Study of surgical anatomy of portal vein of liver segments by cast method and its clinical implications

  • Portal vein provides about three-fourths of liver's blood supply. Portal vein is formed behind the neck of pancreas, at the level of the second lumbar vertebra and formed from the convergence of superior mesenteric and splenic veins. The purpose of this study is to review the normal distribution and variation, morphometry of portal vein and its branches for their implication in liver surgery and preoperative portal vein embolization. It is also helpful for radiologists while performing radiological procedures. A total of fresh 40 livers with intact splenic and superior mesenteric vein were collected from the mortuary of Forensic Department, JSS Medical College and Mysuru Medical College. The silicone gel was injected into the portal vein and different segments were identified and portal vein variants were noted. The morphometry of portal vein was measured by using digital sliding calipers. The different types of portal vein segmental variants were observed. The present study showed predominant type I in 90% cases, type II 7.5% cases, and type III 2.5% cases. Mean and standard deviation (SD) of length of right portal vein among males and females were $2.096{\pm}0.602cm$ and $1.706{\pm}0.297cm$, respectively. Mean and SD of length of left portal vein among males and females were $3.450{\pm}0.661cm$ and $3.075{\pm}0.632cm$, respectively. The difference in the Mean among the males and females with respect to length of right portal vein and left portal vein was found to be statistically significant (P=0.010). Prior knowledge of variations regarding the formation, termination and tributaries of portal vein are very helpful and important for surgeons to perform liver surgeries like liver transplantation, segmentectomy and for Interventional Radiologists.

A Comparative Study of the Literature on Fire Acupuncture (Hwachim) between South and North Korea (남, 북한의 화침 연구문헌에 대한 비교연구)

  • Objectives : The aim of this study was to compare and analyze studies related to fire acupuncture (FA) published in South and North Korea. Methods : FA studies published in South Korea were searched through domestic and overseas databases (KISS, RISS, OASIS, and EMBASE). For North Korean studies, databases from the Information Center on North Korea and Korea Institute of Science and Technology Information were used. Among the searched literatures, clinical studies using FA as a treatment intervention were selected (i.e., randomized controlled trials, and case reports). Finally, information such as the disease for which FA was used, the characteristics of FA, and details of treatment methods for FA were extracted from the screened literatures and the results of South and North Korean studies were compared and analyzed. Results : This review included 28 South Korean studies and 9 North Korean studies. All studies in South Korea, except for the three articles that used traditional FA treatment, applied "warm needling"-like acupuncture (WA) in the form of heating the handle of the needle after inserting the needle into the skin, and most studies were conducted on diseases of the musculoskeletal system. Whereas in the North Korean studies, FA was applied to various diseases such as trigeminal neuralgia, skin diseases, and gastrointestinal diseases, and standardized FA tools were used. Conclusions : Studies on traditional FA treatment have rarely been reported in South Korea. The amount of information described in the FA studies in North Korea was limited. In this review, the characteristics of FA treatment tools in South and North Korean studies were mainly analyzed. In the future, FA studies from more diverse perspectives are needed.

Mitofusin-2 Promotes the Epithelial-Mesenchymal Transition-Induced Cervical Cancer Progression

  • High expression of mitofusin-2 (MFN2), a mitochondrial fusion protein, has been frequently associated with poor prognosis of patients with cervical cancer. Here, we aimed to identify the function of MFN2 in cervical cancer to understand its influence on disease prognosis. To this end, from cervical adenocarcinoma, we performed an MTT assay and quantitative RT-PCR (qRT-PCR) analysis to assess the effects of MFN2 on the proliferation and of HeLa cells. Then, colony-formation ability and tumorigenesis were evaluated using a tumor xenograft mouse model. The migration ability related to MFN2 was also measured using a wound healing assay. Consequently, epithelial-mesenchymal transition (EMT) of MFN2-knockdowned HeLa cells originating from adenocarcinoma. markers related to MFN2 were assessed by qRT-PCR. Clinical data were analyzed using cBioPortal and The Cancer Genome Atlas. We found that MFN2 knockdown reduced the proliferation, colony formation ability, migration, and in vivo tumorigenesis of HeLa cells. Primarily, migration of MFN2-knockdowned HeLa cells decreased through the suppression of EMT. Thus, we concluded that MFN2 facilitates cancer progression and in vivo tumorigenesis in HeLa cells. These findings suggest that MFN2 could be a novel target to regulate the EMT program and tumorigenic potential in HeLa cells and might serve as a therapeutic target for cervical cancer. Taken together, this study is expected to contribute to the treatment of patients with cervical cancer.

