  • Antacid(AM, 600 mg/kg=aluminium hydroxide, magnesium hydroxide, and simethicone with a ratio of 1 : 1 : 0.1) and aceglutamide aluminium(AGA, 263 mg/kg)-Effect of the combined preparation containing on the gastric mucosal hexosamine, sialic acid, and aluminium contents adhered to the gastric wall of the rat was investigated. Severe ulcers were produced in rats by injecting of $30\;{\mu}l$ acetic acid(30%) into the subserosal layer of one position in the corpus. When given orally for 15 consecutive days, AM(1,200 mg/kg), AGA(525, 1,050 mg/kg), and the combined preparation significantly decreased the ulcer area. AGA(525, 1,050 mg/kg) and the combined preparation also increased the amount of hexosamine and sialic acid in the intact and ulcerated areas. On the other hand, the contents of hexosamine and sialic acid were not affected by AM (600, 1,200 mg/kg). The amount of aluminium adhered to the gastric wall of the rat was higher in the combined preparation when compared to the AM(600 mg/kg) and AGA(263 mg/kg). The aluminium contents adhered may play an important role protecting mucosa from aggresive action of gastric juice and potenting defensive factors through the increase of mucosa-forming components by AGA.

  • Effects of aluminum magnesium hydroxide (A) and cimetidine (C) on the pharmacokinetics of minocycline (M) were investigated in female rats. Blood samples were collected at various time intervals until 36 hrs following oral dosing of drugs. Plasma minocycline concentrations were determined by HPLC. Control group (M), $T_1$ group (M+A), $T_2$ group (A+M after 2 hrs), $T_3$ group (M+A after 2 hrs), $T_4$ group (M+C) and $T_5$ group (C+M after 2 hrs) were divided to examine interaction of the drugs with minocycline. Plasma minocyline level-time curves were well described by two-compartment open model with first-order absorption in rats. Antacid treatment was associated with reduced of 71.0, 45.9, 35.7% in minocycline absorption rate $constant(K_{\alpha})$, maximum plasma $concentration(C_{max})$, and relative $bioavailability(F_{rel})$, respectively. Cimetidine treatment group exhibited no significant changes in plasma level-time curve when compared with control group and did not affect minocycline absorption as by any of these three parameters.

  • The relationships involving acid-consuming capacity and X-ray powder diffraction patterns of aluminum hydroxide gel were studied. The aging of antacid efficacy of aluminum hydroxide gel has also been characterized by using X-ray powder diffraction. The acid-consuming capacity decreased with progress of crystallization from amorphous alumino gel, and it was found that aged product at 80.deg. C had crystal structure of bohmite (.alpha.-Al.sub 2/O$_{2}$(OH)$_{2}$). It was also noted that the rate of aging decreased with decreasing pH of aluminum hydroxide gel.

  • In healthy adults, diagnosis of aspiration of foreign body into tracheobronchial tree is not difficult because various symptoms such as dyspnea, coughing, or cyanosis develop when foreign body is aspirated into tracheobronchial tree. But unless a clear history of an aspiration event can be obtained, diagnosis will be delayed. Early complications of tracheobronchial foreign body aspiration include asphyxia, cardiac arrest, dyspnea, laryngeal edema, and cyanosis. Late complications include pneumonia, lung abscess, bronchiectasis, hemoptysis, bronchial stenosis, and polyp. Treatment is removal of foreign body by operation or bronchoscopy. Currently, flexible bronchoscopy is preferred in adults than rigid bronchoscopy. A 36-year-old male visited to Dongkang hospital due to productive coughing and dyspnea. On auscultation, focal inspiratory wheezing was heard. On chest PA, mild emphysematous change was seen Flexible bronchoscopy was done. Bronchoscopically, mucoid impaction, surrounding inflammation, foreign body lodged in the right lower lobe bronchus, and interbronchial fistula(between right middle and lower lobe bronchus) were seen Foreign body($2.4\{times}1.3cm$ sized antacid package) was removed by flexible bronchoscopy. Later, history of aspiration of a piece of antacid package was found. We report a case of recurrent bronchitis with interbronchial fistula as a result of occult aspiration of foreign body with review of the literatures.

