Accuracy of several implant bite registration techniques: an in-vitro pilot study

  • PURPOSE. This study evaluated the accuracies of different bite registration techniques for implant-fixed prostheses using three dimensional file analysis. MATERIALS AND METHODS. Implant fixtures were placed on the mandibular right second premolar, and the first and second molar in a polyurethane model. Aluwax (A), Pattern Resin (P), and Blu-Mousse (B) were used as the bite registration materials on the healing abutments (H) or temporary abutments (T). The groups were classified into HA, HP, HB, TA, TP, and TB according to each combination. The group using the bite impression coping was the BC group; impression taking and bite registration were performed simultaneously. After impression and bite taking, the scan bodies were connected to the lab analogs of the casts. These casts were scanned using a model scanner. The distances between two reference points in three-dimensional files were measured in each group. One-way ANOVA and Duncan's test were used at the 5% significance level. RESULTS. The smallest distance discrepancy was observed in the TB group using the temporary abutments. The Blu-Mousse and HP groups showed the largest distance discrepancy. The TB and BC groups showed a lower distance discrepancy than the HP group (P=.001), and there was no significant difference between the groups using the temporary abutments and healing abutments (P>.05). CONCLUSION. Although this study has limitations as an in-vitro investigation, the groups using the temporary abutments to hold the Blu-Mousse record and bite impression coping showed greater accuracy than the group using the healing abutments to hold the pattern resin record.

Modified FR-4의 임상적용례 (CLINICAL APPLICATION OF MODIFIED FR-4)

  • 전치부 개교합은 상 하악 전치부가 폐구 시 절단 기능에 필수적인 수직피개가 결여되어 있는 상태를 말한다. 일반적인 원인으로는 손가락 빨기, 혀 내밀기, 유아성 연하, 비호흡 부전, 골격성장의 이상 또는 이들이 복합적으로 작용되어 나타날 수 있으며, 어떤 종류의 부정교합과도 함께 나타날 수 있다. 전치부 개교합을 위한 치료 방법은 다양하지만, Rolf Fr$\"{a}$nkel에 의해 고안된 Fr$\"{a}$nkel appliance(FR-4)는 골격성 I급 혹은 II급 부정교합과 함께 나타난 개교합의 치료에 특히 효과적인 것으로 알려져 있다. 서양인에서는 골격성 II급 부정교합이 높은 빈도로 나타나고, 개교합도 이와 함께 나타나는 경우가 많다. 그러나 동양인에서는 골격성 III급 부정교합의 빈도가 높고, 개교합 또한 이와 같은 골격형태와 함께 나타나는 경우가 흔히 있다. 이러한 문제점을 갖는 환자에 있어서 전통적인 FR-4의 사용이 개교합의 교정에 도움이 되었을지라도, 골격성 III급 부정교합을 악화시키는 것으로 나타났다. 두 가지 문제점을 동시에 교정하기 위하여, labial bow를 하악 전치부에, labial pads를 상악 전치부에 위치시킨 modified FR-4를 고안하게 되었다. 다음의 임상례는 전치부 개교합을 동반한 골격성 III급 부정교합을 보이는 환자를 대상으로, 전통적인 FR-4와 modified FR-4를 이용하여 치료한 결과를 비교한 것이다. 첫 번째 임상례는 전통적인 FR-4를 사용한 경우로서, 전치부 개교합은 개선되었으나 골격성 III급 부정교합의 형태가 심해졌다. 그러나 두 번째와 세 번째 임상례에서는 modified FR-4를 사용한 결과, 전치부 개교합과 III급 부정교합이 동시에 개선된 것으로 나타났다.

