Efficacy of Low Dose Barbiturate Coma Therapy for the Patients with Intractable Intracranial Hypertension Using the $Bispectral^{TM}$ Index Monitoring

  • Objective : Barbiturate coma therapy (BCT) is a useful method to control increased intracranial pressure (IICP) patients. However, the complications such as hypotension and hypokalemia have caused conditions that stopped BCT early. The complications of low dose BCT with $Bispectral^{TM}$ index (BIS) monitoring and those of high dose BCT without BIS monitoring have been compared to evaluate the efficacy of low dose BCT with BIS monitoring. Methods : We analyzed 39 patients with high dose BCT group (21 patients) and low dose BCT group (18 patients). Because BIS value of 40-60 is general anesthesia score, we have adjusted the target dose of thiopental to maintain the BIS score of 40-60. Therefore, dose of thiopental was kept 1.3 to 2.6 mg/kg/hour during low dose BCT. However, high dose BCT consisted of 5 mg/kg/hour without BIS monitoing. Results : The protocol of BCT was successful in 72.2% and 38.1% of low dose and high dose BCT groups, respectively. The complications such as QT prolongation, hypotension and cardiac arrest have caused conditions that stopped BCT early. Hypokalemia showed the highest incidence rate in complications of both BCT. The descent in potassium level were $0.63{\pm}0.26$ in low dose group, and $1.31{\pm}0.48$ in high dose group. The treatment durations were $4.89{\pm}1.68$ days and $3.38{\pm}1.24$ days in low dose BCT and high dose BCT, respectively. Conclusion : It was proved that low dose BCT showed less severe complications than high dose BCT. Low dose BCT with BIS monitoring provided enough duration of BCT possible to control ICP.

  • Jukyeo (竹茹; Bambusae Caulis in Taeniam: BCT) is an herbal medicine made from the inner part of the bamboo stem of Phyllostachys nigra Munro var. henonis Stapf ex Rendle or Phyllostachys bambusoides Siebold et Zuccarini. Although medical literature published in China and Korea decades ago introduced BCT as a remedy for reducing vomiting, fever, and hematemesis, decoctions containing BCT as a main herb (君藥) in Korea have been approved only for treating neurologic symptoms. Here, we discuss the anti-inflammatory function of BCT. Combined with the clinical usage of a BCT-containing decoction in treating inflammatory diseases in Japan, we raise the possibility of repurposing the BCT-containing decoctions for treating inflammatory diseases. The anti-inflammatory activity of BCT was mainly assessed by using RAW 264.7 cells. The regulation of NF-κB, Nrf2 and A20 activities was determined by western blot and quantitative RT-PCR. The list of decoctions containing BCT currently approved in Korea was obtained from the Korean Ministry of Food and Drug Safety (KFDA). BCT suppressed the activity of pro-inflammatory factor NF-κB elicited by LPS, activated an anti-inflammatory factor Nrf2, and induced A20 that is known to block several inflammatory pathways simultaneously, suggesting that BCT can suppress inflammation via at least 3 different pathways. KFDA approved 11 decoctions containing BCT as a major herb, including Gamiondam-tang (加味溫膽湯; GOT), for treating neurologic disorders. Interestingly, Jukyeoondam-tang (竹茹溫膽湯; JOT), whose composition is almost identical to GOT except for one herb, has been used to treat inflammatory pulmonary disorders including Covid-19 pulmonary infection in Japan Given the anti-inflammatory function of BCT evidenced by medical literature and experimental results and the clinical usage of JOT in treating inflammatory pulmonary disorders, we suggest a repurposing and extension of the BCT-containing decoctions approved in Korea to treating inflammatory diseases.

  • 본 논문에서는 many-to-many IP 멀티캐스팅을 위한 효율적인 BCT(Backbone Core Tree)생성 알고리즘의 시뮬레이션 방법에 대하여 제안한다. BCT는 기법은 CBT(Core Based Tree)에 기반을 두고 있다. CBT는 공유 트리를 이용하여 멀티캐스트 자료를 전달하기 때문에 Source based Tree에 비하여 각 라우터가 유지해야 하는 상태 정보의 양에 적고, 적용하기 간단하지만, Core 라우터 선택의 어려움과 트래픽이 Core로 집중되는 문제점을 가지고 있다. 이에 대한 보완책으로 BCT기법이 제안되었는데, 본 논문에서는 주어진 네트워크 위상 그래프에서 최소신장 트리를 만들고, 센트로이드(Centroid)를 이용하여 효율적인 BCT를 생성하는 알고리즘을 제안하고 시뮬레이션 방법을 제시한다.

