Clinical Study for Ten Cases, who Complains Abdominal Pain after Surgery -Laparoscopic Gynecological Surgery- (부인과 질환관련 복강경 수술 후 발생한 복통 환자 10예의 임상적 고찰)

  • Purpose: The purpose of this study was to report the effect of TKM (Traditional Korean Medicine) to abdominal pain after laparoscopic gynecological surgery(LGS). Methods: 10 patients who visited the department of gynecology in OO medical center from 1st August 2007 to 31st December 2008 with abdominal pains after laparoscopic operation. They complained abdominal pain and other pains such as back pain, shoulder pain and vaginal bleeding etc. We treated patients with herb medicine, acupuncture and moxibustion treatment. The progress of signs and symptoms was evaluated by checking the change of visual analog scale(VAS). Results: The mean age was 45.1 years(range 38-49), parity 2(0-3) and previous abdominal surgery case was 5. The mean of hospital admitting day was 20 days(range 9- 51) and taken for reducing VAS 10 to 3 were 10 days(range 4-24). After taking TKM, patient's signs and symptoms were alleviated or resolved and Hb, Hct were increased. Conclusion: After laparoscopic gynecological surgery, patients had taken pain such as abdominal pain, shoulder pain, back pain etc. TKM treatment is effective on the recovery after laparoscopic surgery.

A Study about the Satisfaction of Oriental Medical Postpartum Care (산욕 초기 입원치료를 받은 산모들의 한방 치료 만족도 조사)

  • Purpose: 1. To analyze the satisfaction and cognition of postpartum care and to aid the spread of oriental medicine in postpartum care. 2. Get a basic guideline of O.M.D's postpartum care. Methods: We studied the satisfaction and cognition of postpartum care with questionnaire from January 2009 to August 2009, who was treated in ○○ Oriental Medical Postpartum care center during 2 weeks. 61 puerperants answered us. Results: 1. In choosing Postpartum care center, puerperants concerned about new born infants' care(50.8%) and treatment for discomfort after delivery(27.86%). 2. 22 puerperants(36.6%) had a positive view in folk remedy for postpartum care and 34 puerperants(55.73%) thought it would be good to eliminate edema and loose weight. 3. Most of puerperants(80.32%) chose oriental medical postpartum care because they wanted to receive oriental medical treatment. 4. 5 puerperants(8.19%) were very satisfied with oriental medical postpartum care than other type of postparum care which they experienced at first delivery. 47(77.04%) were satisfied, 2(3.27%) were unsatisfied and no one was very satisfied. 5. To the questionnaire about the effect of strength recovery, lochia excretion, edema limination, increasing amount of breastmilk, reducing arthralgia, about 36.8% puerperants showed positive view. 6. The positive view about Acupuncture, cupping, moxibustion, herbal treatment was, respectively, 90.16%, 49.18%, 73.77%, 86.88%. Conclusion: Puerperants were satisfied with oriental medical postpartum care and had positive cognition in oriental medical treatment. There is much capability for extension of oriental medicine in postpartum care. So continuous concern, systemic development and publicity about oriental medical postpartum care are required.

The Role and Policy of Korean Medicine Doctor for the Prevention and Management of Infectious Diseases: A Mixed-Methods Approach (감염병 예방 및 관리를 위한 한의사의 역할과 정책적 과제: 혼합연구방법론을 적용하여)

  • Objectives: This study investigated the role of Korean medicine doctors in the national quarantine system and the status of their duties through questionnaire surveys and in-depth interviews with Korean medicine staff who participated in COVID-19 work. Methods: We developed a questionnaire survey to investigate the status and perception of Korean medicine doctors' participation in responding to infectious diseases, and the results were derived by conducting frequency analysis and T-test using SPSS program. The measurement variables of this study were the status of COVID-19 related work participation and perception of Korean medicine doctors' role in infectious diseases, and the maximum significance level was 0.05. Results: There were 300 respondents to the survey, and as for the COVID-19 related work, Korean medicine treatment for COVID-19 patients accounted for the most(52.7%), and various preventive and treatment tasks such as epidemiological investigations and specimen collection were conducted. Conclusion: According to this study, it was confirmed that there are sufficient legal and administrative grounds for Korean medicine doctors to perform specimen collection and epidemiological investigations. Therefore, it is necessary to prepare a policy framework to solidify the status of Korean medicine doctors in national infectious diseases in the future.

