Hip Protector Design Process for the Korean Elderly

  • This research aims to propose an ergonomic design process for hip protector based on previous studies, existing products, multidisciplinary experts opinion, and wearing test. The elderly are more likely to suffer a hip fracture when they fall due to their physical changes in skeletal form, muscle quantity, bone density, and joint movement. A hip protector is an effective product to prevent hip fractures in the elderly but it also has a problem in that it is uncomfortable. Therefore there is a high chance that it won't be able to prevent hip fractures properly. Since the comfort of a hip protector is one of the most critical elements in preventing a hip fracture, we need to keep improving the hip protector for mobility and usability. Based on the previous studies and limitations of current hip protector products, we need to come up with an optimal design for the Korean elderly. First, knowledge has to be built relating to the ergonomic design of the hip protector considering body shape and size analysis using 3D-scan data, and biomechanical analysis on hip fracture. Second, we need to develop a design process including hip protector pattern design, and wearing evaluation with virtual system. Third, we suggest to reevaluate and verify the design procedure from impact evaluation using testing simulator, virtual evaluation of impact, to wearing comfort and usability evaluation. This design process presented in this study would be expected to contribute to the development of ergonomic hip protector which is suitable for the Korean elderly.

  • 효과적인 재활 시스템을 구상하는데 있어서 훈련 데이터의 정교한 분석은 다음 단계 훈련을 위한 피드백 자료로서 매우 중요하다. 현재 다양한 생체 역학적 실험을 통해 인간의 운동 능력을 평가하고 이로부터 생성된 데이터의 분석을 위한 객관적이고 신뢰성 있는 연구결과들이 발표되고 있다. 그러나 대부분의 기존 연구들은 기초 통계적인 방법에 근거한 정량분석만을 수행함으로써, 획득된 정보를 임상에 적용 하는데 있어서는 충분한 신뢰성을 보장할 수 없다. 데이터마이닝은 대용량 데이터에 들어있는 숨겨진 규칙과 패턴을 탐사함으로써 임상 데이터에 숨어있는 의미 있는 정보추출에 성공적으로 사용되고 있으며, 특히 임상 연구 분야에서는 훌륭한 의사 결정 지원 시스템으로서 점점 그 사용이 증가되고 있다. 본 연구에서는 신뢰성 있는 자세 제어 능력(Postural control ability) 평가를 위해서 측정된 훈련 데이터에 가중연관규칙 탐사를 적용하여 자세 훈련 유형에 따른 근육 활성 패턴과의 연관성을 분석, 효율적인 재활 훈련 규칙을 탐사하였다. 탐사된 규칙은 재활 및 임상 전문가의 의사결정에 더욱 정성적이고 유용한 선험적 지식으로 사용 될 수 있으며, 이를 근거로 환자 맞춤형 최적의 재활 훈련 모델을 구상하기 위한 지표로서 사용될 수 있다.

진자실험을 통한 경직의 정량적 평가에 관한 연구 (A Study on the quantitative Evaluation of Spasticity Implementing Pendulum Test)

  • 본 연구에서는 중추신경계 질환에 의해 발생하는 경직을 정밀하고 정량적으로 평가하기 위하여 생체역학적 측면에서 실험 및 분석을 수행하였다. 객관적인 실험환경을 만족시키기 위해 기존에 검사자가 손으로 수행하던 방식을 개발된 진자실험 장치를 통하여 동일한 방법과 높이가 재현될 수 있도록 하였다. 하지의 진자 운동에서 발생되는 운동 형상학적 데이터들은 3차원 동작 분석장비(Elite. B.T.S. Italy)를 사용하여 측정하였으며, 경직과 관련된 하지의 각도, 각속도 변수에 대한 분석이 수행되었다. 진자 운동시 하지의 운동형상학적 데이터, 근육들의 길이변화와 변화 속도, 동적 근전도신호 등을 동시에 고려할 수 있는 프로그램을 제작하였다. 또한 기존의 분석보다 다양한 각도에서 경직의 정량적 평가가 가능하도록 14개의 새로운 변수를 도출하여 정량적 평가를 위한 총 26개의 변수에 대한 상호 상관성을 검토하였다. 새롭게 제시된 검사방식과 새로운 변수를 통하여 정밀하고 객관적인 경직 판정의 가능성을 제시하였다.

