Clinical Observation on Apgar Score Changing of the Newborn Infants (신생아 Apgar Score의 시간적 변동에 관한 임상적 관찰)

  • This study was attempted for clinical investigation regarding with Apgar score changing phase of newborns and their sucking power related the score and weight. The data for this study obtained from 545 newborns who were born at Dept. of obstetric WooSok Hospital, Korea University from Jan. to Sept.. 1373. It is hoped that this study will contribute to nursing care planning of newborns in nursery. The Apgar score was checked in the delivery room at 1 minute after delivery by Doctor and rechecked at 30 minutes and 1 hour after delivery by nurses. The results obtained were as follows: 1. The total newborn numbers at 1 minute after delivery were 545, at 30 minutes were 542 (3 dead) and at 1 hour were 540. The babies were divided into 3 groups by the Apgar score. The frequency of group 1 (score 1 to 4) at each times were 5.0%, 0.9%, 0.4%, group 2 (score 5 to 7) 11.2%, 4.1%, 1.7% and group 3 (score 8 to 10) 83.8%, 95.0%, 97.9%. 2. The group I was found most frequently among the cases of less than 2, 500g and group 3 was found among the cases over 2.500g. It was found that there was a tendency of delaying in recovery process of low score among the premature and low birth weight newborns. 3. The type of delivery, breech and other I e was found worst and C-Section was the second, however normal spontaneous delivery was found the best. 4. In observation of sucking power of the newborn related its score and weight, good, fair, poor marks were given. But for convenience of statistical analysis. good and fair was collected together and poor alone evaluated. The result of group 1.2 examined statistically by F.E.T shown no relation between newborn weight and sucking power. however group 3, examined by X$^2$test shown very significant relation between newborn weight and sucking power. 5. The mortality rate of newborn while they were in hospital was found 2.6% to 545 of birth and all cases of dead belonged to group 1 at 1 minutes after delivery. 64.3% of the dead was found among the cases less than 2, 500g. 6. It was found that in this observation premature and Newborn asphyxia had influence to low score at birth. Conclusively, good anthemata carr should be emphasized in order to prevent main causes of these and should avoid abnormal delivery as possible.

The Isoflurane Concentration of Precision Vaporizer Goldman Vaporizer According to Room Temperature and Carrier Gas Flow Rate (온도 및 유량에 따른 흡입 마취제 전용 기화기와 Goldman 기화기의 isoflurane 농도 변화)

  • Anesthetic machines may be equipped with either a precision or nonprecision vaporizer. A precision vaporizer is designed to deliver an exact concentration of anesthetic agent. Goldman vaporizer is a low-flow, closed-circle circuit with a low resistance vaporizer, in circuit. Vaporizers used within circle system(VIC) are not usually temperature compensated and this is generally thought to be a disadvantage. As the volatile agent is vaporized, heat is extracted from the liquid and temperature decreases. This cooling of the liquid leads to a decrease in concentration of the anaesthetic agent delivered by the vaporizer. The purpose of this study is to examine the mechanical consistency of the delivery of isoflurane from Goldman vaporizer and precision vaporizer at various gas flow rates and temperatures. And we first studied isoflurane concentration according to room temperature changes delivered by a Goldman vaporizer and precision vaporizer using different gas flow. The room temperature of $15^{\circ}C,$ $20^{\circ}C,$ $28^{\circ}C$ and fresh gas flow rates of 0.5, 1.0, 1.5, 2.0, 3.0 l/min were used. The inspired agent concentration was measured using a Datex-Ohmeda multigas analyzer. As rose in room temperature, the isoflurane concentration of precision vaporizer approximated the dial setting. On the other hand, at a dial setting concentration of 5.0 percent the delivered isoflurane concentration of precision vaporizer was more than the dial setting in high temperature. The isoflurane concentration of precision vaporizer remained constant despite the increase in temperature. The isoflurane concentration of Goldman vaporizer was increased with rise in room temperature and decreased with rise in gas flow.

A Nursing College Women's Meaning of Labor and Delivery : Phenomenological Method (간호학 전공 여대생이 인지한 분만의 의미에 대한 연구)

