Analysis of Class 1 Integrons in Imipenem-resistant Pseudomonas aeruginosa

  • Pseudomonas aeruginosa is an aerobic, Gram-negative, glucose-nonfermenting bacterium, which has emerged as a serious opportunistic pathogen. Recently, outbreaks of carbapenem resistant P. aeruginosa give rise to significant therapeutic challenges for treating nosocomial infections. The genes of metallo-${\beta}$-lactamase (MBL), a powerful carbapenemase, are carried as a part of the mobile gene cassettes inserted into integrons playing an important role in rapid dissemination of antibiotic resistance genes among bacterial isolates. In this study, we investigated the prevalence of integron in imipenem resistant P. aeruginosa isolates. A total of 61 consecutive, non-duplicate, and imipenem resistant P. aeruginosa strains were isolated from a university hospital in the Chungcheong province of Korea. We employed repetitive extragenic palindromic sequence-based PCR (rep-PCR) method for the selection of clonally different P. aerusinosa strains. PCR and DNA sequencing were conducted for the detection of integrons. Twenty-one clonally different P. aeruginosa strains were isolated. Only one (P28) of the strains harbored $bla_{VIM-2}$ that was found as gene cassettes in class 1 integrons. Four of 21 carbapenem resistant P. aeruginosa strains harbored class 1 integron containing aminoglycoside resistance determinant. All of the integrons detected in the study contained more than one resistance gene cassette, which can mediate resistance to multiple antibiotics. To prevent further spreading of the multi-drug resistant P. aeruginosa, conseguent monitoring and clinical polices are required.

Imipenem 비감수성 Carbapenemase 생성 Pseudomonas aeruginosa에 의한 항생제 내성유형과 분자생물학적인 특성 (Patterns of Antimicrobial Resistance and Genotyping of Carbapenemase-producing Imipenem-nonsusceptible Pseudomonas aeruginosa)

  • Pseudomonas aeruginosa are important nosocomial pathogens. Their resistance to carbapenem is increasing and causing concerns in Korea. An increasing prevalence of carbapenem resistance mediated by acquired carbapenemase is being reported. Over a 10 month-period from July 2007 to April 2008, 32 strains of imipenem-nonsusceptible P. auruginosa were isolated from Kangwon National University Hospital. To determine the prevalence and genotypes of the carbapenemase-producing clinical isolates, the antibiotic susceptibility was determined by Microscan Walkaway 96 SI System and the carbapenem activity was detected by the modified Hodge test and the imipenem-EDTA-SMA double-disk synergy test. The metallo-${\beta}$-lactamase gene and OXA-type ${\beta}$-lactamase gene reported in Korea were detected by PCR. As for the result of PCR, 30 isolates of P. aeruginosa were found to have $bla_{IMP-1}$-like and 1 isolate was found to have $bla_{IMP-1}$-like and $bla_{IMP-2}$. No clinical isolates were found to have $bla_{SIM-1}$, $bla_{OXA-23}$-like and $bla_{OXA-24}$-like. Random amplified polymorphic DNA (RAPD)-PCR and dendrogram for genetical similarity to band patterns of each clinical isolates were examined. P. aeruginosa were grouped into 7 clusters of up to 50% of similarity index. In the P. aeruginosa group, PS3 was resistant to the most antibiotics, PS1 was susceptible to the most antibiotics. PS7 was resistant to aztreonam unlike other groups. This is the first report of prevalence of carbapenemase in Chuncheon.

Carbapenem 내성 Klebsiella pneumoniae ST307과 Non-ST307의 분자 특성 및 항균제 내성 비교 (Comparison of Molecular Characterization and Antimicrobial Resistance in Carbapenem-Resistant Klebsiella pneumoniae ST307 and Non-ST307)

