Nurses' Attitude toward Migrant Wives (결혼이민 여성에 대한 간호사의 태도)

  • Purpose: The survey was performed to explore nurses' attitudes toward migrant wives in Korean society. Method: Four hundred thirty-eight respondents were recruited from 2 acute hospitals from 2 different geological areas. The attitude inventory to measure attitude toward migrant wives was utilized, which includes 6 factors such as prejudice, reliability, offensiveness, interests, acceptance, and influence. Result: The overall attitude was slightly positive (3.23) and reliability was the only non-positive scoring factor. Among the general characteristics education level showed a significant difference in 5 factors consistently compared to other characteristics. Conclusion: It can be concluded that Korean nurses tend to take a neutral to slightly positive attitude toward migrant wives. Therefore, any influence from nurses' attitudes related to migrant wives can be ignored. Obstacles or barriers which could hamper nursing care for migrant wives need to be identified and studies of strategies to overcome any existing barriers are called for.

Development of the Transition Shock Scale for Newly Graduated Nurses (신규간호사의 전환충격 측정도구 개발)

  • Purpose: The purpose of this study was to develop a scale to assess the transition shock experienced by newly graduated nurses, and test the validity and reliability of the scale. Methods: The initial items were identified through a review of literature and in-depth interviews with nine newly graduated nurses. Content validation of the items was evaluated by five nurse professors and three nurses. Participants were 269 newly graduated nurses who worked at six acute care hospitals in Busan, Ulsan, and Yangsan, South Korea. Data were analyzed using item analysis, exploratory and confirmatory factor analysis, criterion related validity, and internal consistency. Results: The final scale consisted of 18 items and six factors (conflict between theory and practice, overwhelming workload, loss of social support, shrinking relationship with co-workers, confusion in professional nursing values, and incongruity in work and personal life), which explained 71.3% of the total variance. The six subscale model was validated by confirmatory factor analysis. Cronbach's alpha for the total items was. 89. Convergent validity was evaluated by analyzing total correlation with burnout (r=.71, p<.001) and turnover intention (t=5.84, p<.001). Conclusion: This scale can be used in the development of nursing interventions to reduce the transition shock experienced by newly graduated nurses.

The Impact of the Followership Style on the Organizational Effectiveness in Nursing Organization (간호조직내의 팔로워십[Followership]이 조직유효성에 미치는 영향)

  • Purpose: The purposes of the study were to examine whether nurses' followership style influence organizational effectiveness and to test the model of organizational effectiveness including the factors of nursing organizational culture, leadership style, and followership style. Methods: Data were collected from July 10th to september 20th in 2006 from 451 nurses working in four acute care hospitals. Data analysis were performed by Chi-square test, ANOVA, Duncan test, Pearson Correlation, and maximum likelihood estimation, using the SAS and LISREL 8.30 program. Results: The findings showed that the types of nurses' followership were classified as passive (21.1%), alienated (8.2%), conformist (16.4%), pragmatist (35.9%), and effective style (18.4%). There was significant difference in the organizational effectiveness among the nurses with different followership styles. In testing the path model, transformational, transactional leadership directly influenced followership style. Followership directly influenced organizational effectiveness. Conclusion: The overall fitness of the model was good, evidenced by general fitness index (GFI)=.94, adjusted goodness of fitness index (AGFI)=.82, and root mean square residual (RMSEA)=.06. Based on the findings, it is recommended to develop a program for enhancing effective followership in nursing organization.

A Survey of Drug Intoxication Patients Transferred by 119 Ambulance Service - Seocho area in Seoul - (일 지역 119구급대를 통한 약물중독 사례 고찰 - 서울시 서초지역 중심 -)

  • This study aims to provide information about the acute poisoning patients transferred by 119 ambulance service for nurses and emergency medical technicians. The data is based on 119 ambulance services patients charts of Seocho area in Seoul. The results were as follows: 1. Total number of emergency patients all over the country has been nearly fourtimes in 1995 compared to 1991. Total number of emergency patients were 322.051 in 1994. 2. About $35\%$ of total E.R. patients were transferred by 119 ambulance services in Seoul in the last 5years. Acute drug intoxication patients occupied $2-3\%$ of E.R. patients in Seoul. 3. About $4\%$ of E.R. patients in Seoul were transferred by 119 E.R. services of Seocho area in Seoul in 1995. Among them $2.4\%$ patients were drug intoxication patients. 4. Data were collected from available patients of 119 ambulance services from Jan. 1993 to Dec. 1995 in Seocho area. Total poisoning patients were 184. - The female/male ratio was 69.7:30.4 and most of patients$(69.7\%)$ were in the age of 20th and 30th. - The busy time of calling 119 ambulance service was from 6P.M. to midnight$(37.5\%)$ - It took within 10 minutes for patients$(62.8\%)$ from notification to arrival in hospital. - Regarding poisoning substances. hypnotics $(22.8\%)$, tranquilizer$(14.7\%)$, agricultural agents$(6.0\%)$ rodenticide$(27\%)$ and others $(10.3\%)$ were in order and unknown were $43.5\%$. - Most of the patients or protector $(72.8\%)$ chose hospitals. Among them $(87.3\%)$ were general hospital. - The most predominant symptoms were coma$(22.3\%)$. dizziness$(6.5\%)$ and then allergy. vomiting. gastrointestinal cramps etc. - Airway management and oxygen administration together was the main medical control of prehospital emergency medical services$(33.7\%)$. It is proposed that first, a systematic survey of drug intoxication patients must be conducted to give an appropriate prehospital emergency care for the emergency medical technician and second, a wide and regular public education to improve understanding of first aids should be undertaken.

