The Relationship of Obesity and Related Behaviors among 4th, and 5th Grade-Primary School Children (국민학교 고학년 아동의 비만정도 및 관련행동 연구)

  • The relationship of obesity and related behaviors was analyzed among 361 primary school children, 4th and 5th grade, in one of private school in Seoul in July 1994. Compared to 1985 Korean Children's Growth Standard, the surveyed children have shown remarkable growth, which is the secular trend in growth of Korean children accompanied with rapid economic growth. But the relative incremental weight was far larger than that of height and this trend was more evident among children over 50th percentile, which shows a wide prevalence of obesity of the surveyed children. By WLI index, 15% of the children was overweight and 13% was obese. The higher the WLI, the more frequent and stronger stress they had. The nutrition knowledge score of obese children was higher compared to others. They were especially sensitive to their physical fitness but many normal or underweight-children also experienced stress due to their body weight and have tried to lose weight by incorrect methods. On the other hand many obese children thought they were optimal or underweight. These results reasserts the importance of nutrition education focused on children aheading puberty. They need to know the correct degree of obesity and to get appropriate education through teachers, dietitians, parents and doctors. The findings of this study could be applied to a nutrition implementation policy to ensure better physical fitness of children aheading puberty in the future.

Comparison of the Effects of Internet Diabetic Education on Obesity in Type 2 Diabetic Patients (제 2형 당뇨환자 비만도별 인터넷 당뇨 교육 효과 비교)

  • Purpose: The purpose of this study was to evaluate the effects on obesity of nurse education using the short message service (SMS) of cellular phones and wire Internet. Method: Forty patients in an experimental group, which was assessed pre- and post intervention, completed this study. The patients were divided into two groups according to degree of obesity. The goal of the intervention was to keep blood glucose concentrations close to the normal range. The intervention was applied weekly for 3 months. Participants were requested to input the blood glucose level everyday at http://www.biodang.com by cellular phone or wire internet. The researcher sent optimal recommendations to each patient using SMS of cellular phones and wire Internet. The plasma glucose levels and serum lipids were measured before and after the intervention. Results: After 3 months of education, Glycosylated hemoglobin (HbAlc) decreased by 1.4% in non-obese patients and 0.7% in obese patients. Fasting plasma glucose (FPG) decreased 22.6mg/dl in non-obese patients and 22.3mg/dl in obese patients. Two-hour plasma glucose (2HPG) decreased 97.0mg/dl in non-obese patients and 67.8mg/dl in obese patients. Conclusion: These results indicate that a nurse SMS intervention would improve HbAlc, FPG, and 2HPG in both non-obese and obese patients.

  • Objectives : This study was carried out to evaluate the difference of body mass index(BMI) distribution and the difference of effect of weight reduction program among Sasang constitution. Methods : Sasang constitution was differentiated by QSCC II of 875 cases. Body composition was analysed by bioelectrical impedance analysis(Inbody 3.0, Biospace, Seoul, Korea) in before and after treatment to evaluate the effect among Sasang constitution. The weight reduction program including very low calorie diet, electrical acupuncture and herbal medicine was treated to all cases equally during 1 month. Results and Conclusions : The group of Taeum was more obese and less reducted in body fat mass and % body fat in tools of percent rates of the changes than other type of Sasang constitution. In conclusion, the group of Taeum is more easy to become obese and more difficult to treat obesity

Dietary Behaviors, Health-related Lifestyle and Blood Lipid Profile of Obese Children in Incheon (인천시 비만초등학생의 식행동, 건강관련 생활습관 및 혈중 지질농도)

