Factors Influencing Objective Emergency Nursing Performance of Nursing Students on Simulation Practice (시뮬레이션 실습에서 간호학생의 객관적 응급간호 임상수행능력에 영향 미치는 요인)

  • This study examined the factors affecting the emergency nursing performance of nursing students during a simulation. The variables related to the simulation were the objective and subjective emergency nursing performance, critical thinking, program outcome, simulation satisfaction, simulation confidence, and simulation immersion. The participants were 92 senior students who provided informed consent with an explanation about attending the study. The general characteristics and simulation-related data were obtained from the students before and after the simulation. An assessor evaluated the objective emergency nursing performance during the simulation and program outcome through a report reviewed after the simulation. Data analysis was performed using the number, percentage, mean, standard deviation, X2-test, one-way ANOVA, Pearson correlation, and progressive regression through the SPSS/WIN 22.0 program. Emergency nursing performance by the assessor was negatively associated with the satisfaction of clinical practice (r=-.31, p=.005) and positively related to the subjective clinical performance (r=.23, p=.036). The affecting rate was just 12%, which was low for explaining the objective emergency nursing performance during the simulation. Further studies will be needed to explore other factors affecting the emergency nursing performance.

Development of Evidence-based Guidelines for Nursing Home's Infection Control in Korea (한국형 노인요양시설 근거중심 감염관리 가이드라인 개발)

  • Purpose: The purpose of this study was to develop evidence-based guidelines for infection control in nursing homes in Korea (ENIK). Methods: Three steps were planned for the development which were developing a draft and testing the content validity. First, the draft was based on evidence and developed through focus group interviews with nurses in nursing homes, a comprehensive review of international guidelines and literature, and systematic reviews of interventions for infection control and outbreaks in long-term care facilities. Clinical applicability was established through reviews of nursing records and job assignments in one nursing home. The final step consisted of experts evaluating the content validity. The ENIK was revised to fit Korean nursing homes. Results: The ENIK consisted of recommendations in 9 compositions and a one-page practical algorithm. The principles of infection control were presented by statements and specific strategies were recommended in resident care programs. The infection control practical algorithm was organized into 3 steps: screening at admission, prevention, and control at the early stage. The practice to control infection was composed of a 5-step process. Conclusion: The ENIK will contribute to improving the competency of infection control practice because it provides standardized practice and is tailored to Korean nursing homes.

Assessing educational needs in the development of a simulation-based convergence training program on mental health nursing (시뮬레이션 융합 프로그램 개발을 위한 정신건강간호 교육요구도 조사)

  • This study was to identify the educational needs for mental health nursing among undergraduate nursing students, prioritizing the areas where require extensive convergence learning via simulation. One hundred and forty students completed the survey where data accrued from between March-May 2020. The educational needs for mental health nursing and nursing care schizophrenia patients were assessed using a self-reporting questionnaire, which included 19 and 15 items, respectively. Additional three essay questions were used to obtain information regarding previous simulation education experience. Higher educational needs in nursing care for schizophrenia were identified and the highest-priority need is communication skills. Factors, including realistic experience with standardized patients, teamwork, and feedback from the lecturers, affect positive experiences in mental health nursing simulation. The findings suggest that educational programs to improve communication skills are necessary for better mental health nursing competency. Well-designed and convergence based simulation programs providing real-like clinical experience would ensure optimal learning outcomes.

The Importance and Satisfaction of Intensive Care Unit Health Care Providers with Intensive Care Unit Clinical Nurse Expert's Role (중환자실 전담간호사 역할에 대한 중환자실 의료인의 중요도와 만족도)

  • This study was aimed to identify importance and satisfaction of intensive care unit health care providers for Intensive Care Unit Clinical Nurse Expert (ICU CNE) roles. A questionnaire survey was performed with 138 intensive care unit health care providers between May 3 to 29, 2021. Data were analyzed using descriptive statistics. t-test and Importance-Performance Analysis (IPA). There were statistically significant differences in importance(t=2.33, p=.024) and satisfaction(t=5.43, p<.001) between physicians and nurses. The importance and satisfaction of intensive care unit health care providers were ranked in the order of advanced clinical practice, professional activity, coordination and cooperation, education and counseling and research. In the 'concentrate' area, it was found that 'Professional activity' and 'Education' should be improved. A strategy is needed to strengthen the expertise of ICU CNE and to enhance the competency in areas that need improvement.

Influence of Major Satisfaction, Communication Skills, Image of Nurses on Nursing Professionalism of Nursing Students (간호대학생의 전공만족도, 의사소통능력, 간호사 이미지가 간호전문직관에 미치는 영향)

  • The purpose of this study was to investigate major satisfaction, communication skills, image of nurses and nursing professionalism, and to identify the influencing factors on nursing professionalism of nursing students. Data were collected using questionnaires from 214 students who were 3rd years in the nursing university in Gwangju, June 2019. The collected data was analyzed by descriptive statistics, pearson's correlation coefficient and stepwise multiple regression using IBM SPSS 21.0 program. The mean scores for the major satisfaction(3.71±0.53), communication skills(3.50±0.38), image of nurses(3.80±0.52), nursing professionalism(3.70±0.56) were above average. It was found that there were significant differences in the nursing professionalism depending on age, motivation for nursing clinical practice. Significant relationships were found among major satisfaction(r=.531, p=.000), communication skills(r=.531, p=.000), image of nurses(r=.836, p=.000) and nursing professionalism in nursing students. The results of stepwise multiple regression indicates that image of nurses(β=.705, p=.000) and communication skill(β=.136, p=.001) and major satisfaction(β=.133, p=.002) and predicts 72.8% in clinical practice competency(F=191.248, p<.001). In conclusion, to enhance nursing professionalism for nursing students, it is necessary to develop and utilize a program that enhances image of nurses, communication skills and major satisfaction.