자동차 조립공정에서의 작업자세 특성 (Posture Characteristics in Automobile Assembly Tasks)

  • Many reaearchers have reproted that poor body postures are associated with pains or symptoms of musculoskeletal dissoders. Therefore, the ergonomic evaluation of postural stresses as well as biomechanical stresses is important when a job such as automobile assembly tasks involves highly repetitive and/or prolonged poor body postures. A macropostural classification shema was developed to characterise various body postures occurring in automobile assembly tasks in the study. To specify a postural code and stress level to each body posture, perceived joint discomforts were subjectively evaluated in the lab experiments for the full range of motion in five human body joints. Based on the reaults, a postural classification scheme was developed where the full range of motion in each body joint was classified into several codes repressenting different stress levels. The automobile tasks were clustered into 12 types based on the result walk-in-surveillance and the possible posture codes for each task type are defined. I was exposed that the poor postural problems in automobile assembly tasks were concerned in most part with arms, trunk and neck. Application of te developed schema to seven operations in automobile assembly tasks showed that the schema can be used as a tool to identify the operations and tasks involving highly stressful body postures. The schema can also be utilised as a basis to prioritise the candidate assembly operations for redesign of work methods.

뇌졸중 환자의 경직측정을 위한 휴대형 장치의 유용성 (Clinical Effectiveness of New Portable System for Measuring Spasticity of the Stroke Patients)

  • Spasticity is a motor disorder characterized by a velocity dependent increase in muscle tone with exaggerated tendon jerks, resulting from hyper-excitability of the stretch reflex. The aim of this study is to develop a portable system for quantifying the grade of spasticity which could calculate the biomechanical as well as neurophysiologic parameters, and for determining the relationship between the Tonic Stretch Reflex Threshold (TSRT) and Modified Ashworth Scale (MAS). Eleven patients with stroke participated in the study (6 males and 5 females, the average age of $64.5\pm16.0$ years). As a results, the mean and standard deviation values of the TSRT were $129.8\pm4.2$, $123.4\pm5.2$ and $119.1\pm2.6$ in the MAS 1, MAS 1+ and MAS 2 groups, respectively. Also, there was a negative correlation between the TSRT and MAS (rho=-0.72, p<0.05). This demonstrated that the TSRT could be made clinically available for the more objective and reliable evaluation of the spasticity, instead of using the conventional clinical scales and an isokinetic dynamometer.

  • Evaluation of the methods of processing allogenic bone must be considered in order to make an effective choice of graft materials in oral surgery. Allograft materials processed by the tissue banking industry have varying capacities of bone reconstruction. The biological function of processed bone can be affected by many factors, like particle size, processing parameters, and inclusion or exclusion of mineral and moisture. For example, freeze drying step offers a safe and economical means for packaging, shipping, storage, and preservation of homologous bone. Demineralization of cortical bone using hydrochloric acid can produce a uniform demineralized surface with a capacity for osteoinduction. The objectives of this review were to evaluate the processing methods for allogenic bone and to characterize processed materials for grafting. It is important to understand the biological, biomechanical healing of different types of allografts to make the right choice for allogenic bone on each clinical application and to achieve a successful outcome for alveolar bone reconstruction in oral surgery.

