The effects of pentoxifylline and tocopherol in jaw osteomyelitis

  • Objectives: Pentoxifylline (PTX) is a methylxanthine derivative that has been implicated in the pathogenesis of peripheral vessel disease and intermittent lameness. The purpose of this study was to investigate the effect of PTX and tocopherol in patients diagnosed with osteoradionecrosis (ORN), bisphosphonate-related osteonecrosis of the jaw (BRONJ), and chronic osteomyelitis using digital panoramic radiographs. Materials and Methods: This study was performed in 25 patients who were prescribed PTX and tocopherol for treatment of ORN, BRONJ, and chronic osteomyelitis between January 2014 and May 2018 in Seoul National University Dental Hospital. Radiographic densities of the dental panorama were compared prior to starting PTX and tocopherol, at 3 months, and at 6 months after prescription. Radiographic densities were measured using Adobe Photoshop CS6 (Adobe System Inc., USA). Blood sample tests showing the degree of inflammation at the initial visit were considered the baseline and compared with results after 3 to 6 months. Statistical analysis was performed using the Mann-Whitney test and repeated measurement ANOVA using IBM SPSS 23.0 (IBM Corp., USA). Results: Eight patients were diagnosed with ORN, nine patients with BRONJ, and the other 8 patients with chronic osteomyelitis. Ten of the 25 patients were men, average age was 66.32±14.39 years, and average duration of medication was 151.8±80.65 days (range, 56-315 days). Statistically significant increases were observed in the changes between 3 and 6 months after prescription (P<0.05). There was no significant difference between ORN, BRONJ, and chronic osteomyelitis. Only erythrocyte sedimentation rate (ESR) was statistically significantly lower than before treatment (P<0.05) among the white blood cell (WBC), ESR, and absolute neutrophil count (ANC). Conclusion: Long-term use of PTX and tocopherol can be an auxiliary method in the treatment of ORN, BRONJ, or chronic osteomyelitis in jaw.

  • Statement of problem: Alteration of tooth function is assumed to be changed by stress/strain on the adjacent alveolar bone, producing changes in morphology similar to those described for other load-bearing bones. When teeth are removed, opposing teeth will not be functioned. When edentulous area is restored by implant prostheses, opposing teeth will be received physiologic mechanical stimuli. Purpose: The aim of this study was to evaluate the bone changes around the teeth opposing implant restoration installed mandibular posterior area. Material and method: Eight patients who had mandibular posterior edentulous area were treated with implants. Radiographs of the opposing teeth were taken at implant prostheses delivery(baseline), 3 months, and 6 months later. Customized film holding device was fabricated to standardize the projection geometry for serial radiographs of opposing teeth. Direct digital image was obtained. Gray values of region of interest at each digital image were measured and compared according to time lapse. Repeated measured analysis of variance and post-hoc Scheffe's test were performed at the 95% significance level. Results: Alveolar bone changes around the natural teeth opposing the posterior implant in mandible showed statistically significant difference compared to control group(P<0.05). And gray values of alveolar bone around the teeth opposing implants were increased. There were no statistically significant differences of alveolar bone changes between crestal group and middle group and between mesial group and distal group according to time lapse(P>0.05). There were no statistically significant differences of alveolar bone changes among mesial-crestal group, mesial-middle group, distal-crestal group, distal-middle group, and control group(P>0.05). Conclusion: Alveolar bone around the natural teeth opposing the implant prosthesis showed gradual bony apposition.

수정된 건측 상지 운동 제한 치료가 편마비 아동의 손 기능 향상에 미치는 효과 (The Effects of Modified Constraint Induced Therapy on Upper Extremity Functions of Children With Hemiparesis)

  • The purpose of this study was to investigate the effect of Modified Constraint-Induced Therapy (MCIT) on the effected upper extremity of children with hemiparesis. Four children with hemiparetic upper extremity caused by brain injuries were trained by MCIT for ten weeks. During the same period, all of the subjects were also involved in thirty-minute regular physical therapy and occupational therapy. During the treatment period, the unaffected upper extremities of the subjects were restrained by a specially designed hand splint or a mitten for five hours a day, five days per week. For two hours out of the five-hour restraint period, the affected upper extremities were intensively trained by performing various functional tasks, which were individually structured to emphasize use of the affected arm. A single-subject design with A-B-A reversal was employed in this study. The affected limb motor ability was evaluated by Melbourne Assessment, measuring the time to grasp and release nine pegs, and measuring grasping power. As a consequence of this study, the affected limb motor test scores of all four subjects in the baseline period were improved during the treatment period. Furthermore, the treatment effect was maintained during a one-month follow-up period. The results of this study support the assumption that MCIT is an effective therapeutic method to improve the sensory and motor abilities of hemiparetic children. It also increases the frequency of functional use of the hemiparetic hands of brain-injured children. Based on the results of this study, it can also be assumed that the modified CIT method is especially beneficial to these children by reducing the negative emotional effects of forceful restraint of the unaffected upper extremity. To optimize the functional recovery of the paretic upper extremity by CIT, the restriction period per day should be decided individually, according to the characteristics of the individual.

