In-hospital malnutrition among adult patients in a national referral hospital in Indonesia

  • BACKGROUND/OBJECTIVES: Malnutrition during hospitalization is linked to increased morbidity and mortality, but there are insufficient studies observing clinical factors contributing to weight loss during hospitalization in Indonesia. This study was therefore undertaken to determine the rate of weight loss during hospitalization and the contributing factors. SUBJECTS/METHODS: This was a prospective study involving hospitalized adult patients aged 18-59 yrs, conducted between July and September 2019. Body weight measurement was taken at the time of admission and on the last day of hospitalization. The factors studied were malnutrition at admission (body mass index < 18.5 kg/m2), immobilization, depression (Beck Depression Inventory-II Indonesia), polypharmacy, inflammatory status (neutrophil-lymphocytes ratio; NLR), comorbidity status (Charlson Comorbidity Index; CCI), and length of stay. RESULTS: Totally, 55 patients were included in the final analysis, with a median age of 39 (18-59 yrs) yrs. Of these, 27% had malnutrition at admission, 31% had a CCI score > 2, and 26% had an NLR value of ≥ 9. In all, 62% presented with gastrointestinal symptoms, and depression was documented in one-third of the subjects at admission. Overall, we recorded a mean weight loss of 0.41 kg (P = 0.038) during hospitalization, with significant weight loss observed among patients hospitalized for 7 days or more (P = 0.009). The bivariate analysis revealed that inflammatory status (P = 0.016) was associated with in-hospital weight loss, while the multivariate analysis determined that the contributing factors were length of stay (P < 0.001) and depression (P = 0.019). CONCLUSIONS: We found that inflammatory status of the patient might influence the incidence of weight loss during hospitalization, while depression and length of stay were independent predictors of weight loss during hospitalization.

The Effects of Foot Massage on Fatigue and Depression among Patients with Hematological Malignancies (발마사지가 혈액 암 환자의 피로와 우울에 미치는 효과)

  • Purpose: This study was to investigate the effects of foot massage on fatigue and depression among patients undergoing chemotherapy with hematological malignancies. Method: The subjects were 60 patients hospitalized for undergoing chemotherapy from September 2007 to March 2008, and 30 of them were the experimental group and the other 30 were the control group. To the subjects in the experimental group, foot massage was provided for 30 minutes everyday before the chemotherapy at the same time for 4 days, and the effect was measured on Day 4 and 5 after chemotherapy. Fatigue was measured using the Brief Fatigue Inventory (BFI) developed by Mendoza et al. (1999), and depression was measured by psychiatric rating scale SCL-90-R developed by Derogatis (1977) and translated by Kim Gwang il et al. The data were analyzed through $x^2$-test, Fisher's exact test, unpaired t-test and repeated measures ANOVA using SPSS 12.0. Results: Fatigue and depression were significantly lower in the experimental group than in the control group. Even on Day 5, fatigue and depression were significantly lower in the experimental group than in the control after finish foot massage. Conclusion: The findings of the study demonstrated that foot massage would be an effective nursing intervention to reduce fatigue and depression in patients with hematological malignancies.

Perceptions of primiparas of their Newborns : A Comparision of Koreans and Americans (한국과 미국 초산모의 신생아에 대한 지각 비교)

  • The purpose of this study was to compare Korean and American women as to the perception of their newborns, and to assess factors contributing to a positive mother-infant relationships. American mothers were with their own newborns in the same rooms and could feed and take care of them if they wanted, but Korean mothers were separated from their babies. The subjects of this study were 86 Korean and 86 American primiparas within two-three days after delivery. Data were collected from May to August 1993, using the Neonatal Perception Inventory (NPI) devised by Broussard(1963) with additions by Lee, Ja Hyung(1986). The results of this study are as follows : 1. There was a significant difference in the mothers' perceptions of their babies according to mothers' age(P<0.05, P0.01). Mothers of 20 years and downward had negative perceptions of their babies. 2. There was no difference in the methers' perceptions of their babies according to whether they had a job or not. 3. There was a significant difference in the mothers' perceptions of their newborns according to mothers' education level(P<0.01). Mothers graduated from a junior high school had negative perceptions of their babies. 4. There was no difference in the mothers' perceptions according to their feeding pattern. 5. There was a significant difference between Korean mothers' perceptions and American mothers' perceptions of their babies(P<0.01). 69.7% of Korean mothers and 44. l% of American mothers had positive perceptions. But Korean mothers perceived that it would be more difficult for them to take care of their babies. As seen above, Korean primiparas evaluated their babies higher than Americans. But they perceived that it would be more difficult for them to take care of their babies. The results suggest that there needs a rooming-in system and systematic prenatal educations for the primiparas in Korea.