포대 운반손잡이의 인간공학적 디자인 및 평가 (Ergonomic Design and Evaluation of Carrying Handles for Bag)

  • Various characteristics of the object being lifted are known to affect the biomechanical, physiological, and psychophysical stresses. The object characteristics to be considered in the design process of lifting tasks are weight, shape, stiffness, and availability of handles and similar coupling devices. In this study, a prototype Polypropylene laminated bag with carrying handles was designed to decrease the physical stress of people who handle these bags. Physiological and psychophysical approaches as well as subjective ratings were applied to evaluate the effects of handles provided on the designed PP laminated bag. Statistical analysis showed that the VO2, heart rate, blood pressure, and Borg-RPE score for PP laminated fertilizer bag with carrying handles were significantly lower than those bags without handles. Moreover, Maximum Acceptable Lifting Endurance Time(MALET) measure, newly developed in this study, for bags with handles was significantly higher than those for bags without handles. It is thus recommended that the various types of bags and boxes be equipped with handles to reduce the musculoskeletal, physiological, psychophysical, and subjective perceived stresses.

Clinical and scanning electron microscopic analysis of fractured dental implants: a retrospective clinical analysis

  • Many longitudinal studies have reported the successful osseointegration of dental implants, with survival rates approaching 90-95%. However, implants regarded as a "success" may have also failed to undergo osseointegration. A variety of complications and failures have been observed, including implant fracture - a rare and delayed biomechanical complication with serious clinical outcomes. Given the increasing popularity of dental implants, an increase in the number of failures due to late fractures is expected. This study sought to determine the rate of implant fractures and factors associated with its development. This retrospective evaluation analyzed implants placed at Wonkwang Dental Hospital (from 1996 to the present). In our study we found that the frequency of dental implant fractures was very low (0.23%, 8 implant fractures out of 3,500 implants placed). All observed fractures were associated with hybrid-surface threaded implants (with diameter of 4.0 or 3.75 mm). Prosthetic or abutment screw loosening preceded implant fracture in a majority of these cases.

  • This paper presents a CAD program develpoed on a microcomputer in order to support graphic and computational assessment of ergonomic problems associated with the design of a man-in-cab system. The program is coded to help workspace designers with ergonomic evaluations needed in the design stage. This paper proposed a biomechanical -ergonomic evaluations needed using man and workplace models. The human model is developed to have dimensions obtained from the Korean anthropometric data reported in 1992. Its graphical representation is based on a wire-frame model but, whenever necessary, body segments can be represented by a solid model with hidden line/faces removed and shaded. Workplace models are presented for cabs of the excavator, one of the most popular construction vehicles. A workplace model consists of an operator seat, a steering wheel. two control levers, two pedals, and a control panel. The workplace elements can be modified in their sizes, positions, and orientations by changing the reference point and design parameters. An algorithm for the view test is suggested and loaded to provide a visual evaluaiton of the overall layout of a workplace model.

임신 중 요통에 관한 고찰 (A Review of Pregnancy Related Low Back Pain)

  • Pregnancy and puerperium are associated with significant changes in pschological and physiologic health status. Back and pelvic pain is common in pregnancy with prevalence figures in the range of $48\%-90\%$. The pain starts during pregnancy and often disappears soon after childbirth. But the prevalence of such pain four to six months post postpartum is report to be $25-40\%$. In $10-15\%$ of the case the pain become chronics, that is persisting for more than three months after childbirth. Low back pain and pelvic pain maybe caused by several factors related to changes that occur naturally during pregnancy. Changes in the center of gravity can create a strain on weight-bearing structures in bone. Pregnancy related hormones, relaxin, create general laxity of collagenous tissue. Another factor found to be a possible primary or contributing cause for law back and hip symptoms are leg length inequality, weight gain and changes in foot function. This article outlines the physiological and biomechanical changes that occur during pregnancy which have been reported to be possible causes of low back and pelvic pain And then, examination, diagnosis, evaluation and treatment of the woman during pregnancy are described. Standard treatment for low back pain and pelvic pain in pregnancy includes education in anatomy and kinesiology, back-strengthening exercise, training of the abdominal muscles and body posture correction. So, most woman during pregnancy require individual consultation and physiotherapist.