  • A women's meaning about labor and delivery may influence the process of childbirth and performance of the maternal role. Therefore, negative meaning about the birth experience may have a negative impact on the childrearing. The purpose of this study is to explore the meaning of labor and delivery for the strategies of nursing intervention in positive childbirth experience. The data were collected through the in-depth interviews of 10 nursing college women on Jeju Island from October 2000 to January 2001. The interviews were conducted by investigator in the class room after school hours. Each interview lasted for about 30 minutes on average. Subjects were interviewed one at a time. The interviews were recorded with the consent of the subject. The data were analyzed by means of Giorgi's phenomenological analysis methods and catagorized according to the similarities of its contents. The investigator read the data repeatedly to identify the themes and the main meanings. Eight main meanings were identified as follows : 1)pain 2) shame and disgust 3)load 4)naturality 5)unreality 6)happiness 7)anxiety 8)maturity. Under these main meanings there were seventeen themes. I. Pain : (a) It's too painful (b) I don't want to deliver II. Shame and Disgust : (a) I am shameful (b) I am hateful III. Load : (a) I don't feel free (b) I have responsibility IV. Naturality : (a) It's in order to obtain a child (b) It's natural for women (c) It's a destiny of women V. Unreality : I don't feel real VI. Happiness : (a) I am glad (b) I am happy (c) It's miraculous VII. Anxiety : (a) I am anxious (b) I am worried VIII. Maturity : (a) I understand parents (b) I feel great. The results of the study will provide basic data for positive childbirth experience.

Analysis of the Use of Ambulance Services Among Pregnant, Childbearing, and Postpartum Women Using Data from the Korea Health Panel (한국의료패널을 이용한 구급 이송 서비스 이용 특성 분석: 임신, 출산, 산후기 여성을 중심으로)

  • This study examined the socio-economic and clinical characteristics associated with the use of ambulance services among pregnant, childbearing, and postpartum women based on data from the 2008-2016 Korea Health Panel. The analysis revealed that among the pregnant, childbearing, and postpartum women, the proportion using all ambulance services was 18.9% whereas 12.0% used private and 119 ambulances. Moreover, among those using ambulance services, delivery was the most common reason (38.7%) followed by complications of labor and delivery (20.0%) and pregnancy with abortive outcome (17.3%). There were statistically significant differences between the users and non-users of ambulance services in terms of the average annual household income, emergency arrival time, and delayed arrival at the emergency room. As childbirth becomes more complicated due to low fertility and elderly mothers, the expansion and improvement of ambulance services as a social safety net for pregnant, childbearing, and postpartum women will become increasingly important.

Sexual Behavior and Related Factors of Coitus of Teenage Girls in Korea (십대 여학생의 성행태와 성경험 관련요인)

  • The purpose of this study was to investigate the rate of pregnancy, abortion, delivery and contraceptive use and the relationship between the related factors and the early coitus among Korean teenagers. Method: The subjects of this study were 12,733 teenagers age 13-19 years in 7 major cities and 9 provinces in Korea. Data was collected from 2 Oct., to 28 Oct., 2000 and analyzed by using SPSS 10.0 Program. Results: The results showed pregnancy rate 4.5%, delivery rate 0.5%, abortion rate 0.4%. Adolescent girls who experienced coitus relatively went to vocational school, coeduaction school, night school in the general characteristics (p=0.000). Adolescent girls who experienced coitus tend to go to video room (18.5%), go the singing room (60.7%), the bar (54.8%), petting with the partner during the dates. There were overlapping juvenile delinquencies such as drinking (89.2%), pornography (65.3%), bond inhalation (2.7%), leaving homes (43.7%), smoking (14.5%), cyber sex (70.8%), phone sex (12.9%). Adolescent girls who experienced coitus were more suffering from Wangtta (26.1%) than who had not. They showed significant positive attitudes toward coitus; such as feeling of excitement (6.6%), nature (37.9%), beauty (8.7%). On the other hand, they had more guilty (2.8%) than the inexperienced (p=0.000). The main factors relating to early coitus were 'to confirm love' (46.9%), 'afraid of rejection' (32.2%). They think the benefits of coitus were 'more intimacy with boyfriends' (20.6%), 'satisfaction of curiosity' (16.8%). The losses on coitus were 'worry about pregnancy' (73.7%) and 'unability to refuse sexual contacts' (28.7%) Conclusion: It could be concluded that it is necessary to plan different sex education programs according to general characteristics and delinquency behaviors of teenage girls in Korea.

Environmental Analysis of a Windowless Delivery Swine Building : Temperature and Relative Humidity (무창분만돈사의 온.습도 환경 분석)

  • Recently, local swine producers are rapidly adopting the indoor production system which developed in foreign countries. However, this imported system is reported as not functioning properly because of different climate conditions. The objective of this project was to investigate the environment characteristics of a windowless delivery swine building. The parameters studied were the heating and cooling loads, the daily changes of indoor temperature and relative humidity, the horizontal and the vertical distributions of indoor temperature, and the effect of mist cooling on indoor temperature. From this study, the following are founded : 1. The maximum cooling and heating loads were - 317.0kcal/㎡$.$h and 336.5kal/㎡$.$h in summer and in winter. The large loads seems to be on account of inappropriate operations of ventilating fans. 2. The daily variations of relative humidity in indoor were smaller than those in outside. Those values both in summer and in winter as relative humidities in door was lower than optimum for growing pigs, the additional humidifier might be helpful to increase the relative humidity in indoor. 3. The horizontal distribution of the indoor temperature was found to be uniform in the variation range of 1$^{\circ}C$. 4. The vertical distribution of the indoor temperature was not found to be uniform; the temperature of upper part was higher than that of slot part. 5. Average values of indoor temperature became lower by 3$^{\circ}C$ by mist cooling. But the variation of temperature was found to be larger; The middle part of the room was cooled down, but the corner part of the room was not affected by misting due to uneven nozzle configuration.