Case Series Reporting 7 Somatic Symptom Disorder Cases Diagnosed as Greater Yin Disease by Shanghanlun Provisions (『상한론(傷寒論)』 태음병(太陰病)으로 진단된 신체증상장애 환자 7례 증례 보고)

  • Objectives: This study was conducted to confirm the reproducibility of the diagnosis of greater yin disease (太陰病) and the efficacy of herbal treatment using Gyejigajakyak-tang or Gyejigadaehwang-tang in somatic symptom disorder (SSD). We also further refined the characteristics of greater yin disease in SSD. Methods: We analyzed the demographic and treatment data of seven patients with SSD from four Korean medical clinics. The reliability of diagnosis and the efficacy of the treatments were evaluated by the Beck Anxiety Inventory (BAI), a numerical rating scale (NRS), and the patients' statements. Finally, we analyzed patients according to the diagnostic points of Shanghanlun provision 273. Results: Seven SSD patients took Gyejigajakyak-tang or Gyejigadaehwang-tang. The mean age of the patients was 52.1±15.8 years old, and the herbal medication period was 82.2±38.2 days. At the first visit, the patients showed hypochondriasis, obvious stress, and a severe BAI score (40.4±10.9). The NRS significantly decreased from 8.9±1.6 to 0.7±0.7 after the treatments. There were no side effects reported. As a result of analyzing these 7 cases, hypochondriasis, obvious stress, and anxiety were identified, which had been previously suggested as 自痛, 因爾, and 結硬. Conclusions: We confirmed the reproducibility of greater yin disease in 7 SSD cases from 4 medical institutions. These results imply that there could be a considerable relation between SSD and greater yin disease as defined by the Shanghanlun provisions.

Water balance change at a transiting subtropical forest in Jeju Island

  • Jeju island has a humid subtropical climate and this climate zone is expected to migrate northward toward the main land, Korea Peninsula, as temperature increases are accelerated. Vegetation type has been inevitably shifted along with the climatic change, having more subtropical species native in southeast Asia or even in Africa. With the forest composition shift, it becomes more important than ever to analyze the water balance of the forest wihth the ongoing as well as upcoming climate change. Here, we implemented the Ecosystem Demography Biosphere Model (ED2) by initializing the key variables using forest inventory data (diameter at breast height in 2012). Out of 10,000 parameter sets randomly generated from prior distribution distributions of each parameter (i.e., Monte-Carlo Method), we selected four behavioral parameter sets using remote-sensing data (LAI-MOD15A2H, GPP-MOD17A2H, and ET-MOD16A2, 8-days at 500-m during 2001-2005), and evaluated the performances using eddy-covariance carbon flux data (2012 Mar.-Sep. 30-min) and remote sensing data between 2006-2020. We simulated each of the four RCP scenarios (2.6, 4.5, 6.0, and 8.5) from four climate forcings (GFDL-ESM2M, HadGEM2-ES, IPSL-CM5A-LR, and MIROC5 from ISIMIP2b). Based on those 64 simulation sets, we estimate the changes in water balance resulting from the forest composition shift, and also uncertainty in the estimates and the sensitivity of the estimates to the parameters, climate forcings, and RCP scenarios.

Web-based Agricultural Machinery Rental Business Management System

  • Purpose: This study was conducted to develop a web-based business management system to ensure the efficient operation and transparent management of government-subsidized agricultural machinery rental businesses. Methods: An MS_SQL2000 database management system (DBMS) solution was utilized in the system for high system compatibility and integrated management. This system was targeted to be compatible with Internet Explorer 6.0 or later and to ensure security and seamless web operations. The system administrator is able to manage a fleet of agricultural machinery, including various inventory codes, release and return, fleet registry, and business performance. Users (farmers) may search the database of rental machinery and reserve them. Results: With respect to rental reservations, the system administrator can manage the fleet by setting the rental status to Approved, Released, or Returned. Through the web, the administrator can also create a database that includes machinery specifications, features, and rental rates. In addition, business performance data can be analyzed using a diverse array of tools to streamline the rental business. Without having to go to the rental office, users can save time and money by searching for and renting agricultural machinery through the information available on the website, including availability, specifications, and rental fees. After deploying the system, the time required to analyze monthly performance and create reports was dramatically reduced from 20 days per person to one day per person. Conclusions: Since 2014, AMRB has been installed and is operating in agricultural machinery rental businesses in 31 cities and counties in South Korea. This study recommends continued expansion and dissemination of AMRB for the systematic and efficient management of agricultural machinery rental businesses.