The effects of Banhabaekchulchunma-Tang(BCT) on Dementia induced by focal brain ischemic injury in rats (반하백출천마탕(半夏白朮天麻湯)이 국소(局所) 뇌허혈(腦虛血)로 인(因)한 치매 병태 백서(白鼠)에 미치는 영향(影響))

  • Objective : This study was to investigate the effects of Banhabaekchulchunma-Tang(BCT) on Dementia induced by focal ischemic injury in the rats. Method : Experimental groups were divided into 4 groups ; Normal group, Control group, BCT1 group and BCT2 group. Control group were no treated after focal brain ischemic injury. BCT1 group were administered BCT 0.3 ml/kg to focal brain ischemic injuried rats for 21 days, BCT2 group were administered BCT 1.2 ml/kg to focal brain ischemic injuried rats for 21 days. The present author observed the number of errors on the eight-arm radial maze task , the rate of correct choice on the eight-arm radial maze task, the values of density of Cresyl Violet-stained sections in the hippocampal CA1 and the values of density of Acetlycholin Esterase(AchE) stained nuclei in the hippocampal CA1. Result : 1. The number of errors in the eight-arm radial maze task was significantly decreased in BCT1 group on 1-6th days. And it was significantly decreased in BCT2 group on 1st, 2nd, 3rd and 6th day compared with control group. 2. The rate of correct choice on the eight-arm radial maze task was increased in BCT1 group and BCT2 group compared with control group, but the value was not significant. 3. The values of density of Cresyl Violet-stained sections in the hippocampal CA1 were significantly increased in BCT1 group and BCT2 group compared with control group. 4. The values of density of AchE stained nuclei in the hippocampal CA1 were significantly increased in BCT1 group compared with control group. Conclusion : The present author thought that Banhabaekchulchunma-Tang could be used to cure dementia derived by the phlegm retention of fluid.

Enhanced Vasorelaxation of Banhabackchulchunma-Tang and Involved Mechanism (반하백출천마탕(半夏白朮天麻湯)의 조성에 따른 혈관이완활성과 기전)

  • This study was designed to potentiate the vasodilation effect of Banhabackchulchunma-Tang(BCT) prescription by change of mixture. Six different BCT compositions were made according to mixture of herbs. The vascular relaxation effects of 6 different BCT compositions were examined on phenylephrine(PE)-precontracted rat thoracic aorta. The BCT-1 composition exerted significant relaxation on phenylephrine- or KCI- contracted rat thoracic aorta. Its elaxation was endothelium- independent in both PE- and KCl-induced contraction. Treatment of glibenclamide or tetraethylammonium(TEA) did not affect the relaxation of BCT-1. Vasorelaxation efficacy of BCT-1 was also not influenced by low (25mM) or high (50mM, 80mM) KCl-induced contraction. Furthermore, the contraction by increasing $Ca^{2+}$ concentrations (0.3-10.0mM) to a $Ca^{2+}$-free high K+ (60mM) was significantly reduced by pretreatment with BCT-1 In addition, the relaxant effects were not inhibited by pretreatment of rat aorta with L-NAME, MB, indomethacin and atropine. These results confirm that BCT-1 may exerts its vasodilation effect by endothelium-independent manner. According to the above results, we suggest that the relaxation effect of BCT-1 is endothelium-independent and is related with block of $Ca^{2+}$ influx via $Ca^{2+}$ channel.

Evaluation of Eligibility and Utilization of Breast Conservation Treatment in an Asian Context

  • Background: Breast conservation treatment (BCT) has long been recognised to provide survival outcomes equivalent to mastectomy for the treatment of breast cancer. However, published reports of BCT rates in Asian communities are lower than those from Western countries. This study sought to investigate the eligibility and utilisation of BCT in a predominantly Asian population. Materials and Methods: All patients treated surgically by a single surgeon at a private medical facility between 2009 and 2011 were included in the study. Patients were deemed to have successful BCT if they underwent breast conserving surgery with pathologic clear margins and completed all recommended adjuvant treatment. Those who did not complete adjuvant treatment were excluded from the analysis. Results: Data from a total of 161 patients who underwent treatment during the study period were analysed. The mean age was 48.8 years. One hundred and six patients (65.8%) were of Chinese ethnicity, 12 were Indian (7.5%), 11 were Malay (6.8%), 18 were Caucasian (11.2%) and 14 (8.7%) were of other Asian ethnicity. One hundred and thirty-eight women (85.7%) underwent BCT. Of the 23 (14.3%) who underwent mastectomy, 8 (5.4%) elected to undergo a mastectomy despite being eligible for BCT. In total, it was assessed that 146 of 161 patients (90.7%) were eligible for BCT and utilisation was 94.5%. Conclusions: In this study, eligibility, utilisation of BCT and eventual successful breast conservation rates are similar to published rates in Western communities. Additional research is needed to investigate the reasons for the lower published BCT rates in Asian countries and determine ways to improve them.