Storage of laboratory animal blood samples causes hemorheological alterations : Inter-species differences and the effects of duration and temperature

  • Hemorheological results may be influenced by the time between blood sampling and measurement, and storage conditions (e.g., temperature, time) during sample delivery between laboratories may further affect the resulting data. This study examined possible hemorheological alterations subsequent to storage of rat and dog blood at room temperature ($22^{\circ}C$) or with cooling ($4{\sim}10^{\circ}C$) for 2, 4, 6, 24, 48 and 72 hours. Measured hemorheological parameters included hematological indices, RBC aggregation and RBC deformability. Our results indicate that marked changes of RBC deformability and of RBC aggregation in whole blood can occur during storage, especially for samples stored at room temperature. The patterns of deformability and aggregation changes at room temperature are complex and species specific, whereas those for storage at the lower temperature range are much less complicated. For room temperature storage, it thus seems logical to suggest measuring rat and dog cell deformability within 6 hours; aggregation should be measured immediately for rat blood or within 6 hours for dog blood. Storage at lower temperatures allows measuring EI up to 72 hours after sampling, while aggregation must be measured immediately, or if willing to accept a constant decrease, over 24~72 hours.

Effects of different Discharge Packs given at the Nursery Room on Postpartum Breast-feeding (신생아실에서의 모유 퇴원팩이 산후 모유수유 실천에 미치는 효과)

  • The effects of different discharge packs on the rate of breast-feeding practice were investigated in 2, 4 and 8 postpartal weeks. The subjects were those who have made delivery at Chonnam University Hospital from Jan to Feb 1994. They were divided into three groups by the discharge pack provided at the nursery room : the one group was given with formula discharge pack, another with breast-feeding discharge pack and the other nothing. The formula discharge pack contained formula samples, a feeding bottle and a pamphlet prepared by a formula company, and breast-feeding discharge pack contained a manual pump and a pamphlet made by Korean Nurses Association. Following results were obtained : 1. Different discharge packs significantly affected the rate of breast-feeding practice at 2 week postpartum, while not at 4 and 8 week postpartum. 2. At 2 week postpartum, the rate of breast-feeding practice was significantly higher in the group given with breast-feeding discharge pak than in that given with formula discharge pack. It was also significantly higher in the group given with breast-feeding discharge pack compared with the group given nothing. The breast feeding rate, however, did not significantly differ between the formula discharge pack group and the group given nothing. 3. The most common cause for the artificial feeding was 'lacking breast milk'. The most common cause to select a specific brand of formula milk was 'the same as in the nursery room'. In conclusion, it is suggested that encouraging mothers to perform breast feeding and providing them with a breast-feeding discharge pack instead of a formula pack at the nursery room are helpful in promoting the breast-feeding.

Development ofn Sharing Space Access Management System based on Mobile Key and RCU(Room Control Unit) (모바일 키 및 RCU에 기반한 공유공간 출입관리 시스템 개발)

  • Recently, the importance of non-face-to-face has been emphasized due to COVID-19, and the use of sharing spaces is also expanding. The use of uncontact check-in technology for access control of sharing spaces reduces waiting time and optimizes workers' efficiency, resulting in operational cost savings. In this paper, we propose a sharing space access management system based on a mobile key and RCU (Room Control Unit), access to the facility using a mobile key, and monitor the facility using an RCU. Proposal system is for shared accommodation, rental field (residence, sale-selling hotel), shared office, etc. when there is a one-time visitor on a specific day and time, the corresponding password is delivered to the mobile platform to expose and key the existing password. It is supported by a field-adaptive system that can reduce discomfort such as delivery. In order to test the operation of the proposed integrated system, tests were conducted according to scenarios to understand the overall status of the user's reservation, check-in, and check-out, and a 100% success rate was derived for each item by setting performance indicators to prove test reliability.