발병 3~6개월의 뇌졸중 환자에서 부가적 운동치료가 하지의 기능수행능력에 미치는 영향 (Impact of Additional Therapeutic Exercises on Functional Performance of the Lower Extremities in Stroke Inpatients Within 3 to 6 Months After Stroke Onset)

  • The purposes of this study were to determine the effects of rehabilitation programs on functional performance of the lower extremities and whether additional therapeutic exercise with routine rehabilitative therapy improves functional performance more than just routine rehabilitative therapy by it self for inpatients who have suffered a stroke with below moderate severity within 3 to 6 months after the onset of the stroke. Fifty-eight subjects were divided into two groups. Group I was given routine rehabilitative therapy and group II was given additional therapeutic exercise along with the routine rehabilitative therapy. Each group received 6 weeks of rehabilitation. The timed get-up and go test (TUG), the Fugl-Meyer score (FMS), functional independence measure (FIM), functional reach (FR), gait velocity (GV), and the strength of knee extensor and flexor were selected to measure effect of rehabilitation programs. The main results were measured and analysed at baseline, 3 weeks, and 6 weeks after the start of the rehabilitation programs. The results revealed that all of functional performance of the lower extremities in 3 weeks after the start of the rehabilitation programs were significantly improved compared with before the rehabilitation programs in both groups. In 6 weeks, TUG, FIM, FR, GV, and the strength of knee extensor in group I, TUG, FMS, FIM, FR, GV, and the strength of knee extensor and flexor in group II were significantly improved compared with the results after 3 weeks. At 3 weeks after rehabilitation programs, group II made significantly greater gains in TUG, FR, GV, and the strength of the knee flexor compared to the group I. At 6 weeks, group II made significantly greater gains in TUG, FR, GV, and the strength of the knee extensor and flexor compared to the group I. In conclusion, rehabilitation programs for stroke patients within 3 to 6 months after stroke onset significantly contributed to improve functional performance of the lower extremities. It is desirable for improvement of functional performance of the lower extremities to perform additional exercise with routine rehabilitative therapy.

지역사회 대상의 대사증후군 자가관리 프로그램 중재 효과 - 서울특별시 대사증후군 관리 시범사업 - (Effects of a Self-management Program for Metabolic Syndrome - A Metabolic Syndrome Management Program in Seoul -)

  • Objectives: The aim of the study was to identify the effects of a community-level metabolic syndrome self-care management program. Methods: Data included baseline demographics, abdominal circumference, blood pressure, blood sugar, triglyceride, and HDL-cholesterol collected at 15 public health centers in Seoul from January to March, 2010. A data obtained(584 men and 410 women) from Metabolic Syndrome Management Program in Seoul was analyzed using Paired t-test and Linear mixed model. Results: The group with 6-month intervention resulted in significant decrease in abdominal circumference, systolic and diastolic blood pressure, blood sugar, and triglyceride. In moderate intervention group with 3-month follow-up showed tendency that abdominal circumference(-1.27(${\beta}$)) and diastolic blood pressure(-1.66(${\beta}$)) are significantly reduced. In the group with aggressive intervention, the more counseling led to significant reduction in abdominal circumference(-0.22(${\beta}$)). Conclusions: These results indicate that a self-care for metabolic syndrome management can improve metabolic syndrome status over a short-term period. Further structured research is needed to develop a systematic program on health behaviors focusing on exercise, diet, and intervention.

단일 엔코더 및 디코더를 이용하는 다시점 비디오 시스템 (Multi-View Video System using Single Encoder and Decoder)