  • Carbapenem 내성 Klebsiella pneumoniae (Carbapenem-resistant K. pneumonia, CRKP)는 전 세계적인 공중 보건 문제로 대두되고 있다. 최근 Klebsiella pneumoniae carbapenemase-2 (KPC-2) 생성 sequence type (ST)307은 CRKP의 주요 클론으로 확인되었으며, 국내에서 ST307의 확산이 보고되었다. 본 연구에서는 2020년 3월부터 2021년 12월까지 대전지역의 3차 병원에서 분리된 CRKP 50균주를 대상으로, 분자 특성과 항균제 내성 양상을 조사하였다. 역학적 관계는 multilocus sequence typing (MLST)를 통해 분석하였고, 항균제 감수성 검사는 디스크 확산법을 통해 확인하였다. PCR과 DNA 염기서열분석은 carbapenemase 유전자 확인을 위해 수행하였다. CRKP감염은 남성과 60세 이상의 환자에서 훨씬 더 빈번하게 확인되었다. CRKP 50균주 중 46균주(92.0%)는 다제내성(MDR)을 보였고, 44균주(88.0%)는 carbapenemase-producing K. pneumoniae (CPKP)로 확인되었다. 주요 carbapenemase 유형은 KPC-2 (36균주, 72.0%)였으며, New Delhi metallo-enzyme-1 (NDM-1)과 NDM-5는 각각 7균주(14.0%)와 1균주(2.0%)에서 확인되었다. 특히, KPC-2 생성 K. pneumoniae의 88.9% (32/36)가 ST307에 속한 반면, NDM-1,-5 생성 K. pneumoniae의 87.5% (7/8)가 non-ST307에 속한 것을 확인하였다. 이러한 결과는 ST307의 확산 뿐만 아니라 non-ST307의 발달을 예방하기 위한 적절한 감염관리와 효과적인 감시체계가 필요하다고 사료된다.

Colistin resistance and plasmid-mediated mcr genes in Escherichia coli and Salmonella isolated from pigs, pig carcass and pork in Thailand, Lao PDR and Cambodia border provinces

  • Background: Colistin and carbapenem-resistant bacteria have emerged and become a serious public health concern, but their epidemiological data is still limited. Objectives: This study examined colistin and carbapenem resistance in Escherichia coli and Salmonella from pigs, pig carcasses, and pork in Thailand, Lao PDR, and Cambodia border provinces. Methods: The phenotypic and genotypic resistance to colistin and meropenem was determined in E. coli and Salmonella obtained from pigs, pig carcasses, and pork (n = 1,619). A conjugative experiment was performed in all isolates carrying the mcr gene (s) (n = 68). The plasmid replicon type was determined in the isolates carrying a conjugative plasmid with mcr by PCR-based replicon typing (n = 7). The genetic relatedness of mcr-positive Salmonella (n = 11) was investigated by multi-locus sequence typing. Results: Colistin resistance was more common in E. coli (8%) than Salmonella (1%). The highest resistance rate was found in E. coli (17.8%) and Salmonella (1.7%) from Cambodia. Colistin-resistance genes, mcr-1, mcr-3, and mcr-5, were identified, of which mcr-1 and mcr-3 were predominant in E. coli (5.8%) and Salmonella (1.7%), respectively. The mcr-5 gene was observed in E. coli from pork in Cambodia. Two colistin-susceptible pig isolates from Thailand carried both mcr-1 and mcr-3. Seven E. coli and Salmonella isolates contained mcr-1 or mcr-3 associated with the IncF and IncI plasmids. The mcr-positive Salmonella from Thailand and Cambodia were categorized into two clusters with 94%-97% similarity. None of these clusters was meropenem resistant. Conclusions: Colistin-resistant E. coli and Salmonella were distributed in pigs, pig carcasses, and pork in the border areas. Undivided-One Health collaboration is needed to address the issue.

대전지역의 3차 병원에서 분리된 Carbapenem 내성 Pseudomonas aeruginosa의 병독성 인자 검출 (Molecular Detection of Virulence Factors in Carbapenem-Resistant Pseudomonas aeruginosa Isolated from a Tertiary Hospital in Daejeon)

  • 다제내성 P. aeruginosa의 출현과 확산은 전 세계적으로 중요한 문제가 되고 있다. P. aeruginosa에 의한 발병은 일부 몇몇 세포 관련 및 세포외 병독성 인자의 생성에 기인한다. 본 연구에서는 대전지역의 3차 병원에서 분리된 carbapenem 내성 P. aeruginosa를 대상으로 병독성 인자의 분포와 항균제 내성 양상을 조사하였다. 항균제 감수성 시험은 디스크 확산법으로 확인하였고, 병독성 유전자의 분석을 위해 PCR과 염기서열분석을 수행하였다. 또한, 다제내성 P. aeruginosa의 sequence type (ST)은 multilocus sequence typing (MLST)을 통해 확인하였다. 32균주의 carbapenem 내성 P. aeruginosa 중, 14균주(43.8%)가 다제내성이었으며, 주요 ST는 ST235 (10균주, 71.4.%)임을 확인하였다. 병독성 유전자는 32균주 모두에서 확인되었고, 이 중 가장 높은 빈도로 확인된 병독성 유전자는 toxA, plcN, phzM (100.%)이었다. 또한, 32균주는 모두 8개 이상의 병독성 유전자를 가지고 있었으며, 9균주(28.1%)가 15개의 병독성 유전자를 가지고 있었다. exoU 유전자는 다제내성 P. aeruginosa 균주의 71.4%에서 확인되었다. 이러한 결과는 exoU 유전자가 다제내성 P. aeruginosa 균주의 지속성에 대한 예측 표지자가 될 수 있을 것으로 사료된다.

