A Study on Community-dwelling Elders with Dementia, their Primary Caregiver, and Living Environments (재가 치매노인의 주 수발자와 환경 실태)

  • The purpose of this study was to describe about elders with dementia, their families, and their living environment by visiting the households where demented elders resided. The findings will be used as a basis to develop future individualized adjustment programs for demented elders and their families living in communities. The study participants were 64 demented elders and their families who were registered to a dementia counseling center at Nam-Gu community health center located in Inchon, Korea. Data were collected for two months, from May to June 2001. The length of data collection for each home visit ranged from 1.5 to 2.5 hours. Conclusion are as follows based on these study findings: Those demented elders had more than one chronic health problem in addition to their pre-existing dementia condition. Two thirds of the demented elders were not receiving any specific treatment for dementia. They showed a moderate level of independence in basic ADL, but were mostly residing at home because of lack of ability to perform more delicate and complicate routine daily activities by themselves. In addition, the primary caregivers were not well adjusted to the care-giving activities for their demented family members due to the lack of knowledge and information about dementia. The caregivers were mostly women including daughters-in-law, woman spouses and daughters, over a half of whom perceived their physical and mental health status as poor. Their image toward the demented elders was considerably negative. while their level of knowledge on dementia was moderate. The burden for the care-giving was high, whereas their coping method was passive. As the difference in image toward elderly before and after the onset of dementia in their family member increases, the caregiver burden also increased. The main resource of social support for the caregivers was their children. The caregivers showed high level of needs for knowledge and information on dementia, and day care service was the most preferred type of service by the caregivers. There was lack of safety in the living environments for the demented elders and their families, and in the surrounding environments to prevent dementia-related symptoms. Considering that home-based family care-giving is the most culturally appropriate model of providing care for the demented elders in Korea, we need to develop and apply an individualized adjustment program for the demented elders and their families.

Application Design Planning for the Elderly with Mild Dementia (경증 치매 노인을 위한 애플리케이션 디자인기획)

  • Due to aging society, the prevalence of dementia is continually increasing and, thereby, causing a serious issue. Although Long Term Care Insurance is provided on a national level, it is not available for the elderly with dementia who do not satisfy the requirements. As the use of smartphones becomes widespread, this study investigated an application that can help disease management of the elderly with the early-stage (mild) dementia and communication among the family members based on analysis of the functions found in currently-available applications and survey among the relevant subjects. As a research method, based on review of the relevant theoretical studies, the service environment of applications available for the elderly with mild dementia was analyzed. Through this analysis, it found out that there is no application for the eldery with mild dementia. On the basis of the results, this study proposed a direction for design planning of an application included many functions like managements of taking medicine, physical activity, brain activity, information on dementia and notes. for dementia management aimed to help the patients with mild dementia manage the disease on their own. If this type of services are expanded in addition to the systemic support from the government, the data collected from these applications can contribute to improving management of mild dementia.

Analysis of behavioral management for dental treatment in patients with dementia using the Korean National Health Insurance data

  • Background: The global population is aging rapidly, and accordingly, the number of patients with dementia is increasing every year. Although the need for dental treatment increases for various reasons in patients with dementia, they cannot cooperate during dental treatment. Therefore, behavioral management, including sedation (SED) or general anesthesia (GA), is required for patients with dementia. Thus, this study aimed to investigate the trends and effects of SED or GA in patients with dementia undergoing dental treatment in South Korea based on the Korean National Health Insurance claims data. Methods: This study utilized customized health information data provided by the Health Insurance Review and Assessment Service. Among patients with records of using sedative drugs during dental treatment from January 2007 to September 2019, patients with the International Classification of Diseases-10 code for dementia (F00, F01, F02, F03, and G30) were selected. We then analyzed the full insurance claims data for dental care. Age, sex, sedative use, and dental treatment of patients were analyzed yearly. In addition, the number of cases of GA or SED per year was analyzed, and changes in behavioral management methods with increasing age were investigated. Results: Between January 2007 and September 2019, a total of 4,383 (male, 1,454; female, 2,929) patients with dementia received dental treatment under SED or GA. The total number of SED and GA cases were 1,515 (male, 528 ; female, 987 ) and 3,396 (male, 1,119 ; female, 2,277) cases, respectively. The total number of cases of dental treatment for 4,383 patients with dementia was 153,051 cases, of which 2.22% were under GA and 0.98% were under SED. Midazolam was the most commonly used drug for SED. Conclusion: Although gingivitis and pulpitis were the most common reasons for patients with dementia to visit the dentist, GA or SED for patients with dementia was frequently used in oral and maxillofacial or periodontal surgery.

