해면질골에서 위상속도 및 감쇠계수와 구조적 특성 사이의 상관관계를 고찰하기 위하여 나일론 줄을 평행하게 배열하여 제작된 해면질골 팬텀이 이용되었다. 7개의 해면질골 팬텀의 골소주 간격 (trabecular separation; Tb.SP)은 $300\;{\mu}m$부터 $900\;{\mu}m$까지 분포하였으며, 골용량 비율 (volume fraction: VF)은 1.6%부터 8.7%까지 분포하였다. 해면질골 팬팀의 위상속도 및 감쇠계수는 12.7 mm의 직경 및 1 MHz의 중심 주파수를 갖는 한 쌍의 광대역, 비집속형 초음파 변환기와 함께 수중에서 투과법을 이용하여 측정되었다. 1 MHz에서 측정된 위상속도 및 감쇠계수는 Tb.Sp이 증가함에 따라 거의 선형적으로 감소하였으며, VF이 증가함에 따라 거의 선형적으로 증가하였다. 위상속도 및 감쇠계수를 독립변수로 하고 Tb.Sp 및 VF을 종속변수로 하는 단순 및 다중선형회귀모델로부터 VF 예측을 위한 결정계수가 Tb.Sp 예측을 위한 결정계수에 비하여 높게 나타나는 것을 알 수 있었다. 이와 같이 나일론 줄을 평행하게 배열하여 제작된 해면질골 팬텀에서 나타나는 결과는 사람의 해면질골에서 나타나는 결과와 잘 일치하였으며, 해면질골에서 위상속도 및 감쇠계수 측정으로부터 구조적 특성 평가가 가능하다는 것을 알 수 있었다.
본 연구에서는 해면질골에서 해면소주 두께 (Tb.Th) 및 해면소주 간격 (Tb.Sp)과 같은 구조적 특성에 대한 군속도 및 감쇠계수의 의존성을 살펴보기 위하여 해면질골 팬텀으로서 개포된 구조를 갖는 8개의 구리폼 및 니켈폼이 이용되었다. 구리폼 및 니켈폼의 군속도 및 감쇠계수는 12.7 mm의 직경 및 1.0 MHz의 중심 주파수를 갖는 한 쌍의 광대역, 비집속형 초음파 변환기와 함께 수중에서 투과법을 이용하여 측정되었다. 구리폼 및 니켈폼을 투과한 초음파 신호에서 Biot의 fast wave 및 slow wave가 분리되어 나타나는 것을 관찰할 수 있었다. 구리폼 및 니켈폼의 군속도는 Tb.Th 및 Tb.Sp와 모두 높은 양의 상관관계를 나타냈다. 구리폼의 감쇠계수는 Tb.Th 및 Tb.Sp와 높은 음의 상관관계를 나타내는 반면에 니켈폼의 감쇠계수는 Tb.Th 및 Tb.Sp와 높은 양의 상관관계를 나타냈다. 이와 같은 결과는 해면질골 샘플 또는 팬텀을 이용한 다른 연구자들에 의하여 보고된 결과를 이해하는데 도움이 된다.
Numerous studies reported the significant association between genetic polymorphisms in the vitamin D receptor (VDR) gene and various bone phenotypes such as bone mineral density (BMD) and bone quality, although conflicting results were produced. The objective of this study was to investigate the association between a TaqI RFLP in the VDR gene and calcaneal BMD in Korean vegetarian men, and its interaction with nutrition status as an environmental factor. BUA (broadband ultrasound attenuation), SOS (sound of speed) and stiffness index of the calcaneus were measured using an ultrasound bone densitometer in 266 Korean men (age: $mean{\pm}SD;\;50.9{\pm}12.0$ year), and a TaqI RFLP in the VDR gene analysed by PCR-RFLP method. In total subjects, the distribution of TT, Tt and tt genotypes occurred with frequencies of 90.8%, 8.8% and 0.4%, respectively. There were no significant associations between this polymorphism and osteopenia-osteoporosis or several bone phenotypes in our subjects irrespectively of nutrition status (P>0.05). Therefore, our results suggest that a TaqI RFLP in the vitamin D receptor gene does not contribute to the susceptibility to the calcaneal BMD in Korean men.