Occupational Stress of Workers in the Manufacturing Industry (일부 산업장 근로자들의 직무스트레스 분석)

  • The purpose of this study was to examine which factors affected the job stress of workers in the manufacturing industry. The subjects were 128 workers in automobile component manufacturing companies located in Kyonggi-do province. Data were collected through self-reporting technique for 8 days, October 24-31, 2000. The instrument to test the work related truss was NIOSH's Work-related Stress Inventory revised by Korea Occupational Safety and Health Association(KOSHA) in 1999. The collected data were analysed with SAS. and frequency, percentage, mean value, standard deviation, t-test, ANOVA. Scheffe's post hoc test and Pearson's Correlation Coefficients. The major findings were as follows: 1. Mean job stress score for the job-related stress was $2.6{\pm}0.5$ on the basis of 5.0 points. Job stress was significantly different according to gender. The stress by role conflict showed higher score than the stress by role ambiguity. 2. Discretionary power(freedom to determine) of workers was scored $2.6{\pm}0.7$ on the basis of 5.0 points. And it was significantly different according to gender, duration of work and shift experience. 3. The work environment scored $2.2{\pm}0.3$ on the basis of 3.0 points and significant difference according to gender, age, marital status, type of employment, type of service, and shift work. 4. Role ambiguity showed significant negatively correlate with job control(r=-.32, P=.000) and decision making(r=-.31, P=.000). And decision making showed positively correlated with job control(r=.62, P=.000), and role conflict(r=.26, P=.003).

The Relationship between Psychopathology and Hypoxemia in Hospitalized Pneumoconiosis Patients (진폐증 입원환자에서 저산소혈증과 정신병리의 연관성)

  • Objective : We examined the putative association between the psychological symptoms and hypoxemia in hospitalized pneumoconiosis patients. Methods : One hundred and nine hospitalized pneumoconiosis patients were divided into two groups according to their level of hypoxemia (PaO2); hypoxemic group consisted of 47 patients with PaO2 lower than 80 mmHg, and comparative group consisted of 62 patients with higher than 80 mmHg. All of them were interviewed with the Hamilton Depression Rating Scale (HAM-D), and completed the Symptom Checklist-90items-Revised (SCL-90-R) and the State-Trait Anxiety Inventory (STAI). The medical data (PaO2, PaCO2, Hb, and Hct), number of hospital days and socio-demographic data as well as scores of the three psychological measurements were compared between the two groups. Results : Hypoxemic patients with lower PaO2 showed higher scores in the HAM-D (p=0.01) and stateanxiety subscale of the STAI (p<0.05). Values of hemoglobin and hematocrit were negatively correlated with levels of trait-anxiety (p<0.05). Thirty patients (27.5%) of total subjects suffered from depression (higher than 18 in the HAM-D), and 46.8% of hypoxemic patients were depressed, whereas 12.9% of comparative group patients were depressed (p=0.01). Conclusion : These results suggest that depression and anxiety might be associated with the hypoxemic mechanism in pneumoconiosis patients.

  • Objectives: This research was performed to know severity of depression and anxiety, the psychopathology of hemodialysis patients and kidney transplantation patients using Minneesota Multiphasic Personality Inventory(MMPI) and Zung's Self-rating Depression Scale (SDS), Zung's Self-rating Anxiety Scale(SAS), MMPI Subscales. Methods: We surveyed 31 hemodialysis patients and 119 kidney transplantation patients. 119 kidney transplantation group(KT) was investigated at ; 1) Before kidney transplantation (KT-1), 2) Three days after kidney transplantation(KT-2), 3) Three weeks after kidney transplantation(KT-3),4) Follow up at OPD(F/U). Results: 1) According to dermographic data, mean age was KT 33.1, HD 42.2, Control 33.1 years old and KT, HD were belonged to lower economic states and lower educational level than Control. 2) In the depression scale for SDS, KT-1 was more depressed than F/U and Control but depression scale was significantly decreased at KT-2 in comparison with HD. In the anxiety scale for SAS, KT-1 was more anxious than Control but anxiety scale was not different within IT subgroups and in comparison with HD. 3) In comparison of MMPI scales, Hs, D, Pt, Ma at KT-1, Pd, Pa, Pt, Ma at KT-2, F, D, Pd, Pt, Pa, Sc, Ma at KT-3, Pt at F/U were more high scores than Control.

A Case Study of A Patient Complaining of Disequilibrium Due to Proprioceptive Disorder (고유수용감각이상으로 인한 균형장애를 호소하는 환자 치험 1례)

  • ■Objectives This work reports the case of a patient complaining of disequilibrium due to proprioceptive disorder whose condition improved following Korean medicine treatment. ■Methods The patient was hospitalized for 15 days and treated with herbal medicine, mainly doinseunggi-tang-gami, and acupuncture, electroacupuncture, and moxibustion. Disequilibrium was clinically estimated using Korean dizziness handicap inventory(K-DHI) and the patient's subjective discomfort in this regard was also checked by numeral rating scale. The degree of improvement in blood stasis syndrome was evaluated using modified Blood Stasis Questionnaire II(modified BSQ-II) to assess the clinical effects of the treatment. ■Results After treatment, the K-DHI classification improved from severe to mild, and the patient's subjective discomfort was greatly improved from NRS 6 to NRS1. In addition, the degree of improvement in blood stasis syndrome improved from blood stasis group to blood stasis risk group compared to time of admission. ■Conclusion These results suggest that Korean medicine is effective in treating disequilibrium caused by proprioceptive disorder.