  • In this paper, we introduce the trends in age-related health care services for dementia research due to the rapid aging society. In Korea, the current increase in dementia patients and the disappearance of runaway because social welfare policy is insufficient state and requires a lot of effort in order to protect the patient's guardian. Therefore, In this paper, we propose the need for a monitoring service for dementia patients. The attachment by the wearable sensor and monitor the mobility of the patient causes the protector as possible to trace the location of the patient to monitor the safety of the patient standing with the system provides.

The Influence of Fall-Related Knowledge and Fall Prevention Self-Efficacy of Care-Givers Working in Long-term Care Hospitals with Older Adults with Dementia on Fall Prevention Behaviors and Fall Management Behaviors (요양병원 요양보호사의 치매노인에 대한 낙상 지식, 낙상예방 자기효능감이 낙상예방행위 및 낙상관리행위에 미치는 영향)

  • Objectives: The purpose of this study was to investigate the influence of fall-related knowledge and fall prevention self-efficacy of care-givers working in long-term care hospitals on fall prevention behaviors and fall management behaviors for older adults with dementia. Methods: Participants were 125 care-givers working in long-term care hospitals. Data were collected with structured questionnaires from August 7 to 14, 2018. Data were analyzed by t-test, ANOVA, $Scheff{\dot{e}}$ test, Pearson's correlation coefficients, and multiple regression analysis using SPSS 18.0. Results: Fall prevention self-efficacy influence fall prevention behaviors(${\beta}=.55$, p<.001), while fall-related knowledge impacted fall management behaviors(${\beta}=.43$, p<.001). Conclusions: The results suggest that an education program for fall prevention of older adults with dementia in long-term care hospitals should focus on improving the fall-related knowledge and fall prevention self-efficacy of care-givers to increase both the fall prevention behaviors and fall management behaviors of care-givers.

Severe periodontitis with tooth loss as a modifiable risk factor for the development of Alzheimer, vascular, and mixed dementia: National Health Insurance Service-National Health Screening Retrospective Cohort 2002-2015

  • Purpose: The purpose of this study was to evaluate severe periodontitis with tooth loss as a modifiable risk factor for Alzheimer dementia (AD), vascular dementia (VaD), and mixed dementia (MD) using the National Health Insurance Service-National Health Screening Retrospective Cohort database with long-term follow-up over 14 years. Methods: Multivariate Cox hazards regression analysis was applied to a longitudinal retrospective database, which was updated in 2018, to evaluate the association between severe periodontitis with few remaining teeth and dementia after adjusting for potential risk factors, including sociodemographic factors and comorbid diseases. Results: Among 514,866 individuals in South Korea, 237,940 (46.2%) participants satisfying the inclusion criteria were selected. A total of 10,115 age- and sex-matched participants with severe periodontitis and 10,115 periodontally healthy participants were randomly selected and evenly assigned. The results showed that the risks of AD (hazard ratio [HR], 1.08), VaD (HR, 1.24), and MD (HR, 1.16) were significantly higher in patients with severe periodontitis with 1-9 remaining teeth after adjustment for sociodemographic factors, anthropomorphic measurements, lifestyle factors, and comorbidities. Conclusions: Severe periodontitis with few remaining teeth (1-9) may be considered a modifiable risk factor for the development of AD, VaD, and MD in Korean adults.

