the anterior-superior part than those of the control group (p < 0.05). The nasal cavities of patients with OSA were narrower (p < 0.05) than those of controls. The increasing angle of the first molar palatal root is a compensation of the upper dental arch to improve occlusion. However, for most palatal measurements, there were no significant differences between the OSA and control groups (p > 0.05). The results of 2D and 3D mathematical models were consistent for linear and angular measurements, indicating that 2D and 3D mathematical modeling of the palate is a reliable methodology. Conclusions: OSA is a multifactorial disease; the palates of adults with mild-to-moderate OSA do not have specific morphological features distinct from those of healthy controls.

  • U-healthcare is real-time monitoring of personal biometric information using by portable devices, home network and information and communication technology based healthcare systems, and fused together automatically to overcome the constraints of time and space are connected with hospitals and doctors. As u-healthcare gives health service in anytime and anywhere, it becomes to be a new type of medical services in patients management and disease prevention. In this paper, recent changes in prevention-oriented care is analyzed in becoming early response for Healthcare Information System by requirements analysis for technology development trend. According to the healthcare system, PACS, OCS, EMR and emergency medical system, U-healthcare is presenting the design of a patient-centered integrated client system. As the relationship between the meaning of the terms is used in the ontology, information models in the system is providing a common vocabulary with various levels of formality. In this paper, we propose an ontology-based system for patient-centered services, including the concept of clustering to clustering the data to define the relationship between these ontologies for more systematic data.

Quantitative Microbial Risk Assessment of Clostridium perfringens on Ham and Sausage Products in Korea (햄 및 소시지류에서의 Clostridium perfringens에 대한 정량적 미생물 위해평가)

  • This study was conducted for quantitative microbial risk assessment (QMRA) of Clostridium perfringens with consumption on ham and sausage products in Korea, according to Codex guidelines. Frame-work model as product-retail-consumption pathway composed with initial contamination level, the time and temperature in distributions, and consumption data sets for ham and sausage products and also used the published predictive growth and dose-response models for Cl. perfringens. The simulation model and formulas with Microsoft@ Excel spreadsheet program using these data sets was developed and simulated with @RISK. The probability of foodborne disease by Cl. perfringens with consumption of the ham and sausage products per person per day was estimated as $3.97{\times}10^{-11}{\pm}1.80{\times}10^{-9}$. There were also noted that limitations in this study and suggestion for development of QMRA in the future in Korea.

The Effects of Daekumeumja on Alcohol-induced Muscle Atrophy in Rats (대금음자(對金飮子)가 흰쥐의 만성 알콜성 근위축에 미치는 영향)

  • Chronic alcoholic myopathy is one of the most common skeletal muscle disorders. It is characterized by a reduction in the entire skeletal musculature, skeletal muscle weakness, and difficulties in gait. Patients with alcoholic hepatitis and cirrhosis have severe muscle loss that contributes to worsening outcome. Although the myopathy selectively affects Type II (fast twitch, glycolytic, anaerobic) skeletal muscle fibers, total skeletal musculature is reduced. The severity of the muscle atrophy is proportional to the duration and amount of alcohol consumed and leads to decreased muscle strength. The mechanisms for the myopathy are generally unknown but it is not due to overt nutritional deficiency, nor due to either neuropathy or severe liver disease. Skeletal muscle mass and protein content are maintained by a balance between protein synthesis and breakdown and in vivo animal models studies have shown that ethanol inhibits skeletal muscle protein synthesis. Daekumeumja is a traditional Korean medicine that is widely employed to treat various alcohol-induced diseases. Muscle diseases are often related to liver diseases and conditions. The main objective of this study was to assess that Daekumeumja extract could have protective effect against alcoholic myopathy in a Sprague-Dawley rat model. Rats were orally given 25% ethanol (5ml/kg, body weight) for 8 weeks. After 30 minutes, rats were administrated with Daekumeumja extract. Controls were similarly administrated with the vehicle alone. The weights of gastrocnemius, soleus and plantaris muscles were assessed and the morphologic changes of gastrocnemius and plantaris muscles were also assessed by hematoxylin and eosin staining. In results, The muscles from ethanol treated rats displayed a significant reduction in muscle weight and average cross section area compared to Normal group. Daekumeumja extract treated group showed increased muscle weight and muscle fiber compared to the ethanol treated group. It was concluded that Daekumeumja extract showed ameliorating effects on chronic alcohol myopathy in skeletal muscle.