Development of a Novel Immunochromatographic Assay for Rapid Detection of OXA-23 β-lactamase-producing Acinetobacter baumannii

  • Among the several agents causing carbapenem resistance of Acinetobacter baumannii, the most common cause is OXA-23 ${\beta}$-lactamase, which is known to hydrolyze carbapenem. To effectively control dissemination of carbapenem-resistant Acinetobacter baumannii (CRAB), development of both rapid and easy-to-use detection methods are required. The aim of this study is to develop a novel immunochromatographic assay (ICA) for rapid detection of OXA-23 ${\beta}$-lactamase. Of the seven monoclonal antibodies (mAbs) screened by ELISA, four mAbs (4G6, 4H6, 6G4, 9A4) exhibited high reactivity. Of these four specific antibodies, the combination of 6G4/4G6 showed the greatest reactivity and this combination of mAbs (6G4/4G6 mAbs) was used to develop the OXA-23 ${\beta}$-lactamase ICA. Of 102 A. baumannii isolates tested, the OXA-23 ${\beta}$-lactamase ICA results were consistent with PCR analysis except one false positive and one false negative isolate. The overall sensitivity and specificity were 98.36% and 97.56%, respectively. In conclusion, to the best of our knowledge, we have developed the first specific antibody set to detect OXA-23 ${\beta}$-lactamase using an ICA kit. This novel ICA can be used as a reliable and easy-to-use immunological assay for detection of OXA-23 ${\beta}$-lactamase producing CRAB in clinical laboratories.

항생제 사용량 변화에 따른 그람음성균주의 항생제 내성률의 변화 양상 (The Impact of the Antibiotic Burden on the Selection of its Resistance among Gram Negative Bacteria Isolated from Children)

  • 목적: 국내 소아 그람음성균혈증에서 항생제 내성변화 및 항생제 사용량과의 관계를 분석하였다. 방법: 최근 10년간 18세 이하 입원 환자의 혈액에서 분리된 그람음성균의 항생제 내성률 변화 및 항생제 사용량과의 관계를 분석하였다. 결과: 폐렴막대균, 대장균, 녹농균, 아시네토박터 바우마니의 분리율은 연간 혈액배양 1,000건당 각각 4.6례, 3.5례, 3.4례 및 2.2례였다. 폐렴막대균에서 광범위 세팔로스포린에 대한 내성변화는 없었으나 2010년부터 카바페넴 내성 폐렴막대균이 동정된 후 점차 빈도가 증가하였다. 대장균의 광범위 세팔로스포린 내성이 10%에서 50%로 아시네토박터 바우마니의 카바페넴 내성이 11%에서 71%로 크게 증가하였다(P for trend <0.01). 녹농균은 여러 항생제에 높은 내성을 보였으나 유의한 내성변화를 보이지 않았다. 대장균의 cefepime 내성과 cefepime 사용량 사이에 유의한 양의 상관관계를 보였다(r=0.900, P=0.037). 결론: 국내 소아에서 발생한 그람음성균 균혈증 분석시 카바페넴 및 광범위 세팔로스포린 내성이 증가하였고 일부에서 항생제 사용량과의 관련성을 확인하였다. 이는 원내 경험적 항생제 결정시 중요한 고려 사항이며 추후에도 지속적인 원내 항생제 사용량 및 내성률에 대한 감시가 필요하겠다.

