만성질환 노인의 건강 평형감에 대한 개념분석 (Concept Analysis of Health Equilibrium in the Community-dwelling Elderly With Chronic Diseases)

  • Purpose: The purpose of this study was to define and clarify the concept of 'equilibrium' in community-dwelling elderly persons with chronic diseases. Methods: A hybrid model was used for development of the concept of equilibrium. The model included a field study conducted in Seoul, Korea. Participants in this study included 12 elderly persons with chronic diseases who were taking medicine regularly. Results: The concept of equilibrium was found to be a complex phenomenon having meaning in two dimensions: the family-friendly and social relation-oriented dimensions. Four attributes and nine indicators were defined. Conclusion: Equilibrium was defined as the state of having health recognition, will power, and motive power (family-friendly dimension) and having a feeling of participation (social relation-oriented). Therefore, nurses who work with elderly persons with chronic disease in the community should be aware of the attributes and indicators of equilibrium in order to enhance the equilibrium of their elderly clients with chronic diseases.

일 지역 노인의 사회적 건강결정인자와 건강 관련 삶의 질과의 관계 (The Relationships between Social Determinants of Health and Health-related Quality of Life among the Community-dwelling Elderly)

  • Purpose: The purpose of this study was to examine the social determinants of health (SDH) associated with health-related quality of life (HQOL) among the community-dwelling elderly, based on the conceptual framework of the Commission on Social Determinants of Health (CSDH). Methods: A survey was conducted with 199 elders aged 65 years or older sampled through convenient sampling. Data were collected through face to face interviews by trained interviewers using a structured questionnaire, which included scales of the residential area, social support, sense of community, social network, health behaviors, and HQOL. Data were analyzed by ANOVA and stepwise multivariate regression. Results: Major SDH affecting HQOL included the participants' residential area, sense of community, and health behavior. The HQOL of the elderly residing in the B area with a low rate of basic livelihood security recipients was higher than that of those residing in other areas. Conclusion: CSDH framework was useful to determine the factors associated with HQOL among the community-dwelling elderly. In addition to their health behavior, their sense of community was found to be a SDH of HQOL, indicating the need of health promotion programs tailored to the characteristics of residential areas and strategies to enhance involvement in community activities.

경도인지장애 여성노인의 낙상두려움 예측 요인 (Predictors of the Fear of Falling among Elderly Women with Mild Cognitive Impairment)

  • Purpose: The purpose of this study was to identify the predictors influencing fear of falling in community-dwelling elderly women with mild cognitive impairment (MCI). Methods: A secondary data analysis was performed using data of 65 years or older elderly women with MCI participating in the 7th Korea Longitudinal Study of Ageing of the Korea Employment Information Service. The study subjects included 368 elderly women with MCI. For data analysis, descriptive statistics and logistic regression with complex samples were performed using IBM SPSS ver. 23.0. Results: 89.9% of the elderly women with MCI had fear of falling. There were significant factors such as religion (OR=8.85, 95% CI: 3.39~23.15), restriction of activity (OR=6.84, 95% CI: 2.14~21.90), depression (OR=0.75, 95% CI: 0.62~0.90), and MMSE (OR=1.30, 95% CI: 1.03~1.63), predicting fear of falling in community-dwelling elderly women with MCI. Conclusion: Differentiated strategies should be developed for elderly women with MCI to decrease fear of falling and prevent falls with understanding of contributing factors. This study will provide fundamental information on programming and a policy proposal related to fear of falling for elderly women with MCI.

저소득층 재가노인의 기능상태와 요구되는 요양서비스 유형 분석 (Functional Status and Long-Term Care Services for the Community-Dwelling Low-Income Elderly)

  • Purpose: This study was conducted to explore the functional status and long-term care services for the community-dwelling low-income elderly. Method: A descriptive research design was used in this study. The functional status of the participants was obtained using Minimum Data Set-Home Care Version 2.0 and the long-term care services were identified via Michigan's choice. Total of 154 persons aged 65 years or older completed Korean Minimum Data Set-Home Care Version 2.0 on the community dwelling low-income elderly. Results: The average of Activities of Daily Living was 4.19, and the range was 0-55, while the average of Instrument of Activities of Daily Living was 4.85 and the range was 0-56. Among the subjects, 46.1% belonged to the Information and Referral group and 1.3% to the Nursing Home group. Severe daily pain was reported by 14.9%, and 76.6% of the participants had impaired vision. The Activities of Daily Living was difference according to living with, education, vision, and depression. The long-term care services differed according to gender, pain, vision, hearing, and depression. Conclusion: The support policy for the elderly needed to focus on impaired visual and depression to enhance the activities of daily living. Moreover, there is a need for the Information and Referral group to arrange and develop nursing intervention resources.

