Comparative evaluation of nasal and alveolar changes in complete unilateral cleft lip and palate patients using intraoral and extraoral nasoalveolar molding techniques: randomized controlled trial

  • Objectives: Cleft lip and palate is a common congenital anomaly that impairs the aesthetics, speech, hearing, and psychological and social life of an individual. To achieve good aesthetic outcomes, presurgical nasoalveolar molding (NAM) has become important. Currently, the intraoral NAM technique is widely practiced. Numerous modifications have been made to intraoral NAM techniques, but the original problem of compliance leading to discontinuation of treatment remains unsolved. Therefore, the present study compared an extraoral NAM technique with the intraoral NAM technique. Materials and Methods: Twenty infants with complete unilateral cleft lip and palate were included and divided into two equal groups. Group A received the intraoral NAM technique, and Group B received the extraoral NAM technique. Pre- and postoperative extraoral and intraoral measurements were recorded. Results: Groups A and B did not differ significantly in any extraoral or intraoral parameter. Conclusion: The extraoral NAM technique is as effective as the intraoral NAM technique in achieving significant nasal and alveolar changes in complete unilateral cleft lip and palate patients. Additionally, it reduces the need for frequent hospital visits for activation and the stress associated with the insertion and removal of the intraoral NAM plate, thereby improving compliance.

Effect of Garcinia mangostana L. and propolis extracts on the inhibition of inflammation and alveolar bone loss in ligature-induced periodontitis in rats

  • The purpose of this study was to evaluate the effect of mangosteen extract complex (MEC; Garcinia mangostana L. and propolis extracts) on the inhibition of inflammation and prevention of alveolar bone loss using a ligature-induced periodontitis model. Rat molars were ligatured with silk, and $1{\mu}g/mL$ of lipopolysaccharide of Porphyromonas gingivalis was injected into the buccal and palatal gingivae of the teeth with or without treatment with the MEC. Changes in the expression levels of prostaglandin $E_2$ ($PGE_2$), interleukin-8 (IL-8), inducible nitric oxide synthase (iNOS), matrix metalloproteinase-8 (MMP-8), cyclooxygenase (COX)-1, and COX-2 in gingival tissues were evaluated using enzyme-linked immunosorbent assays. Alveolar bone loss around the ligated molars was examined using micro-computed tomography. The expression levels of $PGE_2$, IL-8, iNOS, MMP-8, COX-1, and COX-2 in gingival tissues were significantly reduced in the group treated with a mixture of $16{\mu}g$ of mangosteen extract powder and $544{\mu}g$ of propolis extract powder (ligation [Lig] + lipopolysaccharide extracted from P. gingivalis KCOM 2804 [L] + MEC 1:34). Additionally, alveolar bone loss was significantly reduced in the Lig + L + MEC 1:34 group compared with that in other groups. These results indicate that the MEC could be useful in preventing and treating periodontitis.

방사선조사에 의한 쥐 폐손상에 방사선보호제로서 Captopril의 역할에 관한 연구 (The Radioprotective Effect and Mechanism of Captopril on Radiation Induced Lung Damage in Rat)

