Yin Yang theory is the first and the last one to interpret diseases apply to treatment in oriental medicine. So it is regarded as the way of heaven and earth, the discipline of all things, the origin of change, the beginning of giving birth and death, the source of spirit. These regulatory passages are needed to compare with western medical physiology and analyzed whether it has scientific bases or not. The Yin Yang theory of traditional oriental medicine are summerized from the descriptions of Scripture of Documents, Ying Wei theory of Huangdi Neijing. Meanwhile up-to-date neurobiological and molecular genetic theories on circadian physiology are reviewed in western medicine. Sunshine is transferred through RGC to SCN, herein the central circadian rhythm is made by zeitgeber and interaction of melatonin secreted from pineal body and orexin from mp pan of hypothalamus. So HPA axis is activated and controlled under the circadian rhythm and affects peripheral tissues and cells of whole body through glucocorticoid hormones. First of all, the circadian rhythm makes the basic patterns of human life in biological and sociological meaning. It is almost same context with the record of the Scripture of Documents. Also the Ying Qi and Wei Qi is basically same with the sleep-awake mechanism, that is melatonin/orexin signal or per/clk/bmal/cry genes with circadian activation and activity of each organ's physiological function. Conclusively it can be said that Yin Yang is a priori principle of living things and the beginning of giving birth and death for activation of them biologically, as is described in Huangdi Neijing.

Background: We analyzed the results of surgical reduction and fixation of ribs under thoracic epidural anesthesia and analgesia (TEA) in patients who had no more than 3 consecutive rib fractures with severe displacement to examine the clinical usefulness of this method. Materials and Methods: From May 2008 to March 2010, 35 patients underwent surgical reduction and fixation of ribs under TEA. We reviewed the indications for this technique, number of fixed ribs, combined surgical procedures for thoracic trauma, intraoperative cardiopulmonary events, postoperative complications, reestablishment of enteral nutrition, and ambulation. Results: The indications of TEA were malunion or nonunion of fractured ribs in 29 (82.9%; first operation) and incompletely ribs under previous general anesthesia in 6 (17.1%; second operation). The average number of fixed ribs per patient was 1.7 (range: 1~3). As a combined operation for thoracic trauma, 17 patients (48.6%) underwent removal of intrathoracic hematomas, and we performed repair of lung parenchyma (2), wedge resection of lung (1) for accompanying lung injury and pericardiostomy (1) for delayed hemopericardium. No patient had any intraoperative cardiopulmonary event nor did any need to switch to general anesthesia. We experienced 3 postoperative complications (8.6%): 2 extrapleural hematomas that spontaneously resolved without treatment and 1 wound infection treated with secondary closure of the wound. All patients reestablished oral feeding immediately after awakening and resumed walking ambulation the day after operation. Conclusion: Thoracic epidural anesthesia and analgesia (TEA) may positively affect cardiopulmonary function in the perioperative period. Moreover, this technique leads to an earlier return of gastrointestinal function and early ambulation without severe postoperative complications, resulting in a shortened hospital stay and lowered costs.

This study investigates the effect of a combination of homeopathic medicine and electro- acupuncture in two patients with breast cancer and severe liver disease who could not receive standard anaesthesia therapy due to liver problems. Specifically, measurable and quantifiable parameters were used to evaluate whether an integrated approach-consisting of electro- acupuncture and a homeopathic medicine diluted above Avogadro's limit (that is, above a potency of 12CH) during the pre-surgical, surgical and post-surgical phases -can improve general well-being of a patient undergoing breast cancer surgery. In breast cancer surgery, we employed an integrated approach consisting of induction with hypnotics and muscle relaxants, followed by maintenance with anaesthetic gas, combined with a homeopathic treatment (Arnica montana 15CH and Apis mellifica 15CH) before and after surgery and an electro- acupuncture treatment performed in the pre- and post-surgical phases without any analgesic/pain relieving medications. Both of the patients treated with the integrated approach improved their overall condition without need for other common pain relieving medicines. Additionally, thanks to their rapid awakening, the patients were not relocated to a protected area and the hospitalization was shorter. A multidisciplinary approach incorporating homeopathic medicine and electro-acupuncture can be a solution for patients who need or ask about a different and/or safer alternative to the standard treatment. This approach can offer a safe, much less expensive, non-invasive and viable alternative for such cases. Moreover it can be useful for an opioids free anesthesia.

