임상 실습을 경험한 간호대학생의 사회적 지지 측정 도구 개발 (The development of a social support scale for nursing students in clinical practice)

  • Purpose: This methodological study developed a scale that reflects the social support characteristics of nursing students who have experienced clinical practice and verified their reliability and validity. Methods: The 37 initial items of the scale were derived based on the results of a previous study that analyzed the concept of nursing students' social support. The initial items were revised through content validity verification, and 29 preliminary items were finally selected. Data were collected from 220 students enrolled in a nursing department in Jeollabuk-do, who had clinical practice experience, and 205 surveys were used for the final analysis. The data collection period was from March 11 to April 26, 2022. An exploratory factor analysis was performed using maximum likelihood factor extraction and varimax rotation. Results: The social support scale for nursing students who have experienced clinical practice consisted of a total of three sub-factors and 17 items. The three sub-factors were 'support from family and friends' (eight items), 'support from school, professors, and clinical instructors' (seven items) and 'support from department seniors' (two items). The reliability of the developed scale was found to be high, with a Cronbach's alpha of .93. As a result of verifying the criterion validity of the developed scale, the correlation between the criterion tool and the scale developed in this study was statistically significant. Conclusion: The significance of this study is that it developed a scale to measure social support for the first time among nursing students in Korea.

신생아중환자실 간호사를 대상으로 한 Neonatal/Infant Braden Q Scale 사용교육이 측정자 간 일치도에 미치는 효과 (The Effect of an Education Program on Inter-rater Reliability of Neonatal/Infant Braden Q Scale for Clinical Nurses)

  • Purpose: This study was aimed to investigate the effect of an education program on inter-rater agreement of Neonatal/Infant Braden Q Scale for clinical nurses working at a neonatal intensive care unit (NICU). Methods: This was single-arm pre and post experimental study. The participants were 12 nurses and 128 hospitalized neonates at a NICU from December, 2012 to March, 2013. Twelve nurses were divided into four different groups; for two groups were assigned nurses with 3 to 5 years of clinical experiences, and for the others with less than 1 year of clinical experience. The interventions were given by one wound ostomy specialist and two NICU nurses with over 5 years of clinical experiences for 1 hour twice. The inter-rater agreement was measured by intraclass-correlation coefficient. Results: Overall inter-rater agreement was improved from .87(95% CI: .80~.92) at the pre-test to .94(.91~.96) at post-test. Each inter-rater agreement except moisture and nutrition was also improved. Conclusion: The developed education program on scoring for Neonatal/Infant Braden Q scale was effective to improve the inter-rater agreement among clinical nurses. We suggest to privide an education for NICU nurse before using the Neonatal/Infant Braden Q scale in clinical settings.

작업치료대학생의 실습만족을 위한 임상수행능력 자기보고식 척도 개발 (Development of Clinical Competency Self-Report Scale for Clinical Satisfaction of Occupational Therapy Student)

  • 본 연구는 작업치료대학생의 임상수행능력을 측정하는 척도를 개발하고 타당화 하는 데 목적이 있다. 임상수행능력 척도 개발은 임상수행능력의 개념과 선행연구를 분석하였으며 초점집단면담(FGI)을 거쳐 구성요인과 예비문항을 제작하였다. 다음으로 이 문항들에 대해 4인의 전문가를 대상으로 내용타당도 검증을 실시하여 36개의 예비문항을 개발하였다. 최종 척도 문항을 선정하기 위해 203명의 작업치료학과 3, 4학년을 대상으로 본 검사를 실시하여 문항분석과 구인타당도를 검증하였다. 문항분석 결과 삭제된 문항은 없었고, 탐색적 요인분석을 실시한 결과 전문의식 요인 8문항, 작업치료 평가요인 11문항, 작업치료 중재요인 4문항, 의사소통 요인 4문항으로 총 4요인 27개 문항으로 추출되었다. 내적일관성 계수인 Cronbach's α를 통해 요인별 신뢰도를 검증한 결과, .87~.94로 나타났으며, 전체 신뢰도는 .96으로 나타나 양호한 것으로 분석되었다. 임상수행능력 척도 총점과 요인들 간 상관은 모두 통계적으로 유의미하게 나타났다. 확인적 요인분석을 실시한 결과 4개 요인 27문항에 대한 요인구조의 적합도 지수는 (χ2=.76, df=.31, CFI=.81, TLI=.80, RMSEA=.79) 수용 가능한 수준으로 볼 수 있다. 임상수행능력 척도는 작업치료대학생을 대상으로 임상실습 전·후의 임상수행능력을 객관적으로 진단하거나 평가하는 데 유용하게 활용될 것을 제언한다.

