임상간호사의 가족간호에 대한 인식 및 수행정도에 관한 연구 (A Study of Perception and Practice on Family Nursing of Clinical Nurse)

  • This is the descriptive investigation study intended to provide basic informations to develop concrete method of nurse arbitration which can improve the quality of nursing care on family by investigating and analyzing the perception and practice on family nursing of clinical nurse. 332 nurses working in 4 university hospitals in Seoul have been the object and the collection of data have been conducted by visiting cooperated by the department of nursing in university hospital from April 4th through April 17th 1998. The measuring instrument of the perception and the practice on family nursing which was written by the researcher was used based on the family nursing arbitration by recently amending Calgary Family Arbitration of the Model of Wright & Leahey. Cronbach's a value of this instrument was .9288 in the perception and .9168 in the practice the collected data have been analyzed by frequency percentage, averaged value. t-test, F-test(ANOVA), Duncan's Multiple Range, Pearson's Correlation Coefficient, and the results are as the follows: 1. The perception on patient's family nursing of clinical nurse showed comparatively high by 3.22 in average(maximum 3.52, minimum 2.82) on the basis of 4 point but the practice showed low by average 2.47(maximum 3.02, minimum 2.11), By providing the patients and their family with "The information about the health problem of the which is the role of giving explanation and information about the disease. the nurse presents the method of their helping patient and in case that the family lack of knowledge about the health problem and crisis of the patient which is the role of education about the method of solving the crisis and change. the nurse educates about the necessity and method of taking care of the crisis and the changes. The third question that the relation of recognizing the difficuly of family and cooperating with them in supporting the patient for mutual function is to be formed showed high in the degree of perception and practice of the necessity. 2. General characteristics of perception about patient's family nursing of the object showed no significant difference except the concerns about the family usually(F=5.472. p<.001) and general characteristics which showed significant difference in the degree of practice were educational background (F=3.177, p<.05), clinical experience (F=2.462, p<.05) and position(F=7.029. p<.001), and attention about patient's family(F=10.603, p<.001), 3. The relation between perception and practice about the nursing on patient's family showed pure correlation but the degree was very low(r=.188, p<.05). The above results showed that the clinical nurses has been high understanding about the necessity of patient's family nursing but the degree of practice has been very low due to the lack of education about the family nursing, having no ways of nurse arbitration for practical duty and lack of political administrative support. Therefore concrete and systematic family situation and arbitration method to be applied clinically are required to be developed and also the education about patient's family nursing and the development of the course for clinical practice are required and political and administrative support for clinical practice about patient's family nursing is required as well.

간호대학생의 임상실습중 언어폭력경험과 임상실습 스트레스와의 관계연구 (A Study on the Relationship between Experience of Verbal Abuse and Clinical Practice Stress during Clinical Practicum of Nursing Students)

  • 본 연구는 간호대학생의 임상실습중 경험하는 언어폭력, 감정반응, 간호전문직관, 임상실습 스트레스 정도를 파악하고 임상실습 스트레스에 영향을 미치는 요인들을 확인하기 위하여 시도되었다. 연구 대상은 4년제 간호대학 4학년에 재학중인 간호대학생 106명이였으며 측정 도구는 언어폭력, 감정반응, 간호전문직관, 임상실습스트레스에 관한 문항으로 구성되었다. 언어폭력, 감정반응, 간호전문직관, 임상실습스트레스 정도는 기술통계로 분석하였고 언어폭력, 감정반응, 간호전문직관, 임상실습스트레스간의 상관관계는 Pearson correlation coefficients, 임상실습스트레스에 영향을 미치는 요인은 다중 선형 회귀분석(Multiple linear regression)을 적용하였다. 임상실습스트레스는 언어폭력, 감정반응과 유의한 정적상관성이 있는 것으로 나타났다(r=.683, r=.573). 유의미한 영향요인으로 언어폭력(𝛽=.487), 감정반응(𝛽=.240)순으로 임상실습스트레스의 49%를 설명하였다. 본 연구의 결과를 토대로 간호대학생들이 임상실습중 언어폭력에 대처하고 임상실습스트레스를 관리하기 위한 전략개발에 기초자료를 제공하고자 한다.