Anti-inflammatory Effect of Haingkyunghonghwatang in cultured RAW264.7 cells and murine models of inflammation (RAW264.7세포주와 염증생쥐모델에서 항염증(抗炎症) 작용(作用)에 대한 행경홍화탕(行經紅花湯)의 효과(效果))

  • Purpose : The Purpose of this research was to investigate the effects of Haingkyunghonghwatang (HKHHT) on anti-inflammatory effects. Methods : As for the parameters of inflammation, levels of several inflammatory cytokines and chemical mediators were determined in mouse lung fibroblast cells(mLFC) and RAW264.7 cells. Also, changes in pathological features by drug treatment were investigated in the in vivo edema-induced rats by carrageenin/arachidonic acid or in the colitis-induced mice by DSS treatment. Results : The cytotoxicity of HKHHT on mLFC and RAW264.7 cells wasn't observed at 100, 50, 10, and $1{\mu}g/ml$ of The treatments. $IL-1{\beta}$, IL-6 and NOS-II mRNA expression of RAW264.7 cells was inhibited by The treatments in a dose-dependent manner. HKHHT treatment of RAW264.7cells(HtRc) inhibited $TNF-{\alpha}$ and COX-2 mRNA expression. HtRc significantly inhibited IL-6 and NO production. HtRc inhibited ROS production. HKHHT inhibited rat's paw edema induced by carrageenin or arachidonate treatment in all concentrations examined. The body weight and colon length of colitis-induced mice were recovered to a normal level by DSS treatment. Clinical disease levels were significantly improved compared to the control animals. HKHHT treatment of colitis-induced mice(HtCm) significantly increased hematological values such as WBC and RBC counts, Hgb and HCT levels, but decreased PLT values. HtCm decreased IL-6 and $TNF-{\alpha}$ production significantly HtCm significantly increased CD3+(T) cell counts. In contrast, HKHHT treatment decreased CD19+ B cell counts and CD3+/CD69+ significantly, and also decreased B/T ratio (%) though not significant. Conclusion : These results indicated that HKHHT could be used for treating diverse female diseases caused by the inflammation.

Ginseng seed oil ameliorates hepatic lipid accumulation in vitro and in vivo

  • Background: Despite the large number of studies on ginseng, pharmacological activities of ginseng seed oil (GSO) have not been established. GSO is rich in unsaturated fatty acids, mostly oleic and linoleic acids. Unsaturated fatty acids are known to exert a therapeutic effect in nonalcoholic fatty liver disease (NAFLD). In this study, we investigated the protective effect and underlying mechanisms of GSO against NAFLD using in vitro and in vivo models. Methods: In vitro lipid accumulation was induced by free fatty acid mixture in HepG2 cells and by 3 wk of high fat diet (HFD)-feeding in Sprague-Dawley rats prior to hepatocyte isolation. The effects of GSO against diet-induced hepatic steatosis were further examined in C57BL/6J mice fed a HFD for 12 wk. Results: Oil Red O staining and intracellular triglyceride levels showed marked accumulation of lipid droplets in both HepG2 cells and rat hepatocytes, and these were attenuated by GSO treatment. In HFD-fed mice, GSO improved HFD-induced dyslipidemia and hepatic insulin resistance. Increased hepatic lipid contents were observed in HFD-fed mice and it was lowered in GSO (500 mg/kg)-treated mice by 26.4% which was evident in histological analysis. Pathway analysis of hepatic global gene expression indicated that GSO increased the expression of genes associated with ${\beta}$-oxidation (Ppara, Ppargc1a, Sirt1, and Cpt1a) and decreased the expression of lipogenic genes (Srebf1 and Mlxipl), and these were confirmed with reverse transcription and quantitative polymerase-chain reaction. Conclusion: These findings suggest that GSO has a beneficial effect on NAFLD through the suppression of lipogenesis and stimulation of fatty acid degradation pathway.

Modeling the potential climate change-induced impacts on future genus Rhipicephalus (Acari: Ixodidae) tick distribution in semi-arid areas of Raya Azebo district, Northern Ethiopia

  • Background: Climate change is believed to be continuously affecting ticks by influencing their habitat suitability. However, we attempted to model the climate change-induced impacts on future genus Rhipicephalus distribution considering the major environmental factors that would influence the tick. Therefore, 50 tick occuance points were taken to model the potential distribution using maximum entropy (MaxEnt) software and 19 climatic variables, taking into account the ability for future climatic change under representative concentration pathways (RCPs) 4.5 and 8.5, were used. Results: MaxEnt model performance was tested and found with the AUC value of 0.99 which indicates excellent goodness-of-fit and predictive accuracy. Current models predict increased temperatures, both in the mid and end terms together with possible changes of other climatic factors like precipitation which may lead to higher tick-borne disease risks associated with expansion of the range of the targeted tick distribution. Distribution maps were constructed for the current, 2050, and 2070 for the two greenhouse gas scenarios and the most dramatic scenario; RCP 8.5 produced the highest increase probable distribution range. Conclusions: The future potential distribution of the genus Rhipicephalus show potential expansion to the new areas due to the future climatic suitability increase. These results indicate that the genus population of the targeted tick could emerge in areas in which they are currently lacking; increased incidence of tick-borne diseases poses further risk which can affect cattle production and productivity, thereby affecting the livelihood of smallholding farmers. Therefore, it is recommended to implement climate change adaptation practices to minimize the impacts.