Genotyping and Molecular Characterization of Carbapenem-resistant Acinetobacter baumannii Strains Isolated from Intensive Care Unit Patients

  • The emergence of multidrug-resistant Acinetobacter baumannii has partly increased treatment failure and patient mortality. Class D β-lactamases is an important mechanism of resistance to beta-lactam antibiotics in this species. This study aimed to investigate the relationship between the presence oxacillinase gene and genetic fingerprints of A. baumannii isolates from the intensive care unit of an Egyptian tertiary care hospital. One hundred and twenty A. baumannii clinical isolates were collected. Multiplex PCR was performed to detect genes encoding oxacillinases (OXA-23, OXA-24, OXA-51, OXA-58 and OXA-143). Molecular typing of all collected isolates was performed using random amplified polymorphic DNA (RAPD)-PCR assay. Out of 120 examined isolates, 92, 88 and 84% were resistant to ertapenem, imipenem and meropenem, respectively. The species-specific, commonly present OXA-51 gene was found in all isolates while OXA-23 showed a high prevalence of 88% of isolates. OXA-24 and OXA-143 genes were detected in 3% and 1% of isolates, respectively. No OXA-58 gene was detected. Five clusters consisting of 19 genotypes were detected using RAPD-PCR. Genotype A was the most prevalent, it was observed in 62% of the isolates followed by genotype B (12%). These results revealed that genotypes A and B are common in the hospital. Results also demonstrate that RAPD-PCR is a rapid and reliable method for studying the clonal similarity among A. baumannii isolated from different clinical specimens.

소아청소년 영역에서의 새로운 항미생물제 (New Antimicrobial Agents for Children)

  • There are relatively few novel antimicrobial agents despite the dramatic increase in antimicrobial resistance and multiple drug resistance of clinical isolates worldwide. Vancomycin is still the most widely used antibiotic for treating resistant Gram-positive coccal infections in children, especially for methicillin-resistant Staphylococcus aureus. For children with Gram-positive coccal infections where vancomycin is not effective or older therapeutic agents cannot be tolerated, linezolid, quinupristin-dalfopristin or daptomycin may be useful in the appropriate clinical setting. For Gram-negative bacterial infections, new carbapenems await clinical application. Tebipenem pivoxil is a novel oral carbapenem undergoing clinical trials for acute otitis media in pediatric patients. Antiviral drug development is now progressing at the pace of antibiotic development 30 years ago. Newer antiviral agents used for the treatment of herpes viruses and hepatitis C virus infections in children are included in this review.

접합가능한다제내성녹농균의출현 (Emergence of Conjugative Multidrug-Resistant Pseudomonas aeruginosa)

  • 다제내성 P. aeruginosa (MRPA)의 출현과 확산은 전 세계적으로 심각한 문제가 되었다. 특히 metallo-β-lactamases (MBLs)의 carbapenem 고도내성 관여 정도는 심각한 수준이며, 특히 P. aeruginosa는 장내세균 속 균종과는 달리 새로운 클론들이 지속적으로 나타나고 기존에 확산되었던 우세 클론을 대체하는 과정이 매우 빠르게 진행되고 있다. 이에 본 연구에서는 2017년 9월부터 2019년 9월까지 부산의 한 종합병원에서 다양한 의료용 시료로부터 분리된 18균주의 P. aeruginosa 균주에 대한 항균제 내성 유전자 분석과 DNA 염기서열 분석을 통한 Integron의 유전자 카세트 분석 및 접합에 의한 Plasmid 전달 분석을 수행하며 이에 대한 역학관계를 조사하고자 하였다. 18균주 모두 XDR 표현형을 보이는 균주였으며, Colistin(100%)을 제외한 대부분의 항생제에 내성을 나타냈으나, aztreonam(22.2%), ceftazidime(16.6%)에 일부 감수성을 보였다. 균주의 66.7%는 다양한 항균제 내성을 나타내는 Class1 integron을 가지고 있었으며, 접합에 의한 Plasmid전달도 성공적으로 이루어졌다(83.3%). 이는 이전의 장내세균에 대한 연구결과보다 25.8% 상향한 결과를 보여 공중보건에 대한 심각한 역학관점의 고찰을 필요로 한다. 특히, IMP-6 ST235 (66.7%)가 주를 이루며, VIM-2 ST357 (16.7), IMP-1 ST446 (16.7)이 확인되었다. 흥미롭게도, 국내에서는 아직까지 보고된 바가 없는 IMP-1 생성 ST446의 확인은 또 다른 MRPA 고위험 클론의 생성과 유행이라는 관점에서 주목할 만하다.