전치부 반대교합 성인의 저작양상에 관한 연구 (Study on masticatory pattern of adult having anterior cross bite)

  • 저작은 기본적으로 Brain system의 central pattern generator(CPG)에 의해 조절되며, CPG로 부터 target 기관까지의 출력(output)은 oral sensory feedback에 의해 변형된다. 전치부 반대교합은 이 feedback mechanism에 영향을 미치어 muscle activity와 jaw movement를 바꾼다. 본연구는 정상교합자와 전치부 반대교합자의 muscle activity와 jaw movement pattern의 차이를 알고자 시행하였다.대상은 각각 성인남녀 30명씩으로 하였으며 측정은 Biopak system의 EMG와 EGN을 사용하여 얻은 결과는 다음과 같다. 1. Resting시 실험군이 정상군보다 모든 근육(좌측 악이복근 제외)에서 높은 근활성도를 보였다. 2. Clenching시는 정상군과 실험군간의 유의차가 없었다.(우측 악이복근 제외) 3. Swallowing시는 정상군이 좌우측 교근과, 우측 후측두근 에서 실험군보다 높은 근활성도를 보였다. 4. 최대 개폐구속도는 정상군이 실험군보다 큰 값을 보였다. 5. 최대개구량과 최대전후방운동량 및 측방변위량은 정상군이 실험군보다 큰 값을 나타냈다.

디지털공제프로그램간의 디지털공제영상 비교 (A comparison of subtracted images from dental subtraction programs)

  • Purpose: To compare the standard deviation of gray levels on digital subtracted images obtained by different dental subtraction programs. Materials and Methods: Paired periapical films were taken at the lower premolar and molar areas of the phantoms involving human mandible. The bite registration group used Rinn XCP equipment and bite registration material, based on polyvinyl siloxane, for standardization. The no bite registration group used only Rinn XCP equipment. The periapical film images were digitized at 1200 dpi resolution and 256 gray levels by a flat bed scanner with transparency unit. Dental digital subtraction programs used for this study were Subtractor (Biomedisys Co., Korea) and Emago (Oral Diagnostic Systems, The Netherlands). To measure the similarities between the subtracted images, the standard deviations of the gray levels were obtained using a histogram of subtracted images, which were then analyzed statistically. Results: Subtracted images obtained by using the Emago program without manual selection of corresponding points showed the lowest standard deviation of gray levels (p<0.01). And the standard deviation of gray levels was lower in subtracted images in the group of a bite registration than in the group of no use of bite registration (p < 0.01). Conclusion: Digital radiographic subtraction without manual selection of reference points was found to be a convenient and superior method.

교합 피개 심도와 과두 위치 (THE RELATIONSHIP OF MANDIBULAR CONDYLAR POSITION TO OVERBITE DEPTH)

  • This study was designed to evaluate the relationship of mandibular condylar position to overbite depth and compare the findings from transcranial radiographs and those, from individualized corrected tomographs in determination of condylar position. The subjects consisted of 20 control subjects (male 8, female 12), and 10 open-bite patients (male 3, female 7) and 23 deep-bite patients (male 17, female 6). The mean age was 23.3 years for the control group, 21.5 years for open-bite group, and 23.2 years for deep-bite group. Transcranial radiographys and individualized corrected tomographys in centric occlusion were taken from right and left temporomandibular joints of each sueject. The results were as follows. 1. In the 20 normal subjects showing no symptoms of TM disorder, the incidence of condylar retrusion was $27.5\%$, middle condylar position $60.0\%$, and anterior displacement $12.5\%$. 2. There was significant correlation between the bite depth and observed condylar position. 3. Only $45.2\%$ of the findings from transcranial radiographs coincided with those from individualized corrected tomographs in determining condylar position.

Botulinum toxin-A injection into the anterior belly of the digastric muscle for the prevention of post-operative open bite in class II malocclusions: a case report and literature review

  • Background: Class II malocclusion patients with hyperdivergent facial types are characterized by short mandibular body lengths and anterior open bite. Accordingly, the treatment for hyperdivergent skeletal class II malocclusion is a lengthening of the mandibular body length and a counterclockwise rotation of the mandible. To prevent post-operative relapse, botulinum toxin-A (BTX-A) injection can be a retention modality. Case presentation: A class II open-bite patient received BTX-A injection to the anterior belly of her digastric muscle for the prevention of post-operative relapse. The relapse was evaluated via a clinical examination and a lateral cephalometric radiograph after the completion of post-surgical orthodontic treatment. The patient showed stable occlusion without any signs of relapse at 15 months post-operatively. Conclusion: In this case presentation, a single injection into the anterior belly of the digastric muscle was sufficient for the prevention of post-operative open bite.

