Implant overdenture using a locator bar system by drill and tapping technique in a mandible edentulous patient: a case report

  • Various options have been introduced for the selection of implant overdenture attachments. Attachment wear due to the repeated insertion and removal of dentures has caused problems such as decreased retention and the requirement for suprastructure remanufacturing. In these cases, a Locator bar system was applied using the drill and tapping technique to achieve total retrievability. In a 55-year-old female patient who showed three degrees of mobility in most of her teeth due to severe alveolar bone loss, a complete denture in the maxilla and an implant supported type overdenture in the mandible were planned after extracting all the remaining teeth. Six implants were placed from canine region to the distal molar region, and the locator was connected to the milled bar using the drill and tapping technique. For a 61-year-old female edentulous patient who complained of poor retention with old denture, a complete denture in the maxilla and an implant-tissue supported type overdenture in the mandible were planned. Four implants were placed in front of mental foramen, and the Locator was also connected to the Hader bar using the drill and tapping technique. With this technique, female parts can be easily replaced, and retention can be continuously maintained.

A Study of the Effect of Platelet-Rich Plasma on the Cellular Proliferation and Differentiation of Osteoblast Cell Line (혈소판농축혈장이 조골세포주의 세포증식 및 분화에 미치는 영향에 대한 연구)

  • The osseointegration in implant therapy is achieved following general wound healing mechanism. Platelet play a major role in wound healing process. In addition to blood clot formation, they secrete many growth factors which regulate the attachment, proliferation and differentiation of nearly all cell types. The use of these growth factors is now known to be very effective methods to improve the cellular activity. Platelet-rich plasma which is made with the newly developed technique concentrating platelets 3-folds or more is also proven to be very effective method to stimulate and accelerate the healing of bone and soft tissue. Previous study proved that platelet-rich plasma enhanced the cellular attachment by inducing fibronectin, vitronectin from osteoblast. So, this study was aimed to investigate the effect of platelet-rich plasma on the cellular proliferation and differentiation in vitro. The effect on the proliferation was evaluated by MTT assay. To evaluate autocrine and paracrine effect, conditioned medium was made and compared. By measuring alkaline phosphatase activity, the effect on the cellular differentiation was evaluated. The results were as following: The cellular proliferation of osteoblast cell line increased depending on the concentration of platelet-rich plasma and conditioned medium. The alkaline phosphatase activity increased depending on the concentration of platelet-rich plasma and conditioned medium. These findings imply that platelet-rich plasma enhance the cellular proliferation and differentiation and maximize the cellular activity by using the autocrine and paracrine effect.

  • Malignant peripheral nerve sheath tumors(MPNSTs) are uncommon sarcomas that mostly arise in the soft tissue. They can develop from the pre-existing neurofibromas or schwannomas, or denovo from the peripheral nerves, or they can occur following the radiation therapy. We report a case of MPNST that developed in the sciatic nerve of the patient with neurofibromatosis type-1(NF-1). The patient was a 39-year-old man with the history of NF-1, who's main symptom was a rapidly enlarging painful mass in his posterior thigh. The well demarcated tumor, $6.5{\times}5{\times}4.5$cm in size, was composed of closely packed spindle cells. Since the patients with NF-1 have a high risk for developing a recurred MPNST, the importance of the clinical follow up is emphasized.

Treatment of Giant Cell Tumor of Distal Radius with Wide Resection and Proximal Fibular Graft (광범위한 절제술 후 근위 비골 이식을 이용한 원위 요골 거대세포종의 치료)

  • Purpose : Treatment of giant cell tumor of distal radius can be treated in several ways according to the agressiveness of the tumor. We treated 3 cases of widely involved giant cell tumor of distal radius with wide resection and proximal fibular graft and report the results with review of literatures. Material and Method : We have treated 3 cases of giant cell tumor of the distal radius since last 1990. Among 3 cases, two cases were grade III radiologically and treated by wide resection of distal radius and vascularized proximal fibular graft, and one case, grade II radiologically, treated by distal radial resection and non-vascularized proximal fibular graft. We followed up clinical results of above three cases 9 years, 12 years and 2 years. Result : In all three cases, tranplanted fibula graft showed solid union but grade III tumors recurred at 4 year and 6 year postoperatively. One of the case which recurred 4 year later was treated with secondary wide resection and wrist fusion with autogenous iliac bone graft, and didn't show any recurrent finding for these 5 years after re-operation. And another grade III, which recurred at 6th post-operative year, is under follow-up for 6 years after recur without 2nd operation. Grade II case didn't show any recurrent findings on 2 year follow-up. Conclusion : Grade III cases recurred at 4 year and 6 year follow-up. The cause of recurrence was thought to be invasion of remaining tumor cell in the soft tissue. To prevent recurrence, complete resection of primary tumor was necessary.