  • 목적 : Captopril을 방사선조사와 병용하여 조기 폐손상을 감소시킬 수 있는 방사선보호제로서의 역할을 확인하고 $TNF\alpha$$TGF\beta1$이 방사선 보호기전에 관여하는지를 알아보고자 하였다. 대상 및 방법 : 실험동물(Sprague-Dawley 흰쥐)은 정상대조군, 실험군(방사선 단독군, Captopril과 방사선 병용군)으로 분류하였다. 실험군은 12.5 Gy의 방사선을 좌측 흉곽에 단일조사 하였다. Captopril과 방사선 병용군은 Captopril (50 mg/kg/d)을 방사선조사 1주전부터 실험종료시인 8주까지 식수에 섞어 경구 투여하였다. 실험결과는 방사선조사 2주와 8주 후에 병리조직 소견을 분석하였고 면역조직화학염색으로 $TNF\alpha$$TGF\beta1$의 발현을 관찰하였다. 결과 : 방사선조사 2주 후에, Captopril과 방사선 병용군은 방사선 단독군에 비해 폐포강내 출혈, 폐포 상피세포 변화, 기관지 상피세포 변화, 혈관변화, 혈관주위 부종과 같은 조직소견의 정도가 현저히 감소되었다. 방사선조사 8주 후에는 기관지 상피세포 변화와 혈관주위 부종이 방사선 단독군에 비해 적었다. Captopril과 방사선 병용군에서 방사선조사 2주후 $TNF\alpha$의 발현은 방사선 단독군과 비교시 폐포 상피세포(p<0.01) 및 폐포강의 대식세포(p<0.01)에서 현저히 감소되었고, 기관지 주변의 림프조직(p=0.06)에서는 감소하는 경향이었다. $TGF\beta1$은 폐포상피세포(p<0.02) 및 폐포강의 대식세포(p<0.02)에서 양성세포가 현저히 감소되었다. 방사선 단독군과 비해 8주후 Captopril과 방사선 병용군의 $TNF\alpha$는 차이가 없었고, $TGF\beta1$의 발현은 폐포강의 대식세포(p=0.09)에서만 감소하는 경향이었다. 결론 : 흰쥐의 폐에 Captopril을 방사선과 병용투여하여 병리조직 소견을 관찰한 결과 방사선에 의한 조기 폐손상이 감소됨을 확인할 수 있었다. 또한 방사선조사 2주 후는 $TNF\alpha$$TGF\beta1$의 발현이 감소하고, 8주 후에는 $TGF\beta1$ 발현의 감소가 관찰되어, Captopril이 조기 폐손상을 억제하는 방사선보호제로서 기전의 일부에 $TNF\alpha$$TGF\beta1$이 관여함을 확인할 수 있었다.

정상골격군에서의 연령에 따른 하악 전치부 치조골 두께의 차이 (Differences in mandibular anterior alveolar bone thickness according to age in a normal skeletal group)

  • 골격성 I급이면서 수직적으로 정상인 환자 160명(남자 80명, 여자 80명)의 측모 두부방사선 계측사진을 대상으로 하악 전치부 치조골의 협설측 두께를 치축을 기준으로 계측하여 연령에 따른 차이를 조사하여 다음과 같은 결과를 얻었다. 하악전치부 치조골의 협설측 두께와 하악 전치부 설측 치조골의 두께는 연령이 증가함에 따라 여자의 CEJ 하방 2 mm 부위를 제외하고는 남녀 모두에서 유의한 감소를 보였다. 반면에, 하악 전치부 협측 치조골의 두께와 하악 이부 최대 풍융부의 두께는 연령이 증가함에 따라 유의한 차이를 보이지 않았다. 이상의 연구를 통하여 한국인에 있어서 수평적으로 I급의 골격형태이면서 수직적으로 정상인 환자에서는 성인보다는 성장기환자에서 하악 전치의 설측 치조골의 두께가 더 두꺼워 발치치료에서와 같은 하악 전치의 설측이동이 유리할 것으로 사료된다.

Alveolar ridge preservation with an open-healing approach using single-layer or double-layer coverage with collagen membranes