국내 상급종합병원의 간호단위 및 임상경력별 핵심간호실무와 교육요구도 (Core nursing practice and educational requirements according to nursing unit and clinical career in tertiary hospital)

  • Purpose: The purpose of this study was to provide fundamental data to develop an educational program for the development of nurses' core competences, and to improve the curriculum in nursing education institutions. Methods: This study, based on a descriptive survey, was conducted with 1,019 nurses working in a tertiary hospital in Seoul. Among 47 core nursing practice items, core nursing practices as perceived by the participants and relevant educational requirements were measured on a four-point scale. The collected data were analyzed using descriptive statistics for the mean and standard deviation, one way ANOVA, and Duncan test as post-hoc test. Results: Regarding the core nursing practice perceived as by the nurses, there were statistically significant differences in 43 out of 47 items according to nursing unit, and in 15 items according to clinical career. Regarding the educational requirements for core nursing practice, there were also statistically significant difference for the same items as perception differences according to nursing unit and clinical career. Conclusion: The results of this study suggest further study on the development of educational programs to advance the clinical expertise of nurses and for the development of curriculum reflecting the actual conditions and requirements in clinical nursing practice fields.

간호사의 비판적 사고성향, 디지털리터러시, 간호정보역량이 근거기반실무역량에 미치는 영향 (Effects of Critical Thinking Disposition, Digital Literacy, and Nursing Informatics Competence on the Competency in Evidence-Based Practice among Clinical Nurses)

  • Purpose: This study aimed to investigate the competency in evidence-based practice, critical thinking disposition, digital literacy, and nursing informatics competence, and to identify the factors influencing the competency in evidence-based practice among nurses. Methods: The participants in this study were 196 nurses from a university hospital in J-city. Data were collected from December 1 to December 30, 2023. The collected data were analyzed with the SPSS 26.0 program using descriptive statistics, independent t-test, one-way analysis of variance, Scheffé test, Pearson's correlation coefficients, and multiple regression analysis. Results: The results showed that the factors influencing the competency in evidence-based practice included nursing informatics competence, critical thinking disposition, digital literacy, and having a master's degree. The participants with a master's degree demonstrated higher competency in evidence-based practice compared to those with a three-year degree or a four-year degree, and a total explanatory power was 53.1%. Conclusion: In this study, critical thinking disposition, digital literacy, and nursing informatics competence were influential factors in competency of evidence-based practice. Therefore, a variety of intervention programs should be developed to enhance the impact of these factors on nurses' competency in evidence-based practice.

기초임상실습프로그램이 간호대학생의 학습동기, 비판적사고 및 임상수행능력 자신감에 미치는 효과 (Effects of Basic Clinical Practice Program in Academic Motivation, Critical Thinking and Clinical Nursing Competence of Nursing Students)

  • 본 논문은 교내 기초임상실습 프로그램이 학생들의 학습동기, 비판적 사고 및 임상수행능력 자신감에 미치는 효과를 확인하고자 시도되었다. 기초임상실습 프로그램은 2주간 30시간의 과정으로 핵심기본간호술과 필수간호교육으로 구성되었다. 대상자는 첫 임상실습을 앞둔 228명의 간호대학생이었고 프로그램 전후에 설문지를 통해 자료를 수집하였다. 자료수집기간은 2013년 2월 18일부터 28일까지 10일 이었다. 연구결과 비판적사고에는 영향을 미치지 않는 것으로 나타났고, 학습동기의 하부영역 중 우울과 시험불안은 유의하게 상승하였고, 학업스트레스는 감소하였으며 자기효능감은 유의하게 상승하였다. 임상수행능력 자신감은 전 영역에서 유의하게 상승한 것으로 나타났다 본 연구결과를 통해 기초임상실습 프로그램이 간호학생들의 임상실습 수행 역량을 강화시키는데 효과가 있음을 확인하였다. 추후연구에서는 중재의 효과를 높이기 위해 기초임상실습 프로그램의 내용을 다양화하고 운영기간을 늘릴 것을 제언한다.