Delayed Foreign Body Reaction Caused by Bioabsorbable Plates Used for Maxillofacial Fractures

  • Background Bioabsorbable plates and screws are commonly used to reduce maxillofacial bones, particularly in pediatric patients because they degrade completely without complications after bone healing. In this study, we encountered eight cases of a delayed foreign body reaction after surgical fixation with bioabsorbable plates and screws. Methods A total of 234 patients with a maxillofacial fracture underwent surgical treatment from March 2006 to October 2013, in which rigid fixation was achieved with the Inion CPS (Inion, Tampere, Finland) plating system in 173 patients and Rapidsorb (Synthes, West Chester, PA, USA) in 61 patients. Their mean age was 35.2 years (range, 15-84 years). Most patients were stabilized with two- or three-point fixation at the frontozygomatic suture, infraorbital rim, and anterior wall of the maxilla. Results Complications occurred in eight (3.4%) of 234 patients, including palpable, fixed masses in six patients and focal swelling in two patients. The period from surgical fixation to the onset of symptoms was 9-23 months. Six patients with a mass underwent secondary surgery for mass removal. The masses contained fibrous tissue with a yellow, grainy, cloudy fluid and remnants of an incompletely degraded bioabsorbable plate and screws. Their histological findings demonstrated a foreign body reaction. Conclusions Inadequate degradation of bioabsorbable plates caused a delayed inflammatory foreign body reaction requiring secondary surgery. Therefore, it is prudent to consider the possibility of delayed complications when using bioabsorbable plates and surgeons must conduct longer and closer follow-up observations.

  • Mesenchymal stem cells (MSC) are pluripotent cells that can be found in umbilical cord blood from new borne babies as well as placenta, bone marrow, adipose tissue, amniotic fluid, muscle, et al. MSC are capable of renewing themselves without differentiation in long-term culture, also can be differentiated into various tissues under specific condition. Formulating a cryopreservation protocol for the MSC is required because these cells cannot survive for long periods under in vitro culture conditions and a new formulation of harmless cryoprotectant is needed for the direct injection of MSC into patients. The undifferentiated MSC were frozen with a vitrification solution of 40% ethylene glycol, 20% Ficoll-70 and 0.3M sucrose. The survival rate after thawing and their proliferation rate were examined and compared with slow rate cooling methods using dimethylsulfoxide (DMSO). The vitrification method showed high survival rate after thawing and proliferation capacity comparable to DMSO. It can be suggested that ultra-rapid cooling method by vitrification is reliable methods for long term preservation of MSC and the vitrification solution with ethylene glycol, Ficoll-70 and sucrose will be more beneficially used for direct transplantation of MSC into patients than DMSO solution.

Affection of Blood Calcium, Ino-Phosphorus and Alkaline Phosphatase Activity Inject with Paraquat on Rats

  • Paraquat (N,N'-dimethyl-4,4'-bipyrrimidinium-dichlorides (PQ): MW 186.6) has been used to killing verierty of hubside plants. For this study were use Sprague-dawely rats (190$\pm$10 gm) and injection 40 mg/kg BW of paraquat (LD$_{50}$) to 24 hrs PQ and 4 days PQ group excepte normal control. For the measurement of blood calcium Ino-phosphorus and activity of alkaline phosphatase (ALP) by HITTACH 746. In serum phosphoruse of normal control were 8.0$\pm$1.2 mg/dl and 24 hrs PQ group were 8.9$\pm$1.0 mg/dl (P<0.05) and 4 days PQ group were 8.8$\pm$0.42 gmm/dl (P<0.05). In serum phosphorus were very sensitive uptaked to 10% within only 24 hours, but 4 days of PQ were similar uptake level than 24 hrs PQ. So this 8.9 mg/dl of sem phosphorus may be thought threshold level becouse does not more encrease. In blood calcium normal of rats were measured 9.51$\pm$0.3 mg/dl and 24 hrs of PQ group were 9.9$\pm$0.51 mg/dl (uptaked 4.5%, p>0.05). This uptake cannot fined meaning mathmatic statistics but 4 days PQ groups of calcium were 10.43$\pm$0.37 gm/dl (uptaked 10%, p<0.05). That 24 hrs PQ groups of caclium dose not reacted sensitive to irritated by PQ. So, when use oxidants of PQ, the blood calcium and Ino-phosphorus were linear correlated uptake that reasone thought may be move out form hardness bone tissue to in blood it does not take feeding to hunger for 4 days. In ALP normal of rats were measured 991$\pm$106 unit/L. In 24 hrs irritated PQ rats were fall down by 629$\pm$91 unit/L (P<0.001) but in 4 days of PQ rats were 792$\pm$85 unit/L (P<0.0011) that the activity of level were mild recovered activity from 629 unit (63%) to 792 unit (80%). So, that the reasone of ALP were very sensitive activity and reverse correlated to blood calcium or phosphorus irritated by PQ.

