Factors affecting maxillary sinus pneumatization following posterior maxillary tooth extraction

  • Purpose: The aims of the present study were 1) to quantitatively evaluate the extent of sinus pneumatization and 2) to determine the factors affecting sinus pneumatization. Methods: Based on implant treatment records, a list of patients who underwent implant placement on the posterior maxilla was obtained. Among them, patients with pre-extraction and post-extraction (before implant placement) panoramic radiographs were selected. After excluding radiographs with low resolution and image distortion, the radiographs before and after extraction were superimposed using computer software. Subsequently, the extent of sinus pneumatization (the vertical change of the sinus floor) was measured. Simple and multiple mixed models were used to determine the factors affecting sinus pneumatization. Results: A total of 145 patients were eligible for the present investigation. The average extent of sinus pneumatization was 1.56±3.93 mm at 176 tooth sites. Male sex, single tooth extraction, extraction of an endodontically compromised tooth, a class I root-sinus relationship, and sinus membrane thickening >10 mm favored pneumatization, but without statistical significance. The maxillary second molar presented the greatest pneumatization (2.25±4.39 mm) compared with other tooth types. This finding was confirmed in the multiple mixed model, which demonstrated a statistically significant impact of the extraction of a second molar compared with the extraction of a first premolar. Conclusions: Maxillary sinus pneumatization was 1.56±3.93 mm on average. The extraction of a second molar led to the greatest extent of pneumatization, which should be considered in the treatment plan for this tooth site.

  • Objectives: Before implementing a competency-based clinical dental hygiene curriculum, it is essential to establish competency development as a foundational educational objective. Therefore, this study aimed to develop the competency of clinical dental hygiene with secured validity using the Delphi survey method. Methods: Dental hygiene competencies were categorized within the dental hygiene process stages, and questions were formulated accordingly. A Delphi survey involving ten qualified experts was conducted to refine the final items based on their review opinions. Results: The expert Delphi survey confirmed that all items met stability criteria, with CVI values of 0.80 or higher, CVR values of 0.60 or higher, and a CV coefficient of variation of 0.5 or less. In total, 42 items were derived. Conclusions: Clinical dental hygiene can contribute to developing specialized dental hygienists if the competencies derived from this study are well applied to the standardized curriculum and operations.

Is dentin biomodification with collagen cross-linking agents effective for improving dentin adhesion? A systematic review and meta-analysis

  • Objectives: The aim of this investigation was to evaluate the effectiveness of collagen cross-linking agents (CCLAs) used in combination with the adhesive technique in restorative procedures. Materials and Methods: In this systematic review, the authors followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist. An electronic search was performed using PubMed, Scopus, Web of Science, Cochrane Library, LILACS, and DOSS, up to October 2020. The gray literature was also researched. Only randomized clinical trials were selected. Results: The selection process yielded 3 studies from the 838 retrieved. The addition of CCLAs in the retention of restorations increased the number of events. The postoperative sensitivity scores and marginal adaptation scores showed no significant difference between the CCLA and control groups, and the marginal pigmentation scores showed a significant increase in the CCLA group. There were no caries events in any group throughout the evaluation period. Conclusions: This systematic review showed that there is no clinical efficacy to justify the use of CCLAs in the protocols performed.

Occlusion for implant-supported fixed dental prostheses in partially edentulous patients: a literature review and current concepts

  • Implant treatment has become the treatment of choice to replace missing teeth in partially edentulous areas. Dental implants present different biological and biomechanical characteristics than natural teeth. Occlusion is considered to be one of the most important factors contributing to implant success. Most literature on implant occlusal concepts is based on expert opinion, anecdotal experiences, in vitro and animal studies, and only limited clinical research. Furthermore, scientific literature regarding implant occlusion, particularly in implant-supported fixed dental prostheses remains controversial. In this study, the current status of implant occlusion was reviewed and discussed. Further randomized clinical research to investigate the correlation between implant occlusion, the implant success rate, and its risk factors is warranted to determine best clinical practices.

  • Traditionally, goals of orthodontic treatment fall into four categories: good tooth alignment, improvement of skeletal profile and good soft tissue, and rehabilitation of the normal function. Electromyography (EMG) has proven as useful tool in evaluating masticatory function among the maxillomandibular relationship and could be useful in accompanying orthodontic treatment objectives. But in pursuit of clinical application, it is necceassry to systemize clinical guideline for diagnosis by EMG records in evaluating the effects of orthodontic, orthopedic, or surgical treatment. This study present 3 orthodontic cases, which treatment results, ie neuromuscular balances of masticatory muscle, could be moitered by EMG procedure.