간호전문직관과 교수효율성이 간호학생의 임상수행능력에 미치는 영향 (The Influence of Nursing Professionalism and Teaching Efficiency on Clinical Competence of Nursing Students)

  • 본 연구는 간호학생의 간호전문직관, 교수효율성이 임상수행능력에 미치는 영향 요인을 분석하였다. D도시에 소재하고 있는 일개 대학 간호학과 4학년 252명을 대상으로 2019년 5월 3일부터 5월 11일까지 설문조사를 실시하였다. 분석방법은 SPSS/WIN 21.0 프로그램을 이용하여 연구대상자의 일반적 특징 분석, independent t-test, one-way ANOVA분석, Scheffe test 사후검정, 상관관계 분석, 단계적 다중회귀분석(stepwise multiple regression)을 실시하였다. 연구결과는 다음과 같다. 간호전문직관 평균 평점은 3.81점, 교수효율성은 3.27점, 임상수행능력은 3.60점으로 나타났다. 임상수행능력에 대한 간호전문직관과 교수효율성은 유의한 양의 상관관계가 있었으며, 성별, 간호전문직관, 교수효율성은 유위한 영향 요인으로 나타났으며, 가장 영향요인이 높은 것은 간호전문직관이었다. 영향요인의 설명력은 19.6%로 나타났다. 본 연구를 통해 간호전문직관과 교수효율성이 간호학생의 임상수행능력에 영향을 미치는 요인임을 규명함으로서 체계적이고 효율적인 임상실습교육을 위해서는 간호전문직관과 교수효율성을 고려한 임상실습교육 프로그램 개발 및 적용이 필요하다고 본다.

간호대학생이 임상실습에서 경험하는 무례함, 소진, 대처와 임상실습만족도의 관계 (Relationship among Incivility, Burnout, Coping and Satisfaction to Clinical Practice experienced by Nursing College Students in Clinical Practice.)

  • 이 연구의 목적은 임상현장에서 간호대학생들이 경험하는 무례함, 소진, 대처와 임상실습만족도와의 관계를 파악하기 위한 서술적 조사연구이다. 간호대학생 160명을 대상으로 인구통계학적 특성, 무례함, 소진, 대처와 임상실습만족도와 관련하여 자기기입식 설문지를 작성하였다. 수집된 자료는 SPSS 18.0을 이용하여 t-test, ANOVA, correlation analysis, and multiple regression analysis을 실시하였다. 평균나이는 21.5세이었으며, 무례함, 소진, 대처 및 임상실습만족도 평균점수는 5점 척도 중 각각 1.18, 3.41, 2.48, 그리고 3.30이었다. 연구 결과, 무례함, 소진, 대처는 서로 유의한 양의 상관관계를 나타내었고, 무례함과 임상실습만족도, 소진과 임상실습만족도는 유의한 역 상관관계를 나타냈다. 또한 대처와 임상실습만족도 사이에는 유의한 양의 상관관계를 보였다. 임상실습만족도에 영향을 미치는 요인으로는 대처, 무례함, 소진이었고, 그 설명력은 26.6% (F=9.868, p<.001) 이었다. 이 연구에서 간호대학생의 임상실습만족도에 영향을 미치는 저해요인과 관련하여 임상현장에서의 대인관계 역량강화 및 임상실습에서 사용할 수 있는 대처방안이나 지지체계 등의 준비된 실습환경조성이 마련되어야 한다. 따라서 임상 현장과의 다각적 협력과 다양한 간호교육프로그램을 개발하여 간호대학생의 임상실습만족도를 높이는 것이 필요하다.