상급종합병원 전문간호사의 임상적 의사결정능력과 직무스트레스 간의 관계에서 역할 명확성의 매개효과 (Mediating Effects of Role Clarity between Clinical Decision-Making Abilities and Job Stress for Advanced Practice Nurses at Tertiary Hospitals)

  • Purpose : The purpose of this study was to identify the mediating effect of role clarity in the relationship between clinical decision-making abilities and job stress among advanced practice nurses at tertiary hospitals. Methods : The participants were 137 advanced practice nurses. The assessment tools were clinical decision-making in nursing scale, role ambiguity scale, and Korean occupational stress scale (KOSS-26). Data were analyzed with the SPSS/24.0 program and mediation analysis was performed according to the Baron and Kenny methods. Results : There were significant relationships between clinical decision-making abilities and job stress (r=-.33, p<.001), and role clarity and job stress (r=-.29, p=.001). Role clarity showed partial mediating effects in the relationship between clinical decision-making abilities and job stress (Z=2.02, p=.043). Conclusion : Therefore, to reduce advanced practice nurses' job stress, it is necessary to develop a program and strategies to increase their clinical decision-making abilities.

수근관 증후군에서 임상양상척도 및 신경전기진단 결과와 정중신경 초음파 소견의 상관관계 (The Correlation between Ultrasonographic Findings of Median Nerve and Clinical Scale and Electrodiagnotic Data in Carpal Tunnel Syndrome)

  • Background: The aim of this study is to identify the correlation between ultrasonographic findings of median nerve and clinical scale and electrophysiologic data in carpal tunnel syndrome. Methods: Forty three patients (79 hands) with electrophysiologically confirmed carpal tunnel syndrome were evaluated. Clinical symptoms were examined by Historical-Objective (Hi-Ob) scale. Electrophysiologic data and Padua scale were used for severity of electrophysiology. In ultrasonographic study, cross sectional area and flattening ratio of median nerve were measured at distal wrist crease level (DWC), 1cm proximal to distal wrist crease level, and 1cm distal to distal wrist crease level. The correlation between Hi-Ob scale, electrophysiologic data and ultrasonography was measured with Spearman rank test. Results: The mean Hi-Ob scale was 2.4. Mean Padua scale was 4.0. In ultrasnonographic study, cross sectional area and flattening ratio were $0.112\;cm^2{\pm}0.025$ and $3.0{\pm}0.6$ at 1cm proximal to DWC level, $0.118{\pm}0.026\;cm^2$ and $2.9{\pm}0.4$ at DWC level, and $0.107{\pm}0.032\;cm^2$ and $3.0{\pm}0.4$ at 1 cm distal to DWC level. Hi-Ob scale was not correlated with cross sectional area and flattening ratio of median nerve. Hi-Ob scale was correlated with Padua scale positively (r=0.44) and correlated with amplitudes of CMAP and SNAP, negatively (r=-0.33; r=-0.30). Cross sectional area of median nerve was significantly correlated with Padua scale, amplitudes and latencies of CMAP and amplitudes of SNAP. Conclusions: Ultrasonographic findings of median nerve and electrodiagnostic data had statistically significant correlation. Consequently, ultrasonography could be an adjunctive method in diagnosis of carpal tunnel syndrome.

간호사의 임상판단 측정도구 개발 (Development of Nursing Clinical Judgment Scale)

  • Purpose: This study aimed to develop a nursing clinical judgment scale (NCJS) and verify its validity and reliability in assessing the clinical judgment of nurses. Methods: A preliminary instrument of the NCJS comprising 38 items was first developed from attributes and indicators derived from a literature review and an in-depth/focus interview with 12 clinical nurses. The preliminary tool was finalized after 7 experts conducted a content validity test based on a data from a preliminary survey of 30 hospital nurses in Korea. Data were collected from 443 ward, intensive care unit, emergency room nurses who voluntarily participated in the survey through offline and online for the verification of the construct validity and reliability of the scale. Results: The final scale comprised 23 items scored on a 5-point Likert scale. Six factors - integrated data analysis, evaluation and reflection on interventions, evidence on interventions, collaboration among health professionals, patient-centered nursing, and collaboration among nurse colleagues - accounted for 64.9% of the total variance. Confirmatory factor analysis supported the fit of the measurement model, comprising six factors (root mean square error of approximation = .07, standardized root mean square residual = .04, comparative fit index = .90). Cronbach's α for all the items was .92. Conclusion: The NCJS is a valid and reliable tool that fully reflects the characteristics of clinical practice, and it can be used effectively to evaluate the clinical judgment of Korean nurses. Future research should reflect the variables influencing clinical judgment and develop an action plan to improve it.