Peer Role-Play in a College of Korean Medicine to Improve Senior Students' Competencies in Patient Care and Communication: A Case Analysis and Proposal for a Model (한의학 전공학생의 진료 및 의사소통 역량 향상을 위한 동료 역할극 모델제안과 사례분석)

  • Objectives: Peer role-play (PRP) has been used in health care training simulations because standardized patient training requires considerable time and expense. This study described the implementation of clinical simulation using PRP and examined the effect. Methods: Final year students from a single college of Korean medicine engaged in PRP as part of clinical skills practice. Education tools from clinical practice guidelines were used to structure the PRP. Communication competency was assessed with the Korean Version of the Self-Efficacy Questionnaire (KSE-12). Whether this training helped to achieve graduate outcomes was evaluated on a five-point scale. Results: Fifty-nine students (53.2%) participated in the survey. Among 12 items on the KSE-12, the score for "How certain are you that you are able to successfully listen attentively to the patient?" was the highest. Further, PRP was found to be helpful for self-directed learning, establishment of one's professional identity, and the ability to communicate and manage patients. Three themes ("Benefits of role-play", "The importance of positive feedback", "Limitations and problems of role-play"), 15 categories, and 16 central meanings were derived by categorizing learners' subjective opinions about PRP. Conclusions: Study findings indicate that PRP may contribute to improving communication skills and establishing a professional identity for future Korean medicine doctors. We suggest using PRP in clinical education in colleges of Korean Medicine.

A Case Report of Tremor Patient Diagnosed as Acute Cerebral Infarction with Dodamhwalhul-tang (도담활혈탕(導痰活血湯)으로 호전된 뇌경색으로 인한 진전 환자 치험 1례)

  • ■ Objectives The purpose of this case study is to report that a tremor patient after Acute Cerebral infarction was treated with 'Dodamhwalhul-tang' and then the symptoms were improved. ■ Methods This patient was 81 years old woman whowas diagnosed with acute cerebral infarction. This patient had hypertension and symptoms of tremor on left lower limb, both side weakness, both lower limb numbness, dizziness and low back pain. She was treated with herb medicines, acupuncture, moxibustion during 22 days of hospitalization. Her tremor was evaluated by the tremor frequency and period and other general conditions were measured. ■ Results After taking Dodamhwalhul-tang, symtoms of tremor and patient's general conditions were improved. ■ Conclusion The result shows that Dodamhwalhul-tang is effective for tremor with acute cerebral infarction.

  • Neuropsychiatric disorder is one of the symptoms developed after traumatic brain injury. The main symptoms of neuropsychiatric disorder are delirium, anxiety, depression and so on. We treated the patient with herbal medicine, acupuncture, moxibustion and other appropriate traditional Korean medical treatment. The patient only appeared neuropsychiatric disorder, not another neurologic symptoms. We consider it is the rare case in traditional Korean medicine hospital. So we report the patient with the review of literatures regarding neuropsychiatric disorder induced by traumatic brain injury.

A Survey on Korean Medicine Doctors' Recognition for Developing Korean Medicine Clinical Practice Guidelines for Gastric Cancer (위암 한의표준임상진료지침 개발을 위한 한의사 인식조사)

  • Objective: The purpose of this study was to provide data for the development of Korean medicine (KM) clinical practice guidelines (CPG) for gastric cancer (GC) by identifying the awareness and knowledge needs of KM doctors (KMD) through an online survey. Methods: A survey questionnaire was produced by referring to the previous recognition survey studies for clinical KMDs. The survey questionnaire was composed of questions regarding the current status of clinical care for GC, the knowledge level of clinicians about GC, and other details that clinicians use during practice. The survey was conducted from September 20 to October 22, 2021. An online survey was conducted with 195 KMDs from the Society of Korean Medicine and the Korean Association of Traditional Oncology of KM. Results: A total of 195 respondents participated in the online survey regarding the current status of GC treatment. Of this number, 10.3% of the respondents specialize in GC, but even if they do not currently specialize in GC, they are more likely to treat patients with GC in the future. In terms of the specifics of KM treatment, there was a demand for information on comprehensive KM treatment methods, such as herbal medicine, acupuncture, moxibustion, cupping, and chuna, as well as on analyzing the effectiveness of combined treatment with chemotherapy and radiation. Conclusion: In this study, we were able to understand the clinical KMDs' perception of GC, knowledge level, and requirements in the CPG. The results of this study will provide the basic data for the development of CPG for GC.