Effects of Home-based Cognitive Occupational Therapy Applied to Dementia Patients in the Initial Stages in Gangwon-do (강원도 지역 초기 치매환자에게 적용한 가정방문 인지작업치료의 효과)

  • Objective : The purpose of this study is to find out the effects of a Home-based cognitive occupational therapy program conducted for the dementia patients in the initial stages living in Taebaek region in Gangwon-do. Methods : This study carried out Home-based cognitive occupational therapy intervention by 30 Occupational Therapy Dept. students and volunteers targeting 30 dementia patients in the initiative stages for eight sessions, once a week. To identify the change of cognitive function of the participants before and after the program, MMSE-K was used for measuring. Results : As a result of applying Home-based cognitive occupational therapy to the dementia patients in the initial stages, overall cognitive function improvement was demonstrated. Especially, statistically significant improvement was exhibited in orientation, memory retrieval, the concentration of attention, and language ability. Conclusion : The program for cognitive function improvement is considered to be usefully applied to intervention in the dementia patients. The development of various Home-based occupational therapy intervention programs is required in order to adopt the Home-based occupational therapy service in the future.

  • The purpose of this study was to provide the customized-service program for the family of dementia patients as an family-function reinforcement program. This study was designed to develop the therapeutic recreation program for the family supporting senior with dementia. The therapeutic program consisted of assessment, planning, implementation, evaluation. This study found the followings. First, this study shows the possibility of implication of the therapeutic recreation for the family supporting senior with dementia. Second, the therapeutic recreation program has a strong theoretical background. Third, the therapeutic recreation program has the clear goal, performing activity, intervention strategy. The therapeutic recreation program for the family supporting senior with dementia that this study provides would be valuable basic data for future studies.

The Prescribing Patterns of Antipsychotic Drugs and Antiparkinsonian Drugs in Elderly Patients with Dementia (노인 치매 환자의 항정신병약물 및 항파킨슨약물 처방 현황)

  • Background: The number of patients with dementia continues to increase as the age of aging continues to grow. Psychiatric symptoms caused by senile dementia are controlled using antipsychotics. However, these antipsychotics can lead to Parkinson's disease, and abuse of dopamine derivatives such as levodopa among Parkinsonian drugs can lead to psychosis. Therefore, we evaluated the patterns of prescribed antipsychotics and antiparkinsonian drugs in patients with senile dementia. Methods: We used data from the sample of elderly patients from the Health Insurance Review and Assessment Service (HIRA-APS-2016). We analyzed the patterns of prescribing antipsychotics and antiparkinsonian drugs including prescribed daily dosage, period of prescription, and number of patients with both antipsychotics and antiparkinsonian drugs for senile dementia. Results: Among the 159,391 patients with dementia included in this analysis, 4,963 patients (3.1%) and 16,499 patients (10.4%) were prescribed typical and atypical antipsychotic drugs, respectively. The most frequently prescribed typical antipsychotic was haloperidol (4,351 patients with dementia), whereas the atypical agent was quetiapine (12,719 patients). The most frequently prescribed antiparkinsonian drugs were in the order of levodopa/carbidopa, benztropine, and ropinirole. In addition, 1,103 and 3,508 patients prescribed typical and atypical antipsychotics, respectively, were co-prescribed antiparkinsonian drugs. Conclusions: Atypical antipsychotics were the preferred prescription in patients with senile dementia. The prescription dose was relatively low; however, the average treatment duration was mostly long-term. Selection of antipsychotics and/or antiparkinsonian drugs should be made carefully in senile dementia and the causal relationship of adverse drug reactions needs further study.

The Systematic Discussion of a Study on the Application of Computerized Cognitive Rehabilitation on Patient with Dementia in Korea (국내 치매 환자를 대상으로 한 전산화 인지 재활 적용 연구에 대한 체계적 고찰)

  • Purpose : This study aimed to investigate a study on the intervention of computerized cognitive rehabilitation applied to patients with dementia in Korea. Thus, a systematic discussion was conducted. Methods : Studies from January 2000 to December 2016 were searched in the Korea Education and Research Information Service. The main key words used were: "dementia," "computerized cognitive rehabilitation," "computerized cognitive training," "computer assisted cognitive therapy," and "computerized cognitive therapy." Result : A total of five studies were selected. The effects of the computerized cognitive rehabilitation therapy applied to patients with dementia resulted in improved cognitive functions, as well as daily living and activities instrumental to daily living. There was also an increase in brain activity. The qualitative aspects of this study showed a low level of internal and external validities due to the small number of subjects, short period of intervention, and limited study environment. Conclusion : In the future, it is necessary to continue a study in which the level of evidence is high to determine whether various cognitive rehabilitation programs are effectively applied to patients with dementia.
