No Association Between MTHFR A1298C Gene Polymorphism and Head and Neck Cancer Risk: A Meta-analysis Based on 9,952 Subjects

  • Objective: Findings for associations between the methylenetetrahydrofolate reductase (MTHFR) A1298C gene polymorphism and head and neck cancer risk have been conflicting. We therefore performed a meta-analysis to derive a more precise relationship. Methods: Ten published case-control studies were collected and odds ratios (ORs) with 95% confidence intervals (CIs) were used to assess the association between MTHFR A1298C polymorphism and head and neck cancer risk. Sensitivity analysis and publication bias assessment also were performed to guarantee the statistical power. Results: Overall, no significant association between MTHFR A1298C polymorphism and head and neck cancer risk was found in this meta-analysis (C vs. A: OR=1.04, 95%CI=0.87-1.25, P=0.668, Pheterogeneity<0.001; CC vs. AA: OR=1.07, 95%CI=0.70-1.65, P=0.748, $P_{heterogeneity}<0.001$; AC vs. AA: OR=1.06, 95%CI=0.88-1.27, P=0.565, $P_{heterogeneity}<0.001$; CC+AC vs. AA: OR=1.06, 95%CI=0.86-1.30, P=0.571, $P_{heterogeneity}<0.001$; CC vs. AA+AC: OR=1.02, 95%CI=0.69-1.52, P=0.910, $P_{heterogeneity}<0.001$). Similar results were also been found in succeeding analysis of HWE and stratified analysis of ethnicity. Conclusion: In conclusion, our meta-analysis demonstrates that MTHFR A1298C polymorphism may not be a risk factor for developing head and neck cancer.

Inorganic As Concentration in Rice Grown Around the Abandoned Mining Areas and its Health Risk Assessment

  • The current study was carried out to investigate total and inorganic arsenic (As) concentrations in 112 rice samples (husked rice and polished rice) grown around the abandoned mining areas and to estimate the potential health risk through dietary intake of rice in Korea. Mean concentrations of total As in husked rice and polished rice were 0.23 and $0.13mg\;kg^{-1}$, respectively. Also, average inorganic As concentrations in husked rice and polished rice were 0.09 and $0.05mg\;kg^{-1}$, respectively. These levels are lower than the standard guideline value ($0.2mg\;kg^{-1}$) for inorganic As in polished rice recommended by Korea Ministry of Food and Drug Safety and Codex. For health risk assessment, the average values of cancer risk probability was $5.7{\times}10^{-5}$ which was less than the acceptable cancer risk of $10^{-6}{\sim}10^{-4}$ for regulatory purpose. Also, hazard quotient values were lower than 1.0. Therefore, these results demonstrated that human exposure to inorganic As through dietary intake of rice collected from abandoned mining areas might not cause adverse health effects.

Health Risk Assessment of Occupants in the Small-Scale Public Facilites for Aldehydes and VOCs (일부 미적용 다중이용시설의 실내 공기 중 알데히드류 및 휘발성유기화합물 노출로 인한 건강위해성 평가)

  • This study was to assess the lifetime cancer and non-cancer risk of exposure of worker and user at public facilities in Korea to volatile organic compounds (VOCs). We measured the concentrations of two aldehydes and five VOCs in indoor air at 424 public buildings that 8 kinds of public facilities (70 movie theaters, 86 offices, 86 restaurants, 70 academies, 22 auditoriums, 30 PC-rooms, 30 singing-rooms and 30 bars) all over the country. There were estimated the human exposure dose and risks with averages of the using-time and frequency for facility users and office workers, respectively. Carcinogens (formaldehyde, acetaldehyde, and benzene) were estimated the lifetime excess cancer risks (ECRs). non-carcinogens (toluene, ethylbenzene, xylene, and styrene) were estimated the hazard quotients (HQs). The average ECRs of formaldehyde and benzene for facility worker and user were $1{\times}10^{-3}{\sim}1{\times}10^{-4}\;and\;1{\times}10^{-4}{\sim}1{\times}10^{-5}$ level, respectively, in all facilities. HQs of four non-carcinogens did not exceed 1.0 for all subjects in all facilities. The estimated ECRs for restaurant and auditorium were the highest, and the PC-room and bar were the next higher facilities. Furthermore, people in a smoking facility had the highest cancer risk. Higher ECRs of formaldehyde and benzene were observed in indoor smoking facilities such as restaurant and auditorium. Higher HQs of toluene and xylene were observed at the restaurant and office building.

  • Radon is a naturally occurring radioactive gas and a major indoor contribution of exposure to ionizing radiation in dwellings. $^{222}Rn$ is a health hazard gas what is responsible for thousand lung cancer deaths every year. In this study, indoor radon concentrations present in thirty representative houses in Mahallat city, Iran, were determined in order to estimate lung cancer risk associated with residential radon exposure. Long-term passive method, using CR-39, was used to measure the radon concentration. The results showed an association between the age of the dwellings and the indoor radon concentration that was found, in that the concentration of radon tended to increase as the age of the dwelling also increased. The indoor radon concentrations were calculated to be within the range of $23{\pm}2$ to $350{\pm}26Bq{\cdot}m^{-3}$, with an average of $158Bq{\cdot}m^{-3}$. The annual effective dose from inhaled radon and its decay products was calculated between $0.8{\pm}0.1$ and $12.3{\pm}0.9mSv{\cdot}y^{-1}$, with an average of $5.5mSv{\cdot}y^{-1}$. By taking into consideration the EPA recommendation and ICRP statement, the average annual risk of lung cancer from inhaled radon was calculated as 0.09%, 0.06%, 0.01%, and 0.03% for current smokers (CS), those who had ever smoked (ES), never smokers (NS) and the general population, respectively.