  • The purpose of this study was to investigate the anthropometric and blood biochemical characteristics, the dietary behaviors and health-related lifestyles of obese children in Incheon. A cross-sectional survey was conducted using anthropometric measurements, biochemical assessments and questionnaire analysis. The subjects included 7,055 obese children residing in the Incheon area (from 106 elementary schools). The degree of obesity was classified using the Obesity Index (OI) as light 20% < OI < 30% ; Medium 30% < OI < 50% ; Severe 50% > OI. The statistical analysis was conducted using SPSS 10.0 program. Most subjects fell within the medium range of obesity. Most subjects had dietary problems such as overeating, unbalanced meals and skipping breakfast. The ratio of boy subjects eating green and yellow vegetables was lowe. when compared to that of the girl subjects. The boy subjects exercised more frequently and longer than the girls. As the Obesity Index increased, hypercholesterolemia significantly increased. The blood cholesterol levels of the subjects were positively correlated with body fat, waist/hip ratio and BMI; HDL cholesterol levels of the subjects were negatively correlated with the anthropometric data and the LDL cholesterol levels of the subjects were positively correlated with body fat. The blood triglycerides levels of the subjects were positively correlated with body weight, body fat, waist/hip ratio and BMI. Therefore, proper nutritional education and intervention are required for an improvement of obese children's dietary behavior, heath-related factors and blood lipid profiles.

Anti-obesity effect of resveratrol-amplified grape skin extracts on 3T3-L1 adipocytes differentiation

  • Resveratrol (3,4,5-trihydroxy-trans-stilbene), a phytoalexin found in grape skin, grape products, and peanuts as well as red wine, has been reported to have various biological and pharmacological properties. The purpose of this study was to investigate the anti-obesity effect of resveratrol-amplified grape skin extracts on adipocytes. The anti-obesity effects of grape skin extracts were investigated by measuring proliferation and differentiation in 3T3-L1 cells. The effect of grape skin ethanol extracts on cell proliferation was detected by the MTS assay. The morphological changes and degree of adipogenesis of preadipocyte 3T3-L1 cells were measured by Oil Red-O staining assay. Treatment with extracts of resveratrol-amplified grape skin decreased lipid accumulation and glycerol-3-phosphate dehydrogenase activity without affecting 3T3-L1 cell viability. Grape skin extract treatment resulted in significantly attenuated expression of key adipogenic transcription factors, including peroxisome proliferator-activated receptor, CCAAT/enhancer-binding proteins, and their target genes (FAS, aP2, SCD-1, and LPL). These results indicate that resveratrol-amplified grape skin extracts may be useful for preventing obesity by regulating lipid metabolism.

Behavioral Strategies for Weight Loss Maintenance (체중감량 이후 장기적인 유지를 위한 다양한 행동전략)

  • Compensatory changes in energy consumption and neuro-hormonal changes following weight loss make it difficult to maintain the reduced weight and may cause weight regain. Therefore, establishing a long-term weight control plan and strategy starting from the initial weight loss period is necessary. Both the patient and doctor should know that weight loss cannot occur continuously, and that maintaining weight after the weight loss period is the basic course of obesity treatment. No single dietary pattern is effective for weight maintenance, and a variety of dietary control methods - such as calorie restriction and healthy proportions of carbohydrates, proteins, fats, and meal replacements - should be used to target an integrated and healthy dietary habit. An increase in physical activity is needed for weight loss and maintenance; however, rather than recommending an excessive amount of exercise, it is better to set realistic and long-term achievable goals. It is necessary to reset the goal according to the patient's weight maintenance stage and continuously apply behavioral therapies, such as self-monitoring and stress management. In previous studies, since the degree of weight loss and changes in behavioral patterns over the course of one year were important factors in maintaining long-term weight loss, obesity therapists should closely examine patient data and behavioral patterns across a period of one year and actively intervene when needed.

Stress and Nutrient Intakes by the Degree of Obesity in Female College Students (비만도에 따른 여대생의 스트레스와 영양소 섭취)

  • This study was conducted to find associations of stress with obesity in female college students in Gwangju. 343 subjects were divided into two groups based on BMI(body mass index). BMI of normal group was $19.3{\pm}1.5$ while that of obese group was $26.4{\pm}3.6$. The measured stress level in obese subjects was higher and they were less capable of controlling obese oriented attitude compared to normal subject. Twenty four hour dietary recall revealed that nutrient intakes of obese subjects were higher in energy, lipid, P and cholesterol. However, they were more deficient in Ca, Fe and vitamin A, compared to those of normal ones. This study suggested severe stress might trigger undesirable dietary behavior leading to increase in food consumption which contribute to obesity. Effective nutrition education program targeting obese female students should be developed and implemented to relieve stress and practice desirable dietary behavior and eating pattern.