  • 인터넷을 통한 데이터 전송 기술의 발전은 다양한 실감 콘텐츠를 확산시키는 역할을 하고 있고, 이러한 실감 콘텐츠의 하나로 다시점 비디오가 연구되고 있다. 다시점 비디오는 여러 대의 카메라로부터 획득한 영상을 사용자에게 전달해 원하는 시점에서 영상을 시청할 수 있도록 하는 차세대 영상이다. 다시점 영상을 공급하기 위해서는 일반적으로 카메라 개수와 같은 개수의 엔코더와 디코더가 필요하며, 또한 상대적으로 큰 전송 대역폭을 필요로 한다. 본 논문에서는 상기 문제점을 해결하기 위해서 단일 엔코더와 디코더만을 이용하는 다시점 비디오 시스템을 제안한다. 부호화에서는 다시점 YUV 비디오를 GOP 단위로 믹싱하여 l 개의 YUV 스트립을 만든 후에, 단일 H.264/ AVC 엔코더로 부호화하여 압축 비트열을 생성한다. 또한 복호화도 단일 디코더 및 이를 제어하는 스케줄러로 구성된다. 스케줄러는 디코더 이용률과 프레임 스킵을 활용하여 각 시점 영상들이 동등한 복호화 프레임 개수를 가지게 한다. 또한 프레임 스킵에서는 비용 함수를 이용하여 H.264/AVC 프로화일에 따른 효율적인 프레임 선택 방법을 제안한다. 다양한 메인 및 베이스라인 프로화일 다시점 영상들을 활용하여 제안 방법의 성능을 검증하였다. 실험 결과는 각 시점 영상이 동등한 디코더 이용률을 얻는 것을 보여준다. 또한 rate-distortion 커브를 이용하여 제안한 방식과 동시 방식의 성능을 검증하였다.

인플루엔자 A 및 폐렴미코플라스마 감염과 병발한 가와사끼병 1례 (Kawasaki Disease with Influenza A Virus and Mycoplasma pneumoniae Infections: A Case Report and Review of Literature)

  • 그 동안 다양한 병원체와 가와사끼병의 관련성이 제기되어 왔으나 아직 확실한 원인으로 증명된 것은 없다. 본 증례는 가와사끼병과 폐렴미코플라스마, 인플루엔자 감염이 동시에 병발한 환자를 소개한다. 27개월 남아가 발열과 기침, 콧물 등의 증상으로 내원하였다. 외래에서 인플루엔자 A 감염을 확인하고 oseltamivir를 복용하였으나 발열이 지속되고 경부 림프절 비대, 양측성 결막 충혈, 입술의 발적과 갈라짐, 딸기혀, BCG 접종 부위의 발진을 보였다. 이에 가와사끼병으로 진단하고 면역글로불린을 정맥주사 하였다. 환아는 혈청 항미코플라스마 IgM 항체가 양성이었고 비인두 도말 중합효소 연쇄반응 검사에서 폐렴미코플라스마 양성으로 나타났다. 본 증례와 더불어 가와사끼병과의 연관성이 거론되었던 병원체들을 살펴보고 가와사끼병의 병인에 대한 가설들을 고찰하여 가와사끼병의 증상이나 관상동맥 병변이 폐렴미코플라스마와 인플루엔자 뿐 아니라 다양한 감염에서 발생할 수 있을 것으로 예상하였다.

과학기술 하부구조 선진화를 위한 대형 연구장비의 수요 조사 (Demand Surveys for Big Research Facilities and Equipments to Advance National S&T Research Infrastructure)

  • This paper deals with demand surveys for big science and technology research facilities and equipments to advance national S'||'&'||'T research infrastructure. We perform surveys thrice based on applied Delphi method on the future demand of big S'||'&'||'T research facilities and equipments among Korean scientists and engineers. We employ the concept of big S'||'&'||'T research facilities and equipments as follows: \circled1 The operating size of it is equivalent to that of an institute or research center, and/or \circled2 The users in various disciplines are many, and/or \circled3 The application areas or spill-over effects are large, and/or \circled4 The scale and scope of research objects is equivalent to that of mega science area such as earth.oceanography.space, and/or \circled5 The expenses for installing and operating it are to be supported by government, and/or \circled5 The facilities are expected as necessary for international joint research, and/or \circled7 It is necessary for promoting creative basic science and developing creative technology. We ask the respondents to answer the following questionnaire: - How to prioritize the equipments according to the degree of importance\ulcorner $\square$ Promotion of basic science and mega science, the development of the technologies to enhance the public welfare, the competitiveness of industrial technologies, the job creation for the S'||'&'||'T personnel, and international cooperation. - Who should be in charge of acquisition and operation of the equipments\ulcorner $\square$ Industry, Government Research Institutes, Academy, ERC and SRC. - When shall we acquire the equipment\ulcorner $\square$ Within 2000, 2002, 2007, 2012, and 2017. - How shall we acquire the equipments\ulcorner $\square$ International Joint Development, Domestic Development, Acquisition from Overseas, - How much will the equipment generate spill-over effects to national competitiveness\ulcorner $\square$ Promotion of basic science, contribution to the economy, supply of S'||'&'||'T personnel, and international cooperation. We suggest the following equipments as prioritized candidates after consulting the officers from MOST, MOE, MIC, MOEN and experts from KBSI and STEPI:(table omitted) where, #1, Korea Advanced Liquid Metal Reactor, #2. 800 MHz Superconduction Fourier-Transform Nuclear Magnetic Resonance Spectrometer, #3. 전침이 복부비만 성인의 비만관련 지표에 미치는 영향-무작위배정 단일 맹검 예비연구- (Effects of Electroacupuncture on Parameters Related to Obesity in Adults with Abdominal Obesity:Three arm Randomized Single Blind Pilot Study)