  • The neutralizing capacities of the antacids, which are frequently used in Korean market, were evaluated in vitro by the methods of Resset and Rice, Fordtran and Collyns, and modified Beekman, respectively. The antacids used in the study are two kinds, the one is preparations from Seoul National University Hospital and the other is products from pharmaceutical companies, and their components are aluminum phosphate, aluminum hydroxide, magnesium aluminum hydroxide, magnesium hydroxide, basic aluminum sucrose sulfate and $2MgO{\times}Al_{2}O_{3}{\times}SiO_{3}$, etc. The hospital preparations, DMC and MAC powders, showed most powerful and sustained neutralizing capacities, i.e. they maintained the pH range from 5 to 8 for 60min, Whereas pharmaceutical products, aluminum hydroxide gel containing magnesium hydroxide and magnesium aluminum hydroxide gel exhibited a moderate capacities, i.e pH ranged from 3 to 6, and aluminum phosphate, $2MgO{\times}Al_{2}O_{3}{\times}SiO_{2}$ and basic aluminum sucrose sulfate displayed a weak activity, pH ranged from 2 to 3. When the therapeutic doses of aluminum hydroxide gel containing magnesium hydroxide and magnesium aluminum hydroxide gel were divided into 2 doses and each dose was used at the interval of 30min., the divided doses kept more prolonged higher pH than the single therapeutic dose. Milliequivalents of neutralizing capacities of each antacid were measured by the method of Fordtran and Collyns. The milliequivalents per 1ml of aluminum hydroxide gel, aluminum hydroxide gel containing magnesium hydroxide, magnesium aluminum hydroxide gel and aluminum phosphate were 2.87, 2.86, 2.57, and 0.67, respectively. The milliequivalents per 100mg of preparations, i.e. MAC powder, dried aluminum hydroxidgel, DMC powder, 2MgO, $Al_{2}O_{3}$. $SiO_{2}$, magnesium aluminum hydroxide and basic aluminum sucrose sulfate were 1.91, 1.68 1.63, 1.45, 1.44, and 0.44, respectively.

  • A score and more kinds of molluscan shells have been used as drugs in oriental medicine. The Korean Pharmacopoeia (K.P.) contains a monograph on oyster shell "Ostreae Testa" as an official drug. A huge deposit of the fossil shell Crassostrea gravitesta eoilensis has been recently excavated in the region of Wolsung, Kyong-Sang-Buk-Do. This study was conducted to determine the applicability of the fossil shell as a substiute for Ostreae Testa, K.P. The fossil shell not only met the K.P. requirements, but also exhibited an appropriate antacid activity. Amino acid survivals were also determined quantitatively with amino acid autoanalyzer.

  • Gastric hemangiomas are rare benign vascular tumors that can cause severe gastrointestinal system bleeding. We presented the case of a neonate with fresh bleeding and melena from the orogastric tube and detected gastric hemangioma in esophagogastroduodenoscopic examination. Propranolol is widely used in treatment of cutaneous hemangiomas and non-gastric gastrointestinal system hemangiomas. However, the surgical approach is preferred for treating gastric hemangiomas, and there are few reports of gastric hemangiomas associated with non-surgical treatment. Gastric hemorrhage decreased with antacid and somatostatin treatment. Propranolol treatment was initiated before the surgery decision. After three weeks of treatment, we observed regression in the hemangioma with endoscopic evaluation. During the course of treatment, the patient's gastrointestinal system bleeding did not recur, and there were no side effects associated with propranolol.

  • Purpose: The purpose of this study was to investigate the difference in the incidence of diarrhea among the subjects given hypertonic and isotonic nutrients to the nasogastric tube feeding patients in the a critical care setting. Methods: This study is aquasi-experimental study with a pre & post-test design. The sample size of 40 was calculated based on Cohen's formula (1988). The total of 40 subjects who signed the informed consent were randomly selected and divided evenly into two groups, experimental and control group. Results: There are no significant differences between the two groups in homogeneity test (sex, age, albumin level, the use of antibiotics, antacid, and $H_2$ blocker). However, the frequencies of occurrence in diarrhea according to the density of nutrients formulation indicated a statistical difference at the level of .005 (p=.001): diarrhea occurred in four of 20 (20%) of the experiment group, but 14 (70%) of the control group. Also the onset date of diarrhea in the experiment group is later than that of the control group. Conclusion: The research findings suggest that we should begin with low density nutrients for nasogastric tube feeding, and increase its density gradually to decrease diarrhea incidence in the critical care setting.
