Purpose: The aim of the present study is to evaluate the effect of repetitive distraction and compression on new bone formation during distraction period. Materials and methods: Sixteen healthy rabbits, weighing about 2.5kg, were used in this experiment. A unilateral mandibular osteotomy was performed in the left mandible and the distractor(Track 1 $plus^{(R)}$, Gebruder Martin $GmbH^{(R)}$, Germany) was fixed with four screws (Cross driver screw $TI^{(R)}$, Gebruder Martin $GmbH^{(R)}$, Germany). After 4 days, the mandibles were distracted at a rate of 0.6mm/day for 10 days to obtain the amount of 6mm distraction in the control group(n=4). In the experimental group A(n=6), they were distracted at a rate of 1.2mm/day for 5 days and then compression of 0.6mm length and distraction of 0.6mm per 12 hours were carried out as counter direction for 5 days, relatively. In the experimental group B(n=6), distraction of 1.2mm length and compression of 0.6mm length per 12 hours were repeated for 10 days to obtain the amount of 6mm distraction finally. The experimental animals were sacrificed at 2 and 4 weeks after surgery and block specimens were obtained. With histologic and histomorphometric analysis, we observed the histologic changes of the cells and bone formation after H-E and Masson- Trichrome staining and then, measured Bone Deposition Rate with TOMORO $ScopeEye^{TM}$ ver. 3.5(Olympus, Japan), Results: Histologically, new bone formation was examined in all experimental groups and the control. But, the ability of bone formation of the experimental group A was somewhat better than any other groups. On the histomorphometric analysis, Bone Deposition Rate was higher in the experimental group A$(50.67{\pm}4.36%)$ than in the control group$(45.94{\pm}3.97%)$ and in the experimental group B$(42.68{\pm}5.70%)$. These data showed significant differences statistically(p<0.05). Conclusion: These results show that the distraction osteogenesis using repetitive compression and distraction force in the early consolidation period may be effective for new bone formation.

Purpose For the reconstruction of craniofacial deformities, the distraction osteogenesis is an useful method which can make new soft tissues as well as new bone. Although the distraction osteogenesis is an effective procedure in quantitative aspects, the new bone formed by this procedure can be coarse in qualitive aspects sometimes. Materials and methods Twelve rabbits, weighing about 2 kg, were included and the prefabricated distraction device were used. After a latency period of 5 days, in the experimental group (n=6), the mandible was distracted at the rate of 1.0 mm/day for 2 days and then compressed with 1.0 mm for the next 3rd day, therefore distracted totally to a length of 1.0 mm for 3 days. This procedure was repeated 5 times and, as a result, the experimental group was distracted to a length of 5.0 mm for 15 days finally. In the control group (n=6), the mandible was distracted totally to a length of 5.0 mm at the rate of 1 mm/day. At 2 weeks and 8 weeks of consolidation in the control group, 3 rabbits in each group were sacrificed and their biopsy specimens from the distracted mandible were taken. Results All animals showed the mandibular elongation clinically and radiographically. Histologically, many blood vessels, osteoblasts and immature bones formed by osteoid deposition were observed in the experimental group at 2 weeks. At 8 weeks, the bony trabeculae were thicker than the ones in the control group and were composed of lamella bones and woven bones in the experimental group. On histomorphometric analysis, the bone deposition area of the distracted site was broader in the experimental group $(273.8{\pm}115.7\;cm^2)$ than the one in the control group $(199.4{\pm}101.4\;cm^2)$. Futhermore, the modified rate of bone deposition area was higher in the experimental group (48${\pm}$20%) than the one in the control group (35${\pm}$18%). However, these data showed no significant differences statistically. Conclusion These results suggest that the distraction osteogenesis by using an alternating distraction/compression protocols is an effective method for increasing new bone formation in distracted areas.
