Back strength and relevance of CPR chest compression (배근력과 심폐소생술의 가슴압박과의 관련성)

  • Purpose : The purpose of the study is to investigate the quality and relevance of back strength or chest compression which is applied by isotonic exercise of hip joint. Methods : Subjects were 37 students who participated in the BLS course and accepted the informed consent from December 7 to 8, 2012. During CPR performance, back strength was measured by the researcher. CPR was used the manikin for practical training with using PC, conducted by standard CPR for 2 minutes, Quality of chest compressions included average chest compression depth, rate, and recoil ratio. Results : Back strength (kg) is related to the chest compression depth (mm) (r =.746, p <.001). The high quality CPR is the most important factor so high quality is full chest recoil of chest compression and chest compression depth (mm) (${\beta}$=.831, p <.001). In this study, chest compression rate and recoil ration were not influenced by back muscle strength. Conclusion : It is necessary to implement the CPR program to improve physical strength and effective performance of CPR.

Effect of Chest Physical Therapy in Pediatric Patients (소아 폐염 환자에서 시행한 흉부 물리치료의 효과)

  • Object: We evaluated the efficacy of chest physical therapy in pediatric patients with pneumonia. Method: Retrospective study was performed in 89 pneumonia patients admitted to pediatric unit. The subjects were divided into two groups: control group; treatment group. Control group included 42 patients who didn't receive chest physical therapy. Treatment group included 47 patients who received chest physical therapy. The chest physcial therapy employed were postural drainage, chest percussion, deep breathing training and enhancement of coughings. The efficacy was evaluated by x-ray outcome before discharge. Results: There were no significant difference in age, sex, type of pneumonia, and symptom duration between two groups. However the duration of fever after admission, duration of antibiotic use and hospital stay were longer in treatment group. In treatment group, longer the day to initiate chest physical therapy, longer the hospital stay. The final outcome was not different between groups. Conclusion: The results suggest that the referred patients for chest physical therapy tends to be of severe cases. Nevertheless, the result that the outcome was not different in two groups means that the chest physical therapy could be used as a effective treatment method in pediatric patient with pneumonia.

  • The community chest has been known as an effective means for private sectors in social welfare by mobilizing community resources. The purpose of this study is to analyze the process of developing community chest system in Korea and to suggest how Korean community chest system can be changed to develop private sectors in social welfare. To obtain this purpose, the author studied as follows. Firstly, The difference between former Korean community chest law and new Korean community chest law were examined. Secondly, the background of establishment of Korean community chest law and the present condition and state of Korean community chest system for private sectors in social welfare were analyzed. Thirdly, the tasks to be solved about developing Korean community chest system for developing private sectors in social welfare were suggested as follows; 1) activating the demonstration work for influencing the formulation of pragmatic partnership between state and private sectors in social welfare 2) utilizing a variety of communication channels for community participation 3) cooperating and coordinating the relationship between community chest system and the most voluntary social welfare agencies 4) raising as much funds as possible for maintaining autonomy of private sectors in social welfare.

  • The desmoid tumor has been reported as the most common histologic subtype of soft tissue sarcoma occuring in chest wall and it known to be highly recurrent. The treatment of choice is a radical wide resection including a safe margin of uninvolved structures around the grossly visible tumor. We report a case of chest wall reconstruction using Marlex sandwich and latissimus dorsi musculocutaneous flap after wide resection of desmoid tumor on the chest wall.

  • This report decribes our experience with operative stabilization of flail chest with the use of Judet`s struts. In a series of 56 patients with flail chest, the method allowed shorter duration of artificial ventilation and decreased functional sequale. We find this technique to be better than previously published methods, since it provides better stabilization and immobilazation of the ribs and thus obviates the need for artificial ventilation and prevents post-traumatic chest deformity.

  • Six cases of broncho-pleural fistula due to complicated liver abscess were experienced at the department of chest surgery, Seoul National University Hospital from October 1967 to March 1970. Amebic liver abscess was the primary cause in the 5 cases and the remaining one case was due to pyogenic liver abscess. Involved lung was right side in all case. The clinical manifestation was fever,chill,cough, sputum, dyspnea, chest pain, hemoptysis and shoulder pain. The methods of the treatment employed were closed thoracotomy [1], thoracotomy & drainage [2],decortication [1],and right lower lobectomy[3]. The average duration of the post-operative course was 19.6 days. There was no operative mortality.

  • We have experienced 131 cases of chest wall tumor, from April 1974 to April 1984 at the Kyung Hee University hospital. We analyzed 131 cases of chest wall tumors, 59 cases were primary chest wall tumors and 72 cases were secondary chest wall tumors. The results were following; 1.Of primary benign chest wall tumors [56], lipomas were most common and located mainly in the scapular area [37/56]. 2.Of secondary malignant chest wall tumors [72], which originated from hung tumors were most common [27/72], & breast [8/72], liver [8/72], prostate [6/72] & stomach [3/72] were followed lowed in orders. 3.We excluded out the Rib tuberculosis.

Iatrogenic Perforation of the Left Ventricle during Insertion of a Chest Drain

  • Chest draining is a common procedure for treating pleural effusion. Perforation of the heart is a rare often fatal complication of chest drain insertion. We report a case of a 76-year-old female patient suffering from congestive heart failure. At presentation, unilateral opacity of the left chest observed on a chest X-ray was interpreted as massive pleural effusion, so an attempt was made to drain the left pleural space. Malposition of the chest drain was suspected because blood was draining in a pulsatile way from the catheter. Computed tomography revealed perforation of the left ventricle. Mini-thoracotomy was performed and the drain extracted successfully.

A Large Dumb Bell Shaped Desmoid Tumor in Chest Wall: A Case Report (흉벽에 발생한 거대한 유건종 1례)

  • Desmoid tumor is found most often in the anterior abdominal wall of parous women. Although it may originate in virtually any musculotendinous structure, those of the chest wall are rare. We experienced a case of large dumb bell shaped desmoid tumor originated in intercostal muscle and invaded anterior chest wall, pericardium, pleura and the lung. The patient was healthy in appearance except a painful swelling on the anterior chest wall. Roentgenographic studies demonstrated a huge homogenous mass in the right anterior chest cavity. He was treated with resection of the tumor including .anterior chest wall, a portion of the pericardium, middle lobe, and part of upper & lower lobes of the right lung because of tumor invasion. The tumor composed with two parts, one [$5{\times}4{\times}3$cm in size] is over the rib cage and another [$10{\times}15{\times}10$cm in size] is in the right chest cavity. Postoperative course was uneventful and there was no evidence of recurrence until last visit, 5 months after surgery.