  • Objectives : To investigate the effects of electroacupuncture on parameters related to obesity in adults with abdominal obesity. Methods : A three arm randomized single blind pilot study was conducted from Jan 4 to March 25, 2010 in Kyung Hee Oriental Medical Hospital. The subjects were 39 adults with abdominal obesity and were randomly divided by computer generated random table into 3 groups; EA(electroacupuncture), sham EA(sham electroacupuncture) and waitlist groups. Acupuncture points located at abdomen($CV_{12}$, $CV_6$, $ST_{25}$, $SP_{15}$, $SP_{14}$) and extremities($LI_4$, $LI_{11}$, $ST_{36}$, $ST_{44}$) were inserted by disposable stainless steel needles and were stimulated 30 minutes with 24Hz, 0.27~1.3mA(tolerable strength), asymmetric biphasic continuous pulse wave form by STN-111 Stratek device in EA group. Two treatment sessions per week for 5 weeks(10 sessions in total) were done in EA and sham EA groups. The primary outcome measurement was WC(waist circumference), and the secondary outcome measurements included WHR(waist hip ratio), ASF(thickness of abdominal subcutaneous fat), and inbody measurements of BW(body weight), BMI(body mass index), BFR(body fat ratio) and VFA(visceral fat area), and also scores of BULIT-R(bulimia test revised), KoQoL(Korean obesity of QoL) and BSQ(body shape questionnaire). Results : All of 39 subjects were included in ITT(intention-to-treat) analysis. There were significant reductions in WC, WHR and ASF after 5-week electroacupuncture treatments and the percentage reductions were significantly greater than sham EA or waitlist group. There were no significant differences between groups in percentage reductions of other parameters(BW, BMI, BFR, VFA, BULIT-R, KoQoL and BSQ). But, there were continuous reductions in BW, BMI, BFR and VFA at 3 weeks after the end of treatment and there was significant reduction in BW compared with the baseline value in EA group. No seriously adverse effects were reported during the period. Conclusions : Electroacupuncture was more effective than sham electroacupuncture or no intervention on the reduction of WC, WHR and ASF in adults with abdominal obesity.

화물자동차 운전자의 운행시간에 따른 사고위험도 분석 (Analysis of Motor Carrier Crash Risk with Driver Hours of Service)

  • 대형 화물자동차의 사고는 일반적인 사고와 달리 심각한 인명피해 등을 동반할 확률이 높다. 장거리 운행이 많은 화물자동차 운전특징으로 인해 운전자의 피로 및 피로로 인한 사고가 높다. 이러한 피로와 관련된 사고를 감소시키기 위해서 외국에서는 운전자의 피로에 영향을 주는 중요변수인 운전시간, 수면시간, 운행형태 등을 고려하여 운전자의 운행시간(Hour-of-Service:HOS)을 법으로 규정하고 있다. 운송업체 또한 운전자들의 사고를 감소시키기 위해 법정 운전자 근무시간 준수를 위해 노력하고 있는 실정이다. 그러나 우리나라는 화물자동차 운전자의 피로와 관련 사고율을 줄일 수 있는 기초적인 법정 운전자 운행시간 조차 제시되지 않고 있다. 본 연구는 우리나라 화물자동차 사고사망자수를 감소시킬 수 있는 운전자의 근로기준법 제정 필요성을 제시하는데 목적이 있다. 이를 위해서 현재 운영 중인 선진외국의 화물자동차 운전자 운전시간 및 운행규정을 살펴보고 실증적 자료를 통해 운전시간에 따른 사고 위험도의 차이를 살펴보았다. 실증적 자료는 미국의 화물자동차 3곳의 사고 자료 231명의 운행일지와 사고가 나지 않은 자료 462명의 운행일지를 수집, 총 693명 운전자의 운행일지를 수집하였다. 운전자의 연속된 운행에 대한 특징을 반영할 수 있도록 time-dependent 로지스틱 회귀모형을 사용하였다. 분석결ㄹ과 운행시간 1시간부터 3시간까지 운전한 운전자 사고위험도 차이는 없는 것으로 나타났다. 운행시간이 10시간인 운전자는 운행시간 1시간인 운전자보다 사고위험도가 약 2.2배 높은 것으로 분석되었다. 결론적으로 운행시간이 증가할수록 사고위험도 또한 증가한다는 것을 알 수 있다. 또한 본 연구는 우리나라의 지역적 특성과 운전자 특성에 맞는 운전자 운전시간 설정을 위한 연구방향 및 향후 연구과제에 대하여 연급하였다.