Implementing Best Practice in Critically Ill Organophosphate Poisoned Patient Through Simulation-Based Learning Program (중환임상중독환자의 상황 인지와 대처 방법에 대한 시뮬레이션 교육의 효과 연구)

  • Purpose: Despite the clinical and socio-economic impact of acute poisoned patients, many of the treatments are not standardized in Korea. Moreover, no formal training that is specifically focused on clinical toxicology exists. Rather, training and education are conducted case by case in various institutions. This study was conducted to develop a standardized simulation-based clinical toxicology training curriculum for healthcare providers. This program will focus on specific assessment and treatment of critical toxicology patients, specifically those who have been poisoned with organophosphate. Methods: The study was performed using a pre- and post-design to determine the effects of implementation of this program. The study was conducted at eight different urban teaching hospitals in a simulated room in the clinical area. The study was targeted to 19 groups composed of emergency residents and nurses. Simulation-based learning was conducted for each group. Results: All 19 groups achieved the minimum passing score of 75%. Implementation of the program led to improved performance rates for overall management and cooperative moods competency (p<0.01). Inter-rater agreement between the two evaluators was excellent. In general, the participants thought the program was realistic and were able to recognize and improve the competencies needed to care for organophosphate poisoned patients. Conclusion: Simulation-based learning is an effective educational strategy that can be applied to improving and understanding proper care for rare but critical patients. This program was effective at improving team performance and cooperative moods when managing an organophosphate poisoned patient in the Emergency Department.

A Study on the Educational Needs of the Kidney Transplant Patients and Educational Importance perceived by Their Nurses (신장이식 환자의 교육요구와 간호사가 지각하는 교육의 중요성 비교)

  • The purpose of this study was to explore the educational needs of kidney transplant patients and educational importance perceived by their nurses to develop a rehabilitational and educational program. Data were collected from January 29, 1999 to July 30, 1999 with interviews using a structured questionnaire. The subjects for this study were 173, of whom 107 were patients who had had a kidney transplant and had visited the out-patient department and 66 were transplant ward nurses who were taking care of the kidney transplant patients at six general hospitals located in Seoul. The questionnaire used for this study was developed by the investigator through a literature review and collected and modified by 11 professional personnel and 3 kidney transplant patients. The data were analyzed using the SAS program for numbers, percentiles, mean, standard deviation, t-test, ANOVA, and $Scheff{\acute{e}}$ test. The results were as follows; 1) In the patient group, the total mean score for educational needs was 154.61 and the item mean score was 3.96. In the nurses group, the total mean score for perceived educational importance was 166.26 and the item mean score was 4.26. In the nurses group, perceived educational needs were scored higher than by the patient group. With regard to domains, both patient and nurses group had the highest educational needs and perceived educational importance in the domain of physical condition and the top five items in the educational needs and perceived educational importance were also in the domain of physical condition. 2) In the patient group, women and the divorce/bereavement group had higher educational needs in the domain of nutritional management, those who had been admitted longer than 4 weeks from their kidney transplant time had higher educational needs in the domains of physical condition and those who were less than 4 years from their transplant had higher educational needs in the domain of follow-up care. In the nurses group, those who were married had higher perceived educational importance in the domain of physical condition.

Burden and Needs of the Family Members of the Intensive Care Unit Patients (중환자실 입원환자 가족의 부담감과 요구도)

  • This study set out to investigate the burden and needs of family members of ICU(Intensive Care Unit) patients facing a crisis situation of a family member being hospitalized in ICU and check their connections. Data were collected from May to October 2013. The subjects include 93 main family caregivers of patients hospitalized in ICU at four general hospitals in the B area for three days or longer. Collected data were put to t-test, ANOVA, Scheff$\acute{e}$ test, and Pearson's correlation coefficient analysis with the SPSS WIN 20.0 program. As a result, the family members of ICU patients scored mean 3.56 points(out of five) on burden and mean 3.58 points(out of four) on needs, recording a moderate or higher level in both the areas. There was significant difference(F=3.463, p=.036) in burden according to the general characteristics with the number of days in the hospital. There was significant positive correlation(r=.332, p<.001) between their burden and needs. Those findings indicate that an active nursing intervention to reduce the burden of the families of ICU patients in a crisis situation and check their needs will be able to mitigate the family crisis and further have positive influences on the recovery of ICU patients.