PURPOSE. This study was performed to characterize the effects of zirconia coated with calcium phosphate and hydroxyapatite compared to smooth zirconia after bone marrow-derived osteoblast culture. MATERIALS AND METHODS. Bone marrow-derived osteoblasts were cultured on (1) smooth zirconia, (2) zirconia coated with calcium phosphate (CaP), and (3) zirconia coated with hydroxyapatite (HA). The tetrazolium-based colorimetric assay (MTT test) was used for cell proliferation evaluation. Scanning electron microscopy (SEM) and alkaline phosphatase (ALP) activity was measured to evaluate the cellular morphology and differentiation rate. X-ray photoelectron spectroscopy (XPS) was employed for the analysis of surface chemistry. The genetic expression of the osteoblasts and dissolution behavior of the coatings were observed. Assessment of the significance level of the differences between the groups was done with analysis of variance (ANOVA). RESULTS. From the MTT assay, no significant difference between smooth and surface coated zirconia was found (P>.05). From the SEM image, cells on all three groups of discs were sporadically triangular or spread out in shape with formation of filopodia. From the ALP activity assay, the optical density of osteoblasts on smooth zirconia discs was higher than that on surface treated zirconia discs (P>.05). Most of the genes related to cell adhesion showed similar expression level between smooth and surface treated zirconia. The dissolution rate was higher with CaP than HA coating. CONCLUSION. The attachment and growth behavior of bone-marrow-derived osteoblasts cultured on smooth surface coated zirconia showed comparable results. However, the HA coating showed more time-dependent stability compared to the CaP coating.
Purpose: To evaluate the effect of compression on the distracted area in new bone formation during mandibular distraction osteogenesis in rabbits. Materials and method: Sixteen rabbits, weighing approximately 2 Kg, and the prefabricated distraction device were used. With the rabbits under general anesthesia, we performed vertical osteotomies between the anterior and posterior mandibular teeth and then placed the distraction device. After a 5 -day latency period, the mandible was distracted to a length of 10.0 mm at a rate of 1 mm/day and then immediately compressed 4 mm in the experimental group (n=8). In the control group (n=8), the mandible was distracted to a length of 6.0 mm at a rate of 1 mm/day. Rabbits in the control group were killed at 2 and 8 weeks during the consolidation period. The specimens were evaluated with light microscope after H & E stain. Histomorphometric analysis was done at 8 week specimens. Results: All experimental animals showed mandibular elongation on the macroscopical and radiographic evaluations. At 2 week, immature bone formation was observed from the surface of the host bone margins with collagen fibers arranged parallel to the direction of distraction in the control group; in the experimental group, immature bone formation was observed adjacent to the host bone, and the collagen fibers were not arranged uniformly. At 8 week, spindle-shaped new bone formation was seen in the direction of distraction in distracted area of the control group, while in the experimental group, the newly formed bone was arranged in a multidirectional manner, like the pattern of trabeculae. In the histomorphometric analysis of 8 weeks, the area of bone deposition was $2.12{\pm}\;0.75\;inch^2$ in the experimental group and $0.87{\pm}0.51\;inch^2$ in the control group (p<0.01). The bone deposition ratio was $29.60{\pm}10.50%$ in the experimental group and $12.10{\pm}7.17%$ in the control group (p<0.01). Conclusion: These results suggest that compression after over-distraction during the mandibular distraction osteogenesis is an effective method of increasing the amount of newly formed bone in distracted area.
