Surgical treatment of esophageal cancer (식도암의 외과적 치료)

  • Between March 1978 and December 1985, 39 patients were admitted to our hospital & surgery was performed to 27 patients. Among 39 patients, male patients were 31 cases, female 8 cases and the age ranged from 42 years old to 69 yrs old with the average of 55 years old. Main symptoms of esophageal cancer were dysphagia [6%], weight loss [20%], retrosternal and epicardial discomfort [18%], hoarseness [13%], and hiccup [5%]. The anatomical locations of esophageal cancer were followed as; 51% in lower esophagus & cardia, 44% in middle, and 5% in upper esophagus. Among 27 cases, 5 cases were managed by feeding gastrostomy and jejunostomy due to inoperability, 19 cases by esophagogastrostomy, and 3 cases by colon bypass with the resection of esophageal cancer. Postoperative complications were noticed in 7 cases, such as anastomotic leakage in 2 cases [7%], respiratory insufficiency in 2 cases [7%], intussusception in 1 case [4%], wound dehiscence in 1 case [4%], and hepatitis in 1 case [4%]. Among 2 respiratory insufficiency, 2 patients die as a result of that complication and operative mortality was 7%.

Inferior Mesenteric Plexus Block Performed in the Lateral Position (측와위에서 시행한 하 장간막 신경총차단)

  • Inferior mesenteric plexus block(IMPB) sa useful nerve block for the relief of intractable lower abdominal and pelvic pain caused by a lower abdominal visceral or a pelvic malignancy. IMPB has been performed in the prone position. But there are many patients who can't lie in the prone position, because ascites is frequently noticed in cancer patients and they also frequently received abdominal operations. We performed IMPB in the lateral position on two patients with lower abdominal pain, Case 1: A 77 year old female who had a right ovarian cancer with metastatic cancer of descending colon and rectum, experienced complete pain relief. Case 2: A 72 year old female who had a far advance pancreatic cancer with intestinal obstruction due to carcinomatosis received right and left celiac plexus block and right and left IMPB. The patient was satisfied with the result of these pain blocks. Conclusion; IMAPB performed in the lateral position on two patients with lower abdominal pain and their results were excellent for pain relief.

  • Mucinous adenocarcinoma (MAC) is a rare malignant neoplasm that occasionally occurs in the large intestine (colon), followed by the pancreas, ovary, lung, prostate, and breast. It is characterized by large amounts of extracellular epithelial mucin that contains tumor cell nests. We herein present a unique case of MAC originating from minor salivary gland, the second to be reported in literature in South Korea. We report a case of MAC in the tongue considered to be developed from minor salivary gland with a review of literature.

A Study of Cancer Incidence Rate due to Photoneutron Dose during Radiation Therapy for Prostate Cancer Patients (전립샘암 환자의 방사선 치료 시 광중성자 선량으로 인한 암 발생률의 연구)

  • The purpose of this study was to study the probability of cancer occurrence due to photoneutron dose exposure of the colon and thyroid gland, which are normal organs, in 3D CRT, IMRT 5 portals, and IMRT 9 portals, which are radiotherapy methods for prostate cancer. The total prescribed dose for prostate cancer was 6600 cGy, 220 cGy per dose, and 30 divided irradiations were applied for the total number of times. After setting up the Rando phantom on the treatment table (couch) of the medical linear accelerator used in the experiment, an optically stimulated luminescence albedo neutron dosimeter was placed on the corresponding area of the large intestine and thyroid gland of the phantom for measurement. During 3D CRT of prostate cancer, the probability of secondary cancer due to photoneutron dose to the colon and thyroid gland, which are normal organs, was 1.8 per 10,000 people. And IMRT 5 portals were 8.7 per 10,000 people, which was about 5 times larger than 3D CRT. IMRT 9 portals derived the result that there is a probability that 1.2 people per 1,000 people will develop cancer. Based on this study, the risk of secondary radiation exposure due to the dose of photoneutrons generated during radiation therapy is studied, and it is thought that it will be used as useful data for radiation protection in relation to the stochastic effect of radiation in the future.