A Coorientation Analysis of Perception on Bionursing between Clinical Nurses and Nursing Professors (기초간호자연과학 교육과 연구에 대한 임상 간호사와 간호학 교수의 상호지향성 인식)

  • Purpose: The purpose of this study was to compare perception on bionursing and satisfaction and importance about bionursing subjects of clinical nurses with that of professors using a coorientation model. Methods: Subjects for this study consisted of 135 clinical nurses at a tertiary hospital and 114 nursing professors. Questionnaire for perception on bionursing consisted of competency of professor, linkage with clinical practice and research of bionursing. Perceptions on bionursing education and research, satisfaction and importance about subjects of bionursing were measured. The data were analyzed by t-test. Results: Perception of clinical nurses on research of bionursing was more positive than professors. Perception of professors on research of bionursing was significantly less than that of professors estimated by clinical nurses. Perception of clinical nurses on linkage with clinical practice and research of bionursing estimated by nursing professor was significantly less than that of clinical nurses. Satisfaction of clinical nurses with the subjects of bionursing was significantly less than that of professors. Clinical nurses perceived anatomy the most important while professors perceived physiology the most important. Conclusion: Perceptions of clinical nurses on bionursing as well as satisfaction and importance about subjects of bionursing were identified to be different from those of professors.

  • Objectives : The purpose of this study is to understand the anatomical basis of the facial muscles and to apply this knowledge on the clinical practice of facial acupuncture. Methods : We searched both contemporary and the latest literatures on the practical application of facial muscle anatomy on Facial Acupuncture. Conclusions : Facial Acupuncture improves skin tone, texture and wrinkling by assisting the circulation of Ki. It stimulates the facial muscles directly to undo the stagnation of the meridians. To practice Facial Acupuncture, thorough understanding of facial anatomy is required. In this study the muscles of the head and neck, appropriate depth and angle of acupuncture needle, etc. were reviewed. The upper facial muscles including frontalis, procerus, corrugator supercilii and orbicularis oculi, the mid facial muscles including auricularis, nasalis, levator labii superioris, zygomaticus and so on, and the lower facial muscles including orbicularis oris, depressor labii inferioris, depressor anguli oris, mentalis and platysma etc. were reviewed in this study. For safer and more effective use of Facial Acupuncture, further study on the objective outcome of the technique should be done.

Clinically Correlated Anatomical Basis of Cricothyrotomy and Tracheostomy

  • Objective : Cricothyrotomy and tracheostomy are performed by physicians in various disciplines. It is important to know the comprehensive anatomy of the laryngotracheal region. Hemorrhage, esophageal injury, recurrent laryngeal nerve injury, pneumothorax, hemothorax, false passage of the tube and tracheal stenosis after decannulation are well known complications of the cricothyrotomy and tracheostomy. Cricothyrotomy and tracheostomy should be performed without complications and as quickly as possible with regards the patients' clinical condition. Methods : A total of 40 cadaver necks were dissected in this study. The trachea and larynx and the relationship between the trachea and larynx and the surrounding structures was investigated. The tracheal cartilages and annular ligaments were counted and the relationship between tracheal cartilages and the thyroid gland and vascular structures was investigated. We performed cricothyrotomy and tracheostomy in eleven cadavers while simulating intensive care unit conditions to determine the duration of those procedures. Results : There were 11 tracheal cartilages and 10 annular ligaments between the cricoid cartilage and sternal notch. The average length of trachea between the cricoid cartilage and the suprasternal notch was 6.9 to 8.2 cm. The cricothyroid muscle and cricothyroid ligament were observed and dissected and no vital anatomic structure detected. The average length and width of the cricothyroid ligament was 8 to 12 mm and 8 to 10 mm, respectively. There was a statistically significant difference between the surgical time required for cricothyrotomy and tracheostomy (p < 0.0001). Conclusion : Tracheostomy and cricothyrotomy have a low complication rate if the person performing the procedure has thorough knowledge of the neck anatomy. The choice of tracheostomy or cricothyrotomy to establish an airway depends on the patients' clinical condition, for instance; cricothyrotomy should be preferred in patients with cervicothoracal injury or dislocation who suffer from respiratory dysfunction. Furthermore; if a patient is under risk of hypoxia or anoxia due to a difficult airway, cricothyrotomy should be preferred rather than tracheostomy.