In bone tissue engineering, polycaprolactone (PCL) is one of the most widely used biomaterials to manufacture scaffolds as a synthetic polymer with biodegradability and biocompatibility. The polymer deposition system (PDS) with four axis heads, which can dispense bio-polymers, has been used in scaffold fabrication for tissue engineering applications. A dual-head deposition technology of PDS is an effective technique to fabricate 3D scaffolds. The electrospinning technology has been widely used to fabricate porous and highly interconnected polymer fibers. Thus, PDS can fabricate nanofiber-combined hybrid scaffolds using fused deposition modeling (FDM) and electrospinning methods. This study aims to fabricate nanofiber-combined scaffolds with uniform nanofibers using PDS. The PCL nanofibers were fabricated and evaluated according to the fabrication process parameters. PCL nanofibers were successfully fabricated when the applied voltage, tip-to-collector distance, flow rate, and solution concentration were 5 kV, 1 cm, 0.1 ml/h, and 8 wt%, respectively. The cell proliferation was evaluated according to the electrospinning time. Scanning electron microscopy was used to acquire images of the cross-sectioned hybrid scaffolds. The cell proliferation test of the PCL and nanofiber-combined hybrid scaffolds was performed using a CCK-8 assay according to the electrospinning time. The result of in-vitro cell proliferation using osteosarcoma MG-63 cells shows that the hybrid scaffold has good potential for bone regeneration.

The present study was aimed to investigate the effect of osseointegration according to implant placement timing in the distracted alveolar bone using intraoral distraction device. Six adult mongrel dogs of either sex, weighing about 15kg, were used. The animals were divided into 4-week and 8-week groups according to the timing of implant installation. The left upper and lower premolars and first molars were extracted and an alveoloplasty was performed to simulate an atrophic ridge. After 12 weeks of healing, a segmental osteotomy was made and an intraoral distraction device which was designed for augmentation of vertical height of the edentulous ridge was applied. Latency period was allowed for 5 days and then distraction was made at a rate of 1.2mm/day for 8 days. Four or eight weeks after distraction, implants were installed. Twelve weeks after implant installation, the animals were sacrificed. Macroscopic, radiographic, and histologic examinations of distracted alveolar ridge were performed. No significant abnormalities such as infection and dehiscence of overlying soft tissue were observed. Radiographically, there was slight bone resorption around the medial and distal edges of the alveolar bone segment, and a new bone deposition was observed in the neighboring alveolar crest area in the both groups. The satisfactory osseointegration was achieved in the distracted gap of the both groups, but fibrous tissue appeared on the buccal side of implant in the distracted gap in 4-week group. These results suggest that proper timing of implant installation is 8 weeks rather than 4 weeks after distraction when dental implant is to be placed onto the distracted bone.
Purpose: Calcium phosphate (CaP)-coated implants promote osseointegration and survival rate. The aim of this study was to (1) analyze the dissolution behavior of the residual CaP particles of removed implants and (2) evaluate bone apposition of CaP-coated machined surface implants at the early healing phase. Methods: Mandibular premolars were extracted from five dogs. After eight weeks, the implants were placed according to drilling protocols: a nonmobile implant (NI) group and rotational implant (RI) group. For CaP dissolution behavior analysis, 8 implants were removed after 0, 1, 2, and 4 weeks. The surface morphology and deposition of the coatings were observed. For bone apposition analysis, block sections were obtained after 1-, 2-, and 4-week healing periods and the specimens were analyzed. Results: Calcium and phosphorus were detected in the implants that were removed immediately after insertion, and the other implants were composed mainly of titanium. There were no notable differences between the NI and RI groups in terms of the healing process. The bone-to-implant contact and bone density in the RI group showed a remarkable increase after 2 weeks of healing. Conclusions: It can be speculated that the CaP coating dissolves early in the healing phase and chemically induces early bone formation regardless of the primary stability.