MDM2 T309G has a Synergistic Effect with P21 ser31arg Single Nucleotide Polymorphisms on the Risk of Acute Myeloid Leukemia

  • Background: The P53 tumor suppressor gene plays a pivotal role in maintaining cellular homeostasis by preventing the propagation of genome mutations. P53 in its transcriptionally active form is capable of activating distinct target genes that contribute to either apoptosis or growth arrest, like P21. However, the MDM2 gene is a major negative regulator of P53. Single nucleotide polymorphisms (SNP) in codon Arg72Pro of P53 results in impairment of the tumor suppressor activity of the gene. A similar effect is caused by a SNP in codon 31 of P21. In contrast, a SNP in position 309 of MDM2 results in increased expression due to substitution of thymine by guanine. All three polymorphisms have been associated with increased risk of tumorigenesis. Aim of the study: We aimed to study the prevalence of SNPs in the P53 pathway involving the three genes, P53, P21 and MDM2, among acute myeloid leukemia (AML) patients and to compare it to apparently normal healthy controls for assessment of impact on risk. Results: We found that the P21 ser31arg heterozygous polymorphism increases the risk of AML (P value=0.017, OR=2.946, 95% CI=1.216-7.134). Although the MDM2 309G allele was itself without affect, it showed a synergistic effect with P21 ser/arg polymorphism (P value=0.003, OR=6.807, 95% CI=1.909-24.629). However, the MDM2 309T allele abolish risk effect of the P21 polymorphic allele (P value=0.71). There is no significant association of P53 arg72pro polymorphism on the risk of AML. Conclusion: We suggest that SNPs in the P53 pathway, especially the P21 ser31arg polymorphism and combined polymorphisms especially the P21/MDM2 might be genetic susceptibility factors in the pathogenesis of AML.

A Critical Evaluation of DNA Adducts as Biological Markers for Human Exposure to Polycyclic Aromatic Compounds

  • The causative role of polycyclic aromatic hydrocarbons (PAH) in human carcinogenesis is undisputed. Measurements of PAH-DNA adduct levels in easily accessible white blood cells therefore represent useful early endpoints in exposure intervention of chemoprevention studies. The successful applicability of DNA adducts as early endpoints depends on several criteria:i.adduct levels in easily accessible surrogate tissues should reflect adduct levels in target-tissues, ii. toxicokinetics and the temporal relevance should be properly defined.iii. sources of inter- and intra-individual variability must be known and controllable, and finally iv. adduct analyses must have advantages as compared to other markers of PAH-exposure. In general, higher DNA adduct levels or a higher proportion of subjects with detectable DNA adduct levels were found in exposed individuals as compared with non-exposed subjects, but saturation may occur at high exposures. Furthermore, DNA adduct levels varied according to changes in exposure, for example smoking cessation resulted in lower DNA adduct levels and adduct levels paralleled seasonal variations of air-pollution. Intra-individual variation during continuous exposure was low over a short period of time (weeks), but varied significantly when longer time periods (months) were investigated. Inter-individual variation is currently only partly explained by genetic polymorphisms in genes involved in PAH-metabolism and deserves further investigation. DNA adduct measurement may have three advantages over traditional exposure assessment: i. they can smooth the extreme variability in exposure which is typical for environmental toxicants and may integrate exposure over a longer period of time. Therefore, DNA adduct assessment may reduce the monitoring effort. ii. Biological monitoring of DNA adducts accounts for all exposure routes. iii. DNA adducts may account for inter-individual differences in uptake, elimination, distribution, metabolism and repair amongst exposed individuals. In conclusion, there is now a sufficiently large scientific basis to justify the application of DNA adduct measurement as biomarkers in exposure assessment and intervention studies. Their use in risk-assessment, however, requires further investigation.

Risk Assessment of Heavy Metals in the Vicinity of the Abandoned Metal Mine Areas (폐금속광산지역 중금속의 위해성 평가)

  • An environmental survey from three abandoned metal mine areas was undertaken on to assess the risk of adverse health effects on human exposure to heavy metals influenced by past mining activities. Tailings contained high concentrations of heavy metals may have a impact on soils and waters around the tailing piles. In order to perform the human risk assessment, chemical analysis data of soils, rice grains and waters for As, Cd, Cu and Pb have been used. The HQ values for heavy metals via the rice consumption were significantly higher compared with other exposure pathways in all metal mine areas. The resulting HI values in three mine areas were higher than 9.0, and their toxic risk due to rice ingestion was strong in these mine areas. The cancer risk of being exposed to As by the rice consumption from the A, B and C mine areas was $5.1\;{\times}\;10^{-3}$, $6.8\;{\times}\;10^{-3}$ and $3.1\;{\times}\;10^{-3}$, respectively. The As cancer risk via the exposure pathway of rice ingestion from these mine areas exceeds the acceptable risk of 1 in 10,000 set for regulatory purposes. Thus, the daily intakes of rice by the local residents from these mine areas can pose a potential health threat if exposed by long-term As exposure.