Primiparas만 Perceptions of Their Delivery Experience and Their Maternal-Infant Interaction : Compared According to Delivery Method (초산모의 분만유형별 분만경험에 대한 지각과 모아상호작용 과정에 관한 연구)

  • One of the important tasks for new parents. especially mothers, is to establish warm, mutually affirming interpersonal relationships with the new baby in the family, with the purpose of promoting the healthy development of the child and the wellbeing of the whole family. Nurses assess the quality of the behavioral characteristics of the maternal-infant interaction. This study examined the relationships between primiparas pereptions of their delivery experience and their maternal infant interaction. It compared to delivery experience of mothers having a normal vaginal delivery with those having a casearean section. The purpose was to explore the relationships between the mother's perceptions of her delivery experience with her maternal infant interaction. The aim was to contribute to the development of theoretical understanding on which to base care toward promoting the quality of maternal-infant interaction. Data were collected directly by the investigator and a trained associate from Dec. 1, 1987 to March 8, 1988. Subjects were 3 random sample of 62 mothers, 32 who had a normal vaginal delivery and 30 who had a non-elective cesarean section (but without other perinatal complications) at three general hospitals in Seoul. Instruments used were the Stainton Parent -infant Interaction Scale(1981) and the Marut and Mercer Perception of Birth Scale(1979). The first observations were made in the delivery room (for vaginally delivered mothers only), followed by day 1, day 2, day 3, and 2 weeks, 4 weeks, 6 weeks and 8 weeks after birth, for a total of 7-8 contacts(Cesarean section mothers were observed on days 4 and 5 but the data not used for analysis). Observations in the hospital were made during the hour prior to scheduled feedings. The infant was placed beside the mother. Later contacts were made at home. Data analysis was done by computer using as SPSS program and indulded X² test, paired t-test, t-test, and Pearson Correlation coefficient ; the results were as follows. 1. Mothers who had a normal vaginal delivery tended to perceive the delivery experience more positively than cesarean section mothers(p=0.002). The finding supported the hypothesis I that perception of delivery would vary according to the method of delivery. Mothers' perceptions of birth were classified into three dimensions, labor, delivery and the bady. There was a significantly different and positive perception by the vaginally delivered mothers to the delivery experience(p=0.000) but no differences for labor or the bady according to the delivery method(p=0.096, p=0.389), 2. Mothers who had a normal vaginal delivery had higher average maternal-infant interaction scores(p=0.029) than mothers who had a cesarean section. There were similar higher scores for the 1st day(p=0.042), 2nd day (p=0.009), and the 3rd day(p=0.006) after delivery but not for later times. The findings supported the hypothesis Ⅱ that there would be differences in maternal-infant interaction for mothers having vaginal and cesarean section deliveries. However these differences deccreased section deliveries. However these differences decreased over time . by eight weeks the scores for vaginal delivery mothers averaged 8.1 and for cesarean section mothers, 7.9. 3. The more highly positive the pereption of the delivery experience, the higher the maternal-infant interaction score for all subjects(F=.3206, p=.006). The findings supported the hypothesis Ⅲ that there would be correlations between perceptions of delivery and maternal-infant interaction. The maternal infant interaction was highest when the perception of the bady and deliery was positive(r=.4363, p=.000, r=.2881, p=.012). No correlations between perceptions of labor and maternal-infant interaction were found(p=0.062). 4. The daily maternal-infant interaction score for the initial contact after birth to 8 weeks postpartum had the lowest average score 5.20 and the highest 7.98(in a range of 0-10). This subjects group of mothers needed nursing intervention to promote their maternal- infant interaction. The daily scores for the maternal-infant over the period of eight weeks. However, there were significantly different increases in maternal-infant interaction only from the first to second day(p=0.000) and from the fourth to sixth weeks after birth(P=0.000). 5. When the eight items of maternal-infant interaction were evaluated separately, “Expresses feelings about her role as mother” had the highest average score, 1.64(ina range of 0-3)and “Speaks to baby” the lowest, 0.9. All items, with the possible exception of “Expresses feelings about her role as mother”, suggested the subjects' need of nursing intervention to promote maternal-infant interaction. 6. There were positive correlations between certain general charateristis, namely, both a higher economic status(p=0.002) and breast feeding(p=0.202) and maternal - infant interaction. There were positive correlations between a mother's confidence in her role as a mother and the perception of the birth experience(p=0.004). For mothers who had a cesarean section, a positive perception of the birth experience was related to the duration of her marriage(p=0.010), a wanted pregnancy (P=0.030) and her confidence in her role as a mother(p=0.000). Pereptions of birth for mothers who had a normal vaginal delivery were positive than those for mothers who had a cesarean section. The level of maternalinfant interaction for mothers delivered vaginally was higher than for cesarean section mothers. The relationship between perception of birth and materanalinfant interaction was confirmed. Cesarean section has an impact on the mother's perceived experience of birth which, in turn, is positively related to maternal-infant in turn, is positively related to maternal-infant interaction. Nursing intervention to enhance maternal-infant interaction should begin in prenatal classes with an exploration of the potential impact of cesarean section on the perceptions of the birth experience and continue throughout the perinatal and post-natal periods to promote the mother's ability to control with this crisis experience and to mobilize social support. Nursing should help transform a relatively negatively perceived experience into an accepted, positively perceived and self affirming experience which enhances the maternal-infant relationship.