신체활동과 우울이 지역사회 거주 노인의 수면의 질에 미치는 영향: 전기노인과 후기노인의 비교 (The Influence of Physical Activity and Depression on Sleep Quality in Community-dwelling Older Adults: A Comparison between Young-old and Old-old)

  • Purpose: The purpose of this study was to identify the influence of physical activity and depression on sleep quality among the young-old and old-old community-dwelling elderly. Methods: Participants were 216 community-dwelling older adults in Korea aged 65 or above. Data were collected using structured questionnaires with face-to-face interviews that included demographic and health-related characteristics, International Physical Activity Questionnaires (IPAQ), the Short Form Geriatric Depression Scale (SGDS) and the Pittsburgh Sleep Quality Index (PSQI). A hierarchical multiple regression was conducted to examine whether physical activity and depression would predict sleep quality under other controlled factors. Results: There were differences in demographic and health-related characteristics, physical activity, and depression by age groups, but not in sleep quality. In the young-old elderly, physical activity (${\beta}=-0.22$, p=.043) and depression (${\beta}=0.31$, p=.002) were significantly associated with sleep quality (F=4.46, p=.001, Adjusted $R^2=.16$). In the old-old elderly, physical activity (${\beta}=-0.29$, p=.001) and depression (${\beta}=0.41$, p<.001) were significantly associated with sleep quality (F=10.79, p<.001, Adjusted $R^2=.29$). Conclusion: These finding highlight physical activity and depression as important contributors to sleep quality in both young-old and old-old elderly.

농촌 마을회관의 노인 복지서비스 공간으로의 활용 방안 - 평면과 이용행태 분석을 중심으로 - (Conceptual Directions of Village Community Center into a Rural Elderly's Welfare Service Space in Chonbuk Province)

  • This purpose was to explore various directions of elderly service welfare space in village community center. Data were collected 3 types that floor plans of village community centers from 24 villages, interview data of these centers use and needs from 24 aged leaders or 96 elderly. The results were as followed: 1) The floor plans of more than 50% was type IV that was composed a basic spaces(entrance, living room, two rooms, kitchen), a toilet space, a heating system space and a storage space. This type was a current basis of the rural village community centers for elderlies. 2) Elderlies used the centers at winter or out of agricultural season, and visited these to pass the time and to promote friendship among them. 3) Also they went these to take a meal together and to save down personal heating expenses. Their needs for centers were a temporary share dwelling space at winter, a maintain social relationship throughout a health, a education and a side work activity, supplying health or sports facilities, and setting up toilet space. Therefore the current centers were needed to add functions of a temporary share dwelling, a energy saving heat system and a social promoting relationship, and so these could be represented to a elderly welfare service center in rural villages.

Wheel of Wellness Counseling in Community Dwelling, Korean Elders: A Randomized, Controlled Trial

  • Purpose: The purpose of this study was to investigate the effects of Wheel of Wellness counseling on wellness lifestyle, depression, and health-related quality of life in community dwelling elderly people. Methods: A parallel, randomized controlled, open label, trial was conducted. Ninety-three elderly people in a senior welfare center were randomly assigned to two groups: 1) A Wheel of Wellness counseling intervention group (n=49) and 2) a no-treatment control group (n=44). Wheel of Wellness counseling consisted of structured, individual counseling based on the Wheel of Wellness model and provided once a week for four weeks. Wellness lifestyle, depression, and health-related quality of life were assessed pre-and post-test in both groups. Results: Data from 89 participants were analyzed. For participants in the experimental group, there was a significant improvement on all of the wellness-lifestyle subtasks except realistic beliefs. Perceived wellness and depression significantly improved after the in the experimental group (n=43) compared to the control group (n=46) from pre- to post-test in the areas of sense of control (p =.033), nutrition (p =.017), exercise (p =.039), self-care (p <.001), stress management (p =.017), work (p =.011), perceived wellness (p =.019), and depression (p =.031). One participant in the intervention group discontinued the intervention due to hospitalization and three in the control group discontinued the sessions. Conclusions: Wheel of Wellness counseling was beneficial in enhancing wellness for the community-dwelling elderly people. Research into long-term effects of the intervention and health outcomes is recommended.