A Case Report of Osuyubujaijung-tang on the Recurrence of Breast Cancer in a Patient Complaining of Coldness and Dizziness (유방암 재발 환자의 장궐 및 현훈이 오수유부자이중탕으로 호전된 치험 1례)

  • A 63-year-old female patient was admitted to the hospital with complaints of coldness in the head and right femur, persistent vertigo, dry mouth, vexation, general weakness, anorexia, and hot flashes. After being diagnosed with left-sided ductal carcinoma in 2017 and undergoing partial resection, she was recommended radiotherapy, chemotherapy, and hormone therapy, but she refused and did not receive any treatment. At a checkup in November 2022, the left-sided invasive ductal carcinoma recurred. She underwent partial resection and was scheduled to receive radiotherapy two months later. During hospitalization, the patient received traditional Korean medicine treatment, including Korean herbal medicine (Osuyubujaijung-tang), acupuncture, moxibustion, and cupping. Subjective symptom changes were evaluated daily. At discharge, the patient's symptoms improved, and her condition varied with the presence of Aconitum carmichaelii. This study suggests that "Osuyubujaijung-tang" cared the coldness and vertigo caused by "Han-dam (寒痰)" in patients with Soeumin visceral syncope pattern (少陰人 臟厥症) and that A. carmichaelii played a major role in this outcome.

Case Report on the Five-year Survival of a Patient with Rectal Cancer Treated with Integrative Medicine After Resection and Chemoradiation Therapy (수술 및 항암 방사선 표준 치료 이후의 한양방 통합치료를 통해 5년 무병생존에 도달한 3기 직장암 환자 1례에 대한 보고)

  • Objectives: This is a five-year survival and complete response (CR) report on rectal cancer treated with western medicine and Korean traditional medicine. Method: A 25-year-old woman diagnosed with rectal cancer visited ⃝⃝ Korean traditional medicine hospital after neoadjuvant concurrent chemoradiation therapy, chemotherapy, and low anterior resection with regional lymph node dissection. She was treated with Korean traditional medicine, including acupuncture, abdominal moxibustion, wild ginseng pharmacopuncture, and herbal medicine, which was based on integrated medicine therapy (IMT), from January 2018 to February 2022. The tumor size was measured by scanning with computed tomography (CT), magnetic resonance imaging, and positron-emission tomography/CT. Adverse events were evaluated using laboratory conclusion and National Cancer Institute Common Terminology Criteria for Adverse Events version 5.0. Result: During four years of treatment, IMT maintained safety. The patient finally reached five-year survival without any recurrence or complication (CR) on March 11, 2022. Conclusion: We suggest that an integrative approach including Korean traditional medicine can be a meaningful treatment option for rectal cancer. Further studies should be performed to establish the proper treatment protocol of integrative medicine for rectal cancer.

Clinical Practice Patterns for Benign Prostatic Hyperplasia: An Online Survey (전립선증식증(Benign Prostatic Hyperplasia)의 한의 임상 진료 현황 조사를 위한 온라인 설문 조사)

  • Objectives: This study investigated Korean medicine doctors' perspectives on clinical practice patterns in the process of developing Korean medicine clinical practice guidelines for benign prostatic hyperplasia. Methods: A questionnaire was developed for Korean medicine doctors. A total of 323 oriental medicine doctors participated in the survey, which was live for a total of 9 days from September 22, 2022, to September 30, 2022. Results: Regarding awareness of treatments for benign prostatic hyperplasia, 63.8% of respondents showed high awareness of Korean medical treatments. However, items such as diagnostic criteria (17.7%), evaluation methods (17.0%), and Western medical treatments (22.9%) showed low recognition rates. In clinical practice, 76.2% of respondents were found to treat five or fewer patients with benign prostatic hyperplasia per month, and the average treatment period was 1 to 3 months for most at 41.2%. Korean medicine doctors diagnosed benign prostatic hyperplasia based on clinical features. The main interventions used were acupuncture, herbal medicine (prescription medicine), and moxibustion. This study has several limitations because of the low response rate for this survey; therefore, the participants are not representative of all Korean medicine doctors. In addition, because the study was conducted broadly on various topics related to benign prostatic hyperplasia, sufficient quality management was not carried out. Further studies that include a larger sample size and more in-depth studies on benign prostatic hyperplasia are needed. Conclusions: It is necessary to develop appropriate and reasonable Korean medicine clinical practice guidelines for benign prostatic hyperplasia.