Maxillary Advancement using Distraction Osteogenesis Devices in Cleft Palate Patients (악정형 장치를 이용한 구순구개열 환자의 상악골 전방 견인)

  • For the treatment of cleft palate patients, orthognathic surgery has been used by standard protocol. Predictable results have been achieved with standard techniques in slight to moderate maxillary hypoplasia. However, limited advancement and high relapse rate was reported in severe cases. The purpose of the present study was to review the clinical results of distraction osteogenesis in the patients with cleft lip and palate. Distraction osteogenesis has improved results in these patients by allowing soft tissue relaxation and gradual bone generation. Therefore, greater movement of the craniofacial skeleton is possible in severe cases of maxillary retrusion with lower relapse rates. In conclusion, distraction osteogenesis for the advancement of hypoplastic maxilla of cleft patients has shown successful treatment method.

  • Synovial sarcoma is an uncommon malignant soft tissue tumor which usually occurs in young-aged person, and frequently involves the lower extremities. Many authors recommended wide excision, adjuvant chemotherapy and radiation therapy. The proposed factors that affect the prognosis are age, size of tumor, site of tumor in the body, depth of tumor, histologic grade and method of treatment. The purpose of the study is to analyze the factors that affect the 5-year survival rate. We retrospectively evaluated 19 cases of synovial sarcoma treated in the Kosin University Medical Center from Jan. 1982 to Dec. 1994. The overall 5-year survival rate was 47.6% and the 5-year survival rates were significantly higher(P<0.05) in the group with smaller size of mass. The significantly lower 5-year survival rates were observed in the patient with deeply located lesion and with higher histologic grade. The 5-year survival rates were higher in the patients treated with wide excision than in the patients treated with amputation. But there was no significant difference between these groups. The adjuvant chemotherapy and radiotherapy were performed. In conclusion, we suggest that the better prognosis will be observed in the patients with smaller size of tumor mass, superficially located and lower histologic grade.

Survival Rate and Prognostic Factors of Liposarcoma (지방 육종의 생존율과 예후 인자)

  • Liposarcoma is second in frequency only to malignant fibous histocytoma among the soft tissue sarcoma. Many different factors which might affect the survival rate of liposarcoma have been reported by many authors. The purpose of this study was to evaluate survival rate of liposarcoma and define the prognostic factors that affected survival rate. The authors analysed retrospectively 17 patients of liposarcoma in extremities from May 1984 to Dec. 1995 who had been treated in department of orthopaedic surgery of Kosin University Medical Center. All cases were resected with marginal or wide margin. There were 9 men and 8 women. The mean age was 48 years. The follow-up period ranged from 15 to 96 months. We compared the prognosis of the patients with several factors; age, sex, surgical staging, size, site, histologic type and treatment modality. At last follow-up, the presence of local recurrence was in 3 cases and the presence of lung metastasis was in 8 cases. The survival rates by Kaplan-Meier product limit method at 2 years and 5 years were 87% and 57% respectively. The statististically significant difference was estimated in histologic type, but was not estimated in age, sex surgical staging, size, site and treatment modality. In conclusion, the histologic type is considered as the most important factor of the prognosis in liposarcoma. Although it was too few patients for the differences to be statistically significant, we consider that surgical staging, site, size, the radio-therapy and chemothrapy in liposarcoma will affect the prognosis.