A Case of Amblyomma testudinarium Tick Bite in a Korean Woman

  • A case of tick bite was found in the inguinal region of a 74-year-old Korean woman. She was attacked by the tick while working in her vegetable garden in the vicinity of mountain located in Suncheon City, the southern coastal area of the Korean Peninsula. On admission she complained of mild discomfort and itching around the bite area. The causative tick was 23 mm long and had slender pedipalps. The scutum was quite ornate and had eyes at the edge. The genital aperture was located anterior to the level of the coxa II. The spiracular plate was comma-shaped and the anus was surrounded posteriorly by the anal groove. The coxa I had subequal 2 spurs; the external one slightly larger. The spur of coxa IV was slightly longer than those of coxae II and III. The tarsus IV had 2 distinct subapical ventral spurs. It was identified as the fully engorged adult female of Amblyomma testudinarium. This is the first human case of Amblyomma bite in Korea.

알루미늄 선삭공정에서 발생되는 음향 신호 특성 (An Investigation of Acoustic Signal Characteristics in Turning of Aluminum)

  • This paper reports on the research which investigates acoustic signals acquired in turning with rough and finish simultaneously. The material is aluminum thin pipe. Two acousto-ultrasonic sensors were set on the finish and the rough bite of the CNC machine. It was first evaluated that one source was affected by the other. It was found that two signals were little affected each other, and that the acoustic signal from the finish bite was more related to the surface defects. Signals from the finish bite only were then analyzed in order to observe several types of surface defects. Second the fundamental experiments were accomplished to study the effects of machine vibration and material state. The signal characteristics due to surface defects were studied from the collected acoustic signals. The analysis was based on real time data, root mean squared average and frequency spectrum by fast fourier transform. As a result, the acoustic signals were made effects by machine condition, material structure. The acoustic signal from the finish bite was closely correlated with surface quality. Two types surface micro defects were then evaluated by the signal characteristics.

Human Bites Over Nose: Management and Reconstruction

  • Background Human bite wounds in emergency department need evaluation in regard of reconstruction. These are due to occlusive bite injuries over face. Most commonly, human bites over face involve ear and nose, and may lead to avulsion injury. Defects over nose can be reconstructed immediately after debridement or delayed till the wound heals and scar becomes supple. Thorough wash and lavage with broadspectrum antibiotic cover has utmost importance in preventing cartilage infection. Methods We report 20 cases of human bite injuries over nose who presented to us in emergency department between 2018 and 2020. At the time of presentation the wound was assessed for closure. If not possible, patient was planned for delayed reconstruction after 3 months. In case delayed reconstruction was planned, the skin and nasal mucosa were approximated at first presentation. The patients underwent paramedian forehead flap after recreation of defect with conchal cartilage graft. Second stage of flap detachment and insetting was done after 3 weeks. After three weeks of second stage, third stage of flap thinning was done. Patients were followed for 3-6 months and subjective satisfaction was noted. Results Nineteen patients underwent delayed staged reconstruction with paramedian forehead flap and one underwent primary wound closure. The flap survival was 100%. The patient satisfaction was excellent in most cases. Conclusion We recommend delayed reconstruction for human bite nasal injuries. For reconstruction, paramedian forehead flap with conchal cartilage graft, if required, provides excellent reconstructive option with good contour and color match and minimal donor site scar.