The current interest in periodontal tissue regeneration has lead to research in bone graft, root surface treatments, guided-tissue regeneration, administration of growth factors, and the use of enamel matrix protein as possible means of regenerating lost periodontal tissue. Several studies have shown that a strong correlation between platelet-rich plasma and the stimulation of remodeling and remineralization of grafted bone exits, resulting in a possible increase of 15-30% in the density of bone trabeculae. The purpose of this study was to study the histopathological results and differences between the use of platelet-rich plasma and the use of enamel matrix $protein(Emdogain^?)$ about bone regeneration at the implant. Implant fixtures were inserted and graft materials placed into the left femur in the experimental group, while the only implant fixtures placed in the control group. In the first experimental group, platelet-rich plasma and xenograft were placed at the supracrestally placed implant site, and in the second experimental group, $Emdogain^{(R)}$ and xenograft placed at the supracrestally placed fixture site. The degree of bone regeneration adjacent to the implant fixture was observed and compared histopathologically at 2, 4, and 8 weeks after implant fixture insertion. The results of the experiment are as follows: 1. The rate of osseointegration to the fixture threads was found to be greater in the experimental group compared to in the control group. 2. The histopathological findings showed that the bone regeneration, the partial osseointegration existed at 4 weeks, and that osseointegration and bone density increaced in the experimental groups at 8 weeks. 3. The results showed that new bone formation and bone remodeling increased in the area near to the fixture in the first and second experimental groups at 8 weeks than at 4 weeks. The results showed that in the area distant from the fixture, new bone formation did not increase and bone remodeling decreased in the first experimental group at 4, 8 weeks, and that new bone formation increased in the second experimental group. 4. The histopathological findings showed that AZ deposition in the first experimental group was remarkable at 2, 8 weeks, and in the second experimental group at 2, 4, 8 weeks in the area distant from the fixture threads.
최근에, 3 차원 인공지지체와 나노섬유는 골 조직 재생을 위해 개발되고 있다. 본 연구에서는, 나노-마이크로 정밀 분사 시스템을 이용하여 하이브리드 인공지지체를 제작하였다. 하이브리드 인공지지체는 마이크로 인공지지체와 나노섬유가 결합하여 제작되었으며, 마이크로 인공지지체와 나노섬유를 얻기 위해 자유 형상 제작 기술과 전기방사 기법이 사용되었다. 마이크로 인공지지체는 정밀한 공극을 고려하여 CAD/CAM 데이터 따라 자유 형상 제작 기술에 의해 제작되었으며, 제작 공정은 $100^{\circ}C$의 온도, 평균 650 kPa의 압력, 그리고 250 mm/sec의 Z 축 이송속도가 적용되었다. 그리고 전기방사법을 통하여 나노섬유를 제작함에 있어서 본 시스템에 적용한 공정 조건은, 5 kV의 전압, 0.1 ml/min의 유량, 그리고 1 mm의 노즐 팁과 콜렉터와의 거리로 설정하였다. 제작된 하이브리드 인공지지체는 MG-63 세포를 이용하여 세포 증식 실험을 진행하였다.
Garre's osteomyelitis is a unique form of osteomyelitis characterized rediographically by localized thickening of the periosteum and deposition of laminated subperiosteal bone. The most common inciting factor is a mandibular infection in permanent first molar with necrotic pulp. This disease occurs primarily in children and to date in all instances it has occured only in mandible. It usually results in hard swelling over the jaws, producing facial asymmetry with little or no pain. The overlying skin is normal but can occasionally be inflammed mostly when pain is present. Palpation reveals a usually smooth, bone-hard lesion which feel like an inherent part of the mandible. Unlike other forms of osteomyelitis, there is no marked increase in fever, white bloods cell count, sedimentation rate or alkaline phosphatase value. The treatment of Garre's osteomyelitis usually consist of elimination of the sourses of infection, i.e., either extration of an offending infected teeth or root canal therapy. This treatment almost always results in resolution of the Garre's osteomyelitis. Resistant cases have involved secondary surgery, i.e., decortication and sequestrectomy. This report presents three cases of Garre's osteomyelitis resolved by endodontic treatment. Cliniqtl examination revealed swelling on the face with no tenderness. Periapical radiograph showed deep caries lesion extending into pulp chamber and periapical radiolucency. Occlusal radiograph showed an enlargement of bone and stretching the periosteum. A clinical diagnosis of the Garre's osteomyelitis was made. Endodontic treatment was accomplished with conventional method and restored facial symmetry. Long-term check-ups are necessary to evaluate the results of endodontic treatment.