  • Purpose: The aim of this prospective pilot study was to compare alveolar ridge preservation (ARP) procedures with open-healing approach using a single-layer and a double-layer coverage with collagen membranes using radiographic and clinical analyses. Methods: Eleven molars from 9 healthy patients requiring extraction of the maxillary or mandibular posterior teeth were included and allocated into 2 groups. After tooth extraction, deproteinized bovine bone mineral mixed with 10% collagen was grafted into the socket and covered either with a double-layer of resorbable non-cross-linked collagen membranes (DL group, n=6) or with a single-layer (SL group, n=5). Primary closure was not obtained. Cone-beam computed tomography images were taken immediately after the ARP procedure and after a healing period of 4 months before implant placement. Radiographic measurements were made of the width and height changes of the alveolar ridge. Results: All sites healed without any complications, and dental implants were placed at all operated sites with acceptable initial stability. The measurements showed that the reductions in width at the level 1 mm apical from the alveolar crest (including the bone graft) were $-1.7{\pm}0.5mm$ in the SL group and $-1.8{\pm}0.4mm$ in the DL group, and the horizontal changes in the other areas were also similar in the DL and SL groups. The reductions in height were also comparable between groups. Conclusions: Within the limitations of this study, single-layer and double-layer coverage with collagen membranes after ARP failed to show substantial differences in the preservation of horizontal or vertical dimensions or in clinical healing. Thus, both approaches seem to be suitable for open-healing ridge preservation procedures.

  • Intra-oral films of 155 patients were taken on the areas of Gold Crown-Bridges, S. P. Bridges and Partial Dentures. And the author observed periodontal ligaments widening rates, the presence of periapical lesions and the alveolar bone patterns on the Partial Denture Saddle portions. The results were as follows : 1. Periodontal ligaments a) Periodontal ligaments widening rate increased obviously with the duration of wearing those prosthetic appliances. b) Periodontal ligaments widening rate was the highest in partial denture wearing patients, and then that of S. P. Crown Bridge patients, Gold Crown Bridge wearing females, Gold Crown Bridge males were the sequences. 2. Periapical lesions a) Periapical lesions increased with the wearing duration of those prosthetic appliances. But they did not increase in proportion to the duration. Periapical lesions were about the same in the teeth with those appliances showing relatively higher rate in Partial Denture abutment teeth and the lowest in Gold-Bridge abutment teeth. 3. Alveolar bone of Partial Denture saddle portion One third of all(32%) were normal with their smooth and compact bone pattern so that they were considered to perform normal function.

서울-경기 지역 2-6세 아동의 발달기적 음운변동에 관한 연구 - 자음을 중심으로 - (A study of phonological regression in 2-6 years of Korean children)

  • This study was designed to investigate the changes of phonological processes in normal Korean children aged from 2- to 6-years. Forty eight children who lived in Seoul or Kyung-Ki do were tested with a picture articulation test and their articulation errors including omissions, additions and substitutions were coded into phonological processes. Those phonological processes were discussed in several ways: syllable structure, place, manner, assimilation, tenseness, and aspiration of sounds. Data were analyzed by two ways: (1) number of subjects who showed each process and (2) percentage of occurrence of each process. Analyses in omission-addition processes demonstrated that postvocalic omission occurred most frequently, followed by velar-, alveolar-, and glottal omission. Analyses in substitution processes showed that fronting (palatal and velar), backing (alveolar), and alveolization occurred most frequently in terms of the place of sounds. In terms of assimilation, alveolar-, stopping, and aspiration assimilation occurred frequently. Analyses by the tenseness and aspiration showed similar occurrences among the 4 processes, with slightly higher occurrences in tensing and aspiration than lanxing and deaspiration. All of the processes decreased by age. The numbers of the processes showed by more than half of the children or exceeded 10% of occurrence were 20 in 2-years of age, 10 in 3-years of age, 1 in 4-years of age, and none in ages of 5 and 6.

Alveolar socket preservation with demineralised bovine bone mineral and a collagen matrix