Anti-inflammation and Anti-cancer Activity of Methanol Extract of Antarctic Lichen, Usnea Aurantiaco-atra (남극 지의류 Usnea Aurantiaco-atra의 메탄올 추출물의 항염증 및 항암 활성)

  • Inflammation by the innate immune system is a protective mechanism of the organism against infection-mediated environmental factors. It is also responsible for the pathogenesis of various human diseases, including the progression of cancer. Lichens are receiving increasing attention as a source of bioactive molecules with therapeutic potential for a variety of diseases. Additionally, the antioxidant, anti-inflammatory, and anticancer potential of lichen and its secondary metabolites have been widely reported. However, the underlying mechanism is still unknown. In the present study, to investigate molecular mechanisms of anti-inflammation and anti-cancer activity in the Antarctic lichen, Usnea aurantiaco-atra, methanol extract of Usnea aurantiaco-atra (MEUS) was used in vitro assays in RAW 264.7 macrophages cell and HCT116 colon cancer cells. Based on our data, MEUS had the anti-inflammatory activity through the modulation of main inflammatory indicators such as COX-2, IL-6, iNOS, TNF-α and NO production in a concentration-dependent manner. In addition, we observed that MEUS had cytotoxic activity against HCT116 colon cancer cells in a concentration-dependent manner, leading to a significantly reduced proliferation of the cancer cells through apoptotic induction by activating caspase-3. Taken together, this work firstly reported the anti-inflammatory and anti-cancer activities of an Antarctic lichen, Usnea aurantiaco-atra, and MEUS may provide a new insight into the molecular mechanisms underlying a link between inflammation and cancer.

Information Needs on Patients with Cancer in Korea (암 환자의 정보요구 분석)

  • Purpose: To explore what particular types of information were important to patients diagnosed with cancer. Methods: Seventy three patients with cancer at an outpatient clinic and hospitalized patients in W Christian Hospital Korea, responded. The structured questionnaire developed by the investigator based on previous studies. Results: There was a significant negative relationship between age and the score of informational need (r=-.307, p<.05). Level of education, and level of monthly income were related to level of informational need. The top three informational priorities according to the time since diagnosis were 'Self care during treatment', 'Health food and diet', 'Likelihood of recurrence', 'Follow up care' and 'Side effects'. The top three informational priorities for patients with breast cancer were 'Likelihood of recurrence', 'Metastasis possibility', 'Treatment options', and 'Side effects. For patients with stomach cancer, they were 'Follow up care', 'Healthy food and diet', 'Likelihood of recurrence', and 'Metastasis possibility', and for patients with colon/rectal cancer, they were 'Side effects', 'Healthy food and diet', 'Likelihood of recurrence', and 'Self care during treatment'. Conclusion: The assessment of information needs based on demographic factors and disease-related factors is critical in helping patients with cancer to manage their illness.

산삼과 산양삼 추출물의 항암 및 항산화 효능

  • Objectives : The aim of this study was to verify anti-cancer and anti-oxidant efficacies of Korean wild ginseng and cultivated wild ginseng of Korea and China. Methods : For the measurement of anti-oxidation, SOD-like activity was evaluated using xanthine oxidase reduction method under in vitro environment. Subcutaneous and abdominal cancer were induced using CT-26 human colon cancer cells for the measurement of growth inhibition of cancer cells and differences in survival rate. Results : 1. Measurement of anti-oxidant activity of ginseng, Chinese and Korean cultivated wild ginseng, and natural wild ginseng samples showed concentration dependent anti-oxidant activity in HX/XOD system. Anti-oxidant activity showed drastic increase at 1mg/ml in all samples. 2. For the evaluation of growth inhibition of cancer cells after hypodermic implantation of CT-26 cancer cells in the peritoneal cavity of mice, Chinese and Korean cultivated wild ginseng and natural wild ginseng groups showed significant inhibition of tumor growth from the 12th day compared to the control group. Similar inhibitory effects were also shown on the 15th and 18th days. But there was no significant difference between the experiment groups. 3. For the observation of increase in survival rate of the natural wild ginseng group, CT-26 cancer cells were implanted in the peritoneal cavity of mice.