Bone Formation by rhBMP-7 Transduced HEK 293 Cells in Nude Mouse (재조합 BMP-7 유전자가 전달된 HEK 293 세포에 의한 누드 마우스에서의 뼈형성)

  • To induce bone formation at ectopic site by tissue engineering and gene therapy, we transplanted collagen sponges containing rhBMP-7 transduced HEK 293 cells in the hypodermis of nude mice. Bone formation was investigated by histological and electron microscopic method at 3, 6, and 9 weeks after transplantation. At 9 weeks after transplantation, eosinophilic bony tissue was observed in the implanted collagen sponge and was confirmed as bone tissue by Von Kossa stain. In the transmission electron microscopic observation, the cells in newly formed bone tissue had eccentrically located nucleus and well developed rough endoplasmic reticulum (rER). Therefore, the cells were evaluated as osteoblasts. Those results suggest that it is possible to form a bone tissue in the ectopic site by transplantation of rhBMP-7 transduced HEK 293 cells. This will be contributed to push more advanced gene therapy for bone formation. However, the HEK 293 cell is unable to apply to the clinical gene therapy. Therefore it is worth to find more compatible cells for clinical application. In addition, collagen sponge is considered as an excellent scaffold and/or carrier for gene therapy and a good biomaterial for tissue engineering.

Comparison of micro-computerized tomography and cone-beam computerized tomography in the detection of accessory canals in primary molars

  • Purpose: This study was performed to compare the accuracy of micro-computed tomography (CT) and cone-beam computed tomography (CBCT) in detecting accessory canals in primary molars. Materials and Methods: Forty-one extracted human primary first and second molars were embedded in wax blocks and scanned using micro-CT and CBCT. After the images were taken, the samples were processed using a clearing technique and examined under a stereomicroscope in order to establish the gold standard for this study. The specimens were classified into three groups: maxillary molars, mandibular molars with three canals, and mandibular molars with four canals. Differences between the gold standard and the observations made using the imaging methods were calculated using Spearman's rho correlation coefficient test. Results: The presence of accessory canals in micro-CT images of maxillary and mandibular root canals showed a statistically significant correlation with the stereomicroscopic images used as a gold standard. No statistically significant correlation was found between the CBCT findings and the stereomicroscopic images. Conclusion: Although micro-CT is not suitable for clinical use, it provides more detailed information about minor anatomical structures. However, CBCT is convenient for clinical use but may not be capable of adequately analyzing the internal anatomy of primary teeth.

New insights into pathways of the dorsal scapular nerve and artery for selective dorsal scapular nerve blockade

  • Background: The aim of this study was to clarify the topographical relationships between the dorsal scapular nerve (DSN) and the dorsal scapular artery (DSA) in the interscapular region to identify safe and convenient injection points related to DSN blockade. Methods: Thirty shoulders of embalmed Korean cadavers and 50 live subjects were used for dissection and ultrasound (US) analysis. Results: The running patterns of the DSA and DSN in the interscapular region were classified into 3 types. Type I was defined as nerves that were medial to the artery and parallel without changing location (80.0% of specimens). In type II (13.3%), the nerve and artery traversed one another only one time over their entire length. In type III (6.7%), the nerve and artery traversed one another, resembling a twist. Above the level of the scapular spine, the nerve was always medial to the artery. Below the scapular spine, the number of arteries was obviously decreased. Most of the arteries were lateral to the medial border of the scapula, except at the level of the superior angle of the scapula artery (SA). The positional tendency of the DSN toward the medial or lateral sides from the medial border of the scapula was similar. In US imaging of live subjects, the DSA was most observed at the level of the SA (94.0%). Conclusions: Results of this study enhance the current knowledge regarding the pathway of the DSN and DSA and provide helpful information for selective diagnostic nerve blocks in the interscapular region.

Analysis of Tonification and Sedation Methods in the 『Yeongsochimgugyeong』 (『영소침구경』 보사법 분석)

  • Objectives : Although the tonification and sedation method such as mountain-burning fire method and heaven-penetrating cooling method, is one of the important techniques that have been claimed to increase the effect of acupuncture, research on this has been sparse. The aim of this study is to provide an understanding of tonification and sedation methods in the "Yeongsochimgugyeong". Methods : We analyzed the similarities and differences by comparing the contents of the tonification and sedation method described in books "Yeongsochimgugyeon" and "Complete Compendium of Acupuncture and Moxibustion" Results : A total of 28 out of the 33 tonification and sedation methods described in "Yeongsochimgugyeon" were compared with those in "Complete Compendium of Acupuncture and Moxibustion". As a result, we found that many of the tonification and sedation methods recorded in "Yeongsochimgugyeon" were based on more detailed and specific clinical cases. Conclusions : The tonification and sedation methods in "Yeongsochimgugyeon" were not only descriptive and consistent in describing that methods, but also contains valuable information for clinical use. In the future, based on this research, we will be able to clinically implement these tonification and sedation methods and quantitatively evaluate them.