상급종합병원과 종합병원의 근거기반 임상간호실무지침의 활용도 (Utilization of Evidence-Based Clinical Nursing Practice Guidelines in Tertiary Hospitals and General Hospitals)

  • Purpose: The purpose of this study was to investigate the actual utilization of clinical practice guidelines developed by Hospital Nurses Association. Methods: The subjects were 70 nurses who were in charge of guideline distributions in 70 advanced general hospital and general hospitals with 500 beds or more nationwide. Data were collected between June and August, 2020 by mail (return rate: 88.6%). Data were analyzed using descriptive statistics, t-test, and ANOVA with SPSS/WIN 24.0. Results: Among the clinical practice guidelines developed by Hospital Nurses Association, 72.9~90.1% were placed with book and electronic file in nursing department and 24.3~35.8% were placed with book and electronic file in each nursing unit at hospital. The average number of utilized clinical practice guidelines were 3.96±3.88, and average score of guideline utilization was score 2.85±0.79 which means 'use sometimes'. Conclusion: To improve the distribution and utilization of the clinical practice guidelines, it is necessary to enhance the recognition of values of evidence based nursing practice targeting head of nursing department and to stimulate the distribution and utilization of the clinical practice guidelines using diverse education programs for staff nurses.

간호학생의 영성과 임상실습 스트레스 (Spirituality and Stress of Clinical Practice among Nursing Students)

  • 본 연구는 간호학생의 영성과 임상실습 스트레스 정도를 평가하고 이들 간의 관계를 파악하여 간호학생들의 영성증진과 임상실습 스트레스 중재방안으로 영성의 활용가능성을 확인하고자 시도하였다. 2개 대학의 간호학생 203명을 편의 추출하여 자가 보고식 설문지를 사용하여 자료를 수집하였다. 연구결과, 본 연구대상자들의 영성은 5점 만점에 3.18점, 임상실습스트레스는 5점 만점에 3.42점으로 중간이상의 수준이었다. 하위영역별로는 영성의 경우 '삶의 의미와 목적'이 가장 높았고, 임상실습 스트레스에서는 '실습교육환경'에 대한 스트레스가 가장 높게 나타났다. 일반적 특성에 따른 차이에서는 영성은 종교와 전공만족도에 따라, 임상실습 스트레스는 성별과 전공만족도에 따라 유의한 차이를 보였다. 영성과 임상실습 스트레스는 유의한 부적상관관계가 있었다. 결론적으로, 간호학생들의 임상실습 스트레스를 감소시키기 위해서는 영성증진을 위한 교육프로그램 개발과 이를 효과적으로 교육과정에 접목시킬 수 있는 방안을 구축할 필요가 있다.

시뮬레이션기반 임상추론 실습교육 프로그램이 간호학생의 간호역량에 미치는 효과 (Effects of a Simulation based Clinical Reasoning Practice Program on Clinical Competence in Nursing Students)

  • Purpose: The purpose of this study was to evaluate the effects of a simulation based clinical reasoning practice program on clinical competence in nursing students. The program was based on the theoretical frameworks of simulation models and experiential learning theory. Methods: The program consisted of eight scenarios which includes three main symptoms (abdominal pain, changes in mental status, dyspnea), for improvement of clinical competencies in nursing students. A nonequivalent control group pretest-posttest design was used for evaluation of the effects of the program. Fifty-two junior nursing students in Y University participated in the experimental group (n=25) or control group (n=27). Critical thinking was measured using a self-administered questionnaire. Clinical judgment and clinical performance were measured by a rater using the Rubric. Descriptive analysis, t-test, Mann-Whitney U, Wilcoxon signed rank test was used for data analyses. Results: Clinical judgment and clinical performance increased in the experimental group, but there were no significant differences in critical thinking. Conclusion: Results indicate that the program developed in this study is a useful strategy to enhance clinical judgment and clinical performance in nursing students. However, the program did not significantly enhance critical thinking disposition, and further study is needed to measure integrated clinical competence including critical thinking skills.