간호학생이 임상실습에서 경험하는 무례함 한국어판 측정도구의 타당도와 신뢰도 (Validity and Reliability of the Korean Version of a Tool to Measure Uncivil Behavior in Clinical Nursing Education)

  • Purpose: This study aims to develop a Korean version of a tool to measure uncivil behavior in clinical training to examine the experiences of nursing students. Methods: The "Uncivil Behavior in Clinical Nursing Education Scale" was developed by Anthony and Yastik in 2011. This study procedure was based on DeVellis' instrument development guidelines. Data were collected from 220 senior-year nursing students from four different universities in four different locations. Two hundreds surveys were analyzed using SPSS software and AMOS. Results: Out of 20 questions, 13 were selected after reviewing the content validity, face validity, construct validity, and reliability. The factors of the Korean version scale were specified as "exclusion", "contempt", and "refusal." The general characteristics of the subjects that showed significant differences in the occurrence of incivility were gender, age, transfer student status, level of satisfaction with clinical training, and level of satisfaction with the clinical training environment. Conclusion: The "Korean-Uncivil Behavior in Clinical Nursing Education Scale" was partially modified to account for differences in language and culture, but its validity and reliability were verified. We suggest that nurse educators and supervisors will be able to better understand the relationship between nurses and nursing students in clinical training.

걸음마기 아동의 정신건강 위험요인 선별척도의 개발 및 타당화 (The Development and Validation of a Scale for the Mental Health Screening of Toddlers)

  • Development of the 3 scales comprising the present research was based on review of literature, existing screening scales, and empirical research pertaining to (Scale I) the development of social and emotional problems of children, (Scale II) parent-child relations, and (Scale III) assessment of children's environment. Professionals in each area approved a draft of the new screening scale. The clinical group was classified into normal and at-risk groups based on the Denver II scale and the Child Behavior Check List(ages 1.5-5). The clinical groups were administered the newly developed screening scale to see whether the same classification pertained. Results proved the cross-validity of the new scale.

신규간호사의 조직사회화 영향 요인 (Factors Associated with Organizational Socialization in New Nurses)

  • Purpose: The aim of this study was to examine the relationship between calling, clinical performance, role conflict, and organizational socialization and to evaluate the factors associated with organizational socialization of new nurses. Methods: This study design was cross-sectional correlational study. The participants of this study were new nurses 171 at general hospitals in Jeollabuk-do. Data was collected between August 1 to 20, 2018 from questionnaire responses and were analyzed with descriptive statistics, independent t-test, one-way ANOVA, pearson's correlation coefficient, and multiple regression analysis. Results: The average calling rating was 2.47±0.59 (rated on a scale of 1~4), clinical performance rating was 3.55±0.52 (rated on a scale of 1~5), role conflict rating was 3.29±0.59 (rated on a scale of 1~5), and organizational socialization rating 3.19±0.41 (rated on a scale of 1~5). Calling, clinical performance, role conflict, gender, and work in the desired unit were the significant factors influencing organizational socialization of new nurses. Conclusion: These findings indicate that strategies are needed to improve organizational socialization of new nurses to ensure effective management of personnel. Moreover, it is necessary to develop a program to cultivate the nursing calling, enhance clinical performance, and resolve role conflicts among new nurses.

간호 대학생의 긍정적 정서, 셀프리더십 및 임상실습 만족도간의 관계 (Relationship between Positive Affectivity, Self-Leadership, and Satisfaction of Clinical Practice among Nursing Students)

  • Purpose: This study aimed to identify the relationship between positive affectivity, self-leadership, and satisfaction of clinical practice among nursing students. Methods: This study was a descriptive correlational study. A total of 205 participants were obtained by convenience sampling from third- and fourth-year nursing students in three nursing schools. The data were collected by using a self-reporting questionnaire. Collected data were analyzed with SPSS Win 18.0. Results: Mean scores were 3.40 (5-point scale) for positive affectivity, 3.68 (5-point scale) for self-leadership, and 3.30 (5-point scale) for satisfaction of nursing clinical practice. There was a significant positive correlation of nursing students among positive affectivity (r=.43, p<.001), self-leadership (r=.41, p<.001), and satisfaction of clinical practice. The predictors were levels of satisfaction about nursing major, grade, positive affectivity, and rehearsal, which accounted for 34% of the variance in satisfaction with clinical practice nursing students. Conclusion: It is necessary to enhance the levels of satisfaction about nursing major, the level of positive affectivity, and self-leadership in nursing students. The results of this study delineate the need for the development and application of diverse educational programs that can increase positive affectivity and self-leadership for clinical practice.