An atypical case of rare salivary malignancy, hyalinizing clear cell carcinoma

  • As an uncommon, malignant salivary gland tumor with female predominance, hyalinizing clear cell carcinoma (HCCC) is regarded as an indolent tumor. The diagnosis of this rare tumor is challenging, and it depends on microscopic and immunohistochemical (IHC) studies. Although it is regarded as an indolent tumor, there are reports of unconventional forms with aggressive clinical courses. We report an atypical case of this rare tumor, HCCC, in a male patient who had a relatively large-sized mass ($3.8{\times}3.0$ cm) on the right mouth floor with ipsilateral neck node metastasis. The clinical, radiological, pathological, and IHC features together with the clinical course are described.

  • Therapeutic use of antibiotics in the pulpal disease should reduce the clinical symptoms and pathogenic microflora in the pulp. The purpose of this study was focused on local drug delivery into the root canal in endodontic therapy. Monolithic films from polycaprolactone and polyethylene glycol preparations with minocycline were prepared, and then the antimicrobial effect of these films on necrotic canal was tested. A total 17 necrotized pulp teeth were sampled before and 1 week after insertion of film into the pulp. Bacterial culture were performed with anaerobical condition and seeded in the 5 selective and non - selective media for 7 days in $37^{\circ}C$ of anaerobic chamber. Bacterial identification were performed with Gram staining, biochemical test, and API kit There was significantly decreased of anaerobic and aerobic microflora of 13 among 17 patients after therapy. Among the identified microflora, Streptococcus species, black - pigmented species and Fusobacterium species were significantly reduced 1 week after treatment with local delivery antibiotics. Furthermore, clinical symptoms included in pain and local swelling were continuousely reduced after therapy. These results suggest that local drug delivery with minocycline is effeicient to treat the endodontic involved teeth for releasing clinical symptoms and microbiological shifting.

  • Achieving an aesthetic implant-supported restoration in the single tooth missing case can be challenging when the implant site is in e anterior region. The objective of this report is to focus on presurgical evaluation of implant site and systematic development of related prosthetic modalities. An accurate diagnostic evaluation, a systematically developed pesurgical plan, and knowledge and clinical skill of the various related therapeutic modalities are indispensible. Collection of patient's information, appropriate abutment selection, soft tissue contour, implant axis, and occlusion need to be discussed for aesthetic clinical outcome. For aesthetic restoration, such as surgical guide stent for precise implant positioning customized provisional restoration for development of optimal periimplant soft tissue contours, and fabrication customized abutment (mesiostructure) for veriable emergence profile, are recommended.

  • The author has studied retrospectively the meniscectomy with temporary silastic implant. The study material used was 4 joints that were possible to be followed up in 9 joints of out of all 145 joints who were diagnosed as internal derangement and have been operated in Yongdong Severance hospital from March 1988 to February 1990. We concluded that the use of temporary silastic implant would prevent the complication which would be arised after meniscectomy alone. And we could obtain the results of the relief of clinical symptoms and maintenance or improvement of joint space when preoperative and postoperative clinical and radiographic findings were compared. The larger number of patients and long term follow-up will be needed for further the investigation.

Antimicrobial effect of calcium hydroxide as an intracanal medicament in root canal treatment: a literature review - Part II. in vivo studies

  • The first part of this study reviewed the characteristics of calcium hydroxide ($Ca(OH)_2$) and summarized the results of in vitro studies related to its antimicrobial effects. The second part of this review covers in vivo studies including human clinical studies and animal studies. The use of $Ca(OH)_2$ as an intracanal medicament represented better histological results in animal studies. However, human clinical studies showed limited antimicrobial effects that microorganisms were reduced but not eliminated through the treatment, and that some species had resistance to $Ca(OH)_2$. Most of clinical outcome studies supported that there is no improvement in healing of periapical lesions when $Ca(OH)_2$ was applied between appointments. Further studies are required for the antimicrobial effects of $Ca(OH)_2$, and search for the ideal material and technique to completely clean infected root canals should be continued.