The SPM Study on the Change of Titanium Surface Roughness following Airpowder Abrasive and Application Time of Citric Acid (공기-분말 연마와 구연산의 적용시간에 따른 임프란트 표면 거칠기의 변화에 관한 주사탐침현미경적 연구)

  • The Peri-implantitis causes inflammation of periodontal tissue and bone loss. It contaminates surface of implants. Therefore, guided bone regeneration has been used for the treatment of this disease. For the reosseointegration of the exposed surface, various mechanical and chemical methods have been used for cleaning and detoxication of implant surface. Among these methods, air-powder abrasive and oversaturated citrate are known to be most effective. However, these treatments may deform implant surface. In this research, changes of surface roughness they were examined. 10 experimental machined titanium cylinder models were fabricated to be used for control groups. Each of them was air powder abraded for 1 minute and they were named group 1. And then, group 1 were burnished with cotton pellets soaked with citrate for 30 seconds(Group 2), 1 minute(Group 3), 3 minutes(Group 4), and 5 minutes(Group 5) burnishing were applied for grouping respectively. Each group were examined with SPM, and their surface roughness were measured and analyzed. 1. Surface roughness of titanium decreased when it was air-powder abraded for 1 minute. It was statistically significant. 2. When Air-powder abraded titanium were treated with citrate for 3 minutes, Their surface roughness was the lowest. Titanium treated for 1 minute was the second lowest and 30 seconds was the third and titanium burnished for 5 minutes was the highest. 3. Surface roughness of titanium which was treated with citrate was decreased till 3 minutes, which was statistically significant. There was no statistical significance from 30 seconds to 1 minute and from 1 minute to 3 minutes, and there was statistical significance from 30 seconds to 3 minutes. 4. Oxide layer was formed when titanium is exposed to air, and it was removed when air-powder abraded. It was made when treated with citrate. It is thought that citrate treatment is necessary after the air-powder abrasion, and 1 minute is clinically and qualitatively adequate for burnishing time of citrate.

Greater Lymph Node Retrieval Improves Survival in Node-Negative Resected Gastric Cancer in the United States

  • Purpose: Guidelines in Western countries recommend retrieving ${\geq}15$ lymph nodes (LNs) during gastric cancer resection. This study sought to determine whether the number of examined lymph nodes (eLNs), a proxy for lymphadenectomy, effects survival in node-negative disease. Materials and Methods: The US National Cancer Database (2003-2011) was reviewed for node-negative gastric adenocarcinoma. Treatment was categorized by neoadjuvant therapy (NAT) vs. initial resection, and further stratified by eLN. Kaplan-Meier and Weibull models were used to analyze overall survival. Results: Of the 1,036 patients who received NAT, 40.5% had ${\leq}10eLN$, and most underwent proximal gastrectomy (67.8%). In multivariate analysis, greater eLN was associated with improved survival (eLN 16-20: HR, 0.71; P=0.039, eLN 21-30: HR, 0.55; P=0.001). Of the 2,795 patients who underwent initial surgery, 42.5% had ${\leq}10eLN$, and the majority underwent proximal gastrectomy (57.2%). In multivariate analysis, greater eLN was associated with improved survival (eLN 11-15: HR, 0.81; P=0.021, eLN 16-20: HR, 0.73; P=0.004, eLN 21-30: HR, 0.62; P<0.001, and eLN >30: HR, 0.58; P<0.001). Conclusions: In the United States, the majority of node-negative gastrectomies include suboptimal eLN. In node-negative gastric cancer, greater LN retrieval appears to have therapeutic and prognostic value, irrespective of initial treatment, suggesting a survival benefit to meticulous lymphadenectomy.

Mapping the Geographic Variations of the Low Birth Weight cases in South Korea: Bayesian Approaches (우리나라 저체중아 출생의 공간적 변동성 지도화: 베이지언적 접근)

  • This study reviewed and compared methods for mapping aggregated low birth weight (LBW) and geographic variations in LBW in South Korea. Based on this review, we produced LBW maps in South Korea. Standardized mortality/morbidity ratios (SMRs) and crude mortality rates have been widely used for many years in epidemiological research. However, SMR-based maps are likely to be affected by sample size of unit area. Therefore, this study adopted a model-based approach using Bayesian estimates to reduce noisy variability in the SMR. By using a Bayesian model, we can calculate a statistically reliable RR values. We used the full Bayes estimator, as well as empirical Bayes estimators. As a result, variations in the two Bayes models were similar. The SMR-based statistics had the largest variation. The result maps can be used to identify regions with a high risk of LBW in South Korea.