Comparative Proteomics Analysis of Colorectal Cancer

  • Background and Objective: Protein expression in colon and rectal cancer (CRC) and paired normal tissues was examined by two-dimensional gel electrophoresis (2-DE) to identify differentially expressed proteins. Materials and Methods: Five fresh colorectal cancer and paired adjacent normal tissues were obtained and differentially expressed protein spots were determined using PDQuest software, with identification on the basis of MALDI-TOF mass spectra. Results: Compared with normal colorectal mucosa, protein abnormal expression of 65 spots varying more than 1.5 times were found in 2-DE gels from colorectal cancer samples (P<0.05); forty-two proteins were up-regulated and 23 were down-regulated; twelve protein spots were identified using mass spectrometry, of which 8 were up-regulated, includimng HSPB1and Annexin A4, while 4 were down-regulated, the results being consistent with Western blot findings. Conclusions: Two-dimensional electrophoresis reference maps for CRC tissues and adjacent normal mucosa (NMC) were established and 12 differentially expressed proteins were identified. Up-regulated HSPB1 and Annexin A4 may play many important roles in the pathogenesis of colorectal cancer.

Interleukin-32: Frenemy in cancer?

  • Interleukin-32 (IL-32) was originally identified in natural killer (NK) cells activated by IL-2 in 1992. Thus, it was named NK cell transcript 4 (NK4) because of its unknown function at that time. The function of IL-32 has been elucidated over the last decade. IL-32 is primarily considered to be a booster of inflammatory reactions because it is induced by pro-inflammatory cytokines and stimulates the production of those cytokines and vice versa. Therefore, many studies have been devoted to studying the roles of IL-32 in inflammation-associated cancers, including gastric, colon cancer, and hepatocellular carcinoma. At the same time, roles of IL-32 have also been discovered in other cancers. Collectively, IL-32 fosters the tumor progression by nuclear $factor-{\kappa}B$ ($NF-{\kappa}B$)-mediated cytokines and metalloproteinase production, as well as stimulation of differentiation into immunosuppressive cell types in some cancer types. However, it is also able to induce tumor cell apoptosis and enhance NK and cytotoxic T cell sensitivity in other cancer types. In this review, we will address the function of each IL-32 isoform in different cancer types studied to date, and suggest further strategies to comprehensively elucidate the roles of IL-32 in a context-dependent manner.

Gefitinib induces anoikis in cervical cancer cells

  • Gefitinib exerts anticancer effects on various types of cancer, such as lung, ovarian, breast, and colon cancers. However, the therapeutic effects of gefitinib on cervical cancer and the underlying mechanisms remain unclear. Thus, this study aimed to explore whether gefitinib can be used to treat cervical cancer and elucidate the underlying mechanisms. Results showed that gefitinib induced a caspase-dependent apoptosis of HeLa cells, which consequently became round and detached from the surface of the culture plate. Gefitinib induced the reorganization of actin cytoskeleton and downregulated the expression of p-FAK, integrin β1 and E-cadherin, which are important in cell-extracellular matrix adhesion and cell-cell interaction, respectively. Moreover, gefitinib hindered cell reattachment and spreading and suppressed interactions between detached cells in suspension, leading to poly (ADP-ribose) polymerase cleavage, a hallmark of apoptosis. It also induced detachment-induced apoptosis (anoikis) in C33A cells, another cervical cancer cell line. Taken together, these results suggest that gefitinib triggers anoikis in cervical cancer cells. Our findings may serve as a basis for broadening the range of anticancer drugs used to treat cervical cancer.
