Time efficiency and operator convenience of using a micro-screw in image registration for guided implant surgery (마이크로스크류가 가이드 임플란트 수술을 위한 영상정합 과정에서 작업시간과 술자편의성에 미치는 영향)

  • Purpose: The image registration of radiographic image and digital surface data is essential in the computer-guided implant guide system. The purpose of this study was to examine the effects of using micro-screw on the working time and convenience of operators in the process of image matching for guided implant surgery. Materials and methods: A mandibular dental model was prepared in partial edentulism for Kennedy class I classification. Two micro-screws were placed on the each side of retromolar area. Radiographic and scan images were taken using computed-tomography and digital scanning. The images were superimposed by 12 operators in software in two different conditions: using remaining teeth image alone and using teeth and micro-screws images. Working time, operator convenience and satisfaction were obtained, and analyzed using the Mann-Whitney U test (${\alpha}=.05$). Results: The working time was not statistically different between image registration conditions (P>.05); however, operator convenience and satisfaction were higher in the teeth and micro-screw assisted condition than in the teeth-alone assisted condition (P<.001). Conclusion: The use of micro-screw for the image registration has no effect in working time reduction, but improves operator convenience and satisfaction.

Development of the Oral Health Education Program for North Korean Defectors in South Korea (북한이탈주민 대상 구강보건교육 프로그램 개발)

  • To improve the oral health level of North Korean defectors, it is necessary to improve awareness of oral health care along with treatment of oral diseases. The purpose of this study is to develop an oral health education program that can be applied to North Korean defectors who have settled in local communities. To develop the program, three steps were taken: literature review, in-depth interview with experts, and program development applying the Dick&Carey model. As a result of in-depth expert interviews, what should be considered in oral health education for North Korean defectors is the need to understand North Korean defectors in advance, to familiarize themselves with the daily language of North Korea, to develop a synchronization strategy suitable for North Korean defectors, and to connect with whole body health. It was found that there was a need to establish the contents of education and to seek realistic education methods that reflect the characteristics of local communities. As a result of developing the program, fifty learning goals were developed, and ten sessions were planned. The oral health education program for North Korean defectors developed in this study will be of practical help to dental hygienists planning oral health education for North Korean defectors in the future and North Korean defectors.

The effect of policy on Korean personal assistance service for persons with disabilities of labor market participation (장애인활동지원서비스제도의 노동시장 참여에 대한 정책효과)

  • This study used data from the 6rd and 12th year of the Korean Welfare Panel to evaluate the effects of the Personal Assistance Service(PAS) system on the labor market of PAS users' participation. For the purpose of this study, 64 program groups using the Korean PAS and 344 control groups not using the Korean PAS were selected using Caliper matching among the propensity score matching. A chi-square test was used for the difference in characteristics between groups, and a simple difference-in-differences (DID) model and a double-difference multiple regression analysis of DID were performed to estimate the effect of thepolicy before and after the Korean PAS. As a result of the study, it was found that statistically, PAS had no significant effect on the labor market. This is due to the low number of system users, resulting in low post-hoc power, incomplete matching and limited availability of PAS Assistants for Disabled People. Therefore, In order to demonstrate the effectiveness of the Personal Assistance Service(PAS) system, specialized services and systems that meet the needs of the disabled and household members should be implemented.

Comparative accuracy of implant impression techniques with different splinting materials (임플란트 인상채득 방법과 인상용 코핑 연결 고정에 따른 정확성 비교)

  • Purpose: We investigated the effect of open tray impression using pick-up impression coping and close tray impression using transfer impression coping on the accuracy of impression in edentulous patients on their mandibular parts. the effect of material types of pick-up type impression copings for splinting in open tray impression on the accuracy of impression was also evaluated. Materials and Methods: Two implant fixtures were implanted in parallel in the left molar of the mandibular in the shape of a mandibular partial edentulous model. The 40 individual trays were fabricated using 3D printer. The prepared individual trays were classified into 4 groups (i.e., PN, PG, PH, and TN groups), and a total of 40 impression-takings were conducted. A master cast was connected to a Scan Body. The converted STL file was super-imposed on the scan images of the various groups. Results: The order of standard deviation values decreased as follows: PN (0.2343 ± 0.0844 mm), TN (0.2192 ± 0.0840 mm), indicating that the high accuracy of impression for TN group. In addition, for the comparison results between the material types used in splinting the open tray impression, the PH group showed a relatively lower standard deviation (0.1910 ± 0.1176 mm) than that of the PN group (0.2343 ± 0.0844 mm), PG group (0.2556 ± 0.1082 mm). Conclusion: The acrylic resin synthesized by light-induced polymerization exhibited a higher accuracy of impression taking than that of autopolymerizing acrylic resin. Meanwhile, the accuracy of impression taking was not dependent on the implant impression taking method or the presence of connection/fixation of impression copings.