The prevalence of obesity and the level of adherence to the Korean Dietary Action Guides in Korean preschool children

  • The objective of this study was to investigate the associations between the prevalence of overweight and obesity and the degree of adherence to the Korean Dietary Action Guides for Children (KDAGC). In a cross-sectional study based on a child care center-based survey in Seoul, Korea, we collected parental-reported questionnaires (n = 2,038) on children's weight and height, frequency of fruit and vegetable consumption, and the quality of dietary and activity behaviors based on the 2009 KDAGC Adherence Index (KDAGCAI) which was developed as a composite measure of adherence to the KDAGC. Overweight and obesity were determined according to age- and sex-specific BMI percentile from the 2007 Korean national growth chart. Associations were assessed with generalized linear models and polytomous logistic regression models. Approximately 17.6% of Korean preschool children were classified as overweight or obese. Obese preschoolers had lower adherence to the KDAGCAI compared to those with lean/normal weight. Preschoolers with a high quality of dietary and activity behaviors had a 51% decreased odds ratio (OR) of being obese (highest vs. lowest tertile of KDAGCAI-score, 95% CI 0.31, 0.78; P = 0.001); the associations were more pronounced among those who were older (P = 0.048) and lived in lower income households (P = 0.014). A greater frequency of vegetable consumption, but not fruit, was associated with a borderline significant reduction in the prevalence of obesity. Our findings support the association between obesity prevention and high compliance with the Korean national dietary and activity guideline among preschool children.

Weighting Method based on Experts Opinions for Obesity Syndrome Differentiation Questionnaire (전문가 가중치 부여를 통한 비만변증설문지 적용)

  • Objectives We have investigated a questionnaire on syndrome differentiation pertaining to obesity. To calculate data from this questionnaire, we can simply sum up the degree of symptoms. However, this does not reflect the difference in contribution of syndrome differentiation. In order to improve the level of precision of this questionnaire, we gather the weight of each symptom from experts and apply them to overweight persons. Method Nine Experts from The Society of Korean Medicine for Obesity Research nominated weights for the symptoms. We created a program based on weight survey results and applied to 1487 overweight persons and 26 oriental medical doctors. The concordance rate between the result obtained from the oriental medical doctors and that obtained using three methods was analyzed. Results 1. The reliability of this questionnaire is very high (Cronbach' ${\alpha}$=0.963). 2. The concordance level between diagnosis by oriental medical doctors and the result of general calculation is 0.347, between diagnosis by oriental medical doctors and the result of weighted calculation by syndrome differentiation is 0.362, between diagnosis by oriental medical doctors and the result of weighted calculation by symptoms is 0.1. Conclusions Weighted calculation by syndrome differentiation is relative more appropriate among three methods studied.

Obesity Related Glomerulopathy Progressed to Chronic Renal Failure (만성 신부전으로 진행된 비만 연관 사구체병증 1례)

  • Obesity-related glomerulopathy (ORG) is a secondary form of focal and segmental glomerulosclerosis (FSGS) manifesting as proteinuria and progressive renal dysfunction that results from maladaptive glomerular response to increasing adiposity. Reports of ORG progressing to end stage renal diseases in rare in the pediatric population. We report a 9-year-old boy with obesity (body mass index $35\;kg/m^2$) who was diagnosed with ORG presenting with proteinuria. He was diagnosed with obesity-related glomerulopathy based on the laboratory, urinary, and kidney biopsy finding. In spite of treatment with angiotensin- converting enzyme (ACE) inhibitor and/or, angiotensin-receptor blocking agent, the degree or amount of proteinuria increased and renal function declined continuously. His BMI did not decrease and eventually progressed to chronic renal failure. Consequently, obese patients should be monitored for proteinuria, which may be the first manifestation of FSGS, a lesion that may be associated with serious renal sequelae.