The purpose of this study is to apply a concept of open structure to clothing design and to verify the characteristics found in the various types of clothing which has open structure. The literatures from various academic fields including philosophy, literature, social science, architecture, and fine arts are investigated to define the concept of openness and to analyze it from the perspectives both of the visual and of the moaning of openness. This paper is to identify the types and the characteristics of clothing by future intention, complexity, discontinuity of open structure. By closely examining fashion design after 1980s found in fashion collection publications and designer's websites, the results of this study are as follows: first, the concept of openness can be classified into two different levels, that is, visual and meaning, secondly, in clothing the concept of open structure is applied to the meaning side by future intention, by complexity and by discontinuity. Open structure through future Intention has new content and interpretation and must have the possibility of intelligence awakening, future guidance and basic contents. Open structure through complexity has secondary function exists concurrent with the shape key example is the smart clothes with the digital functions. It has functions of amusement, supplement and protective, and is future clothes which satisfies with health, welfare, desire of beauty. Open structure with discontinuity is clothes with dramatic changes in system, structures and states. Structure can be changed by silhouette, detail, or fabric, material, or dramatic and practical function as tools in terms of productions and environment. This study can help to formulate and to integrate the concept of open structure in clothing with various phases and enhance the value of clothes by showing an application of the concept of openness to the clothing in meaning level.
Objectives: We evaluated the effect of the traditional Korean medical therapy of catgut-embedding therapy in patients with carpal tunnel syndrome (CTS). Methods: Thirteen patients(20 hands) with CTS underwent catgut-embedding therapy once per week for a total of 1-4 sessions. We measured the time to tingling in the fingers on performing Phalen's test. We also had patients use a visual analog scale (VAS) to indicate the severity of tingling. We also asked about the awakening time at night due to pain and tingling. We controlled the treatment as being limited to four sessions. Even if a patient had not been treated for four sessions, we ended the patient's treatment if symptoms resolved. Results: Patients who received catgut-embedding therapy showed significant improvements in the clinical symptoms of CTS. After treatment, in a total of 19 hands, the status of fourteen was changed from positive to negative in Phalen's test and the average of tingling occurrence time in that test was delayed. Among the 20 hands, the VAS in eight hands changed to zero and the average of VAS score decreased. Furthermore, in nine patients, four could sleep well without the use of a splint. Conclusions: Patients undergoing this traditional Korean medical therapy showed alleviation of finger tingling after even a single treatment. We thus concluded that this traditional Korean catgut-embedding therapy, may be effective for treating CTS. However, to confirm the effects of catgut-embedding therapy on CTS, further tests are required, including under the defined, strict conditions of a randomized controlled trial.
Objective : Assuming that the characteristic of meridian system has been similar to this of electric potentials in human body and that measurements of electric potential at well(井穴) and sea(合穴) points in branches of the twelve meridians will be representative of measurements of the twelve meridians, to measure the electric potentials in twenty aged and fifty aged healthy volunteers groups at sleeping(SG) and awakening(AG), and then to find out the characteristic of meridian system among 2 groups. Methods : We selected who thirty healthy volunteers were diagnosed by a blood test, urine examination and differentiation of syndromes by five viscera(五臟辨證) among volunteers. Their electric potential of well and sea points in the meridians were simultaneously measured by physiograph. Results : Measurements were analyzed by statistical factor analysis, we obtained that the both left and right side electric potential of well and sea points in branches of the twelves meridians were unclearly divided into four factors according to age and whether sleeping or not, which were the three Yin meridians of the hand, the three Yang channels of the hand, the three Yin meridians of the foot, and the three Yin meridians of the foot. Conclusion : In conclusion, using the sequently measuring method, we obtained that electrical potentials of well and sea points in branches of the twelves meridians were divided into two factors, but the simultaneously measuring method, those were divided into four factors. The latter result means that the electrical potentials of twelve meridians were reflected by the function of the viscera and bowels.
Sleep is a necessity for survival. Disruption of sleep leads to numerous adverse physiological and psychological consequences. These could be particularly undesirable for older patients, who are subject to many additional factors. But there is limited research related to hospitalized elderly in Korea. The purpose of the study is to explore sleep patterns and disturbing factors of before and after hospitalization, in order to present basic information regarding elderly sleep to develop nursing intervention. The sample consisted of 32 elderly men and women between the ages of n and 87 years. Data collection was done from September to November 1997. Measures of sleep patterns and related factors were obtained from self-reported sleep questionnaires. Analysis of data was done by use of t-test, paired t-test, ANOVA, and Pearson Correlation Coefficient. The results of this study were summarized as follows : 1. In comparision between before and after admission of their sleep pattern, “sleep onset” tends to be delayed and nocturnal sleep time was significantly reduced. So, hospitalized elderly reported less total sleep time than before admission. 2. Regarding the sleep disturbing factors, medication(hypnotics ; 37.5%), physiological factor (snoring ; 59.4%) environmental factor (pillow ; 78.1%), emotional factor(anxiety related to disease ; 37.5%), and illness factor(fatigue ; 34.7%) were reported. 3. Significant differences in gender were found. Men had more disturbances in sleep than women owing to difficulty in falling a sleep and lack of nocturnal sleep. Women consumed more sleep inducing drugs. Significant increase was reported in napping during the day with increasing age. 4. Significant differences between good sleepers and poor sleepers were found for the following variables : nocturnal sleep time, total sleep time, bed time, sleep onset latency time, sleep latency time after nocturnal awakening, time spent in bed upon arousal, environmental factors, and emotional factors. In conclusion, it was found that the quantity and quality of sleep were significantly altered in hospitalized elderly, but adequate strategies for better sleep were not practiced. Further research is needed to develop Intervention strategies to promote sleep and to prevent sleep problems.
This study aims to describe actual condition of residents' involvement of Sk$\"{a}$rk$\"{a}$ll ecovillage in Sweden by in-depth interviews with five residents. This study, hopefully, could contribute to offer useful information for forming ecovillage, as well as awakening about ecovillage movement to the public in Korea. The interviews were fulfilled during spring of 2010 in G$\"{o}$teborg and Sk$\"{a}$rk$\"{a}$ll ecovillage in Sweden. The results are as follows: 1) In terms of overall residents' involvement by self-evaluation everybody agrees with that residents' involvement was quite excellent during forming stage of the village, while it has been notably decreased now in running stage. It reveals there is a gap between ideology and reality. 2) Concerning decision making method, they use majority decision instead of consensus, since only 9 households of 20 are residing there all year around, it is hardly possible to assemble all the residents in the meeting. In Accordance with problematic conflicts in the village recently, development of socialization with others and education of communication skill for residents probably could be helpful to resolve conflicts among residents in long term. 3) In terms of evaluation of ecovillage movement in Sk$\"{a}$rk$\"{a}$ll ecovillage, it is the fact that many residents live less self-sufficiently, more energy consumed than the ideology pursuits. Also, they live with close connections with conventional society. Nevertheless, the efforts they are willing to practice 3R principles (Recycle, Reuse, Reduction) have to be highly respected, if we consider its educational influence to environmental movement for the conventional society.