Institutionalization of a Patient-Sitter Program in Acute Care Hospitals (보호자 없는 병원 제도화 방안)

  • In an effort to resolve the burden of patients hiring patient sitters, this study sought to review the Ministry-of-Health-and-Welfare-initiated pilot program of running hospitals without patient-sitter to identify its background, operation method, performance results, and limitations. Based on the review, the study derived the necessity of introducing a hospital system without patient-sitter as well as its operation and systemization methods. The ministry-initiated pilot programs were conducted twice: in 2007, and in 2010. A review of the 2007 pilot program revealed that the patients and families' satisfaction score with nursing services was 9.1 points (on a 10-point scale), their intention to reuse the service was 97.8%, and their intention to recommend the service was 98.0%, all high scores. Appropriate nursing manpower, derived from the 2007 pilot project, indicated 2.3 patients per nurse and 4.0 patients per nurse aid. The 2010 pilot project results indicated that the patients and families' satisfaction was high at 8.0-9.1 points (on a 10-point scale), and that the intention to reuse and recommend the service was also high. Compared with the 2007 pilot project, however, the types of medical institutions and the nurse to patient ratios were diverse, offering limitations. In conclusion, to systemize hospitals without patient-sitter, it is necessary to develop policies designed to establish criteria for the appropriate nurse to patient ratio and skill-mix, to standardize the work, to prepare finances for securing nursing staff, to evaluate the nursing demands, and to monitor the quality management.

Survey of Under-Reporting Rate and Related Factors after Blood and Body Fluid Exposure among Hospital Employees (병원직원의 환자 혈액 및 체액 노출 후 미보고 및 관련 요인)

  • Purpose: The purpose of this study was to examine the under-reporting rate and related factors after blood and body fluid (BBF) exposure among hospital employees. Methods: Fifteen hundred l employees were conveniently sampled from ten university and acute care hospitals. The survey questionnaire consisted of 37 items. Data were collected from September 10 to November 30, 2008. Results: The survey response rate was 88.7%. The 47.9% (638/1,331) of hospital employees were exposed to BBF and the mean number of exposure was $4.7{\pm}5.942$ within the previous year. Under-reporting rate after BBF exposure was 69.4% (443/638). By multi-variate logistic regression analysis, the exposure number, exposure type, infectious disease and hospital were independently related to the under-reporting of BBF among hospital employees. Conclusion: The under-reporting rate after being exposed to blood and body fluids was relatively high. To address this problem, educational programs are needed to decrease the under-reporting rate for healthcare workers. Further, it might be helpful if other factors related to under-reporting be investigated in future studies.

A Clinical Study of the Pediatric Patients Who Visited in Emergency Room of Oriental Medical Hospital (모 한방병원 응급실에 내원한 소아 환자에 대한 임상적 연구)

  • Objectives The purpose of this study was to revitalize pediatric emergency care in oriental medical hospital. Methods The study was composed of 281 pediatric patients who visited emergency room of the ${\bigcirc}{\bigcirc}$ university oriental hospital, from March 2006 to February 2007. Results The age distribution showed that 1 to 3 year's patients(40.6%) are the most New pediatric patients(59.1%) were more than who received medical treatment in this place. The seasonal distribution of between month's section and the 24 solar terms's section made no differences and pediatric patients who visited in emergency room was most in spring, least in autumn. Moreover the changes of season's patients were more than others. Pediatric patients who visited during the weekends and holidays(50.9%) were more than during the weekdays(49.1%). Systemic division of the major problems were the digestive symptoms(44.5%) and nervous symptoms(30.6%), respiratory symptoms(12.5%), musculoskeletal symptoms(5.3%), dermatological symptoms(1.8%), cardiovascular symptoms(0.7%) and others(4.6%) followed. The time interval between arrival and onset : within 6 hours were the most(54.5%). Pediatric patients who had digestive, nervous, musculoskeletal diseases within 6 hours were the most, but in the case of patients who had respiratory diseases, most of them were visited within 48 hours. Acting and herb-med treatment(77.9%) were the most in medical treatments. Pediatric patients(69.0%) who didn't revisit in this hospital after treatment in emergency room were more than who revisited(29.9%). Conclusions Pediatric patients who visited in emergency room of oriental medical hospital were most not acute form of a serious diseases, patients most visited in disease prefer to oriental medicine; such as dyspepsia, convulsions, crying, ankle sprain. Further studies will be needed for the actual circumstance's reflecion from this hospital and other oriental hospitals nearby or other western emergency rooms for the accurate studies.