  • Purpose: The aim of the present study was to evaluate the healing of post-extraction sockets following alveolar ridge preservation clinically, radiologically, and histologically. Methods: Overall, 7 extraction sockets in 7 patients were grafted with demineralised bovine bone mineral and covered with a porcine-derived non-crosslinked collagen matrix (CM). Soft tissue healing was clinically evaluated on the basis of a specific healing index. Horizontal and vertical ridge dimensional changes were assessed clinically and radiographically at baseline and 6 months after implant placement. For histological and histomorphometric analysis, bone biopsies were harvested from the augmented sites during implant surgery 6 months after the socket preservation procedure. Results: Clinically, healing proceeded uneventfully in all the sockets. A trend towards reduced horizontal and vertical socket dimensions was observed from baseline to the final examination. The mean width and height of resorption were 1.21 mm (P=0.005) and 0.46 mm (P=0.004), respectively. Histologically, residual xenograft particles ($31.97%{\pm}3.52%$) were surrounded by either newly formed bone ($16.02%{\pm}7.06%$) or connective tissue ($50.67%{\pm}8.42%$) without fibrous encapsulation. The CM underwent a physiological substitution process in favour of well-vascularised collagen-rich connective tissue. Conclusions: Socket preservation using demineralised bovine bone mineral in combination with CM provided stable dimensional changes of the alveolar ridge associated with good reepithelialisation of the soft tissues during a 6-month healing period.

Bone changes in the mandibular incisors after orthodontic correction of dental crowding without extraction: A cone-beam computed tomographic evaluation

  • Purpose: This study aimed to evaluate changes of the alveolar bone and interdental bone septum of the mandibular incisors through cone-beam computed tomography (CBCT) after orthodontic treatment of mandibular dental crowding without dental extraction. Materials and Methods: The sample consisted of 64 CBCT images(32 pre-treatment and 32 post-treatment) from 32 adult patients with class I malocclusion and an average age of 23.0±3.9 years. The width and height of the alveolar bone and interdental septum, the distance between the cementoenamel junction (CEJ) and the facial and lingual bone crests, and the inclination of the mandibular incisors were measured. Results: The distance between the CEJ and the marginal bone crest on the facial side increased significantly (P<0.05). An increased distance between the CEJ and the bone crest on the facial and lingual sides showed a correlation with the irregularity index (P<0.05); however, no significant association was observed with increasing mandibular incisor inclination (P>0.05). The change in the distance between the CEJ and the marginal bone crest on the facial side was correlated significantly with bone septum height(P<0.05). Conclusion: Bone dehiscence developed during the treatment of crowding without extraction only on the incisors' facial side. Increasing proclination of the mandibular incisor was not correlated with bone dehiscence. The degree of dental crowding assessed through the irregularity index was associated with the risk of developing bone dehiscence. The interdental septum reflected facial marginal bone loss in the mandibular incisors.

Effectiveness of miniscrew assisted rapid palatal expansion using cone beam computed tomography: A systematic review and meta-analysis

  • Objective: This study aims to examine the effectiveness of miniscrew assisted rapid palatal expansion (MARPE) treatment in late adolescents and adult patients using cone-beam computed tomography (CBCT). Methods: Literature search was conducted in five electronic databases (PubMed, Embase, Scopus, Web of Science, and Cochrane Library) based on the PICOS keyword design focusing on MARPE. Out of the 18 CBCT screened outcomes, only nine parameters were sufficient for the quantitative meta-analysis. The parameters were classified into three main groups: 1) skeletal changes, 2) alveolar change, and 3) dental changes. Heterogeneity test, estimation of pooled means, publication bias, sensitivity analysis and risk of bias assessment were also performed. Results: Upon database searching, only 14 full-text articles were qualified from the 364 obtained results. Heterogeneity test indicated the use of the random-effects model. The pooled mean estimate were as follows: 1) Skeletal expansion: zygomatic width, 2.39 mm; nasal width, 2.68 mm; jugular width, 3.12 mm; and midpalatal suture at the posterior nasal spine and anterior nasal spine, 3.34 mm and 4.56 mm, respectively; 2) Alveolar molar width expansion, 4.80 mm; and 3) Dental expansion: inter-canine width, 3.96 mm; inter-premolar width, 4.99 mm and inter-molar width, 5.99 mm. The percentage of expansion demonstrated a skeletal expansion (PNS) of 55.76%, alveolar molar width expansion of 24.37% and dental expansion of 19.87%. Conclusions: In the coronal view, the skeletal and dental expansion created by MARPE was of the pyramidal pattern. MARPE could successfully expand the constricted maxilla in late adolescents and adult patients.