Fabrication of complete denture using digital technology in patient with mandibular deviation: a case report (하악 편위 환자에서 디지털 방식을 이용한 총의치 제작 증례)

  • Recently, digital technology and computer-aided design/computer-aided manufacturing (CAD/CAM) environment have changed the clinician treatment method in the fabrication of dentures. The denture manufacturing method with CAD/CAM technology simplifies the treatment and laboratory process to reduce the occurrence of errors and provides clinical efficiency and convenience. In this case, complete dentures were fabricated using stereolithography (SLA)-based 3D printing in patient with mandibular deviation. Recording base were produced in a digital model obtained with an intraoral scanner, and after recording a jaw relation in the occlusal rim, a definitive impression was obtained with polyvinyl siloxane impression material. In addition, facial scan data with occlusal rim was obtained so that it can be used as a reference in determination of the occlusal plane and in arrangement of artificial teeth during laboratory work. Artificial teeth were arranged through a CAD program, and a gingival festooning was performed. The definitive dentures were printed by SLA-based 3D printer using a Food and Drug Administration (FDA)-approved liquid photocurable resin. The denture showed adequate retention, support and stability, and results were satisfied functionally and aesthetically.

  • Recently, through the development of CAD/CAM technology, it is also being used for fabricating dentures. Compared to conventional methods, when digital dentures are fabricated, the fabrication process is facilitated, and the number of visits to hospitals is reduced and errors are reduced. In this case, the vertical dimension was increased using a CAD program in a patient who needed vertical dimension recovery due to the use of old dentures, and the final denture was fabricated using a monolithic disc through the milling method. The centric relation was recorded using existing dentures, and using the information from the intraoral scan and the existing denture model scan, a trial denture was fabricated and delivered to the patient to evaluate the midline and occlusion. Based on the evaluation of the trial denture, the final denture was fabricated using a milling method and a monolithic disc, and the final denture showed satisfactory results functionally and aesthetically.

  • Purpose: The purpose of this study was to review the outcomes of a series of studies on tissue regeneration conducted in multiple institutions including the Department of Periodontology, College of Dentistry, Yonsei University. Materials and Methods: Studies were performed divided into the following three subjects; 1) Development of three-dimensional nano-hydroxyapatite (n-HA) scaffold for facilitating drug release and cell adhesion. 2) Synergistic effects of bone marrow-derived mesenchymal stem cells (BMMSC) application simultaneously with platelet-rich plasma (PRP) on HA scaffolds. 3) The efficacy of silk scaffolds coated with n-HA. Also, all results were analyzed by subjects. Results: Hollow hydroxyapatite spherical granules were found to be a useful tool for the drug release and avidin-biotin binding system for cell attachment. Also, BMMSC simultaneously with PRP applied in an animal bone defect model was seen to be more synergistic than in the control group. But, the efficacy of periodontal ligament cells and dental pulp cells with silk scaffolds could not be confirmed in the initial phase of bone healing. Conclusion: The ideal combination of three elements of tissue engineering-scaffolds, cells and signaling molecules could be substantiated due to further investigations with the potentials and limitations of the suggested list of studies.

Factors influencing the axes of anterior teeth during SWA on masse sliding retraction with orthodontic mini-implant anchorage: a finite element study (교정용 미니 임플랜트 고정원과 SWA on masse sliding retraction 시 전치부 치축 조절 요인에 관한 유한요소해석)

  • Objective: With development of the skeletal anchorage system, orthodontic mini-implant (OMI) assisted on masse sliding retraction has become part of general orthodontic treatment. But compared to the emphasis on successful anchorage preparation, the control of anterior teeth axis has not been emphasized enough. Methods: A 3-D finite element Base model of maxillary dental arch and a Lingual tipping model with lingually inclined anterior teeth were constructed. To evaluate factors influencing the axis of anterior teeth when OMI was used as anchorage, models were simulated with 2 mm or 5 mm retraction hooks and/or by the addition of 4 mm of compensating curve (CC) on the main archwire. The stress distribution on the roots and a 25000 times enlarged axis graph were evaluated. Results: Intrusive component of retraction force directed postero-superiorly from the 2 mm height hook did not reduce the lingual tipping of anterior teeth. When hook height was increased to 5 mm, lateral incisor showed crown-labial and root-lingual torque and uncontrolled tipping of the canine was increased.4 mm of CC added to the main archwire also induced crown-labial and root-lingual torque of the lateral incisor but uncontrolled tipping of the canine was decreased. Lingual tipping model showed very similar results compared with the Base model. Conclusion: The results of this study showed that height of the hook and compensating curve on the main archwire can influence the axis of anterior teeth. These data can be used as guidelines for clinical application.