본 연구에서는 진단 방법의 정확성이 아직까지 구체적으로 규명되지 않고 있는 어린이의 성장 발육을 체계적이고 과학적으로 측정할 수 있는 방법을 제시하고자 시도하였다. 현재 많이 이용되고 있는 성장 진단 방법은 X-ray 영상을 이용한 각 관절 부위의 성장판의 개폐 분석이다. 그러나 X-ray 방식은 성장 진단에서 요구되는 주기적이고 반복적인 측정을 하지 못함과 동시에 방사선 노출이라는 단점을 가지고 있다. 따라서 인체에 무해한 초음파 영상을 이용한 성장판 개폐 분석 대체 가능성을 평가하기 위해 종골의 초음파 광역 감쇄 영상을 이용한 프로파일 분석 및 성장판 영상 분석 알고리즘을 제시하였다. 본 연구에서는 7세~16세의 어린이 269명에 대해서 X-ray를 이용하여 무릎, 손가락, 종골에서의 성장판 영상을 얻었다. 또한 초음파 방식을 이용하여 종골의 성장판 영상을 얻었다. 각 관절에서의 성장판 개폐 유무 시기는 각 개인별로 측정 방법에 따라 조금의 차이는 있으나 전반적으로 일치하였다. 또한. X-ray방식과 초음파 방식에 의한 성장판 영상을 비교한 결과 상호 높은 연관성을 나타내었다. 이와 같이 성장판을 정확하게 측정하기 위해 도입된 초음파 프로파일 분석 알고리즘은 기존의 X-ray 영상 방식에 의한 성장판의 측정을 대체할 수 있을 것으로 사료된다.
본 연구는 최대 골질량에 도달하는 20대의 대학생 1039명(남학생 532명, 여학생 507명)을 대상으로 골초음파 상태와 영양소 섭취 및 생화학적 영양 상태를 나타내는 지표들과의 관계를 분석하였다. 대상자의 골밀도 수치에 따라 t-score$\underline{\geq}$-1인 정상 골밀도군과 t-score<-1 골감소군으로 분류하여 비교한 결과 남학생의 경우 정상 골밀도군이 90.4%, 골감소군이 9.6%이며 여학생의 경우 정상 골밀도군이 94.1%, 골감소군이 5.9%의 분포를 보였다. 체적지표인 BMI를 아시아-태평양 비만기준에 의거하여 분리하였을 때는 남학생은 저체중 3.4%, 정상+과체중 70.7%, 비만 25.9%, 여학생은 저체중 16.2%, 정상+과체중 76.5%, 비만 7.3%의 분포를 보였다. 정상 골밀도군과 골감소군의 골밀도와 생화학적 영양 상태는 남학생 여학생 모두 통계적으로 유의한 차이가 없었다. 전체대상자들의 영양소 섭취상태를 살펴 본 결과, 남학생의 경우 정상 골밀도군과 골감소군 간의 유의적인 차이가 나타나지 않았으나 여학생의 경우 동물성 단백질과 동물성 철분 섭취량이 정상 골밀도군에서 골감소군보다 유의하게 높았다. 평균 에너지 섭취량은 동일한 연령대의 한국인영양섭취기준(KDRIs)과 비교하였을 때, 각 남학생 정상골밀도군 약 74%, 골감소군 67%, 여학생 정상골밀도군 약 89%, 골감소군 81%의 섭취수준을 나타내었다. 평균 단백질섭취량은 남녀 두 군 모두 100%를 상회하는 섭취 양상을 나타냈으며, 강한 뼈를 유지하거나 각종 신진대사에 필수적인 영양소이기 때문에 골격건강에 가장 큰 영향을 미치는 칼슘의 경우 남학생과 여학생 모두 KDRIs와 비교하여 권장섭취기준의 약 63%정도로 부족한 것으로 나타났다. 골 건강 상태에 영향을 미치는 영양인자의 로지스틱 회귀분석 결과 연령, 성별 및 BMI를 교란변수로 통제하였을 경우 동물성 단백질, 동물성 철분 및 아연섭취는 골감소를 예방하는 보호 작용이 있는 것으로 나타났다. 동물성 단백질과 이들 미량 영양소의 섭취정도는 높은 식사의 질을 반영하므로 질적으로 우수한 식생활이 최대 골질량에 도달하는 시기인 20대의 골격형성과 건강을 위해 필수적인 것으로 사료된다.