재가 관절염 노인의 낙상 예방행위 예측모형 구축 (A Structural Equation Model of Fall Prevention Behavior among Community-dwelling Older Adults with Osteoarthritis)

  • Purpose: The purpose of this study was to explain fall prevention behaviors of community-dwelling elderly with osteoarthritis based on the Health Belief Model. Methods: A total of 200 older adults with osteoarthritis residing in community was recruited from July 10 to August 30, 2013. The direct and indirect effects of perceived fall risk, perceived benefits and barriers, and self efficacy of fall prevention were examined on fall prevention behaviors. Data were collected with structured questionnaires and analyzed using SPSS/WIN 20.0 and AMOS 20.0 program. Results: The hypothetical model was a good fit for the data based on the model fit indices. Among socio-demographic variables, age and fall knowledge showed significant direct effects on fall prevention behaviors. The constructed model explained 34.2% of the variance of fall prevention behaviors, including perceived fall risk and efficacy of fall prevention behaviors as significant predictors. Conclusion: The findings revealed the need to develop an effective nursing intervention to promote fall prevention behaviors of community-dwelling elderly with osteoarthritis by focusing on perceived fall risk and efficacy of fall prevention behaviors. Knowledge about fall can also be increased by an age-based education program.

지역사회 노인의 죽음준비에 영향을 미치는 요인: 2014년도 노인실태조사를 중심으로 (Factors Related to Death Preparedness among Community-dwelling Older Adults in Korea: Using Korean National Survey on Elderly 2014)

  • Purpose: This study examined the current state of death preparedness and factors related to the death preparedness among community-dwelling older adults in Korea. Methods: A total of 6,879 older adults' data, selected from the Korean National Survey on the Elderly 2014 data, were analyzed. The criteria for selection were responding to the questionnaires directly and normal cognitive function. Stratified sampling logistic regression analysis was conducted on the compiled data. Results: It was seen that 37.3% of older adults were prepared for death; specifically, 28.6% had arranged for resting places, 10.7% had purchased burial clothes, 6.8% had subscribed to mutual aid societies, 0.7% participated in death education, and 0.6% had signed a will. The results of logistic regression indicated that the older adults who were females, older age, religious, highly educated, more satisfied with their economic status, living in rural areas, and participating in more social activities were more likely to prepare for their deaths compared to their counterparts. Conclusion: Education programs that prepare for death need to target marginalized groups in the community, such as male older adults and whose participation in social activities or satisfaction with their economic status are relatively low.

시설노인과 지역사회 거주노인에서 사회적 유리에 따른 인지기능과 신체기능 (Cognitive and Physical Function by the Social Disengagement in Institution and Community Living Elderly)

  • Purpose: This study was conducted to compare the level of cognitive and physical function by the level of social disengagement in institutions and community dwelling elderly. Method: The study subjects were 209 people whose age ranged from 65 to 94. The data were surveyed with social disengagement index, MMSE-K, and ADL, and analysed by SPSS 11.0 using frequency, t-test, ANCOVA and ANOVA. Results: 1) The level of social disengagement was 1.67 in institution and 3.25 in community elderly, and showed statistically significant difference (F=41.07, p<.001). 2) There were statistically significant difference in MMSE-K (F=15.51, p<.000), and activity of daily living (F=9.65, p=.002) between two residence after control the age, gender, religion, allowance and number of children. 3) There were statistically significant difference in MMSE-K (F=6.55, p=.002) by social disengagement in community dwelling elderly. Conclusions: Programs for successful aging to reduce social disengagement should be developed that stimulate social activities and then improve cognitive function for the elderly. For these, establishing social policies in nation and local government is needed. Especially, these policies should be conducted among the elderly with low-income living in institutions.