최대 교합력과 안면 골격 형태에 관한 연구 (Relationship between maximum bite force and facial skeletal pattern)

  • 본 연구의 목적은 최대 교합력을 측정하여 전후방적, 수직적 및 횡적인 안면 골격 요소들과의 관련성을 연구하는 것이다. 연구 대상으로 남녀 대학생들을 각각 26명과 14명씩 총 40명을 선택하였다. 본 연구를 위하여 개발된 strain gauge를 이용한 장치로 좌우 제1대구치와 전치부에서 최대 교합력을 측정하였다. 두부방사선 사진을 촬영하여 여러 계측 항목들을 측정하고, 통계분석을 시행하여 다음의 결과를 얻었다. 1. 제 1대구치 최대 교합력은 남자는 $68.0\pm13.9kg$, 여자는 $55.6\pm10.5kg$ 이었고 (p<0.05), 전치에서 남자는 $8.4\pm4.9kg$, 여자는 $5.1\pm3.4kg$이었다(p<0.05). 2. 저작을 선호하는 쪽의 교합력이 더 크게 나타나는 경향을 보였지만 통계적으로 유의한 차이는 없었다(P>0.05). 3. 최대교합력이 강한 군과 약한군 간에 여러 항목들에서 유의한 차이를 나타내었는데, 제1대구치 최대 교합력에 대해서는 N-S-Ar, S-Ar-Go, FH-H1, IMPA 그리고 MMO 항목들이 유의한 차이를 보였고, 전치부 최대 교합력에 대해서는 N-S-Ar 과 FH-H1 항목들이 유의한 차이를 보였다(p<0.05). 4. 최대 교합력과 유의성 있는 상관 관계를 보인 항목들은 제1대구치 최대 교합력에 대해서는 N-S-Ar, S-Ar-Go, UGA, FH-H6, FH-H1, body weight 그리고 MMO, 전치부 최대 교합력에 대해서는 Go-Me, P-1 그리고 IMPA등 이었다(p<0.05). 이상의 결과들을 종합해 볼 때, 제1대구치의 최대 교합력은 수직 전안면 고경이 짧을수록 증가하였다. 전치의 최대 교합력은 하악체 길이가 길수록, 상악 전방 치조부 고경이 클수록, 그리고 하악 절치들이 설측으로 경사져 있을수록 증가하였다(P<0.05).9\%$ 수준이었으며, 인장결합강토(TBS)에 대해서는 $52\%$ 수준이었고, Chessboard base에서 최저 Peel bond strength$(_{60}PBS)$는 전단결합강도(SBS)의 $34\%$ 수준이었으며, 인장결합강도(TBS)에 대해서는 $61\%$ 수준이었으며, Non-etched Foil-Mesh base에서 최저 Peel 결합 강도$(_{60}PBS)$는 전단결합강도(SBS)의 $34\%$ 수준이었으며, 인장결합강도(TBS)에 대해서는 $55\%$ 수준이었다. 4. 단위 면적 당 결합강도에 있어서 전단결합강도(SBS)와 인장결합강도(TBS) 및 $75^{\circ}\;와\;90^{\circ}$ peel 결합강도는 Micro-Loc base와 Chessboard base에서 차이 가 없었으며 Non-etched Foil-Mesh base에서 가장 작았고(p<0.05), $0^{\circ},\;15^{\circ},\;30^{\circ},\;60^{\circ}$ peel응력을 적용한 결과 Chessboard base에서 가장 큰 Peel결합강도를, Non-etched Foil-Mesh base에서 가장 작은 결합강도를 보였다(p<0.05).았다. 6. 주사전자현미경으로 본 표면은 모든 제품에서 생산과정 중에 보이는 압흔과 pitting이 관찰되는데, 진성기업의 Stainless Steel은 가늘고 긴 압흔이 있으며 비교적 매끄러운 표면을 보이고, Unitek사의 경우 압흔과 함께 pitting 이 관찰되며, Ormco Stainless Steel의 경우 불규칙한 pitting이 다수 존재했다.수술 시행 시기별의 차이를 보이지 않고 고른 분포를 보였다. 10. 내원한 환자를 순구개열