Development of OSCE by Nurse Managers of One University-Affiliated Hospital for Skill Test of Nurse Recruitment Process (간호사 채용을 위한 간호관리자의 OSCE 개발)

  • To recruit nurses with skills, the hospital must evaluate the skills of the applicants. But objective evaluation of the skills is a difficult. Many hospitals are using written examination or oral test only and are not testing the nursing skills directly when they recruit new nurses. OSCE(Objective Structured Clinical Examination) is a method developed for the evaluation of clinical skills objectively. In this study, 16/20 nurse managers of one university-affiliate hospital were assembled for the project of developing 7 OSCE modules by themselves, and then they ran OSCE using those modules for 40 nursing students. They scored nursing skills of the students using checklists they designed. The whole process was proceeded phase by phase using step-wise approach model. The project was successful; inter-personal consistency of the scores were high; nurse managers became confident in objective skill evaluation; subjective responses for this project were favorable. The hospital adopted OSCE in recruitment process thereafter. This experience might be applied to other hospitals who want to recruit nurses with skills.

Job Description of the Nurses Who Work in Operating Room Using DACUM Technique (데이컴 직무분석 기법을 이용한 수술실 간호사의 직무분석)

  • Purpose: Developing a curriculum(DACUM) is a method of analyzing job focused competency, which is obtained from the data of an expert belonging to a certain career. In this study the DACUM method was used to analyze the jobs of operating room nurses. Method: Through the DACUM workshop which was arranged by two DACUM facillitators, a definition of the role of operating room nurses was developed and then duties and tasks of operating room nurses' were identified. For the workshop, a DACUM committee with 10 operating room nurses was organized. Finally, the duties and tasks which were identified were validated by 422 nurses for importance, difficulty and frequency. Results: Thirteen duties and 105 tasks were identified on the DACUM chart, where importance, difficulty, and frequency of tasks were represented by alphabet letters A, B, and C as higher degree of importance. The determinant coefficient(DC) showed that the most important duty was assisting with operations(DC=6.61), and the least, managing operating materials(DC=4.22). For tasks, the most important ones were assisting in orthostatic surgery(DC=7.60), and assisting in thoracic surgery(DC=7.38), and the least important making gauze ball(DC=2.39), and saving of operation site((DC=3.27). Conclusion: The results suggest a need to develop an education program using the DACUM chart as a basis for the development and as a clinical career ladder and for curriculum of operating room nursing.

Factors related to the undergraduate nursing students' metacognition (간호대학생의 메타인지 영향요인분석)

  • Metacognition is a core element for nurses' clinical competency as a result of the learner's development and evaluate goals and plans for problem solving. The study aimed to analyze factors related to metacognition among undergraduate nursing students. The 205 nursing students participated from August 15 to October 19, 2017 to measure metacognition, self-directed learning, grit, learning environment and learning style. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and multiple regression with the SPSS/Win 22.0 program. Metacognition represented a positive correlation with self-directed learning, grit, learning environment. The self-directed learning and grit were significant factors on undergraduate nursing students' metacognition. As a result of this study, it is necessary to develop curriculum that can improve metacognition level by increasing self-directed learning ability and grit of undergraduate nursing student.

  • The purpose of this study is to confirm the effects of the contents of the recording lectures through the automatic recording system of the nursing students on the program outcomes of the pediatric nursing. The study was designed using a nonequivalent control group pretest-posttest design. the subject of this study consisted of 104 nursing students(the experimental group of 52 and the control group of 52). Data were analyzed by ${\chi}^2$ test and independent t-test using SPSS 22.0. The effect of the first program outcome(PO1. We can integrate nursing skills based on various the liberal arts and the major knowledge into practice) of the experimental group was not confirmed, but the fifth program outcome(PO5. We can apply critical thinking based nursing processes and implement clinical inferences) was significantly higher than that of the control group. It is significant that this study presented the data to develop the class design method to improve the program outcomes of the nursing student at the present time which is operating the program outcomes based curriculum for nurse with core competency.

The Relationship Between Educational Needs and the Caregiving Burden in Stroke Patients' Primary Caregivers (뇌졸중 환자를 돌보는 주 보호자의 교육 요구도와 돌봄부담과의 관계)

  • Purpose: The aim of the present study was to identify the relationship between educational needs and the caregiving burden of primary caregivers with stroke patient. Methods: This cross-sectional and descriptive study was conducted in 2016 with 115 primary caregivers for stroke patients at a university hospital. The caregiving burden and educational needs were investigated using structured questionnaires via interviews. Data were analyzed with descriptive statistics, the t-test or ANOVA, and Pearson's correlation analysis using IBM SPSS Statistics version 23.0. Results: The scores of caregiving burden and educational needs of primary caregiver with stroke patient were $77.65{\pm}1.66$ and $123.33{\pm}2.37$, respectively. The caregiving burden was associated with health status in primary caregivers' general characteristics. The caregiving burden and educational needs of primary caregivers have a significant correlation (r = .44, p <.001). Conclusions: Educational needs of primary caregivers with stroke patient are associated with their caregiving burden. Therefore, it is necessary to develop a primary caregiver centered intervention program considering educational needs to improve their caregiving burden. Also, to promote quality of nursing, there is the need to increase the educating competency of nurse and nursing professionalism of clinical nurse using various educational training program.