The Effect of Barbiturate Coma Therapy for the Patients with Severe Intracranial Hypertension : A 10-Year Experience

  • Objective : Barbiturate coma therapy (BCT) has been known to be an useful method to control increased intracranial pressure (IICP) refractory to medical and surgical treatments. We have used BCT for patients with severe IICP during the past 10 years, and analyzed our results with review of literatures. Methods : We analyzed 92 semicomatose or comatose patients with Glasgow coma scale (GCS) of 7 or less with severe IICP due to cerebral edema secondary to parenchymal damages irrespective of their causes. Forty patients who had received BCT with ICP monitoring from January 1997 to December 2006 were included in BCT group, and fifty-two patients who had been managed without BCT from January 1991 to December 1995 were divided into control group. We compared outcomes with Glasgow outcome scale (GOS) and survival rate between the two groups. Results : Good outcome (GOS=4 and 5) rates at 3-month after insult were 27.5% and 5.8% in BCT and control group, respectively (p<0.01). One-year survival rates were 35.9% and 12.5% in BCT and control group, respectively (p<0.01). In BCT group, the mean age of good outcome patients ($37.1{\pm}14.9$) was significantly lower than that of poor outcome patients ($48.1{\pm}13.5$) (p<0.05). Conclusion : With our 10-year experience, we suggest that BCT is an effective treatment method for severe IICP patients for better survival and GOS, especially for younger patients.

An Analytical Validation of the GenesWellTM BCT Multigene Prognostic Test in Patients with Early Breast Cancer (조기 유방암 환자를 위한 다지표 예후 예측 검사 GenesWellTM BCT의 분석적 성능 시험)

  • GenesWell$^{TM}$ BCT is a 12-gene test suggesting the prognostic risk score (BCT Score) for distant metastasis within the first 10 years in early breast cancer patients with hormone receptor-positive, HER2-negative, and pN0~1 tumors. In this study, we validated the analytical performance of GenesWell$^{TM}$ BCT. Gene expression values were measured by a one-step, real-time qPCR, using RNA extracted from FFPE specimens of early breast cancer patients. Limit of Blank, Limit of Detection, and dynamic range for each of the 12 genes were assessed by serially diluted RNA pools. The analytical precision and specificity were evaluated by three different RNA samples representing low risk group, high risk group, and near-cutoff group in accordance with their BCT Scores. GenesWell$^{TM}$ BCT could detect gene expression of each of the 12 genes from less than $1ng/{\mu}L$ of RNA. Repeatability and reproducibility across multiple testing sites resulted in 100% and 98.3% consistencies of risk classification, respectively. Moreover, it was confirmed that the potential interference substances does not affect the risk classification of the test. The findings demonstrate that GenesWell$^{TM}$ BCT have high analytical performance with over 95% consistency for risk classification.

(A Centroid-based Backbone Core Tree Generation Algorithm for IP Multicasting) (IP 멀티캐스팅을 위한 센트로이드 기반의 백본코아트리 생성 알고리즘)

  • In this paper, we propose the Centroid-based Backbone Core Tree(CBCT) generation algorithm for the shared tree-based IP multicasting. The proposed algorithm is based on the Core Based Tree(CBT) protocol. Despite the advantages over the source-based trees in terms of scalability, the CBT protocol still has the following limitations; first, the optimal core router selection is very difficult, and second, the multicast traffic is concentrated near a core router. The Backbone Core Tree(BCT) protocol, as an extension of the CBT protocol has been proposed to overcome these limitations of the CBT Instead of selecting a specific core router for each multicast group, the BCT protocol forms a backbone network of candidate core routers which cooperate with one another to make multicast trees. However, the BCT protocol has not mentioned the way of selecting candidate core routers and how to connect them. The proposed CBCT generation algorithm employs the concepts of the minimum spanning tree and the centroid. For the performance evaluation of the proposed algorithm, we showed the performance comparison results for both of the CBT and CBCT protocols.

Characterization and Antioxidant Activity of Gold Nanoparticles Synthesized using Bambusae Caulis in Taeniam extract (죽여 추출물로 합성한 금 나노 입자의 특성과 항산화 활성)

  • Green synthesis of gold nanoparticles(GNPs) considered more ecofriendly and cost effective than other chemical methods use of dangerous reagents and solvents, improved material and energy efficiency and enhanced design of non-toxic products. In this study, we developed a green synthesis method for using Caulis in Taeniam (BCT). BCT were characterized by UV-vis, Zetasizer, TEM, XRD, and FTIR. The antioxidant activity of BCT was determined by DPPH and ABTS radical-scavenging assays, and heme oxygenase-1 induction in RAW 264.7 macrophages. The resulting BCT appeared spherical with an average diameter of 67.171.39 nm The aAntioxidant activity was increased in a dependent manner. To conclude, the green synthesis of BCT-GNPs was successful, and it appers to be useful in the for future applications.