The objective of this study is to determine the effectiveness of 16-month milk consumption as a part of the health promotion community program for women in Asan. Subjects included 313 women belonging to the milk group (mean age = 69.1, range $47{\sim}89 y$) and 66 women to the control (mean age=43.6, range $20{\sim}69 y$) group. For those in the milk group, one cup (200 ml) of partially lactose-digested low-fat milk was provided everyday for 16 months. Each subject was interviewed to assess calcium intake by a 24-h recall after fasting blood was obtained for analyzing bone turnover markers, and calcaneus broadband ultrasound attenuation (BUA) was measured by quantitative ultrasound (QUS) on the left heel before and after the milk supplementation. After 16 months, the calcium intake levels changed from 55% of recommended dietary allowance (RDA) to 85% RDA in the milk group and from 73% RDA to 84% RDA in the control group. BUA were reduced from $67.9{\pm}8.1$ to $64.7{\pm}17.5$ dB/MHz for milk and from $90.4{\pm}13.0$ to $87.2{\pm}15.2$ dB/MHz for control groups. Paired t-test showed the changes of BUA for both groups (-3.24 and -3.15 dB/MHz for milk and control groups, respectively) were significant, but the two groups did not show any differences in absolute changes. When post-BUA was analyzed after age, initial BUA and menopausal status were controlled as covariates in ANCOVA model, the milk group showed significantly (p < 0.05) smaller changes than the control group (-3.50 vs -6.71 dB/MHz, respectively). According to a multiple regression analysis, milk consumption and initial BUA showed significant interaction meaning that those with lower initial BUA showed higher milk effects. We conclude that one-cup a day milk consumption for 16 month can prevent further bone loss and significantly improve calcium intake.

Nutrition factors play an important role in the pathogenesis of osteoporosis. The purpose of this study is to investigate the relationship between nutritional status and bone mineral density of elderly women. Three hundred thirty five elderly women (over 65 years) in Asan were divided into three age groups (group 1, less than 70 y ; group 2, from 70 to 75 y ; group 3, 75 y or more). Total alkaline phosphatase and serum calcium (Ca) were analyzed using spectrophotometric procedure. Data for food and nutrient intakes were obtained by a 24-hour recall method. Bone density was measured by broadband ultrasound attenuation (BUA) using QUS-2. Age differences were tested with the X 2 test for categorical variables and with ANOVA and Tukey's test for continuous variables. Correlation was conducted to test the association between bone density and nutrient intake. The subjects in age groups 1,2 and 3 were $36.7\%$, $32.8\%$ and $30.4\%$, respectively. Height and body weight of the subjects were significantly decreased with age. Average bone density of the subjects in group 3 was lower than the other age groups. Osteoporosis determined by t-score is $17.9\%$ for group 1, $24.5\%$ for group 2 and $55.9\%$ for group 3 (p < 0.001). The serum Ca level of the subjects in group 2 was significantly lower than that of group 1 although mean values in all age groups are within the normal range. Dietary Ca intake, nutrient adequacy ratio (NAR) and index of nutrient quality (INQ) were decreased with age. Bone density was negatively correlated with age (p < 0.001), while body mass index (p < 0.01) was positively related with bone density. Although partial correlation did not reveal the significant correlation of BMD and dietary calcium after controlling for age, since calcium intake was very poor compared to sodium and phosphorous intakes, recommendation of more calcium intake for elderly women especially those over 75 years must be continuously emphasized.
This study was designed to investigate the effect of bone demineralization and tibia lead on blood lead in retired lead workers. Two hundred thirty five(126 females and 109 males) retired lead workers who worked in 4 different lead factories and 101 non-occupationally lead exposed subjects(51 females and 51 males) were recruited from March 2004 to October 2004. Bone mineral density(BMD) was measured at left calcaneous bone area by broadband ultrasound attenuation(BUA) method with QUS-2(Metra Biosystems Inc, USA). The BUA value transformed into T-score by WHO standard conversion criteria. Tibia bone lead was measured for skeletal bone lead with K-xray fluorescence(K-XRF) and blood lead was analyzed with flameless atomic spectrophotometer. Hemoglobin, hematocrit, serum calcium and iron were also analyzed. In addition, information for smoking and drinking status and basic personal data such as age, gender and lead exposure were also collected using questionnaire inquiry. Blood lead was correlated with tibia lead (r=0.664) and these two variables were negatively correlated with BMD in bivariate analysis. BMD showed significant main effect on the change of blood lead independent to tibia lead without any effect modification of age or gender; the one T-score unit decrease of mineral bone density made $0.43{\mu}g/dl$ increase of blood lead. On the other hand, tibia lead showed effect modification with gender on blood lead; the slope of tibia lead on blood lead in male was steeper than in female and crossed at around zero of tibia lead. In the multiple regression analysis of blood lead and tibia lead on BMD after adjustment of related covariates, only blood lead showed statistically significant effect on BMD. This study confirmed that BMD and blood lead were significantly associated. To verify the causal association of BMD on blood lead and vice versa, further longitudinal studies are needed.