Changes of the surface roughness depending on immersion time and powder/liquid ratio of various tissue conditioners (수종의 조직 양화재의 침수시간과 분액비에 따른 표면 거칠기의 변화)

  • Statement of problem: Volume stability, microstructure reproducibility and fluidity along with compatibility with dental stone must be in consideration in order to use tissue conditioner as a material for functional impression. There are few studies concerning the influence of time factor in oral condition on surface roughness of the stone and optimal retention period in the oral cavity considering such changes in surface roughness. Purpose: The purpose of this study was to find out the influence of various kinds of tissue conditioner, its powder/liquid ratio and immersion time on surface roughness of the stone. Material and methods: Materials used in this study were the three kinds of tissue conditioners(Coe-Comfort, Visco-Gel, Soft-Liner) and were grouped into three: group R-mixed with standard powder/liquid ratio that was recommended by the manufacturers, group M-mixed with 20% more powder, group L-mixed with 20% less powder. Specimens were made with the size of 20 mm diameter and 2 mm width. Each tissue conditioner specimens were subdivided into 5 groups according to the immersion time(0 hour, 1 day, 3 days, 5 days, 7 days), completely immersed into artificial saliva and were stored under $37^{\circ}C$. Specimens of which the given immersion time elapsed were taken out and were poured with improved stone, making the stone specimens. Surface roughness of the stone specimens was measured by a profilometer. Results: Within the limitation of this study, the following results were drawn. 1. Major influencing factor on surface roughness of the stone model made from tissue conditioner was the retention period(contribution ratio($\rho$)=62.86%, P<.05) of the tissue conditioner in oral cavity to make functional impression. 2. In case of Coe-Comfort, higher mean surface roughness value of the stone model with statistical significance was observed compared to that of Soft-Liner and Visco-Gel as immersion time changes(P<.05). 3. In case of group L(less), higher mean surface roughness value of the stone model with statistical significance was observed compared to that of R(recommended) and M(more) group as immersion time changes(P<.05). Conclusion: We may conclude that as the retention period of time in oral cavity influences surface roughness of the stone model the most and as the kind of tissue conditioner and its P/L ratio may influence also, clinician should well understand the optimal retention period in oral cavity and choose the right tissue conditioner for the functional impression, thus making the functional impression with tissue conditioner usefully.

Reproducibility and accuracy of tooth size measurements obtained by the use of computer (컴퓨터를 이용한 치아크기 계측시 재현도와 정확도에 관한 연구)

  • The purpose of this study was to evaluate the availability of computer system for the measurement of tooth size in the model analysis through the comparison of two measurements: One was to use a computer; and the other was to use vernier calipers. Twenty sets of casts were used, which showed a moderate degree of crowding and full eruption of all teeth. The mesio-distal width of 12 teeth from the left central incisor to the left first molar at each set of the casts were measured twice with vernier calipers and a computer respectively. This measurement was repeated two weeks later. First, for the reproducibility analysis, the two computer measurements were compared then the vernier calipers measurements were compared. Second, all the teeth were sepapated into the region of mesiodistal contact points and its width was measured by a micrometer to obtain standard measurements. For the accuracy analysis, these standard measurements were compared with the measurements from the dental casts using two methods. The difference between them was defined as the measurement error. To investigate the cause of measurement error, an examination was made for the presence and degree of contact point deviation on each tooth from the upper and lower occlusograms, and the mesio-distal angulation of each tooth was measured with TARG. Following results were obtained through statistical analysis. 1. In the analysis for reproducibility; the measurements with vernier calipers showed significant differences in three out of twelve teeth while the computer measurements showed significant differences in one out of twelve teeth. 2. In the analysis for accuracy; compared with the standard measurements, the measurements with vernier calipers showed significant differences in three out of twelve teeth while the computer measurements showed significant differences in two out of twelve teeth. 3. Compared with the standard measurements, the measurements with vernier calipers were apt to be larger at the upper first molar, and smaller at the lower first molar The computer measurements, however, were apt to be larger at both upper and lower first molars. 4. The measurements with vernier calipers showed the largest error at the lower first molar and the degree of error was variable according to the tooth while the difference of error was small in the computer measurements. 5. In the analysis for the correlation of the degree of measurement errors with the contact point deviation index and the mesio-distal crown angulation of each tooth, the measurements with vernier calipers did not show significant correlation while the measurements with computer showed slight Positive correlations. The results of this study indicate that a computer system may be useful for the measurement of tooth size in the model analysis.