Outcomes Based on Risk Assessment of Anastomotic Leakage after Rectal Cancer Surgery

  • Purpose: Anastomotic leakage (AL) is associated with high morbidity and mortality, high reoperation rates, and increased hospital length of stay. Here we investigated the risk factors for AL after anterior resection for rectal cancer with a double stapling technique. Patients and Methods: Data for 460 patients who underwent primary anterior resection with a double stapling technique for rectal carcinoma at a single institution from 2003 to 2007 were prospectively collected. All patients experienced a total mesorectal excision (TME) operation. Clinical AL was defined as the presence of leakage signs and confirmed by diagnostic work-up according to ICD-9 codes 997.4, 567.22 (abdominopelvic abscess), and 569.81 (fistula of the intestine). Univariate and logistic regression analyses of 20 variables were undertaken to determine risk factors for AL. Survival was analysed using the Cox regression method. Results: AL was noted in 35 (7.6%) of 460 patients with rectal cancer. :Median age of the patients was 65 (50-74) and 161 (35%) were male. The diagnosis of AL was made between the 6th and 12th postoperative day (POD; mean 8th POD). After univariate and multivariate analysis, age (p=0.004), gender (p=0.007), tumor site (p<0.001), preoperative body mass index (EMI) (p<0.001), the reduction of TSGF on 5th POD less than 10U/ml (p=0.044) and the pH value of pelvic dranage less than or equal to 6.978 on 3rd POD (p<0.001) were selected as 6 independent risk factors for AL. Assessment of the Reliability of a Novel Self-sampling Device for Performing Cervical Sampling in Malaysia

  • Background: The participation of women in cervical cancer screening in Malaysia is low. Self-sampling might be able to overcome this problem. The aim of this study was to assess the reliability of self-sampling for cervical smear in our country. Materials and Methods: This cross-sectional study was conducted on 258 community dwelling women from urban and rural settings who participated in health campaigns. In order to reduce the sampling bias, half of the study population performed the self-sampling prior to the physician sampling while the other half performed the self-sampling after the physician sampling, randomly. Acquired samples were assessed for cytological changes as well as HPV DNA detection. Results: The mean age of the subjects was $40.4{\pm}11.3years$. The prevalence of abnormal cervical changes was 2.7%. High risk and low risk HPV genotypes were found in 4.0% and 2.7% of the subjects, respectively. A substantial agreement was observed between self-sampling and the physician obtained sampling in cytological diagnosis (k=0.62, 95%CI=0.50, 0.74), micro-organism detection (k=0.77, 95%CI=0.66, 0.88) and detection of hormonal status (k=0.75, 95%CI=0.65, 0.85) as well as detection of high risk (k=0.77, 95%CI=0.4, 0.98) and low risk (K=0.77, 95%CI=0.50, 0.92) HPV. Menopausal state was found to be related with 8.39 times more adequate cell specimens for cytology but 0.13 times less adequate cell specimens for virological assessment. Conclusions: This study revealed that self-sampling has a good agreement with physician sampling in detecting HPV genotypes. Self-sampling can serve as a tool in HPV screening while it may be useful in detecting cytological abnormalities in Malaysia.

Environmental Impact Assessment of Nuclear Power Plant Accident using Spatial Information Modeling: A Case Study of Chernobyl (공간정보 모델링을 이용한 원전 사고의 환경 영향 평가: 체르노빌 사례연구)

  • This paper demonstrates the effectiveness of advanced spatial modeling techniques for environmental monitoring and impact assessment through a case study of Chernobyl nuclear accident occurred in 1986. Land-cover types changed after the accident are analysed by a post classification comparison method using bi-temporal Landsat TM data acquired in 1986 and 1992 near the accident site. Spatial modeling including various kriging algorithms are also applied to analyze the relationships between Cesium concentrations in soil and thyroid cancer incidence rates in Belarus, which was greatly damaged by the accident. The change detection results clearly showed the decrease of croplands and the increase of abandoned lands, and concrete structures were newly built around the nuclear plant to prevent the spread of radioactive contamination. In Belarus, high Cesium concentrations were observed in southern areas with high thyroid cancer risk estimated by Poisson kriging. Geographically weighted regression, which could account for geographic variations of independent variables including Cesium concentrations and distances from the Chernobyl nuclear power plant, was applied to extract the relationships between the independent variables and the thyroid cancer risk. The estimated risk values showed a correlation coefficient value of 0.98 with respect to the thyroid cancer risk values, which implied that the thyroid cancer risk in Belarus was affected by the accident. In conclusion, it is expected that advanced spatial modeling techniques applied in this study would be useful for environmental impact assessment and public health research.
